내장 신경 차단후 생긴 일시적 편측 하지마비 (Transient Unilateral Paraplegia after Splanchnic Nerve Block)

  • Two cases are described transient unilateral paraplegia following a splanchnic nerve block (SNB) among 683 procedures in 708 patients from 1968 to 1992. The first case, 64 year-old male, had bilteral splanchnic nerve block twice with pure alcohol 4 months and 2month ago. The paraparesis of right leg was developed 4 minutes after the pure alcohol injection during the third SNB and was completely recovered after 37 minutes. The second case, 60 year-old male, had also a first SNB with temporary relief of pain for a week. The Second SNB with 95% alcohol was followed by paraplegia of left leg. He showed almost complete improvement but expired 18 days later. The probable cause was the spread of alcohol onto the lumbar plexus or paravertebral space unilaterally, because of adhesion or paravertebral space due to previous alcohol injections.

Transient paraplegia after neurolytic splanchnic block in a patient with metastatic colon carcinoma

  • We present a patient with metastatic colon carcinoma who developed paraplegia following a neurolytic splanchnic block. A 41-year old man with metastatic adenocarcinoma of the colon received a splanchnic neurolytic block using alcohol because of severe abdominal pain. Bilateral motor weakness and a sensorial deficit in both legs developed after the procedure. Diffusion magnetic resonance imaging revealed spinal cord ischemia between T8 and L1. The motor and sensorial deficits were almost completely resolved at the end of the third month. We think that anterior spinal artery syndrome due to reversible spasms of the lumbar radicular arteries using alcohol have resulted in transient paraplegia. The retrograde spread of alcohol to neural structures may have also contributed.

Mid-Term Results of Thoracic Endovascular Aortic Repair for Complicated Acute Type B Aortic Dissection at a Single Center

  • Background: Complicated acute type B aortic dissection is a life-threatening condition with high morbidity and mortality. The aim of this study was to report a single-center experience with endovascular stent-graft repair of acute type B dissection of the thoracic aorta and to evaluate the mid-term outcomes. Methods: We reviewed 18 patients treated for complicated acute type B aortic dissection by thoracic endovascular aortic repair (TEVAR) from September 2011 to July 2017. The indications for surgery included rupture, impending rupture, limb ischemia, visceral malperfusion, and paraplegia. The median follow-up was 34.50 months (range, 12-80 months). Results: The median interval from aortic dissection to TEVAR was 5.50 days (range, 0-32 days). There was no in-hospital mortality. All cases of malperfusion improved except for 1 patient. The morbidities included endoleak in 2 patients (11.1%), stroke in 3 patients (16.7%), pneumonia in 2 patients (11.1%), transient ischemia of the left arm in 1 patient (5.6%), and temporary visceral ischemia in 1 patient (5.6%). Postoperative computed tomography angiography at 1 year showed complete thrombosis of the false lumen in 15 patients (83.3%). Conclusion: TEVAR of complicated type B aortic dissection with a stent-graft was effective, with a low morbidity and mortality rate.

Proteomic Changes in Chick Brain Proteome Post Treatment with Lathyrus Sativus Neurotoxin, β-N-Oxalyl-L-α,β-Diaminopropionic Acid (L-ODAP): A Better Insight to Transient Neurolathyrism

  • Neurolathyrism is a neurodegenerative disorder characterized by spastic paraplegia resulting from the excessive consumption of Lathyrus sativus (Grass pea). ${\beta}$-N-Oxalyl-L-${\alpha},{\beta}$-diaminopropionic acid (L-ODAP) is the primary neurotoxic component in this pea. The present study attempted to evaluate the proteome-wide alterations in chick brain 2 hr and 4 hr post L-ODAP treatment. Proteomic analysis of chick brain homogenates revealed several proteins involved in cytoskeletal structure, signaling, cellular metabolism, free radical scavenging, oxidative stress and neurodegenerative disorders were initially up-regulated at 2 hr and later recovered to normal levels by 4 hr. Since L-ODAP mediated neurotoxicity is mainly by excitotoxicity and oxidative stress related dysfunctions, this study further evaluated the role of L-ODAP in apoptosis in vitro using human neuroblastoma cell line, IMR-32. The in vitro studies carried out at $200{\mu}M$ L-ODAP for 4 hr indicate minimal intracellular ROS generation and alteration of mitochondrial membrane potential though not leading to apoptotic cell death. L-ODAP at low concentrations can be explored as a stimulator of various reactive oxygen species (ROS) mediated cell signaling pathways not detrimental to cells. Insights from our study may provide a platform to explore the beneficial side of L-ODAP at lower concentrations. This study is of significance especially in view of the Government of India lifting the ban on cultivation of low toxin Lathyrus varieties and consumption of this lentil.

허혈성 척수 손상의 동물실험모델에서 Trimetazidine의 척수 보호효과 (Protective Effects of Trimetazidine in a Rabbit Model of Transient Spinal Cord Ischemia)

  • 배경: 흉부 및 흉복부 대동맥 수술시에 발생하는 척수의 허혈성 손상에 의한 신경학적 합병증은 발병 전의 예측이 어려울 뿐만 아니라 중증의 장애를 남기게 된다. 본 연구에서는 허혈성 심근 질환 치료에 쓰이고 있는 Trimetazidine(이하 TMZ)의 척수의 허혈성 손상에 대한 보호 효과를 동물실험모델에서 실험하였다. 대상 및 방법: 다 자란 New-Zealand White Rabbits 33마리를 대조군(Group 1, N-17)과 실험군(Group 2, N=16)으로 나누어 실험하였다. 수술은 대조군에서는 전신마취 후 정중개복하여 좌측 신동맥 기시 직하부위에서 복부대동맥을 혈관 점자로 30분간 폐쇄하여 척수 허혈을 유발하였으며, 실험군에서는 TMZ 3mg/kg을 대동맥 겸자전 투여하였다. 수술 2시간 후, 24시간 후, 48시간 후에 Modified Talrov scale에 의한 운동능력을 평가하였으며 술 후 48시간에 요천추부 척수를 적출하여 조직검사를 시행하였다. 결과: 대조군과 실험군에서 각각 7 마리(17마리 중 10마리 사망)와 11마리(16마리 중 5마리 사망)가 실험 종료가지 생존하여 척수 조직을 채취하였다. Modified Talrov scale은 대조군과 실험군에서 각각 수술 2시간 후 1.13 $\pm$ 1.25와 3.20 $\pm$ 0.77, 24시간 후 1.45 $\pm$ 1.57와 3.50 $\pm$ 0.76, 48시간 후 1.86 $\pm$ 1.86와 3.91 $\pm$ 0.30이었다(p$\leq$0.05). 척수의 조직학적 검사에서는 신경학적 결손이 큰 대조군(Group 1)의 척수조직에서 허혈성 손상이 더 심하게 일어난 것이 관찰되었다. 결론: TMZ은 동물실험에서 척수의 일과성 허혈성 손상에 대하여 통계적으로 유의한 척수 보호효과를 나타내었다.
