The endoscopic assisted approach for the treatment of condylar fracture is a less invasive alternative treatment modality and is considered to be able to overcome the limited access to the operation field to obtain an accurate reduction and fixation. Six patients with condylar neck and subcondylar fracture underwent the endoscopic assisted open reduction and internal fixation through the transoral approach at the Department of Oral and Maxillofacial Surgery, Seoul National University Dental Hospital. The endoscope was inserted through an intraoral incision and the reduction of fracture fragment was performed via a transbuccal approach with two transcutaneous stab incisions. Five patients showed anatomic reduction without any complications. One patient, whose fracture site was fixed with a single plate, showed displacement of fractured condylar segment during the follow up period. No patient had any facial nerve damage.
Purpose: The purpose of this study was to compare and analyze the effects of gargling using normal saline or essential oil on oral bacterial colonization of the subjects who did transoral endotracheal intubation for general anesthesia. Methods: A repeated measures, non-equivalent control group pretest-posttest design was used in this study. The subjects of the study included 58 people; the gargling group with normal saline (n=19); the gargling group with essential oil (n=20); and the control group (n=19). Data were collected from University hospitals in a Korean province from August 13-31, 2012. The collected data were analyzed with $X^2$-test, t-test, ANOVA and Scheff$\acute{e}$ test using SPSS 19.0. Results: Although statistically significant differences among the three groups did not appear in the change of the aerobic bacterial colony before and after the experiments, the aerobic bacterial colony of the gargling group with normal saline ($71.58{\pm}143.39$) and the group with essential oil ($6.95{\pm}332.07$) have increased less compared to the control group ($145.42{\pm}385.01$). The change of the anaerobic bacterial colony before and after the experiments, the control group was ($167.58{\pm}483.58$) and the gargling group with essential oil was ($169.70{\pm}291.60$) and increased, while the gargling group with normal saline ($-42{\pm}331.09$) reduced, but statistically significant differences have not appeared. Conclusion: These findings indicated that oral gargling with normal saline is effective in reducing anaerobic bacterial colonization.

Neurofibroma is characterized as a benign, slow growing neoplasm, originating from Schwann cells or fibroblast in peripheral nerve sheaths. It may appear as a solitary tumor or have multiple localizations in von Recklinghausen disease. They are commonly found in the gastrointestinal tract and laryngeal neurofibromas are extremely rare, accounting for only 0.03 to 0.1% of benign tumors of the larynx. The aryepiglottic fold and arytenoid are the common site of occurrence for laryngeal neurofibroma, because the branch of the superior laryngeal nerve is involved. We present a case of solitary plexiform neurofibroma arising from the laryngeal surface of epiglottis in a 55-year old female who found the lesion incidentally. We removed the tumor completely by transoral laser surgery and no recurrence was found after 7 months. The case of solitary neurofibroma arising from laryngeal surface of epiglottis has not been reported in Korea. We report this case regarding the diagnosis and treatment with review of literatures.

In Fibrous dysplasia(FD) of the jaws, the majority of cases can await the cessation of growth before surgical intervention, and it seems prudent to delay surgery whenever possible until growth has ceased. In craniofacial FD, however, the dangers of dystopia, dystopia and loss of vision may require early surgery to prevent or control cranio-orbital complications. Delaying surgery in those circumstances may be significantly detrimental to such patients. Conservative surgical management of FD is widely practised and we advocate an extension to this conservative treatment by combining surgical recontouring with appropriate osteotomies if indicated, to achieve an optimal esthetic and functional results in craniofacial FD. One case will be presented to illustrate the feasiblility of such combined treatment, to report the uneventful healing of osteotomies in the FD of the jaws, and to demonstrate the use of titanium miniplate fixation in dysplastic bone. The other case had expansile disease of the left facial and fronto-temporal bones and osteolytic change left mandible. This patient complained of severe spontaneous bleeding of left mandibular premolar area and it was suspected as central hemangioma of the left mandible and craniofacial FD. Angiogram disclosed generalized dilation of the external carotid artery and its branches, especially terminal branches of the left facial and inferior alveolar arteries. But no specific abnormalities, such as A-V shunt, venous lake, or early venous drainage, was seen. So it was diagnosed craniofacial FD with hypercellularity and generalized bony recontouring was performed via coronal and transoral approaches.

Angioleiomyoma is benign smooth muscle tumor originating from the vascular wall. While they can occur in various anatomical locations, they are rarely reported in the vallecula region of the oropharynx. We present a case of a 58-year-old female patient with a five-year history of progressive dysphagia and throat discomfort. Laryngoscopy revealed a large, soft, mobile mass located on the right side of the vallecula. Radiological imaging further characterized the lesion as a well-circumscribed, heterogeneous mass. Surgical intervention in the form of Transoral Videolaryngoscopic Surgery (TOVS) was performed, leading to the successful removal of the mass. Histopathological analysis confirmed the diagnosis of angioleiomyoma.
Purpose: Fibrillar collagens like type I collagen, are the major constituent of the extracellular matrix and structural protein of bone. Also, it can be a scaffold for osteoblast migration. The purpose of this study is to estimate the effects of absorbable atelo-collagen sponge (Teruplug$^{(R)}$, Terumo biomaterials Co., Tokyo, Japan) insertion in tooth extraction sites on periodontal healing of the second molar, healing of the fractured mandibular bone and new bone formation of third molar socket after the extraction of the impacted third molar with mandibular angle fracture. Methods: In our study of six cases of mandibular angle fractures, all of them underwent the extraction of the third molar tooth & absorbable atelo-collagen sponge insertion in tooth extraction site. Three of them had a intraoral infection & oral opening to fracture site, two of the six had dental caries, and only one had reduction problem due to third molar position. Six consecutive patients with noncomminuted fractures of the mandibular angle were treated by open reduction and internal fixation using one noncompression miniplates and screws placed through a transoral incision. Results: All of the patients have showed good postoperative functions and have not experienced complications requiring second surgical intervention. There was well healing of the mandibular bone and the most new bone formation of third molar socket after the extraction of the impacted third molar with mandibular angle fracture. Conclusion: The results of this study suggest that absorbable atelo-collagen sponge is relatively favorable bone void filler with prevention of tissue collapse, food packing, and enhance periodontal healing. Thus, the use of atelo-collagen sponge and one noncompression miniplate seems to be relatively easy, safe, and effective for the treatment of fractures of the mandibular angle and third molar extraction.
