거북목을 가진 사람에게 메켄지 폄운동과 어깨안정화운동이 앞쪽머리자세에 미치는 영향 (Effect of Shoulder stabilization exercise and McKenzie Exercise on Forward Head Posture)

  • 본 연구의 목적은 거북목을 가진 사람에게 메켄지 폄운동과 어깨안정화운동이 앞쪽머리자세에 미치는 영향에 대해 알아보는 것이다. 연구대상자는 30명의 거북목을 가진 평범하고 건강한 사람들로 이루어져 있다. 대상자는 어깨안정화운동과 메켄지 폄운동으로 나누어(인원=30, 남자 15명, 여자 15명) 무작위로 나누어 6주 동안 실시되었다. 본 연구는 마름근, 아래등세모근의 운동프로그램과 깊은목굽힘근, 위등세모근, 어깨올림근의 신장프로그램을 통한 목뼈의 머리척추각(Cervical Vertebra Angle) 값과 각 근육별 근전도의 차이를 알아보았다. 앞쪽머리자세와 경우에는 목과 등근육의 활성도가 감소하고 피로도는 증가함을 보였다. 이러한 결과는 목과 머리의 불안정한 상태를 안정시키기 위해서는 위등세모근, 깊은목굽힘근, 어깨올림근의 신장과 마름근, 아래등세모근의 운동을 통해 거북목의 개선에 긍정적인 변화를 줄 것으로 보인다. 이러한 근육들의 불균형한 수축과 과도한 긴장으로 근육의 경직과 피로가 누적되며 이러한 상태가 계속 유지되게 되면 만성화상태가 되어 근육의 불균형으로 인해 통증을 유발할 수 있으며, 피로의 누적으로 인하여 자극에 대한 반응과 근력을 약화시켜서 목 디스크가 발생할 확률이 높아질 것으로 사료된다.

목 디스크 환자에게 그라스톤을 이용한 기계적 자극 시 동일 척수 분절의 통증 유발점 압통 역치 및 근 긴장도에 미치는 영향 (The Effects of Mechanical Stimulation using Graston on Changing Trigger Point Pressure Pain Threshold and Muscle Tone of the Same Spinal Segment in Neck Disk Patient)

  • 본 연구의 목적은 목 디스크 환자에게 기계적 자극을 적용하여 피부 감각의 변화를 유도하고 동일한 척수분절의 통증 역치와 근 긴장도 전-후 변화를 확인하고자 하였다. 이에 목 디스크 질환을 가지고 있는 남성 10명, 여성 20명으로 구성된 대상자를 총 30명 선정하고 실험을 진행하였다. 기계적 자극 그룹은 Graston 장비를 이용하여 5분간 피부 감각 변화를 유도하였고, 대조군은 아무런 조치를 받지 않았다. 피부 감각 변화는 Von Frey filament를 통해 검사를 진행하였고, 압통 역치의 변화는 압통 역치 측정기를 통해 측정하였고, 근 긴장도의 변화는 Myotone pro를 사용하여 측정하였다. 기계적 자극 적용 시 피부 감각의 역치는 통계적으로 유의하게 증가하였다(p<0.05). 동일한 척수 분절의 신경 지배를 받는 오른쪽 가시아래근의 압통 역치와 근 긴장도는 통계적으로 유의하게 증가하였고(p<0.05), 오른쪽 등세모근의 압통 역치는 통계적으로 유의하게 감소하였다(p<0.05). 하지만 다른 근육들의 압통 역치와 근 긴장도는 통계적으로 유의한 변화가 나타나지 않았다. 또한, 대조군에서는 피부 감각, 압통 역치, 근 긴장도의 변화가 통계적으로 유의하게 나타나지 않았다. 결과적으로 기계적 자극에 의해 유도된 피부 감각 변화는 동일 척수 분절의 압통 역치와 근 긴장도의 변화시켰다. 따라서 동일 척수 분절에 대한 기계적 자극의 적용을 통해 목 디스크 환자의 압통 역치, 근 긴장도의 증가와 통증 조절을 위한 새로운 재활 방법으로 임상적 의의가 있다고 판단된다.

로잉운동 시 상지 운동궤적에 따른 근육활성 전략에 관한 연구 (Study on the Strategy of Muscular Activity for Motor Track of Upper Limbs during Rowing Exercise)

  • 본 논문에서는 로잉 운동 시 팔의 운동 궤적별 상 하지 근육활성화와 근력변화를 고찰하였다. 피험자는 로잉운동이나 운동관련 상해경험이 없는 20대 남성 20명을 대상으로 진행하였다. 피험자들은 기존의 운동궤적으로 운동을 하는 대조군과 새로운 운동궤적으로 운동을 하는 훈련으로 각각 10명씩 나누어 실험을 진행하였다. 운동은 총 8주간 주 3일 운동을 실시하였다. 운동 중 실시간 근전도를 측정하였으며 매주 1회 관절토크를 측정하였다. 실시간 근전도 결과 운동궤적에 따라 서로 다른 근육활성개시구간을 나타냈다. 이는 운동궤적에 따라 운동추진력을 다른 근육에서 활성화 하는 것을 의미하며, 관절토크 변화에서도 운동궤적에 따라 다른 변화를 나타냈다. 직선궤적운동에서는 상지 보다는 하지의 토크 변화가 컸으며 이는 운동 추진력을 하지 위주로 발생되는 것을 의미한다. 타원궤적운동에서는 상지, 요추, 하지 모두에서 큰 토크 변화가 나타났으며 이는 운동 추진력을 단관절이 아닌 다관절에서 발생되는 것으로 판단된다.

저작근 및 경부근에서 압력통각역치와 기능시 근활성도 간의 관계 (Relation between Pressure Pain Threshold and Functional Activity in Masticatory and Cervical Muscles)

  • This study aimed at evaluating tile relationship between pressure pain threshold(PPT) and electromyographic(EMG) activity in 30 healthy volunteers and 30 patients with temporomandibular disorders. PPTs were determined with electric pressure algometry over masseter, temporalis anterior, sternocleidomastoid, and trapezius muscle during resting and clenching. To obtain more reliable result, two examiners measured PPTs two times on each muscles, randomly. Resting and clenching EMG activity and tenderness of all muscles were measured to evaluate the relationship with PPTs. The collected data were processed by SAS/STAT program. The obtained results were as follows : 1. Pressure pain thresholds in tendered muscles were lower than those in non-tendered muscles. This tendency was more prominant in masticatory muscles than in cervical muscles and in clenching than in resting. 2. There ware a tendency of higher PPTs in unaffected side than in affected side, but there were no significant difference statistically. 3. There wert no difference of PPTs between in preferred side and in ipsilateral side, significantly. 4. There was a significantly positive correlation between resting and clenching PPTs, but there were no correlation between PPTs and EMG activities. 5. Comparison of EMG activity between in higher group and in lower group by resting PPTs didn't show any significant difference. But resting EMG activity in the higher group of clenching PPTs were lower, on the contrary, clenching activity were higher than those in lower groups of clenching PPTs. From this results, the author concluded that the clenching PPTs was more important than resting PPTs in diagnosis of muscle dysfunction. And the author recommanded that all of three following test were performed for the proper diagnosis and treatment of orofacial Pain : muscle tenderness, EMG activity, and PPT.

두부자세에 따른 근활성과 측모두부방사선계측치의 변화에 관한 연구 (Changes of Muscle Activity and Cephalometric Variables Related to Head Posture)

  • This study was performed to investigate the factors affecting muscle activity and cephalometric variables according to change of head postures. For this study, 150 patients with temporomandibular disorders and 80 dental students without any signs and symptoms of temporomandibular disorders were selected as the patients group and as the normal group, respectively. Head position to body-midline in frontal plane and upper quarter posture to body plumb line in sagittal plane were observed clinically and electromyographic(EMG) activity of anterior temporalis, masseter, sternocleidomastoideus, and trapezius on clenching were recorded with $BioEMG^{(R)}$ in four head postures, which were natural head posture(NHP), forward head posture(FHP), $20^{\circ}$ upward head posture(UHP), and $20^{\circ}$ downward head posture(DHP). Cephaloradiographs were also taken in the same head postures as in EMG taking, but that was taken only in NHP for the patient group. Cephalometric variables measured were SN angle, CVT angle, atlas inclination angle, occlusal plane angle, Me-C2 angle, pharyngeal width, occiput~axis distance, area of pharyngeal space, and cervical curvature. The data were analyzed by SAS statistical program. The results of this study were as follows : 1. Between the patient and the normal group, there were significant difference in distance from plumb line to acromion, eye-tragus angle, electromyographic activity of the four muscles, and cephalometric variables of linear measurement. 2. There was no consistent pattern of correlation between upper quarter posture, EMG activity and cephalometric variables in any case without relation to cervical curvature and head position in frontal plane. 3. Sternocleidomastoid muscle only showed variation of electromyographic activty with changes of head postures, but all the muscles did show correlation with head postures. 4. All the cephalometric variables measured in this study showed difference of mean value by head posture, and CVT angle, pharyngeal width, occiput-atlas distance, and area of pharyngeal space showed correlation between these variables with change from NHP to FHP, and from NHP to UHP.

The Effect of Visual Feedback of Head Angles With Using a Mobile Posture-Aware System on Craniocervical Angle and Neck and Shoulder Muscles Fatigue During Watching the Smartphone

  • Purpose: To find the effect of visual feedback of head angle with using a mobile posture-aware system on craniocervical angle and neck and shoulder muscles fatigue for preventing or decreasing the forward head posture. Methods: Twenty-four healthy young adults in Chungbuk to participate in this study. The subjects started to watch a movie clip for 10 minutes with visual feedback in $0^{\circ}$, $30^{\circ}$, and $60^{\circ}$ of head angles. During the task, surface electromyography (EMG) was used to collect data from the upper trapezius (UT), sternocleidomasetoid muscle (SCM), cervical erecter spinae (CES) during watching the smartphone. Craniocervical angles were measured using a sagittal-view photograph of the subject in a sitting posture. A one-way repeated analysis of variance with a significant level of 0.05 used for statistical analysis. Results: Craniocervical angle with $0^{\circ}$ visual feedback was significantly greater than $30^{\circ}$ and $60^{\circ}$. Craniocervical angle with $30^{\circ}$ visual feedback was significantly greater than $60^{\circ}$. In addition, MDF of UT muscles in $0^{\circ}$ and $30^{\circ}$ of visual feedback was significantly greater than $60^{\circ}$. Conclusion: We concluded that $0^{\circ}$ visual feedback of head angle with using a mobile posture-aware system would be beneficial to prevent or decrease forward head posture during watching a smartphone. We also could recommend using of $30^{\circ}$ visual feedback in case of caring UT muscle fatigue primarily.

Reproducibility of Electromyography Signal Amplitude during Repetitive Dynamic Contraction

  • Objective: The aim of this study is to evaluate the fluctuation of signal amplitude during repetitive dynamic contraction based on surface electromyography(EMG). Background: The most previous studies were considered isometric muscle contraction and they were difference to smoothing window length by moving average filter. In practical, the human movement is dynamic state. Dynamic EMG signal which indicated as the nonstationary pattern should be analyzed differently compared with the static EMG signal. Method: Ten male subjects participated in this experiment, and EMG signal was recorded by biceps brachii, anterior/posterior deltoid, and upper/lower trapezius muscles. The subject was performed to repetitive right horizontal lifting task during ten cycles. This study was considered three independent variables(muscle, amplitude processing technique, and smoothing window length) as the within-subject experimental design. This study was estimated muscular activation by means of the linear envelope technique(LE). The dependent variable was set coefficient of variation(CV) of LE for each cycle. Results: The ANOVA results showed that the main and interaction effects between the amplitude processing technique and smoothing window length were significant difference. The CV value of peak LE was higher than mean LE. According to increase the smoothing window length, this study shows that the CV trend of peak LE was decreased. However, the CV of mean LE was analyzed constant fluctuation trend regardless of the smoothing window length. Conclusion: Based on these results, we expected that using the mean LE and 300ms window length increased reproducibility and signal noise ratio during repetitive dynamic muscle contraction. Application: These results can be used to provide fundamental information for repetitive dynamic EMG signal processing.

근막통증증후군 환자에게 통증점 체외충격파 치료가 경추의 가동 범위, 통증, 근육의 기계적 특성에 미치는 영향 (Effects of Extracorporeal Shock Wave Therapy in Pain Point on Range of Motion, Pain and Mechanical Muscle Properties in Myofascial Pain Syndrome)

  • Background: To evaluate whether extracorporeal shock wave therapy (ESWT) in the pain point is a more effective treatment than the trigger point for myofascial pain syndrome (MPS) of the upper trapezius. Objects: The purpose of this study was to compare the most effective areas when applying extracorporeal shock wave therapy. Methods: A total of 30 patients with MPS were randomly assigned to the trigger point in the ESWT (n = 15) and pain point ESWT (n = 15) groups. Interventions in both groups were performed in one session, i.e., 2,000 shocks with 1.5 bar intensity. Pain and function were assessed using the visual analog scale (VAS) and cervical range of motion (ROM) and based on mechanical muscle properties. Statistical analysis was performed using the repeated measures two-way analysis of variance to determine the significance probability between pre- and post-test. Results: Changes in mechanical muscle properties were not statistically significant between the two groups. However, VAS and cervical ROM showed statistically significant differences at pre- and post-intervention, regardless of the group (p < 0.05). Conclusion: Although no significant difference was observed in the intervention effect, applying an extracorporeal shock wave to the pain point rather than the pain trigger point should be considered in order to save time in effectively and accurately identifying the pain trigger point and site.

Effect of 4 Weeks of Pilates Exercise in Women using Light LED EMG Feedback System on Posture and Balance; Randomized Controlled Trial

  • Objective: This study aimed to assess the impact of using the LED light electromyographyfeedback system (EMG-light) during Pilates exercises in women to maintain consistent muscle contraction in the abdominal external oblique muscles and reduce muscle contraction in the upper trapezius muscles. This study compared Pilates training using the EMG-light with Pilates training only in healthy women for 4 weeks. Design: This study was conducted as a cross-sectional study. Methods: A total of 17 healthy women were divided into an experimental group (n=9) and a control group (n=8). Both groups performed Pilates exercises as assigned, twice per week for four weeks, with each session lasting fifty minutes. The experimental group were used the EMG-light feedback system during pilates exercise while the control group did not use EMG-light. We used cervical vertebral angle (CVA), shoulder tilt angle to evaluate neck posture and standing balance with closed eye for 30s before and after exercise. Results: The control group exhibited a significant change in CVA (p<0.05). Both groups showed significant changes in shoulder tilt, center of pressure (COP) path-length, and COP velocity during eyes-closed conditions (p<0.05). However, there were significant differences between the experimental and control groups in terms of CVA, shoulder tilt, COP path-length, and COP velocity. Conclusions: This study demonstrated that Pilates exercises had positive effects on shoulder posture and balance. The use of EMG-light provided real-time visual feedback on muscle contraction during Pilates exercise. However, the experimental group did not show significant improvements compared to the control group, which performed Pilates exercises without feedback.

경부안정화 운동이 민간 경비원의 목통증, 경부장애지수, 관절가동범위에 미치는 효과 (Effects of Cervical Spinal Stabilization Training in Private Security on Chronic Neck Pain and Cervical Function, Neck Pain, ROM)

  • 이 연구는 만성 경부통을 가진 민간 경비원들을 대상으로 경부 안정화 운동을 실시하여, 목통증, 경부장애지수, 관절가동범위, 근육통증, 재위치감각 변화에 미치는 영향을 비교 분석 하고자 실시하였다. 연구 대상자는 경기 지역 민간경비업체에 종사하는 직원 중 만성 경부통을 호소하는 21명을 대상으로 경부 안정화 운동그룹, 자세교정 운동그룹으로 무선 할당하여, 주3회, 8주간 운동을 실시하였다. 실험 전 후 설문지를 이용하여 목통증과 경부장애지수를 조사하였으며, 관절가동범위, 근육통증, 재위치감각 검사를 실시하여 실험 전 후 변화된 값을 비교 측정하여 다음과 같은 결과를 얻었다. 통증강도와 경부장애지수는 경부 안정화 운동그룹이 실험 후 유의하게 감소하였으며(p<0.05), 관절가동범위, 근육통증, 재위치감각은 경부 안정화 운동그룹에서 실험 후 유의한 향상이 있었으나, 자세교정 운동그룹은 유의한 차이가 없는 것으로 나타났으며(p>0.05), 그룹 간 비교에서도 경부 안정화 운동그룹이 통증, 경부장애지수, 관절가동범위, 근육통증, 재위치감각에서 유의한 향상이 있었다(p<0.05). 본 연구 결과를 볼 때, 만성 경부통을 호소하는 민간 경비원들을 대상으로 경장근과 다열근과 같은 심부근육 강화에 초점을 둔 경부안정화 운동프로그램이 일반적인 자세교정 운동프로그램보다 통증 및 목 기능개선에 효과적인 것으로 나타났다. 따라서 경부 안정화 운동이 만성 경부통을 가진 민간 경비원들의 목 통증 감소와 기능 회복에 효과적이라고 할 수 있겠다.
