  • The purpose of this study was to investigate of fishery consciousness for fisheries high school teachers. The data was collected from 61 fisheries high school teachers in Kyungbuk Province. The treatment of materials are classified by level of academic careers, teaching careers. This research evaluated percentage to the frequency of reponses to each question and carried out $\chi^2$ - test, etc. The statistically significant level accounted for p<.05 and made use of SPSS10.0. The conclusion of the study were as follows: Strategic development of fisheries high school on fishery consciousness need to program with fishery characteristic, to promote maritime affairs had better systematic publicity activities. On educational system policy need to keep consistency of occupational education policy, change of titles of school and reconstruction of department, expand of carrier guidance direction. On high self-conceit of Hsheries high school teachers need to many discussion be based program of school itself and academic society. And director and vice-principle station region of islands, etc.

Does Serum Osmolarity Change as a Result of the Reflex Neuroprotective Mechanism of Cerebral Osmo-Regulation after Minor Head Trauma?

  • Objective : It is well known that changes in cerebral hemodynamics occur after traumatic brain injury (TBI). Osmo-regulation in the brain is important for maintaining a constant milieu in the central nervous system. Nevertheless, to our knowledge, early osmolarity changes after minor head injury have not been studied until now. Methods : In this study, serum osmolarity was measured in 99 patients with minor head trauma. As a control group, blood samples were drawn from 99 patients who had a minor trauma in an extremity. Serum osmolarity was estimated using a fully automatic biochemical autoanalyzer within the first 3 hours after the trauma. Results : The mean serum osmolarity levels were $286.08{\pm}10.1\;mOsm/L$ in the study group and $290.94{\pm}5.65\;mOsm/L$ in the control group (p<0.001). However, after age adjustment between the study and control groups, this statistical significance was found to be valid only for patients over 30 years of age. Conclusion : It was noted that serum osmolarity levels decrease in the first 3 hours following minor head trauma in patients over 30 years of age. Further studies into this area could provide guidance for the management/treatment of elderly patients.

  • Purpose: In maxillofacial surgery, proper preoperative diagnosis is very important in achieving good postoperative results. Although conventional CT scans are useful for visual representations of fractures, they cannot provide direct guidance for reconstructing facial bone fractures. However, the recent technology of multislice scanning has brought many clinical benefits to CT images. Direct correlations can be made between preoperative imaging data and operative planning. The aim of the current study is to evaluate the differences between conventional CT and multidetective three-dimensional CT(3D MDCT) measurements in craniofacial deformities. Methods: From January 2005 to November 2005, MDCT scans of 41 patients were evaluated by comparing them with conventional CT scans. The 3D MDCT images were assessed and reviewed by using a simple scoring system. Results: The 3D MDCT scans offered easy interpretation, facilitated surgical planning, and clarified postoperative results in malar complex fractures, mandibular fractures, and extensive maxillofacial fractures and cranioplasty. However, 3D MDCT images were not superior to conventional CT scans in the diagnosis of blowout fractures. Conclusion: In spite of its limitations, the 3D MDCT provided additional and more comprehensive information than the conventional CT for preoperative assessment of craniofacial deformities. Therefore, the 3D MDCT can be a useful tool for diagnosis and systematic treatment planning in craniofacial skeletal deformities.

  • Objective: The purpose of this study was to understand the role of specialists in child studies by analyzing child-related laws and policies and their service systems. Methods: We analyzed the aims, background, history, and related 5-year-plans in child-related laws and policies and their service delivery systems. Results: There were many difficulties in enforcing consistent policies. First, there was no unified age for implementing child and youth policy. Second, there was no comprehensive long-term policy. Third, many departments were involved. Conclusion: The study findings suggest the future role of specialists in child studies. First, they must take steps to introduce a monitoring system for the proper implementation of the first master plan for child policy. Second, they need to put effort into improving treatment of child-related workers. Third, they have to expand the academic area of child studies by reforming university curricula in a more pragmatic way. Fourth, the procedure to acquire different certifications in child studies and youth studies needs to be affiliated.

Comparison of dental information websites in some nations (일부 국가의 치과 웹사이트 비교)

  • Objectives : The purpose of this study was to compare the dental information websites in some nations and to provide the basic data for clinical application. Methods : Two hundred twenty-four dental websites were chosen by using Yahoo search engine. The websites included 59 from the United States, 50 from the United Kingdom, 54 from Canada, and 61 from Korea. The survey was conducted from August 15 to September 30, 2011. Results : The guidance of medical treatment expenses was the most sustainable in the United Kingdom and followed by the United States, Canada, and Korea(p<0.001). Korean dental information website provided the visitor's message. The United Kingdom provided the best type of FormMail and the United States, Canada and Korea also provided(p<0.001). Korean dental information website showed the best types of messages and was followed by the United States, Cadana and the United Kingdom. Korean dental information had the best type of FormMail(p<0.001). Conclusions : In order to make the accurate access to the dental health information website, it is necessary to provide the easy access and continuous research efforts to the information system.

Magnetic Resonance-Guided Focused Ultrasound : Current Status and Future Perspectives in Thermal Ablation and Blood-Brain Barrier Opening

  • Magnetic resonance-guided focused ultrasound (MRgFUS) is an emerging new technology with considerable potential to treat various neurological diseases. With refinement of ultrasound transducer technology and integration with magnetic resonance imaging guidance, transcranial sonication of precise cerebral targets has become a therapeutic option. Intensity is a key determinant of ultrasound effects. High-intensity focused ultrasound can produce targeted lesions via thermal ablation of tissue. MRgFUS-mediated stereotactic ablation is non-invasive, incision-free, and confers immediate therapeutic effects. Since the US Food and Drug Administration approval of MRgFUS in 2016 for unilateral thalamotomy in medication-refractory essential tremor, studies on novel indications such as Parkinson's disease, psychiatric disease, and brain tumors are underway. MRgFUS is also used in the context of blood-brain barrier (BBB) opening at low intensities, in combination with intravenously-administered microbubbles. Preclinical studies show that MRgFUS-mediated BBB opening safely enhances the delivery of targeted chemotherapeutic agents to the brain and improves tumor control as well as survival. In addition, BBB opening has been shown to activate the innate immune system in animal models of Alzheimer's disease. Amyloid plaque clearance and promotion of neurogenesis in these studies suggest that MRgFUS-mediated BBB opening may be a new paradigm for neurodegenerative disease treatment in the future. Here, we review the current status of preclinical and clinical trials of MRgFUS-mediated thermal ablation and BBB opening, described their mechanisms of action, and discuss future prospects.

Effects of Nature-Based Programs for Workers in Korea: A Systematic Review

  • The aim of this study was to review previous literature to determine the effects of nature-based program for workers. This systematic review was conducted in accordance with National Evidence-based Healthcare Collaborating Agency's guidance for undertaking systematic reviews for intervention. Literature search was performed using National Assembly Digital Library, Korean Studies Information Service System, and Korea Education & Research Information Service for literature published until March 2019. The participants were full-time workers, and intervention of nature-based programs was conducted in the outdoor, indoor, and indirect nature contact exposures, with comparators in the control group who did not receive the treatment. The results showed that the programs were effective in physical, psychological, and social health. The methodological quality of randomized controlled trials (RCT) was assessed using the Cochrane Risk of Bias(RoB) tool, while non-randomized controlled trials (N-RCT) were assessed using the Cochrane Risk of bias assessment tool for non-randomized studuies (RoBANS). A total of 16 studies were selected for assessment: two RCTs, 10 N-RCTs, and four one-group pretest-posttest designs. Most interventions were provided at the workplace and in the community. There were many kinds of nature-based interventions, and forest therapy and horticultural therapy programs were most common. Various interventions for workers effectively improved job stress, depression, serum cortisol and stress-response. However, the included studies lacked methodological rigor. Future research is needed to evaluate the long-term effectiveness of nature-based programs for workers using rigorous research designs.

Barriers to Counseling on Advance Directives Based on Counselors' Experiences: Focus Group Interviews

  • Purpose: In Korea, since the Act on Hospice and Palliative Care and Decisions on Life-Sustaining Treatment for Patients at the End of Life was implemented in February 2018, advance directives (ADs) have become legally effective and should be documented after sufficient explanation by a registered counselor. However, little is known regarding the adequacy of current AD counseling. This qualitative study aimed to explore the barriers to AD counseling based on counselors' experiences. Methods: We conducted focus group interviews using purposive sampling. Seven counselors working at hospitals, community health institutions, and non-profit organizations participated in this study. They were asked about the challenges and problems they encountered during AD counseling. Results: Three themes emerged from this study; 1) issues regarding consistency in AD counseling, 2) issues regarding AD counselors' competency and work environment, and 3) issues regarding the adequacy of the service system. The interviewees stated that the lack of a manual for standardized service made AD counseling inconsistent. The limited competency and poor work environment of counselors were pointed out as major barriers. The interviewees also stated that a proper service system considering individual circumstances is absent. Conclusion: The goals of AD counseling should be clarified and guidance should be implemented for providing standardized services. Further efforts to enhance the competency of AD counselors and to improve their working conditions are needed. Establishing an integrated framework for an adequate service system is also essential to overcome systematic barriers to AD counseling.

Sensitivity Analysis of Initial Pressure and Upper Control Limit on the Pressure Decay Test for Membrane Integrity Evaluation (압력손실시험을 이용한 막 완결성 평가에서 초기압력 및 UCL 도출인자 민감도 분석)

  • Recently domestic drinking water industry has recognized membrane-based technology as a promising alternative for water treatment. To ensure successful application of membrane processes, the integrity of membrane systems should be maintained. According to US EPA guidance, the pressure decay test based on the bubble point theory is recommended to detect any membrane defection of which size is close to the smallest diameter of Cryptosporidium oocysts, $3{\mu}m$. Proper implementation of the pressure decay test is greatly affected by initial test pressure, and the interpretation of the test results is associated with upper control limit. This study is conducted to investigate various factors affecting determination of initial test prtessure and upper control limit, including membrane-based parameters such as pore shape correction factor, surface tension and contact angle, and system-based parameters, such as volumetric concentration factor and total volume of system. In this paper, three different hollow fibers were used to perform the pressure decay test. With identical initial test pressure applied, their pressure decay tendency were different from each other. This finding can be explained by the micro-structure disparity of those membranes which is verified by FESEM images of those membranes. More specifically, FESEM images revealed that three hollow fibers have asymmetry, deep finger, shallow finger pore shape, respectively. In addition, sensitivity analysis was conducted on five parameters mentioned above to elucidate their relation to determination of initial test pressure and upper control limit. In case of initial pressure calculation, the pore shape correction factor has the highest value of sensitivity. For upper control limit determination, system factors have greater impact compared to membrane-based parameters.

CYBERKNIFE RADIOSURGERY FOR INOPERABLE RECURRED ORAL CANCER (사이버나이프를 이용한 수술 불가능한 재발성 구강암의 치험례)

  • CyberKnife is a stereotactic radiosurgery system which could be used to treat many tumors and lesions. It provides the surgeon unparalleled flexibility in targeting using a compact light linear accelerator mounted on a robotic arm. Advanced image guidance technology tracks patient and target position during treatment, ensuring accuracy without the use of an invasive head frame. CyberKnife with Dynamic Tracking Software is cleared to provide radiosurgery for lesions anywhere in the body when radiation treatment is indicated. It has often been used to radiosurgically treat otherwise untreatable tumors and malformations. Moreover, this instrument treats tumors at body sites, most of which are unreachable by other stereotactic systems. Compared with conventional radiotherapy, it is fundamentally different that using non-invasive, frameless, no excessive radiation exposure to normal tissue. In oral malignant neoplasm, surgical excision and radiation therapy should be tried first, additionally chemotherapy could be considered. However, after failure of conventional therapies, patients had poor systemic condition and surgical limitation. So, CyberKnife could be a suitable therapy. A 49 years man was referred in recurred mandibular cancer treated by radiotherapy. The tumor was considered inoperable, because of extensive invasion and was not expected to good response to conventional therapies. We experienced a case of CyberKnife after 4 cycle chemotherapies, so we report it with review of literature.
