  • To understand the trends in research in the fields of physical therapy, all issues of the Journal of Korean Society of Physical Therapy were reviewed from volume 1 in 1989 to volume 13, number 2 in 2001 in regard to research themes, research objects, and research designs. The number of articles dramatically increased in recent years. Although exercise physiology, biomechanics, and therapeutic exercise were major subjects of interest, alternative treatment modalities such as massage, taping, acupuncture were fields of increasing interests. There were also increasing interests in medical engineering and basic science. Cerebrovascular accidents, elderly people, sportsmen were research objects of increasing frequency. In regard to research designs, experimental research designs such as randomized controlled designs and nonequivalent controlled designs were increasingly adopted in clinical research.

Initial Report for the Radiation Effects Research Foundation F1 Mail Survey

  • To study the full health effects of parental radiation exposure on the children of the atomic bomb survivors, the Radiation Effects Research Foundation developed a cohort of 76,814 children born to atomic bomb survivors (F1 generation) to assess cancer incidence and mortality from common adult diseases. In analyzing radiation-associated health information, it is important to be able to adjust for sociodemographic and lifestyle variations that may affect health. In order to gain this and other background information on the F1 cohort and to determine willingness to participate in a related clinical study, the F1 Mail Survey Questionnaire was designed with questions corresponding to relevant health, sociodemographic, and lifestyle indicators. Between the years 2000 and 2006, the survey was sent to a subset of the F1 Mortality Cohort. A total of 16,183 surveys were completed and returned: 10,980 surveys from Hiroshima residents and 5,203 from Nagasaki residents. The response rate was 65.6%, varying somewhat across parental exposure category, city, gender, and year of birth. Differences in health and lifestyle were noted in several variables on comparison across city and gender. No major differences in health, lifestyle, sociodemographics, or disease were seen across parental exposure categories, though statistically significant tests for heterogeneity and linear trend revealed some possible changes with dose. The data described herein provide a foundation for studies in the future.

A Trend Analysis of Case Reports and Case Series on Hwa-byung in Korea

  • Objectives: This study aimed to provide a basis for treatment of Hwa-byung, a mental disorder frequently found among Koreans, by analysing published case reports and case series. Methods: In June 2015, a literature search for case reports and case series describing interventions for Hwa-byung was conducted in 7 databases for research publications in Korea, using Korean and English terms. Results: Thirty-two papers, reporting 439 cases of Hwa-byung, met the inclusion criteria. Since the first case report on Hwa-byung in 1996, case reports were continuously published until 2012. A total of 22 interventions were described, of which herbal medicine (71.9%), acupuncture (65.6%), and moxibustion (34.4%) were the most frequently described interventions. Twenty-five papers reported patterns, for which liver qi depression (28%) was most frequently used. A total of 48 outcome measures were used in 32 papers, of which Beck Depression Index (46.9%) and Symptom change (43.8%) were used in at least 10 papers. Conclusions: Since only 32 case reports/case series have been published on Hwa-byung in Korea, a strong basis for recommending standardized treatments is lacking. Therefore, a sufficient number of clinical studies, in particular randomized clinical trials (RCTs), are warranted to provide a clinical basis for treatment of Hwa-byung.

IMPACT OF FOURWING SALTBUSH ON FEED AND WATER INTAKE AND ON BLOOD SERUM PROFILE IN SHEEP

  • Sixteen Harnai males were used to evaluate the influence of varying levels of fourwing saltbush hay on feed and water intakes as well as the blood serum mineral status in a completely randomized design. The animals were grouped randomly into four, with four animals in each. The four groups were randomly allotted low, medium, high and very high levels of saltbush hay supplementation in addition to wheat straw. The animals were given fresh water at free of choice. Weekly body weight was recorded for each individual animal. Blood serum was collected for mineral contents. The experiment lasted for eight weeks. The inclusion of saltbush leaves in the diet showed a non-significant effect on the total dry matter intake. There has been a significant increase in the water intake when very high levels of saltbush were included in the ration. Lower levels showed no effect on the water intake. The animals maintained their body weight from week 1 to week 8. No treatment by weeks interactions on the potassium and sodium levels were detected. However higher levels of saltbush increased significantly the potassium and sodium contents in the serum. Calcium contents were significantly(p<0.01) lowered with the inclusion of saltbush leaves in the diet. Whereas Phosphorous contents showed an increasing(p<0.05) trend with the higher levels of saltbush. No clinical or sub-clinical toxicological symptoms were observed in the sheep with the higher mineral contents.

The Literary Trends of Herbal Prescription Ukgan-san and its Application in Modern Traditional Korean Medicine (동의보감에 수록되지 않았던 억간산의 문헌상 흐름과 현대 한의학에서의 응용 현황)

  • Objectives: Ukgan-san does not appear in Traditional Korean Medicine classics. However, it has been used in research and clinical applications in Korea. In this paper, we are aiming to examine the records concerned with Ukgan-san to the present, and announce the present usage state in Korea. Methods: We searched and summarized the contents of previous medical books about Ukgan-san. In order to investigate the current status of utilization of Ukgan-san in Korea, a bibliographic database of Korea, a database of clinical researches, and a database of pharmaceuticals of the Korea Food and Drug Administration were searched. Results: As a result of searching Ukgan-san related documents, Ukgan-san has been developed in China (Ming dynasty). However, its clinical application has been expanded by modern Japanese kampo medicine doctors. In Korea, cases of Parkinson's disease, other movement disorders, and psychiatric disorders have been reported, and randomized controlled trials had been done and planned. In addition, some extracts which are consisted of Ukgan-san are also available. Conclusions: In Korea, Ukgan-san has been influenced by Japan and has been used for clinical and research purposes. The utilization rate in Korea is still small, but the utilization rate will increase according to the results of clinical studies.

Clinical Research Trends in Respiratory Diseases Related to Particulate Matter (미세먼지 관련 호흡기질환 임상연구 동향)

  • Objectives: This study aimed to review the clinical research trends in respiratory diseases related to particulate matter (PM) to help design clinical studies on herbal medicine that protects against PM. Methods: We searched three international databases (Pubmed, EMBASE, and CENTRAL) to investigate clinical studies on respiratory diseases related to PM and to analyze their study design, respiratory-related disease, inclusion/exclusion criteria, study period, outcome measure, study results, measurement method of PM and range of PM. Results: A total of 18 studies were finally selected, including 13 observational studies and 5 randomized controlled trials. The selected studies showed an increasing trend from 1985 to 2018 and were conducted mostly in North America, followed by Europe and Asia. Subject disease and age were decided in variety by each study objective, although asthma accounted for the majority. For the outcome assessment, pulmonary function test was mostly used for lung function. Quality of life questionnaires and biomarkers in blood and sputum were also used. Conclusion: A well-designed clinical study on herbal medicine that protects against PM is needed, and this study is expected to be used as base data.

Trend analysis of new health technology in dentistry (치의학 분야의 신의료기술 동향 분석)

  • Objectives : Currently, it is being reported that new health technology (HT) is introduced and spread throughout Korea at a very fast rate. However, the current status of new HT related to dentistry has not been known or studied. Therefore, this study aims to understand the present state and awareness on dentistry-related domestic new HT via surveys and research, and thus apply this as a reference to construct an effective new HT-related system in the field of dentistry. Methods : The assessment status of New Health Technology Assessment (nHTA) committee was searched on the website for any updates on new HT5). The cases were searched in the range between Jan 1 2012 and Jul 31 2016 by submission date and were analyzed by year, division of technology, progress and major operating practitioner. Among those, technologies utilized by dentists were additionally analyzed. The survey result was obtained by combining responses of 25 participants from the workshop for new HT application and 11 members of Korean Association of Oral and Maxillofacial Surgeons (KAOMS). The workshop was co-hosted by KAOMS and Clinical Translational Research Center for Dental Science (CTRC) under Seoul National University Dental Hospital (SNUDH). Results : Most of the respondents to the survey have heard of new health technology but were unaware of the application procedure. 'Safety and effectiveness' was selected the most for things to consider when introducing new HT, followed by 'scientific evidence of the technology.' For new dental HT status, submissions between 2012 and Jul 31 2016 were analyzed by major operating practitioner, including cases where there are multiple practitioners. Yet, only 19 cases (17 %) of dentists were reported. Among 385 cases approved by nHTA, only 2 cases included dentists. Conclusions : Active publicity by the government on new HT and its application procedure, and continued interest and research in the dental and medical community is likely to be required for the development of dentistry.

A Review of Sa-am Acupuncture used in Korean Obstetrics & Gynecology (한방 부인과질환에 사암침법을 활용한 국내 연구 동향 분석)

  • Objectives: The Sa-am Acupuncture Method which is originated from 17th century is one of Korean traditional acupuncture methods and most widely adopted techniques used by clinicians. This study aims to investigate research trend of Sa-am acupuncture on Korean Obstetrics and Gynecology. Methods: We searched for papers which had Sa-am acupuncture used in women's disease in Korean journals published from 1987 to March 2021. Results: 33 papers were found. 5 clinical studies and 28 case studies were categorized to women's disease groups described in Korean Obstetrics and Gynecology textbook. In most of the studies, Sa-am acupuncture was used with other treatments. In the case reports, each case was diagnosed individually and Sa-am acupuncture was used according to the diagnoses. The most frequently used Sa-am acupuncture method in women's diseases was Sojang jung-geuk. Conclusions: Sa-am acupuncture could be a good treatment for women's diseases. But evidence of relation between Sa-am acupuncture and the disease is insufficient, so more research will be needed.

Detection of Onset and Offset Time of Muscle Activity in Surface EMG using the Kalman Smoother

  • A visual decision by clinical experts like physical therapists is a best way to detect onset and offset time of muscle activation. The current computer-based algorithms are being researched toward similar results of clinical experts. The new algorithm in this paper has an ability to extract a trend from noisy input data. Kalman smoother is used to recognize the trend to be revealed from disorderly signals. Histogram of smoothed signals by Kalman smoother has a clear boundary to separate muscle contractions from relaxations. To verify that the Kalman smoother algorithm is reliable way to detect onset and offset time of muscle contractions, the algorithm of Robert P. Di Fabio (published in 1987) is compared with Kalman smoother. For 31 templates of subjects, an average and a standard deviation are compared. The average of errors between Di Fabio's algorithm and experts is 109 milliseconds in onset detection and 142 milliseconds in offset detection. But the average between Kalman smoother and experts is 90 and 137 milliseconds in each case. Moreover, the standard deviations of errors are 133 (onset) and 210 (offset) milliseconds in Di Fabio's one, but 48 (onset) and 55 (offset) milliseconds in Kalman smoother. As a result, the Kalman smoother is much closer to determinations of clinical experts and more reliable than Di Fabio's one.

Physicochemical Properties of Freeze-dried Corn Starch Sponge Matrix (동결 건조된 옥수수 전분 스펀지 매트릭스의 이화학적 특성)

  • The focus of the current study was to investigate the physicochemical properties of a corn starch-sponge matrix prepared at a low concentration below gel forming by freeze-drying. The effect of variables(starch concentration, heating temperature, and heating hold time) on the physicochemical properties of the samples was analyzed by response-surface methodology. Regression models on the properties of samples such as hardness, springiness, and water solubility index(WSI) showed high correlation coefficients(r>0.95) and significant F values, but regression models for the other properties(swelling power, apparent viscosity, reducing sugar content, and digestibility) showed them to have relatively low significance. Sample hardness of sample showed the highest value at condition of $90^{\circ}C$ and 5%, whereas springiness was at a maximum at $130^{\circ}C$ and 5%. Also, at 1% of starch concentration, mechanical properties were greatly decreased as the relative humidity increased, compared with the 3% and 5%, especially in the hardness of samples. The WSI showed an increasing trend with heating temperature regardless of starch concentration. Overall, the physicochemical properties of freeze-dried corn starch-sponge matrix were influenced much more by starch concentration and heating temperature than by heating hold time. The results of this study show that the basic properties of freeze-dried corn starch-sponge matrix can be used for the specific food applications or as a functional material for its stability.
