Self-Care Education Programs Based on a Trans-Theoretical Model in Women Referring to Health Centers: Breast Self-Examination Behavior in Iran

  • Background: Breast cancer is one of the most common cancers and a major public health problem in developing countries. However, early detection and treatment may be achieved by breast self-examination (BSE). Despite the importance of BSE in reducing the incidence of breast cancer and esultant deaths, the disease continues to be the most common cause of cancer death among women in Iran.This study aimed to determine the effects of self-care education on performance of BSE among women referring to health centers in our country. Materials and Methods: This quasi-experimental interventional study with pretest/posttest control group design was conducted on 168 women referred to health centers. The data were collected using a validated researcher-made questionnaire including demographic variables and trans-theoretical model constructs as well as a checklist assessing BSE behavior. The instruments were administered to groups with and without self-care education before, a week after, and 10 weeks after the intervention. Then, the data were entered into the SPSS statistical software (version 19) and analyzed using independent sample t-tests, paired sample t-test, repeated measures ANOVA, Chi-square, and Friedman tests (p<0.05). Results: The results showed an increase in the intervention group's mean scores of trans-theoretical model constructs (stages of change, self-efficacy, decisional balance, and processes of change) and BSE behavior compared to the control group (p<0.001). Conclusion: The study confirmed the effectiveness of aneducational intervention based ona trans-theoretical model in performing BSE. Therefore, designing educational interventions based on this model is recommended to improve women's health and reduce deaths due to breast cancer.

Research on the motion characteristics of a trans-media vehicle when entering water obliquely at low speed

  • This paper proposes a single control strategy to solve the problem of trans-media vehicle difficult control. The proposed control strategy is just to control the vehicle's air navigation, but not to control the underwater navigation. The hydrodynamic model of a vehicle when entering water obliquely at low speed has been founded to analyze the motion characteristics. Two methods have been used to simulate the vehicle entering water in the same condition: numerical simulation method and theoretical model solving method. And the results of the two methods can validate the hydrodynamic model founded in this paper. The entering water motion in the conditions of different velocity, different angle, and different attack angle has been simulated by this hydrodynamic model and the simulation has been analyzed. And the change rule of the vehicle's gestures and position when entering water has been obtained by analysis. This entering water rule will guide the follow-up of a series of research, such as the underwater navigation, the exiting water process and so on.

간호학생의 운동변화 단계 요인 (Influencing Factors of Exercise Behavior Change in Nursing Students)

  • Purpose: This study was to predict the stage of exercise behavior change in undergraduate students and to identify the factors affecting exercise behavior change by using the trans theoretical Model. Method: The subjects were 193 undergraduate students at K and D university in D city. Data were collected by questionnaire surveys using convenience sampling and analyzed by SPSS 12.0 program. Results: 77.7% of 193 nursing students do not exercise or they exercise irregularly. Exercise self-efficacy score and exercise benefits score were decreased with advancing exercise stage, on the other hand exercise barriers score and the process of exercise behavior change score were increased. In addition to this, the results revealed that stage of exercise behavior was significantly correlated with the process of exercise behavior change, exercise benefits, exercise barriers and exercise self-efficacy. Conclusion: These results suggested that the process of exercise behavior change, exercise decisional balance and exercise self-efficacy can be potential affecting factors for the stages of behavior change in university students. Therefore, these findings give useful information for educating nurse to construct nursing service programs focused on the stage of behavior change.

산업장 남성 근로자의 금연행위 변화단계별 특성 (The Characteristics of Smoking Cessation Behavior by the Stage of Change in Industrial Workers)

  • Purpose: This study was to identify the stages of change in smoking cessation behavior and factors associated with the stages of smoking cessation behavior according to the trans-theoretical model. Methods: The subjects were 154 industrial workers working at H Industry in N City, Chonnam Province who were currently smoking and had smoked in the past. Data were analyzed by descriptive statistics, ANOVA, and Duncan's multiple comparison test using SAS Version 10.0. Results: The subjects were distributed among the stages of change in smoking cessation behavior: there were 28 subjects (18.2%) in the precontemplation stage, 71 (46.1%) in the contemplation stage, 21 (13.6%) in the preparation stage, 8 (5.2%) in the action stage, and 26 (16.9%) in the maintenance stage. The amount of smoking per day, self-efficacy, and advantages (pros) of smoking were significantly associated with the stage of change in smoking cessation behavior. Conclusion: This study suggested that the stage of change in smoking cessation behavior of the subject should be identified prior to the application of intervention programs, nursing intervention strategies should be considered to reduce the amount of smoking per day, and the disadvantages of smoking should be perceived.

기혼간호사 대상 자궁경부세포 검진의 변화 단계에 따른 관련요인 (Relating Factors in Pap Smears by Stages of Change among Married Nurses)

  • Purpose: This study was to examine the differences in health belief variables and HPV (Human Papillomavirus) knowledge in Pap smears according to the stages of change using Trans-theoretical Model (TTM) among married nurses. Methods: The research design was a cross sectional survey. Participants were 387 married nurses working in general hospital located metropolitan city. Measurement variables were perceived threatening, perceived severity, benefits of Pap, HPV knowledge and stages of the change in Pap smear. Results: Classified stages were pre-contemplation 3.9%, contemplation 17.8%, relapsing 29.5%, action 31.0%, and maintenance 17.8%. Accuracy rate of HPV knowledge were low ranging from 15% to 45%. Among the variables, perceived threatening (F=3.56, p=.007), perceived severity (F=9.73, p<.001) and HPV knowledge (F=7.52, p<.001) were significantly different by stages of change. Conclusion: Application of TTM to Pap smears was efficient to know the nurse's level of health behaviors, Health belief variables and HPV knowledge were main factors to identify the stages of change. The continuation of Pap smears and the education in relation to HPV knowledge should be encouraged for married nurses.

범이론 모형을 적용한 고혈압 환자 약물순응 중재 프로그램 개발 및 적용 효과 (The Development and Effects of a Medication Adherence Intervention Program for Hypertensive Patients)

  • Purpose: This study was to estimate the effectiveness of an intervention program using the trans-theoretical model on the stages of change, the process of change, decisional balance, self-efficacy, the levels of medication adherence, and blood pressure difference. Methods: A similar experiment was conducted using a nonequivalent control group pre- and post-test design. The study subjects were 108 hypertensive patients. The program was administered to the experimental group while educational materials were given to the control group. Descriptive analysis, 2-test, t-test, and repeated measures ANOVA were used with SPSS 17.0 to analyze collected data. Results: After treatment with this program, the experimental group showed stage of change ($x^2=52.917$, p<.001), cognitive (F=13,528, p<.001), behavioral process of change (F=10.808, p<.001), benefits (F=5.569, p=.012), loss of medication adherence (F=15.661, p<.001), self-efficacy (F=5.407, p=.011), levels of medication adherence (F=51.442, p<.001). Conclusion: There was a significant improvement in the experimental group who participated in the TTM medication adherence intervention program. Given that this intervention program motivated and stimulated hypertensive patients to adhere to the prescribed medications, the TTM medication adherence intervention program is expected to be an effective and practical intervention method for health improvement.

현미경 관찰을 통한 Seip 직물의 섬유 형태 연구 (Morphological Analysis of Fibers of the Seip Textiles through the Microscopic Examination)

  • The fiber morphologies of the textiles from the burials of the Seip group of Ohio Hopewell (ca. 100 B.C. to A.D. 500)mounds were examined using light and scanning electron microscopy. A theoretical model was proposed which delineates the rate of morphological change in fibers through time and the cultural activities which occur during the life of a textile element. The Seip textiles were categorized according to their visual and structural characteristics. The microscopic examination of the Seip textiles yielded the evidence of animal hair fibers and bast fibers. The animal hair fibers of the seip textiles were identified as the hair of rabbit or hare. The bast fibers of the Seip textiles exhibited variation in morphological features such as the degree of bundle separation and the presence of transverse striations, fibrillations, or trans-verse cracks.

Relationship of Smoking with Self-perceived Health and Selected Health Behaviors

  • The purpose of this study was to analyze variables associated with smoking and selected health behaviors. This study targeted adult men and women over 20 of age. The sample population was drawn from the national sample, and the samples were chosen from the telephone book. A total of 1,500 cases were collected through a telephone based interview survey. As a tool for this study, a structured questionnaire was developed. the variables included self-perceived health status, and selected 7 health behaviors; smoking behavior, physical activities, eating habit, weight control, alcohol consumption, stress management, and cancer exam. The healthy life practice actions of Trans-theoretical model have been classified into five stages. Smokers were more likely than non-smokers to positive attitudes toward smoking and the impact of smoking in this study. This means that smokers's awareness toward the impact of smoking is very week. Smoking behavior was significantly related to other health related behaviors based on the correlation analysis. However, gender, engaging in regular physical activities, moderate consumption of alcoholic beverages, and receiving a cancer examination were the variables that can explain and predict a person's stages of adopting a non-smoking behavior. Self perceived health status was closely related to other health behaviors. Gender, age, and monthly household income were important demographic variables that have significant relationship with the self-perceived health status. Among the health behaviors, regular physical activities and weight control were significant predictable variables. Similar results have been found in the National health and Nutrition examination survey in Korea. As a result of this study, it was found that among the selected health behaviors, engaging in physical activity was an important variable to increase positive sense of health and non-smoking behaviors. Educational and policy level input is needed to increase awareness and provide chances to participate in fitness activities. To those who maintain exercise, the environmental support and diffusion of knowledge information and education data should be strengthened. To change attitudes toward smoking, more in-dept awareness campaign and education should be provided according to people's different behavioral status. In addition, not only diffusing health education data and delivering knowledge information through related programs, but also environmental support system that helps an individual maintain his/her action is required. Such a support system means settlement of the health enhancement base of school, workplace and community should be carried out, and the policy level support and regular programs should be provided and spread to the unit of community.

소음작업장 근로자의 청력보호구 사용단계와 관련요인 (Change of Stages and Related Factors for Wearing of Hearing Protection Device among Noisy Workplace-workers)

  • Purpose: This study was done to identify the distribution and related factors for stage of change for wearing hearing protection devices (HPDs) by workers in environments with high noise. Predictors of Use of Hearing Protection Model and Trans-theoretical Model were tested. Methods: The participants were 755 workers from 20 noisy work places in Busan and Gyeongnam. Data were collected from January to April 2008 using self-administered questionnaires, and analyzed using multiple logistic regression. Results: There were significant differences in social mode (OR=1.35, 95% CI: 1.06-1.73) between precontemplation/contemplation and preparation stage, in males (OR=2.36, 95% CI: 1.24-4.51), workers with high school education or less (OR=1.39, 95% CI: 1.28-2.78), shift workers (OR=1.50, 95% CI: 1.02-2.21), workers who previously worked in noisy places (OR=1.39, 95% CI: 1.20-2.34), and workers who had previous hearing examinations (OR=1.89, 95% CI: 1.25-2.85), in the social model (OR=1.59, 95% CI: 1.42-1.78), and self-efficacy (OR=1.05, 95% CI: 1.02-1.08) between workers in preparation and action stages, in length of time working in noisy work places (OR=2.26, 95% CI: 1.17-4.39), social model (OR=1.66, 95% CI: 1.33-2.08), and perceived benefit (OR=0.95, 95% CI: 0.93-0.97) between action and maintenance stage. Conclusion: Social model was a common factor showing differences between two adjacent stages for wearing HPDs. The results provide data for developing programs to encourage workers to wear HPDs and application of these programs in work settings.

Evaluation of a theory-based community intervention to increase fruit and vegetable intakes of women with limited incomes

  • The study objectives were to increase both the stage of readiness to eat fruit and vegetables as well as the intakes of women who participated in the Expanded Food and Nutrition Education Program (EFNEP) for families with limited incomes. The intervention was to enhance the currently used curriculum, Eating Right Is Basic III (ERIB3), with stage-specific processes based on the Trans-Theoretical Model of readiness to change. Trained EFNEP community workers taught the enhanced curriculum to 90 mothers in the experimental county and to 53 mothers in the control county. Pre- and post-intervention measures included stages of readiness to eat fruit and vegetables and to intake as assessed by 24-hour dietary recalls and staging questions. Most women recruited into EFNEP were in Action and Preparation Stages (53.5%). Fruit and vegetable intakes showed a linear trend along with the Stage of Change for fruit and vegetable. After intervention, some combination of the ERIB3 and the fruit and vegetables-enhanced ERIB3 resulted in a reported 1/2 servings/day increase in fruits and vegetables in both the control and the experimental counties, although changes were not significant. EFNEP women also moved along the Stage of Change Continuum for fruits and vegetables in both counties. The percentage of people who ate 5 or more servings of fruit and vegetables was significant, however, only in the experimental group. We encourage health professionals to apply lessons learned from this intervention and to continue to pursue theoretically based interventions to change dietary behaviors.
