골격성 III급 부정교합자의 임상적 특성에 따른 심리상태 (Psychological state of the patients according to skeletal class III malocclusion symptoms)

  • Objectives: The purpose of the study is to investigate the psychological state of the patients according to skeletal class III malocclusion symptoms. Methods: The subjects were 200 skeletal class III malocclusion patients. The questionnaire consisted of general characteristics, clinical manifestation, and T score of Korean version of self-rated Symptom Checklist-90-Revision modified by Jae-hwan Kim. The data were analyzed using SAS version 9.2 and t-test, ANOVA, and ANCOVA were used. Clinical manifestation included subjective recognition and radiological analysis. The subjective recognition of the patients consisted of self-satisfaction of the appearance, phonation, mastication, and temporomandibular joint pain. T score consisted of somatization, obsessive-compulsive, interpersonal sensitivity, depression, anxiety, hostility, phobic anxiety, paranoid ideation, and psychoticism. Results: The most serious factor in 9 scales was the severe TMJ pain by ANCOVA. Male patients had a higher T score in phobic anxiety, psyochoticism, somatization and depression than female patients. Those having low appearance satisfaction had the problem in interpersonal sensitivity. Obsessive-compulsive symptoms were conspicuous in phonation difficulty and temporomandibular pain. Conclusions: The patients with skeletal class III malocclusion have more satisfaction with appearance, pronunciation, and phonation than those with skeletal class III malocclusion and overjet. Proper dental treatment will improve the communication and quality of life.

Correlation between skeletal and dental changes after mandibular setback surgery-first orthodontic treatment: Cone-beam computed tomography-generated half-cephalograms

  • Objective: To investigate skeletal and dental changes after application of a mandibular setback surgery-first orthodontic treatment approach in cases of skeletal Class III malocclusion. Methods: A retrospective study of 34 patients (23 men, 11 women; mean age, $26.2{\pm}6.6years$) with skeletal Class III deformities, who underwent surgery-first orthodontic treatment, was conducted. Skeletal landmarks in the maxilla and mandible at three time points, pre-treatment (T0), immediate-postoperative (T1), and post-treatment (T2), were analyzed using cone-beam computed tomography (CBCT)-generated half-cephalograms. Results: The significant T0 to T1 mandibular changes occurred $-9.24{\pm}3.97mm$ horizontally. From T1 to T2, the mandible tended to move forward $1.22{\pm}2.02mm$, while the condylar position (Cd to Po-perpendicular plane) shifted backward, and the coronoid process (Cp to FH plane) moved vertically. Between T1 and T2, the vertical dimension changed significantly (p < 0.05). Changes in the vertical dimension were significantly correlated to T1 to T2 changes in the Cd to Po-perpendicular plane (r = -0.671, p = 0.034), and in the Cp to FH plane (r = 0.733, p = 0.016), as well as to T0 to T1 changes in the Cp to Po-perpendicular plane (r = 0.758, p = 0.011). Conclusions: Greater alterations in the vertical dimension caused larger post-treatment (T2) stage skeletal changes. Studying the mandibular position in relation to the post-surgical vertical dimension emphasized the integral importance of vertical dimension control and proximal segment management to the success of surgery-first orthodontic treatment.

유치열기 골격성과 비골격성 3급 부정교합 환아의 제1대구치 성숙도 비교 (Maturation of the First Molars in Primary Dentition with Class III Malocclusion)

  • 유치열기의 3급 부정교합은 조기 치료가 추천되지만 정확한 진단이 쉽지 않다. 3급 부정교합의 골격성 특성과 치아 발육상태 사이에 연관성이 있다면 골격성 3급 부정교합의 감별 진단을 위한 평가 항목으로 치아성숙도가 활용될 수 있을 것이다. 본 연구는 유치열기의 골격성 혹은 비골격성 3급 부정교합으로 진단받은 환아를 대상으로 치아 석회화도 및 제 1대구치의 맹출률을 비교, 분석하여 상하악의 치아성숙도 차이가 유치열기 3급 부정교합의 감별 진단시 평가 항목으로 사용될 수 있는지 알아보기 위한 목적으로 시행되었다. 전치부 반대교합을 주소로 부산대학교 치과병원 소아치과에 내원한 유치열기 아동 중 비골격성 3급 부정교합군 18명과 골격성 3급 부정교합군 34명을 연구대상으로 선정하였다. 파노라마 방사선사진상에서 치령 및 제1대구치 맹출률을 비교, 분석하여 다음과 같은 결과를 얻었다. 골격 및 성별에 따른 역령과 치령의 차이는 존재하지 않았으며, 두 군 모두 역령에 비해 치령이 높게 나타났다(p < 0.05). 비골격성군과 골격성군의 상하악 맹출률의 차이는 각각 16.53%, 18.91%로 통계학적으로 유의한 차이를 보여(p < 0.05), 3급 부정교합의 감별 진단을 위한 평가 항목으로서 제1대구치 치아성숙도의 활용 가능성을 시사한다.

서울시내에 거주하는 13세-15세 청소년들의 부정교합에 관한 역학적 연구 (PREVALENCE AND SEVERITY OF MALOCCLUSION IN CHILDREN 13 TO 15 YEARS OF AGE LIVING IN SEOUL)

  • Childrens between the ages of 13 and 15 years, living in Seoul, were examined in order to determine the prevalence and severity of malocclusion in the permanent dentition. This survey encompassed 981 children and an individual chart was prepared for each subject recording an original HMAR score and classification of occlusion according to Angle. Also, subjective evaluation of "treatment needs" was carried out in 581 children. The results were as follows: 1. Of the 981 children in this survey, 12,1 percent showed excellent occlusion. (0 point) 2. The 14 percent of the children who had a score of 24 and above all appeared to belong to the "treatment highly desirable" or "treatment mandatory" category. 3. The incorporation of SAR (Supplementary Assessment Record) into the HMAR can provide more sensitive method for evaluating severity of malocclusion. 4. According to Angle's classification 77.4 percent of all malocclusion belonged to Class I, 1.3 percent to Class II, Division 1, 0.9 percent to Class II, Division 2, and 11.3 percent to Class III. 5. The mean HMAR score for Class I was significantly lower than for either Class II, Division 1 or Class III. (P < 0.05) 6. A close relationship was found between the degree of "treatment needs" and the value obtained by the HMAR scoring. (chi-square test, p < 0.05) The differences between the mean HMAR scores of the various subjectively defined categories were statistically significant. (P < 0.001)

3급 치근분지부 골결손에서 미세전류자극이 치주조직의 재생에 미치는 영향에 관한 실험적 연구 (Experimental study on the effect of direct microcurrent to periodontal regeneration in class III furcation defects)

  • Several experimental studies showed that the application of small amounts of electric current to bone stimulated osteogenesis at the site of the cathode and suggests that the application of electrical currents to periodontal defects could promote bone and cementum formation. The purpose of this study was to determine the effect of direct microcurrent to the periodontal regeneration of class III furcation defects in dogs. Class III furcation defects were surgically created on the third and the fourth premolars bilaterally in the mandibles of nine mongrel dogs. Experimental periodontitis were induced by placing small cotton pellets into the created defects for 3 weeks. The experimental sites were divided into three groups according to the treatment modalities: Group I-surgical debridement only; Group II-allogenic demineralized freeze dried bone grafting; Group III-allogenic demineralized freeze dried bone grafting and electrical stimulation. For fluorescence microscopic evaluation, calcein, oxytetracycline HCI and alizarin red were injected 2, 4 and 8 weeksfS days prior to sacrifice) after surgery. The animals were sacrificed in the 1st, 2nd, 4th and 8th week after periodontal surgery and the decalcified and undecalcified specimens were prepared for histological and histometrical examination. After the first and the second weeks, gingival recession was more severe in group I than groups II and III. After the fourth and the eighth weeks, there was no difference in the width of junctional epithelium and connective tissue attachment among the three groups, but the width of connective tissue attachment increased in group II at the eighth week, compared to the fourth week. The amount of bone repair in new attachment was significantly greater in group III, compared to groups I and II. New attachment formation was significantly greater in group III, compared to groups I and group II. These results suggest that electrical stimulation using microcurrent generator could be a useful tool for periodontal regenerative therapy in class III furcation defect.

  • Purpose : The purpose of this study is to setup a standard treatment protocol in surgical correction of skeletal malocclusion with Angle Class III patients. We asessed the soft tissue changes according to skeletal changes of patients who have undergone orthognathic surgery with bilateral split sagittal ramus osteotomy (BSSRO) and advancing genioplasty. Materials & Methods : The soft tissue change of 9 skeletal Class III patients was assessed after BSSRO and advancing genioplasty. The patient group was skeletal Class III who was surgically treated by BSSRO & advancing genioplasty. The average follow up period is 13 months with the range of 6 and 30 months. All patients have undergone preoperative and postoperative orthodontic treatment. The assessment was devided into two groups. One was antero-posterior relationship and the other was vertical relationship of dimensional changes of soft tissue after orthognathic surgery. Results : In antero-posterior dimensional changes after surgery, the percentage of soft tissue change in comparison to hard tissue was 89%. Vertical ratio after surgery, 86% soft tissue changes were assessed.

Differences in positions of cone-beam computed tomography landmarks in patients with skeletal Class III facial asymmetry according to midsagittal planes

  • Objective: This study aimed to clarify differences in the positions of cone-beam computed tomography (CBCT) landmarks according to different midsagittal planes (MSPs) in patients with skeletal Class III facial asymmetry. Methods: Pre-treatment CBCT data from 60 patients with skeletal Class III were used. The patients were classified into symmetric (menton deviations of < 2 mm) or asymmetric (menton deviations of > 4 mm) groups. Six MSPs were established based on previous studies, and three-dimensional analyses were performed for the planes in both the groups. The measurement outcomes were compared statistically. Results: A statistically significant interaction (p < 0.01) was observed between MSPs and facial asymmetry. No significant differences were observed among MSPs in the symmetric group. However, significant differences in linear measurements were identified among MSPs in the asymmetric group. Specifically, the upper facial MSP revealed both maxillary and mandibular transverse asymmetries. On the other hand, anterior nasal spine (ANS)-associated MSP could not identify maxillary asymmetry. Furthermore, the menton deviation was approximately 3 mm lower when estimated using the ANS-associated MSP than that using upper facial MSP. Conclusions: The choice of MSP can significantly affect treatment outcomes while diagnosing patients with asymmetry. Therefore, care should be taken when selecting MSP in clinical practice.

한국인 성장기 III급 부정교합의 TTBA 치료 후 상악골 견인 효과 (Maxillary protraction effects of TTBA (Tandem Traction Bow Appliance) therapy in Korean Class III children)

  • 이 논문의 목적은 성장기 골격성 III급 부정교합의 치료를 위하여 새로이 고안된 Tandem Traction Bow Appliance(TTBA)의 상악골 견인 효과를 평가하는 것이다. 이화여자대학교 목동병원 치과 교정과에 내원한 성장기 골격성 III급 부정교합 환자 중 TTBA로 치료받은 88명(소년 42명, 소녀 46명)을 대상으로 하였다. 치료 시작 시 연령은 7세 6개월${\pm}$1년 6개월이었으며, 평균 치료 기간은 13개월${\pm}$3개월이었다. 치료 전과 후의 측모두부방사선사진계측이 동일 조사자에 의해 투사 및 중첩되었으며, modified McNamara analysis과 pitchfork analysis로 분석하였다. 치료 전후의 변화를 알아보기 위하여 paired sample t-test를 실시하였으며 다음과 같은 결론을 얻었다. 상악과 상악치열은 전방으로 이동하였다. 하악은 후방으로 이동하였으나, 이 양은 통계학적으로 유의하지 않았다. 하악치열은 전방으로 이동하였다. Net dental changes와 apical vase change가 복합되어 나타난 결과, III급 부정교합 치료에 유리한 방향으로 total molar relationship correction이 일어났다. Total molar relationship correction에 대한 기여 정도는 net dental movement 26%, apical vase change 74%였다. 이상의 결과는 TTBA 치료가 상악골 결핍을 가진 성장기 골격성 III급 부정교합에서 유의한 상악골 견인 효과를 가진다는 것을 의미한다.

  • The patient, 18-year-old girl, had a class III malocclusion with the lateral compression of the maxilla due to the premature loss of the canines, mild mandibular prognathism, and mouth breathing habit. The treatment plan consisted of 1) rapid maxillary expansion 2) a period of retention 3) extraction of the first mandibular premolars instead of surgical correction 4) completion of orthodontic treatment with multibanded system. The maxilla was separated in the midline by the application of orthopedic forces via a cemented rapid maxillary expansion device. After 18 months, She gained functional overbite-overjet relationship, good interdigitation of buccal segments, and facial harmony due to the retraction of lower anterior teeth.
