Recent Findings on the Mechanism of Cisplatin-Induced Renal Cytotoxicity and Therapeutic Potential of Natural Compounds

  • The efficacy and side effects associated with anticancer drugs have attracted an extensive research focus. Onconephrology is an evolving field of nephrology that deals with the study of kidney diseases in cancer patients. Most renal diseases in cancer patients are unique, and management of renal disease can be challenging especially in the presence of continuing use of the nephrotoxic drugs. Cisplatin is one of the most important chemotherapeutic agents used in the treatment of various malignancies, such as head, neck, ovarian, and cervical cancers. The major limitation in the clinical use of cisplatin is its tendency to induce adverse effects, such as nephrotoxicity. Recently, plant-derived phytochemicals have emerged as novel agents providing protection against cisplatin-induced renal cytotoxicity. Owing to the diversity of phytochemicals, they cover a wide spectrum of therapeutic indications in cancer and inflammation and have been a productive source of lead compounds for the development of novel medications. Of these agents, the effectiveness of triterpenoids, isolated from various medicinal plants, against cisplatin-induced renal cytotoxicity has been reported most frequently compared to other phytochemicals. Triterpenes are one of the most numerous and diverse groups of plant natural products. Triterpenes ameliorate cisplatin-induced renal damage through multiple pathways by inhibiting reactive oxygen species, inflammation, down-regulation of the MAPK, apoptosis, and NF-κB signaling pathways and upregulation of Nrf2-mediated antioxidant defense mechanisms. Here, we reviewed recent findings on the natural compounds with protective potential in cisplatin-induced renal cytotoxicity, provided an overview of the protective effects and mechanisms that have been identified to date, and discussed strategies to reduce renal cytotoxicity induced by anticancer drugs.

Immediate Debridement and Reconstruction with a Pectoralis Major Muscle Flap for Poststernotomy Mediastinitis

  • Background : Poststernotomy mediastinitis is a rare, but life-threatening complication, thus early diagnosis and proper management is essential for poststernotomy mediastinitis. The main treatment for mediastinitis is aggressive debridement. Several options exist for reconstruction of defects after debridement. The efficacy of immediate debridement and reconstruction with a pectoralis major muscle flap designed for the defect immediately after the diagnosis of poststernotomy mediastinitis is demonstrated. Methods : Between September 2009 and June 2011, 6 patients were referred to the Department of Plastic and Reconstructive Surgery and the Department of Thoracic and Cardiovascular Surgery of Ajou University Hospital for poststernotomy mediastinitis. All of the patients underwent extensive debridement and reconstruction with pectoralis major muscle flaps, advanced based on the pedicle of the thoracoacromial artery as soon as possible following diagnosis. A retrospective review of the 6 cases was performed to evaluate infection control, postoperative morbidity, and mortality. Results : All patients had complete wound closures and reduced severity of infections based on the erythrocyte sedimentation rate and C-reactive protein levels and a reduction in poststernal fluid collection on computed tomography an average of 6 days postoperatively. A lack of growth of organisms in the wound culture was demonstrated after 3 weeks. There were no major wound morbidities, such as hematomas, but one minor complication required a skin graft caused by skin flap necrosis. No patient expired after definitive surgery. Conclusions : Immediate debridement and reconstruction using a pectoralis major muscle flap is a safe technique for managing infections associated with poststernotomy mediastinitis, and is associated with minimal morbidity and mortality.

Effect of Medicinal Plant Extracts on Mealy Bugs (Maconellicoccus hirsutus Green) Affecting Mulberry

  • Efficacy of aqueous leaf extracts of medicinal plants viz., Eucalyptus globulus, Ocimum sanctum and Piper betle were evaluated against the hatching of eggs, mortality of nymphs and mortality and fecundity of adult mealy bugs (Maconellicoccus hirsutus Green) under in vitro conditions. Totally seven concentrations 1, 5, 10, 20, 50, 70 and 100 percent were tested against mealy bugs. The results revealed that with the increase in the concentration of extracts, there was a corresponding decrease in hatching, nymphal & adult mortality and fecundity in all the extracts tested. The hatching of eggs was minimum at 100 percent concentration in E. globulus (20.00%) followed by O. sanctum (30.42%) and P. betle (36.06%) over control (96.73%). The growth and development of nymphs and adults were adversely affected after the treatment. The nymphs did not attain the adult stage and there was total mortality of nymphs in different treatments. The duration of nymphal stage was reduced by 2-10 days in E. globulus, 1-7 days in O. sanctum and 1-2days P. betle at higher concentrations. However, at lower concentrations it was prolonged by 2-4 days in all the three extracts. At lower concentrations the mortality of adults was on par with the control. Similarly the adult duration was also reduced by 3-4 days at 100 percent concentration and prolonged by 5-6 days at lower concentrations in all the extracts. With the increase in concentrations of the extracts there was decrease in the fecundity of eggs. Thus, the leaf extracts of E. globulus was found to be most effective followed by O. sanctum and P. betle against mealy bugs.

Preliminary Evaluation of Paecilomyces lilacinus HY-4 to Control Tetranychus urticae

  • The virulence against the two-spotted spider mite, Tetranychus urticae Koch, was evaluated with entomopathogenic fungus Paecilomyces lilacinus (Thom) Samson HY-4 which is isolated from coleopteran insect and registered to Korean and US patents. Virulence tests were conducted with conidial suspensions ($1.5{\times}10^6conidia/ml$) of P. lilacinus HY-4 against T. urticae adults and positive results were recorded in laboratory conditions. The spraying device was also developed for the efficient and exact evaluation of treatment. The developed spraying device was named as SD-tower sprayer and its efficacy of spraying conidia was evaluated. The accumulated mortality caused by P. lilacinus HY-4 using SD-tower spray was about 73% at 6 days after inoculation. This suggest that the isolate P. lilacinus HY-4 may be considered as promising for a new approach to prevent adult infestations by T. urticae.

Is Local Anesthesia Necessary in Ketamine Sedation for Pediatric Facial Laceration Repair?: A Double-Blind, Randomized, Controlled Study (소아 안면 열상 봉합을 위한 케타민 진정시 국소 마취가 필요한가: 전향적 무작위대조, 이중맹검 연구)

  • Purpose: The aim of this study was to assess the clinical efficacy of combined treatment with local anesthesia and ketamine procedural sedation for pediatric facial laceration repair in the Emergency Department (ED). Methods: Patients aged 1 to 5 years receiving ketamine for facial laceration repair were prospectively enrolled in a double-blind, randomized, and controlled study at an ED. All patients were to receive intravenous ketamine (2 mg/kg). The local anesthesia group (LA group) received a local anesthetic along with ketamine, whereas the no local anesthesia group (NLA group) received only ketamine. The total time of sedation, the patients' movements and groans, adverse events, and the satisfaction ratings of physicians, nurses, and parents were recorded. Results: A total of 186 patients were randomized (NLA group: 90, LA group: 96). The total time of sedation (30.5 minutes for the NLA group, 32.6 minutes for the LA group; p=0.660), patients' groans (26 (28.9%) versus 23 (24.0%); 0.446) and movements (27 (30%) versus 35 (36.5%); p=0.350) was not affected by the addition of local anesthesia. Other adverse events were similar between the two groups. Also, the satisfaction ratings of physicians (median 4 for the NLA group versus 4 for the LA group (p=0.796)), nurses (2 versus 2.5 (p=0.400)), and parents (4 versus 4 (p=0.199)) were equivalent between the two groups. Conclusion: In this study, we found that local anesthesia was not required along with ketamine sedation for pediatric facial laceration repair.

Therapeutic Effect of Dioctahedral Smectite on Diarrhea Caused by E. coli and Salmonella in Piglets (E. coli와 Salmonella에 의한 자돈설사에 있어서 Dioctahedral Smectite의 치료효과)

  • The objectives in the present study were to evaluate the therapeutic effect of dioctahedral smectite (smectite) against piglet diarrhea caused by E. coli and Salmonella. Of this study, twenty piglets (body weight, $6.60{\pm}0.34\;kg$) with diarrhea were used to estimate the efficacy of smectite on piglet diarrhea with 20% smectite suspension in PBS. Pigs were administered with 1 ml smectite suspension three times a day after feeding, and fecal samples were collected at the gate of treatment and on 1st, 2nd, 3rd, and 4th day after administration. Changes in body weight, diarrhea index, and shedding of E. coli and Salmonella were monitored during the experimental period. The group treated with smectite reduced diarrhea index (p < 0.01), and increased body weight compared to control group. In the treated group, the number of E. coli and Salmonella was significantly decreased compared to each control group (p < 0.01). From results of the present study, 20% smectite suspension had therapeutic effect on diarrhea caused by E. coli and Salmonella in piglets.

Enhanced Vasorelaxation of Banhabackchulchunma-Tang and Involved Mechanism (반하백출천마탕(半夏白朮天麻湯)의 조성에 따른 혈관이완활성과 기전)

  • This study was designed to potentiate the vasodilation effect of Banhabackchulchunma-Tang(BCT) prescription by change of mixture. Six different BCT compositions were made according to mixture of herbs. The vascular relaxation effects of 6 different BCT compositions were examined on phenylephrine(PE)-precontracted rat thoracic aorta. The BCT-1 composition exerted significant relaxation on phenylephrine- or KCI- contracted rat thoracic aorta. Its elaxation was endothelium- independent in both PE- and KCl-induced contraction. Treatment of glibenclamide or tetraethylammonium(TEA) did not affect the relaxation of BCT-1. Vasorelaxation efficacy of BCT-1 was also not influenced by low (25mM) or high (50mM, 80mM) KCl-induced contraction. Furthermore, the contraction by increasing $Ca^{2+}$ concentrations (0.3-10.0mM) to a $Ca^{2+}$-free high K+ (60mM) was significantly reduced by pretreatment with BCT-1 In addition, the relaxant effects were not inhibited by pretreatment of rat aorta with L-NAME, MB, indomethacin and atropine. These results confirm that BCT-1 may exerts its vasodilation effect by endothelium-independent manner. According to the above results, we suggest that the relaxation effect of BCT-1 is endothelium-independent and is related with block of $Ca^{2+}$ influx via $Ca^{2+}$ channel.

Antifungal Effects of Cinamon Ramulus, Pulsatillae Radix, Dictamni Radicis Cortex, Paeoniae Radix, Arecae Semen, Artemisiae Capillaries Herba against Candida albicans (Candida albicans에 대한 계지(桂枝), 백두옹(白頭翁), 백선피(白鮮皮), 백작약(白芍藥), 빈낭, 인진(茵蔯)의 항진균효과)

  • The purpose of this thesis is to dedicate to get the objectivity of herbal medicine treatments by choosing herbs likely to work as antifungal agents against Candida albicans which is the causes of Candida vaginitis, making experiments on them and getting the significant results. Each herb's efficacy on control the number of Candida vaginitis is noticed by using Disk Susceptibility test with six herbs medicine and Broth dilution assay of the culture. Cinamon Ramulus, Pulsatillae Radix, Dictamni Radicis Cortex, Paeoniae Radix, Arecae Semen, Artemisiae Capillaries Herba extracts experimented on the efficiency of antifungal activities against Candida albicans by disk susceptibility test. Pulsatillae Radix has the strongest efficiency. In Dictamni Radicis Cortex never appeared antifungal effect on treatment. MIC (minimal inhibition concentration) in vitro antifungal agents of Cinamon Ramulus, Pulsatillae Radix, Paeoniae Radix, Artemisiae Capillaries Herba extracts against Candida albicans was determined by broth dilution assay. MIC against Candida albicans is that Pulsatillae Radix is 2.5 mg/ml and Cinamon Ramulus is 5 mg/ml and Paeoniae Radix is 10 mg/ml and Artemisiae Capillaries Herba is 40 mg/ml.

Effects of Gamicheungpyehwadam-tang on Immune-cell Regulation in Association with Bronchial Asthma in OVA-induced Mouse Model (가미청폐화담탕이 천식 유발 병태 모델에서 천식 관련 활성 면역세포에 미치는 영향)

  • These studies were investigated the effects of Gamicheungpyehwadam-tang (CPHDT) on immune-cell regulation in association with bronchial asthma in OVA-induced mouse model. The administration of 400 mg/kg CPHDT significantly reduced the number of total cells in lung, peripheral lymph node and spleen in OVA-induced bronchial asthma mouse model. The administration of 400 mg/kg CPHDT significantly reduced $CD3^+,{\;}CD19^+$and $CD3^+,{\;}CD69^+$ cell numbers separated from lung, peripheral lymph node and spleen in OVA-induced bronchial asthma mouse model. CPHDT significantly reduced $CD3^+/CCR3^+,{\;}CD4^+,{\;}B220^+/IgE^+$, and $CD3^+/DX5^+$ cell numbers separated from lung, peripheral lymph node and spleen in OVA-induced bronchial asthma mouse model in a dose dependent manner, However, CPHDT significantly reduced $CD8^+$ cell numbers from only lung and spleen. The administration of CPHDT significantly reduced $NK^+$ cell numbers separated from lung of OVA-induced bronchial asthma mouse model in all concentrations, but 200 mg/kg CPHDT reduced $NK^+$ cell numbers separated from peripheral lymph node. These results suggest that CPHDT has anti-asthma and anti-allergy effects. In addition to, CPHDT may be useful treatment of asthma based on the further studies about the individual efficacy search of the components of CPHDT and the adding of variety drugs to CPHDT.

  • The purpose of this thesis is to dedicate to get the objectivity of herbal medicine treatments by choosing herbs likely to work as antibacterial agents, Moutan Cortex Radicis, Caesalpiniae Lignum, Houttuyniae Herba, Forsythiae Fructus, Prunellae Herba, Scrophulariae Radix against Gardnerella vaginalis which is associated with Bacterial vaginosis, making experiments on them and getting the significant results. Each herb's efficacy on control the number of Bacterial vaginosis is noticed by using Disk Susceptibility test with six herbs medicine and Broth dilution assay of the culture. Disk Suseptibility Test : The efficiency strength is as follows in a row : Caesaipiniae Lignum, Moutan Cortex Radicis, Prunellae Herba, Forsythiae Fructus, Houttuyniae Herba, Scrophulariae Radix. MIC Prunellae Herba is 1.5 mg/ml, Caesalpiniae Lignum is 5 mg/ml, Forsythiae Fructus and Moutan Corex Radicis is 10mg/ml for G. vaginalis. As a results, herbmed which tested in this study have an antibacterial effects against G. vaginalis and that may be used for treament of Bacterial vaginosis. Assistant use of external medicine to p.o medicine is expected to bring the good effect of treatment which is used for the origin of this disease.
