해외 사례 고찰을 통한 지자체 노인요양시설 인증 체계 및 조사원 고도화 방안 (A Study on the Advancement of Accreditation Systems and Surveyors' Expertise for Long-term Care Facilities: Focusing on Overseas Cases)

  • Purpose: This study aimed to suggest strategies for advancing local-government-based accreditation systems and surveyor training in long-term care facilities in Korea. Methods: A comprehensive review of the literature including research papers and official reports issued by governments from the United States, Australia, and Japan was conducted to explore domestic and international policies related to long-term care facility certification and accreditation systems. Results: The USA has two types of care quality assurance systems including mandatory certification (5-star rating system) by the Centers for Medicare and Medicaid Services and voluntary accreditation by the Joint Commission on Accreditation of Healthcare Organizations. Australia operates a government-based mandatory accreditation system for all long-term care facilities through the Australian Aged Care Quality Agency. Japan, particularly the Tokyo district, operates a third-party evaluation system that involves the voluntary participation of long-term care facilities. Conclusion: This study provides several strategies to enhance accreditation processes and surveyors'expertise. For instance, motivating facilities to voluntarily participate in accreditation is necessary by 1) providing sufficient and continuous consultations and feedback about how to improve care quality, 2) differentiating accreditation domains and indicators from the national health insurance certification system, and 3) actively utilizing accreditation results and providing incentives.

노인장기요양기관에 근무하는 간호사의 근무실태 및 직무만족도 영향 요인 (A Study on Working Conditions and Factors Related to Job Satisfaction of Nurses in the Long-term Care facilities for the Elderly)

  • Purpose: Th study is to explore the working conditions and factors related to job satisfaction of nurses in long-term care facilities for the elderly. Methods: A total of 127 nurses working long-term care facilities, from June to November 2014, completed questionnaires. Face to face interviews were conducted. The questionnaire consists of a total of 32 questions, including 5 questions related to general characteristics, 15 questions related to working conditions, and 12 questions related to job satisfaction(likert scale of 1 to 5). Results: The majority of nurses were employed in institutional care facilities, in urban area, and in privately owned facilities. On average, employed nurses worked 170.7 hours per month. Only 40.9% of them got more than two million won for their monthly salary. They were satisfied with the nature and worth of their work and the workplace relationship, but not with their wages and promotion opportunity 40.2% of nurses reported satisfaction with job. In multiple logistic regression analysis, age was a statistically significant influencing factor on general job satisfaction. Conclusion: The findings suggest that policies must be to improve nurses' working conditions, for wages, and to support them with education and training for skills development.

한국 봉제기술자의 직무 환경 만족도와 교육 실태 파악을 위한 현장 조사 (On-Site Survey of Satisfaction with Job Environments and Training Situation of the Korean Sewing Technicians)

  • Domestic sewing industry is showing signs of a redevelopment and the demands of the sewing technician are gradually increasing. This study would investigate the opinions of sewing technicians about job environments and training situation in order to solve the shortage of sewing manpower. As a result of a survey with 100 sewing technicians, of the pay systems, the salary system showed the highest satisfaction and preference. Their satisfaction with pay was low. There was a tendency that they work on Saturdays and national holidays, and their satisfaction with the number of holidays was low. Most worked for 10 to 11 hours per day, and they showed low satisfaction with their working hours. There were no places to rest in the company. They showed law satisfaction with the air and vent. The sewing technicians were concerned about respiratory and musculoskeletal diseases because of the working and workplace environment and needed places to rest, vent facilities, and spacious and comfortable spaces. The total lead-time that they were sent to the field as skilled workers was 1 to less than 2 years; they received training from seniors; and the training period, mostly, was 6 months to 2 years. Most of the sewing technicians surveyed had the expertise, did not feel the necessity of retraining related to their tasks, and did not have any willingness to take reorientation due to a lack of time even with their tasks.

전북지역 유치원교사의 식생활교육 실태와 연수 요구도 (Status and Training Demand on Dietary Education of Kindergarten Teachers in Jeonbuk Province)

  • The purpose of this study was to investigate the kindergarten teachers operation and demand of dietary education in Jeonbuk province. The study was carried out using a self administered questionnaire and the subjects were 148 kindergarten teachers. The questions were general characteristics of the subjects, operating status as frequency, place, education time, and training demand on dietary education. The results are as follows. All subjects were female(100.0%), more than half were 20's(62.8%) in their age, teaching career was 31.8% in less than 3 years, and 68.2% in more than 3 years. Frequency of the dietary education operation was 3 times per week(53.4%), place of education were classroom(81.9%), and education time was lunch time(87.5%). Twenty three point eight percent of the subjects recognized the facilities for education was lack, 32.6% of them did the education materials was lack. The desirable frequency of dietary education was 1~2 times per week(40.4%). About two third of the subjects(73.6%) recognized the objects of the dietary education was not only preschool children but also their parents. Most of the subjects(83.8%) had willing to get training, the proper training program was 10 hours(87.8%), and they prefer semester weekday(64.9%) than semester weekend(30.5%). Proper ratio in the core value of dietary education as environment : health : thanks was 31.8% : 40.9% : 27.2%. It is concluded that the demand on dietary education of kindergarten teachers is necessary for their education program in kindergarten.

닭 날개 혈관을 이용한 혈관문합술의 교육 (Training of Microanastomosis with Chicken Wing Brachial Artery)

  • Purpose: Microsuturing is a difficult job for beginners of microsurgery. It is because they are not familiar with microscopic environment and, it needs much time for them to get used to microanastomosis. Before the real microsurgery, sometimes a surgeon wants rehearsal. But, microsurgical exercise has been performed with surgical glove, silastic drain or rat femoral artery. Rat femoral artery is a very good training material. But, it needs animal laboratory, anesthesia and its keeping facilities. And the surgeon should appoint the time to exercise with the laboratory. Methods: We used chicken wing brachial artery for education material of microsuturing. The artery is 5 cm long and the diameter is about 1 mm. Monofilament 10-0 was used for suture material. Results: Six persons of Seoul National University medical school students and one resident attended in this program. Each of them performed arterial anastomosis ten times. They were satisfied with chicken wing brachial artery for anastomosis training under the magnification environment. Conclusion: We think that chicken wing brachial artery is a very cheap and an effective training material for the beginners of microsurgery.

다중협업이 가능한 AR 기반 화학공정 운전원 교육 시뮬레이터(OTS-Simulator) 개발 (Development on AR-Based Operator Training Simulator(OTS) for Chemical Process Capable of Multi-Collaboration)

  • 인적오류로 발생하는 화학사고를 예방하고자 첨단 기술을 응용한 화학사고 예방 및 대응 훈련 프로그램을 개발하였다. 기존에 구축된 파일롯 플랜트(pilot plant)를 바탕으로 가상의 공정을 설계한 후, 화학사고 대응 컨텐츠를 개발하였다. 컨텐츠 구현을 위하여 파일롯 설비 일부를 개조하여 원격제어기능을 부여하였다. 또한, 가상환경에서 설비를 제어할 수 있는 DCS 프로그램을 개발하였으며, AR과 연동하여 최종적으로 가상의 화학사고를 대응할 수 있는 화학공정 운전원 교육(OTS)을 개발하였다. 이를 통해 훈련자가 직접 장치를 조작해봄으써 운전역량을 쌓을 수 있고, 가상의 화학사고를 대응함으로써 비상시 대처능력을 기를 수 있었다. 본 연구와 같은 차세대 OTS가 화학산업에 널리 보급된다면 인적오류에 의한 화학사고를 예방하는데 크게 기여할 것으로 기대된다.

훈련예제 병합을 이용한 자동차 차량번호판 문자인식 성능 향상 방안 (Vehicle License Plate Recognition Using the Training Data's Annexation)

  • 자동차 수의 급증으로 야기되는 교통혼잡, 교통사고, 주차난 등의 많은 문제에 효율적으로 대응하기 위해서는 제한된 인력과 비용을 사용하는 자동차 관리가 필수적인데 이를 위한 많은 연구들이 국내외적으로 현재 진행되고 있다. 현재 진행되고 있는 여러 연구 분야 중에서 특히 자동차의 차량번호판인식 기술은 법규위반 차량 식별, 통행료 징수, 자동차세 징수, 도난 도주 차량 확인 및 주차 관리 등의 많은 분야에 응용되고 있다. 자동차의 차량번호판 문자 인식 문제와 같이 훈련예제 수집 비용이 많이 드는 경우에 제한된 수의 훈련예제를 최대한 활용하여 분류성능을 향상시키기 위한 방안의 하나로, 수집된 훈련예제들로부터 가상의 예제를 생성하고, 생성된 가상예제를 훈련예제로 추가하여 학습하는 여러 연구가 수행된 바 있다. 본 논문에서는 차량번호판 문자 인식의 성능 향상을 위해 수집된 예제들을 적절히 병합하여 가상의 예제를 생성하는 방안에 관해 기술하고, 문자인식 분야에서 일반적으로 많이 사용되는 여러 알고리즘에 대하여 다양한 가상예제 생성방안 및 다양한 생성비율에 따른 실험을 통해 그 효용성을 확인한다.

노인주간보호시설의 프로그램운영과 공간구성관계연구 - 경기도 노인주간보호시설을 중심으로 - (Study on service and Use the Actual conditions of Day care Center for the elderly - with A Focus on Adult Day Care Facilities -)

  • As the country's elderly people who are 65 years or older recently exceeded 10% of the total population with development of medical technology and improvement of living standards, Korea has turned into an aging society. Especially in Gyeonggi-do, as of late December 2012, elderly people who were 65 years or older were 1,135,242 persons, taking up 18.98% of the region's population and registering the largest number of elderly people in the nation's cities or provinces. Due to such a sharp rise in elderly population, support for the elderly is increasing the burden on families and communities. The study aims to take as its subjects elderly people staying at authorized elderly welfare facilities, who are weak in mind and body and have difficulty in daily life with disabilities, or adult day care facilities that take care of elderly people during the day or at night, examine the concept of adult day care facilities and instances in foreign countries, and study the status of the adult day care facilities located in Gyeonggi-do, their services, and safety by figuring out space arrangement based on program implementation. Spacial arrangement in program operation should satisfy fuction and purpose from the manager and user's perspective, and a desirable program operation should provide separate spaces for the elderly with Alzheimer's and those without Alzheimer's. Compared to residential care facilities, adult day care facilities incur less financial burden and, compared to other authorized services, have many right functions that can upgrade the quality of users and satisfy their desires. Major countries like Japan, the UK, and Sweden recognize the right functions of day and night care services and aggressively support and utilize adult day care facilities. For adult day care facilities, quality services should be developed and use and choices should be enhanced as regards services. Development of special programs for the elderly with dementia and stroke, instead of simple protective functions of a program, must be actively promoted, while manpower training is required for program operation, conveniences, and safety. By developing and providing space arrangement models that focus on efficiency, convenience, and safety of program operation, adult day care operation can be revitalized, while quality of elderly care may be enhanced and welfare budget can be saved.

어린이급식관리지원센터 직원의 업무수행 난이도 인식 (Center for Children's Foodservice Management (CCFSM) Employees' Perception of Difficulties in Performing Tasks)

  • 본 연구는 전국의 어린이급식관리지원센터 직원(센터장을 제외한 팀장, 팀원)을 대상으로 업무 수행 시 난이도 인식 및 효율적 업무 수행 방안에 대해 조사하여 어린이급식관리지원센터 사업의 효과를 높이고 직원들의 직무만족을 높일 수 있는 방안을 제시하고자 하였다. 2013년 이전에 개소한 기존센터 19개, 154명의 직원, 2013년 이후에 개소한 신규 센터 11개, 73명으로 총 30개 센터, 227명의 직원을 대상으로 이루어졌다. 출장의 빈도가 높은 영양 및 위생 안전관리 순회방문업무, 어린이, 조리원 대상 방문교육 업무를 담당하고 있는 직원들에게서 공통으로 '지도를 위한 출장 후 체력 소모가 커 업무에 집중이 어렵다'가 높게 응답되었다. 영양 및 위생 안전관리 순회방문업무, 조리원 대상 방문교육 업무를 담당하고 있는 직원들에서는 '어린이 급식소에 상주하는 영양사가 아니기 때문에 개선이 필요한 부분에 대한 계속적인 피드백이 어려워 행동 변화를 이끌어내기가 어려움'이 높게 응답되었다. 대상별 집합 교육에 대한 의견으로 '집합 교육 시간이 원장, 조리원, 교사의 업무 시간과 겹쳐 참여율이 저조'가 3.97점, 재정관리에 대한 의견으로 '센터 위탁 기관 내 규정, 지자체의 예산 사용 규정, 식약처 규정이 달라 혼선이 옴'이 4.13점으로 높게 나타났다.

교정시설 내 응급의료체계 발전방안 (The Developmental Program of Emergency Medical System in Correctional Facilities)

  • 응급의료체계(Emergency Medical System)의 목적은 응급환자에게 신속하고 적절한 응급처치를 제공함으로써, 환자의 생명을 구하고 환자의 상태를 최단 시간 내에 정상상태로 회복시켜서 사회로 복귀시키는 것이며 교정행정(矯正行政)의 목적은 범죄자를 사회로부터 격리하여 이들 수형자에게 형기동안 교육, 교화활동 및 직업훈련 등을 실시함으로써, 이들이 출소 후 사회에 복귀하여 다시는 범죄를 저지르지 않고 성공적으로 사회에 정착하도록 하는 것이다. 이렇듯 교정행정과 응급의료체계의 목적이 국민을 안전하게 사회에 복귀시키는 것으로 상호 부합된다 할 것이다. 따라서 본 연구는 교정시설 내 하위문화의 특수성으로 인한 신체적 손상과 질병에 노출될 기회가 많은 수용자와 그 곳에서 근무하고 있는 직원들의 안전을 위해 교정시설 내 다양한 응급상황 발생요인과 제도, 인력, 시설, 장비, 예산현황 및 문제점 등을 통해 효율적인 교정시설 내 응급의료체계 발전방안을 다음과 같이 제시하고자 한다. 첫째, 각 교정시설 보안근무체계에 맞게 적정인원의 응 급구조사의 채용을 확대하여야 한다. 둘째, 수용자의 중증도분류에 따른 이송체계를 완비하고 응급의료장비를 갖추어야 한다. 셋째, 교정공무원과 수용자에 대해 실습 위주의 현실적이고 체계적인 응급처치교육을 실시하여야 한다. 넷째, 1339응급의료정보센터와 같은 사회 내 응급의료체계와 협력체계를 구축하여야 한다. 다섯째, 보건복지부는 응급의 료기금의 지원을 교정시설로 확대하여야 한다.
