Detection of mcr-1 Plasmids in Enterobacteriaceae Isolates From Human Specimens: Comparison With Those in Escherichia coli Isolates From Livestock in Korea

  • Background: The emerging mobile colistin resistance gene, mcr-1, is an ongoing worldwide concern and an evaluation of clinical isolates harboring this gene is required in Korea. We investigated mcr-1-possessing Enterobacteriaceae among Enterobacteriaceae strains isolated in Korea, and compared the genetic details of the plasmids with those in Escherichia coli isolates from livestock. Methods: Among 9,396 Enterobacteriaceae clinical isolates collected between 2010 and 2015, 1,347 (14.3%) strains were resistant to colistin and those were screened for mcr-1 by PCR. Colistin minimum inhibitory concentrations (MICs) were determined by microdilution, and conjugal transfer of the mcr-1-harboring plasmids was assessed by direct mating. Whole genomes of three mcr-1-positive Enterobacteriaceae clinical isolates and 11 livestock-origin mcr-1-positive E. coli isolates were sequenced. Results: Two E. coli and one Enterobacter aerogenes clinical isolates carried carried IncI2 plasmids harboring mcr-1, which conferred colistin resistance (E. coli MIC, 4 mg/L; E. aerogenes MIC, 32 mg/L). The strains possessed the complete conjugal machinery except for E. aerogenes harboring a truncated prepilin peptidase. The E. coli plasmid transferred more efficiently to E. coli than to Klebsiella pneumoniae or Enterobacter cloacae recipients. Among the three bacterial hosts, the colistin MIC was the highest for E. coli owing to the higher mcr-1-plasmid copy number and mcr-1 expression levels. Ten mcr-1-positive chicken-origin E. coli strains also possessed mcr-1-harboring IncI2 plasmids closely related to that in the clinical E. aerogenes isolate, and the remaining one porcine-origin E. coli possessed an mcr-1-harboring IncX4 plasmid. Conclusions: mcr-1-harboring IncI2 plasmids were identified in clinical Enterobacteriaceae isolates. These plasmids were closely associated with those in chicken-origin E. coli strains in Korea, supporting the concept of mcr-1 dissemination between humans and livestock.

  • 본 연구는 네덜란드 동부 라흐닝언과 그 주변에 위치한 식품산업 클러스터인 푸드밸리의 클러스터 구조 특성과 존립기반을 트리플 힐릭스 혁신체계 관점에서 분석한 것이다. 푸드밸리는 대학을 기반으로 성장한 클러스터이나 산 학 연 관 네트워크 체계에 기반하여, 이른바 트리플 힐릭스 혁신체계를 효과적으로 구축함으로써 세계적인 식품산업 클러스터로 발전하게 되었다. 푸드밸리의 트리플 힐릭스 혁신체계는 '기반구축기' $\rightarrow$ '관계형성기' $\rightarrow$ '발전기' $\rightarrow$ '정착기'에 이르는 4단계의 발전과정을 거치면서 혁신 클러스터로 성장하였다. 이 과정에서, 세계적인 교육 및 연구역량을 보유한 라흐닝언 대학 연구센터(WURC)를 중심으로 한 대학 및 연구기판들은 지역 내외의 기업들과 활발한 지식 이전 및 공유 활동을 통해 파트너십을 형성하는데 중추적 역할을 했다. 아울러 지역 기업들은 국제적 경쟁력을 가진 다국적 농식품 기업들과 기술혁신형 창업기업들이 중심이 되어 푸드밸리소사이어티와 같은 학습 커뮤니티를 형성하여 활발한 지식 창출, 이전 및 공유 활동에 참여하고 있다. 여기에 정부 및 지원기관들은 클러스터의 네트워크 촉진자로서 산 학 연의 주체별 혁신역량과 대학과 기업의 지식 창출, 이전 및 공유 메커니즘을 조성하는데 중요한 역할을 담당하고 있다.

다기능 보조기구와 체중부하검사 보조기구의 X선 화질 비교 (Comparison of X-ray Image Quality Between Multi-Function Device(MFD) and Weight Bearing Platforms(WBPs))

  • 본 연구의 목적은 여러 가지 하지입식검사에 적용할 수 있는 Multi-Function Device(MFD)를 제작하여 기존에 사용하던 Weight Bearing Platforms(WBPs)들과 X 선 영상의 비교를 통해 임상 활용을 제안하고자 한다. MFD는 단상(보조기구)를 이용한 Weight Bearing Foot/Ankle, Hindfoot Alignment View 검사시 X선 장치의 X선 관구의 최하 조절 높이를 고려하여 제작하였다. 그리고 Foot/Ankle Phantom으로 MFD와 WBPs에서 Weight Bearing Lateral Foot과 Hindfoot Alignment View를 검사하여 X선 영상의 해상력을 Quick MTF(modulation transfer function)프로그램으로 비교하였다. 연구결과 MFD와 WBPs에서 검사한 두 가지 영상 모두 MTF 50(50% Contrast Spatial Frequency)과 10-90%(10-90% of Maximum Energy Rise Distance) Rise의 C/P(Cycles Per Pixel)평균과 LPH(Lines Per Image Height)평균 모두 유의한 차이는 없었다(p>0.05). 연구에서 제작한 MFD는 환자의 안전과 여러 가지 검사를 복합적으로 수행 할 수 있는 유용한 보조기구로 임상 활용을 제안한다. 또한 정책적으로 보조기구의 활용을 권장한다면 전문 업체의 활성화를 돕는 길이며 의료의 질도 더욱 향상할 수 있을 것이다.

공공기관의 생산현황통보에 관한 연구 (A Study on the Records Production Report of Public Institutions)

  • 생산현황통보는 국내 기록관리의 주요 성과이자 특징으로, 전 세계적으로 유례를 찾아보기 힘든 한국적 기능이다. 1999년 "공공기관의 기록물 관리에 관한 법률" 제정 당시, 법률에 생산현황통보 제도를 포함시키기 위한 여러 노력이 있었고, 2006년 법률개정 이후에도 여전히 그 기능이 존재하고 있다. 그러나 생산현황통보에 대한 시스템, 서식 등의 업무적 지원이 미비할 뿐만 아니라, 형식적인 기능으로 변질되고 있는 실정이다. 이에 본 연구에서는 생산현황통보 제도의 제정목적과 함께 현재 실무 현황 및 업무지원을 위한 시사점을 도출하고자 한다. 우선 생산현황통보 제도 전반을 이해하고, 취지와 목적을 확인하기 위해 법률 제정 관계자와 생산현황통보제도를 집행하는 영구기록물관리기관 관계자와의 면담하였다. 제도를 실무 현장에서 수행하고 있는 현장의 기록물관리 전문요원과의 면담도 수행하였다. 이를 통해 생산현황보고에 대한 전문가 집단의 견해와 함께 상호간의 의견 차이를 확인하였고, 시스템을 통한 실무지원이 가능하도록 현재의 기능을 평가하고, 향후 개선점을 제안하였다.

BGO:Eu 섬광체의 방사선 저항 (Radiation Resistance of BGO:Eu Scintillator)

  • 섬광검출기로 널리 사용되고 있는 BGO 결정을 Czochralski 방법으로 육성하였다. 그리고 들뜸빛띠, 방출빛띠 및 형광수명시간을 측정하여 엑시톤과 $Eu^{+3}$ 이온의 전하전달상태 사이에 에너지가 전달되고, 그 전달효율이 결정의 온도가 높아질수록 증가함을 확인하였다. 이것은 BGO 섬광체에 Eu를 첨가함으로써 방사선 저항을 증가시키는 과정에 대한 하나의 메카니즘이 된다. 한편 Eu를 많이 첨가할수록 방사선 저항은 증가되지만 섬광으로 부적당한 $Eu^{3+}$ 이온의 $^{5}D_{0}$, 준위에서 방출되는 형광이 커졌다. BGO 결정에 Eu를 0.1몰% 첨가하면 방사선유도 색중심밀도가 약 20배 줄고, 섬광에 이용되는 $Bi^{3+}$ 이온과 엑시톤이 방출하는 형광량에 비해서 형광수명시간이 $^{5}D_{0}$ 형광방출량은 1% 미만이었다.

Development of a Flow Analysis Code Using an Unstructured Grid with the Cell-Centered Method

  • A conservative finite-volume numerical method for unstructured grids with the cell-centered method has been developed for computing flow and heat transfer by combining the attractive features of the existing pressure-based procedures with the advances made in unstructured grid techniques. This method uses an integral form of governing equations for arbitrary convex polyhedra. Care is taken in the discretization and solution procedure to avoid formulations that are cell-shape-specific. A collocated variable arrangement formulation is developed, i.e. all dependent variables such as pressure and velocity are stored at cell centers. For both convective and diffusive fluxes the forms superior to both accuracy and stability are particularly adopted and formulated through a systematic study on the existing approximation ones. Gradients required for the evaluation of diffusion fluxes and for second-order-accurate convective operators are computed by using a linear reconstruction based on the divergence theorem. Momentum interpolation is used to prevent the pressure checkerboarding and a segregated solution strategy is adopted to minimize the storage requirements with the pressure-velocity coupling by the SIMPLE algorithm. An algebraic solver using iterative preconditioned conjugate gradient method is used for the solution of linearized equations. The flow analysis code (PowerCFD) developed by the present method is evaluated for its application to several 2-D structured-mesh benchmark problems using a variety of unstructured quadrilateral and triangular meshes. The present flow analysis code by using unstructured grids with the cell-centered method clearly demonstrate the same accuracy and robustness as that for a typical structured mesh.

  • As the WTO system launches through the agreement of Uruguay Round, the Government has to revise the office regulations or reform the system. Also, Integrating and Coordinating the like affair in health care (i. e., children's home, industry health, school health, health manpower, the administration of health center, the administration on food hygiene, health environmental education, and so on.) which is now scattered into some government departments like the Ministry of Labor, the Ministry of Education, the Ministry of Home Affairs, the Ministry of Agriculture, and the Ministry of Environment, the Government has to prevent unspecialty, inefficiency, inconsistency, and uneconomy. The Government has to review and adopt above suggested the Proposal 1),2),3),4) of the Health Centers on the basis of the local autonomy law and it will help the successive settlement of the local autonomy system in Korea. According to the suggested proposal, the Central Government mainly takes charge of the Macro affairs as hardware, and transfer the Micro affairs as software into the Local Governments to attempt the appropriate functional allocation. To achieve it successfully, the Central Government also has to do the financial support, manpower training and technical support, allocation of health care resources, direction and control, research and development and the health care plan on the macro level. Local Governments which divided into the wide local government and basic local government also have to do their best for health improvement of the community societies like plan of health care program, implementation of health care service program, taking charge of the affairs of health insurance, activation of community residents' participation and security of health care resources etc. To achieve this goal, the Government have to be more active and reformative, the related social and health agencies and educational agencies have to cooperate and support for the goals, and especially, the community residents have to participate actively and voluntarily, When all these conditions promote, local health care administration will be developed, and health level of community residents will be secured. And going one step forward, the country and people will be more healthy

서비스무역 커리큘럼 개발에 관한 연구 (A Study on the Curriculum Development for the Trade in Services)

  • The importance of trade in services has been increasing day by day, but the research on it is insufficient so far in terms of basic information, statistics, influence, industrialization and so on. To foster of professional trader in services, we need curriculums, textbooks and training centers like academy or college in advance. We have well developed curriculums for trade in goods since 1960's in Korea, so we can transfer the trade in goods' curriculum to trade in services. There are some differences between trade in goods and trade in services basically, but we can borrow a lot of idea from trade in goods in terms of basic framework like international economics, international business and international commercial transactions. This study propose the basic framework for trade in services' curriculum. First, trade in services economics handle the basic concept, statistics, characteristics, theories etc. Second, trade in services business treat the global companies to expand their business to global market, so characteristics of service companies, marketing plan and strategies and so on. Third, international commercial transaction of trade in services concentrate for procedures and contracts in terms of formation, implementation and finish of contract. Finally, Services industries can be a future strategic industry to any contries, so there are some national and corporates' strategy for expanding their business. This study acts on the initial idea for curriculums of trade in services, so I am looking forward to many criticism and development from another researchers to develop the model curriculums and textbook for education of specialized trader in services.

지역 응급의료기관의 아동 간호중재 분석 -4차 개정 Nursing Intervention Classification을 적용하여- (Analysis of the Characteristics and the Nursing Interventions for Children in Regional Emergency Departments -Using the Nursing Intervention Classification-)

  • Purpose: The purpose of this study was to provide descriptive data about the characteristics of pediatric patients and nursing interventions in Regional Emergency Medical Centers (REMC). Methods: A retrospective design was used to examine the medical records of 4,310 children. The clinical data and nursing terminologies of REMC were analyzed using the Nursing Intervention Classification (NIC). Results: Male toddlers dominated the sample. The mean age of the children was 3.51 yr. In more than half of the visits, patients arrived between from 7 a.m. to 3 p.m., on a weekday. There were only 189 ambulance transports to REMC, (4.4% of visits). The most frequent injuries were due to falls (28.0%) and contusions (27.3%), but the most common reasons for visits were non-injury (73.4%): fever, cough/shortness of breath, seizures, and abdominal pain. Of the 4,310 visits, 27.8% spent 6-24 hours in the REMC, while 33% resulted in hospital admission and 2.1% in transfer to another hospital. Of the 17,929 nursing interventions, 17,909 elements (99.9%) were classified under NIC. All the listed NIC interventions, however, were not reflected in the level of practice demonstrated by REMC nurses. Conclusion: These results can enhance the understanding of pediatric emergency nursing interventions and can make NIC more applicable.

Bimetallic Co/Zn-ZIF as an Efficient Photocatalyst for Degradation of Indigo Carmine

  • Cobalt-incorporated zeolitic imidazolate framework ZIF-8 was synthesized by a simple one-pot synthesis method at room temperature. Powder X-ray diffraction patterns and energy dispersive X-ray spectrum confirmed the formation of the bimetallic Co/Zn-ZIF structure. UV-Vis diffuse reflectance spectra revealed that the bimetallic ZIF had a lower HOMO-LUMO gap compared with ZIF-8 due to the charge transfer process from organic ligands to cobalt centers. A hydrolytic stability test showed that Co/Zn-ZIF is very robust in aqueous solution - the most important criterion for any material to be applied in photodegradation. The photocatalytic efficiency of the synthesized samples was investigated over the Indigo Carmine (IC) dye degradation under solar simulated irradiation. Cobalt incorporated ZIF-8 exhibited high efficiency over a wide range of pH and initial concentration. The degradation followed through three distinct stages: a slow initial stage, followed by an accelerated stage and completed with a decelerated stage. Moreover, the photocatalytic performance of the synthesized samples was highly improved in alkaline environment rather than in acidic or neutral environments, which may have been because in high pH medium, the increased concentration of hydroxyl ion facilitated the formation of hydroxyl radicals, a reactive species responsible for the breaking of the Indigo Carmine structure. Thus, Co/Zn-ZIF is a promising and green material for solving the environmental pollution caused by textile industries.
