객담 전처리 방법에 따른 객담 항산균 도말 및 배양 양성률 비교연구 (Comparison of Smear and Culture Positivity using NaOH Method and NALC-NaOH Method for Sputum Treatment)

  • 연구배경: NaOH (sodium hydroxide) 기법을 이용한 전처리 객담과 NALC-NaOH (N-acetyl-L-cysteine-sodiumhydroxide) 기법을 이용한 객담의 항산균 도말 및 결핵균배양 양성률 그리고 배지 오염률을 비교하여 실험실 검사과정 중의 일부분을 개선함으로써 검사 결과의 개선이 가능한 지 확인하고자 하였다. 방 법: 2007년 6월부터 2008년 6월까지 국립마산병원에서 객담 검사가 시행된 환자를 대상으로 환자에게서 객담배출 요령을 충분히 교육한 후 두개의 객담 검체를 채취하여 통상적인 NaOH 기법과 NALC-NaOH 기법을 각각 적용한 후 도말 및 배양 결과 그리고 배지 오염률을 비교하였다(n=436). 결 과: 항산균 도말 검사에서는 NaOH 기법에 비하여 NALC-NaOH 기법이 다소 높은 양성률(33.0% vs. 39.0%)을 보였으나 통계적인 유의성은 없었다(p=0.078). 결핵균배양 검사에서는 배지 오염률(culture contamination)은 각각 3.2%와 3.0%로 유의한 차이는 보이지 않았다. 배양양성률은 NALC-NaOH 기법이 통계적으로 유의하게 높은 배양 양성률(39.7% vs. 28.0%, p=0.0003)을 보였으며, 배양 결과를 도말 검사 결과와 연관 지어 분석하였을 때 도말 검사상 음성인 경우 NaOH 기법과 NALC-NaOH 기법은 각각 7.2%와 15.8% (p=0.0017), scanty를 보인 객담의 경우 각각 42.9%와 60.8% (p=0.036)의 배양 양성률을 보여 통계적으로 의미 있는 차이를 보였다. 결 론: 도말 검사에서 음성이나 scanty를 보여 상대적으로 결핵균의 수가 적은 객담의 경우 NALC-NaOH를 이용함으로써 배양 양성률을 개선 시킬 수 있었으며 이는 임상적으로 폐결핵의 진단과 치료 경과 관찰에 직접적인 도움을 줄 수 있음을 보여주었다.

일부 농촌지역의 결핵 치료 환자에 대한 실태 조사에 관한 연구

  • This is a study of 21 tuberculosis patients receiving medical treatment at the Public Health Center in Kyongi Do, Pu Chun Gun and at the General Hospital. The results cover the findings of the period from May, 1969 to November 1970. The information obtained is based on personal interviews with the patients, and symptomatic diagnosis made from observations. The following statistics when not equalling 100% contain only the responses of the two extremes in each case. The findings of the research are as follows: 1. 52.3% of the patients in the study are males and 47.7% are females. 28.6% of the subjects are between 20 and 29 years of age and an equal percent are between 30 and 39 years. 2. 47.5% of the subjects had graduated from primary school, while only 4.8% had graduated from high school. 3. 57.1% of the patients said they had no religions beliefs, while 4.8% professed to being Buddhists or believing in superstition. 4. 47.3% of the people said they were unemployed, while 4.8% classified themselves as labourers. 5. In response to how tuberculosis was first detected in their respective cases, 52.6% became aware of their disease through X-ray results, while 4.8% were discovered to have tuberculosis when being treated for other diseases at the hospital. 6. When asked how many of the patients knew anything about their disease when treated, 57.1% knew nothing about tuberculosis when they received treatment, while 42.9% had some knowledge of the disease. 7. Of those who knew something about tuberculosis, 61.9% learned about from doctors and nurses, while 4.8% learned from other people. 8. 57.1% of the patients knew that tuberculosis is a communicable disease, while 42.9% did not know. 9. 52.4% of the patients did not know the cause of tuberculosis while 4.9% believed the disease was caused by a curse. 10. When asked about the extent of treatment, 52.4% responded that they had undergone continuous treatment, while 4.8% had not received treatment. 11.The reasons given for not continuing treatment were the following: economic factors 55.6%; side reactions to the treatment, lack of knowledge of how to get treatment, of the need for treatment, or of the positive effects of treatment 11.1%. 12. 61.9% of the subjects usually took the medical treatment at home, 9.5% took it in the mountains or at the beach. 13. 42.9% of the patients received drugs for treatment at the local public health center, while 4.8% received them at the hospital 14. 33.3% of the patients received P.A.S+I.N.H.+S.M. for treatment of tuberculosis, while 4.8% received P.A.S.+S.M.. and some secondary drug. 15. Of the patients who took some extra medicine for tuberculosis, 38.1% took a Chinese drug, while 9.5% took herb medicine. 16. 38.1% of the patients had continued treatment for three years, 4.8% had interrupted the treatment. 17. When asked about the development of the disease after treatment, the patients gave the following information: after one month, 90.5% thought the treatment helped, while 9.5% weren't sure; after one year, 55.6% thought it was good, while 5.5% thought it was not; after three years, 63.6% had a very bad condition. while 4.8% didn't know. 18. 61.9% of the patients were unconcerned about covering their mouths when they coughed, while 38.1% covered their mouths. 19. 57.2% were unconcerned they spit, while 23.8% spit into a waste basket. 20. 66.7% were unconcerned about sterilizing tableware, while 9.5% handled it separately. 21. 66.7% were unconcerned about ventilating their room, while 9.5% ventilated the room twice a week.

폐 Aspergillosis 의 외과적 치료 (Surgical treatment of pulmonary aspergillosis)

  • Since tuberculosis was a common pulmonary disease in Korea, Aspergillosis was easily misdiagnosed as tuberculosis and an acute form of Aspergillosis was misinterpreted as pneumonia because of their similarities in the X-ray findings. This investigation is designed to illustrate the clinical features and preoperative diagnosis and surgical role in the management of this disease. In a retrospective review of operative cases from Jan. 1963 through Dec. 1983, 36 cases were analyzed. Peak age incidence lies in the 3rd decade [41.7%]. All cases had a history of treatment with antituberculous drugs under diagnosis of pulmonary tuberculosis and the most common chief complaint was hemoptysis [69.5%]. Only nine cases [25%] showed cavitary lesions with mycetoma and preoperative sputum study for fungus showed low positive valve [42.3%]. Anatomical location of lesion was located mainly upper lobe [66.7%] and most of cases were managed by lobectomy. We experienced 7 cases of complication; they were postoperative empyema, hepatic failure, esophageal varix bleeding. Postoperative pathologic findings showed that 29 cases [80.5%] were combined with tuberculosis 3 cases were combined with bronchiectasis and 4 cases were not combined with other disease. In conclusion, when the patient has a longstanding history of pulmonary tuberculosis and has a hemoptysis, he must be suspected fungus super infection. Resectional surgery is the treatment of choice for symptomatic localized disease and needed resection in asymptomatic patient to prevent possible fatal sequelae in the future.

한(韓).양방(洋方) 겸치(兼治)로 치유된 폐결핵 환자 1례 (Clinical Report on One Case of Pulmonary Tuberculosis Using Oriental Medicine with Western Medicine)

  • Objective : This study was designed to evaluate the treatment effects and the prevention of side effects from antituberculosis drugs by using oriental medicine on a pulmonary tuberculosis patient. Methods : The clinical data was analyzed on a patient with pulmonary tuberculosis whose main symptoms were general weakness, anorexia, weight loss, tachycardia, night sweats, fever, coughing and chest pain. The patient was treated from June 17, 2008 to April 1, 2009, using herbal medicine (Cheongpyebyeolgab-tang) with antituberculosis drugs. Results : After treatment, the symptoms disappeared faster than with western medicine treatment only. Laboratory examinations and radiograph impressions also improved. No side effects of the antituberculosis drugs were noted. Conclusions : This suggests that oriental medicine therapy is effective for pulmonary tuberculosis and prevents the side effects of the antituberculosis drugs.

  • Pulmonary hypertension (PH) is a condition of increased blood pressure in the pulmonary arteries and is diagnosed with an increased a mean pulmonary artery pressure ≥25 mm Hg. This condition may be associated with multiple clinical situations. Based on pathophysiological mechanisms, clinical presentation, hemodynamic profiles, and treatment strategies, the patients were classified into five clinical groups. Although there have been major advances in the management of PH, it is still associated with significant morbidity and mortality. The diagnosis and treatment of PH have been performed mainly by following European guidelines, even in Korea because the country lacks localized PH guidelines. European treatment guidelines do not reflect the actual status of Korea. Therefore, the European diagnosis and treatment of PH have not been tailored well to suit the needs of Korean patients with PH. To address this issue, we developed this guideline to facilitate the diagnosis and treatment of PH appropriately in Korea, a country where the consensus for the diagnosis and treatment of PH remains insufficient. This is the first edition of the guidelines for the diagnosis and treatment of PH in Korea, and it is primarily based on the '2015 ESC/ERS Guidelines for the diagnosis and treatment of pulmonary hypertension.' with the acceptance and adaptation of recent publications of PH.

결핵 환자의 초치료 중단위험 사정도구 개발 (The Development of a Scale Assessing the Risk of Discontinuation of Tuberculosis Treatment)

  • Purpose: This study identified the reasons why tuberculosis (TB) patients withhold treatment in a bid to develop a assessment scale to select patients who needs nursing intervention in the early stage and decrease the risk of discontinuation of treatment. Sample: There were two samples. A sample of 191 patients with TB and having primary treatment and a second sample of N who were under re-treatment Methods: The study design included qualitative and quantitative methods. Qualitative data were collected from in-depth interviews of TB patients under re-treatment. The quantitative data were collected from 191 patients with TB under primary treatment. Results: Exploratory factor analysis revealed 11 factors explaining 69.6% of total variance. These factors were categorized into four subgroups. A depression scale was used to establish concurrent validity. The depression scale had a positive relationship (r=54) with the discontinuing of primary treatment. The internal consistency reliability for the four subgroups was over .84. The confidence coefficient was Cronbach's ${\alpha}$ .95. The final scale was a self-reported four Likert scale including 50 items. Conclusion: Reliability and validity was established for the scale and the scale can be used to examine the risk of treatment discontinuation for TB. The scale is an important resource for nursing interventions in identifying and treating high risk clients.
