Tear Patterns in Traumatic and Atraumatic Meniscal Injuries of the Knee (외상과 비외상에 의한 슬관절 반월상 연골의 파열 형태)

  • We reviewed the charts and photos taken during arthroscpy of 218 knees of 214 patients(240 menisci) retrospectively. The male was 156 cases(73%) and the female 58 cases(27%). The mean age of the patients was 35 years(range, 7-68). The patients who had definite trauma history were classified as trauma group(Group 1), and the patients who had no or could not recall trauma history were as atrauma group(Group 2). The trauma group was subclassified into the the patients with sports injury, traffic accident, fall down, slip down, direct injury, and miscellaneous according to the causes of the trauma. The patterns of meniscal tear were classified into longitudinal, bucket-handle, horizontal, transverse, flap, complex, and degenerative tear on the basis of O'Connor's classification. The aim of this study was to compare the meniscal tear patterns between trauma group(Group 1) and atrauma group(Group 2) and between the patients before and after the age of 40. The results were as follows ; 1) The difference in the incidence of tear between medial and lateral meniscus was not significant statistically. 2) In Group 1, 60% of the cases showed the longitudinal and bucket-handle tear and 52% of the cases of Group 2 were horizontal tear. 3) In the patients before the age of 40, the longitudinal and bucket-handle tear were 52% of the cases and in the patients over 40, tear patterns which were thought to be related to degenerative change, horizontal and degenerative tear were more than half of the cases (51%).

Clinical Evaluation of the Fracture of Talar Neck (거골 경부 골절에 대한 치료)

  • The fracture and fracture-dislocation of the neck of the talus (Hawkins' type I-IV) are uncommon injuries and represent only 0.12 to 0.32% of all fracures. Authors clinically evaluated in 12 cases Whom treated fracures of the neck of the talus, at department of orthopaedic surgery, Sun General Hospital, from 1990 to 1996, and the following results are obtained. 1. Of 12 cases, there were 11 males and 1 female, average age was 30 years. 2. Causes of fracture was fall down injury in 7 cases(58%), traffic accident in 4 cases(33%), direct trauma in 1 case(8%). 3. According to the classification by Hawkins' type I in 2 cases(17%), type II in 7cases (58%), type III in 3cases(25%). 4. Associated injuries were calcaneal fracture in 3 cases, fracture-dislocation of talus in 3 cases, subtalar dislocation in 3 cases, medial malleolar fracture in 5 cases, soft tissue injury in 3 cases, femur and tibia fracture in 1 case, and lumbar Spine compression fracture in 1 case. 5. Average time to operation after injury was 2.5 days. 6. In 2 cases were treated conservatively and 10 cases were treated open reduction and internal fixation with screw or K-wire. 7. Complications were avascular necrosis in 4 cases, post traumatic arthritis in 2 cases, skin necrosis in 4 cases, and then ankle fusion was done in 2 cases. 8. High rate of complication was seen in the talar neck fracture associated with calcaneal fracture. In the analysis of above results, evaluated by Hawkins' scoring system were excellent to fair in 75%.

Car-tire-related Crushing Injury of the Lower Leg in Children (자동차 바퀴에 의한 소아 아래다리의 압궤 손상)

  • Purpose: Crushing injuries by car tires result from a combination of friction, shearing, and compression forces and the severity of injury is influenced by the acceleration. Because car-tire injuries of the lower leg in children are common these days but they have received little attention; thus, our purpose was to look closely into this problem. Methods: A retrospective analysis was conducted of data from children under 15 years old age who visited an emergency department because of a car-tire-related crushing injury to the lower leg in pedestrian traffic accident from January 2008 to September 2012. The patient's age, sex, site of injury, degree of injury, associated injuries, type of surgery, and complications were reviewed. Results: There were 39 children, the mean age was 8.0 years, and 71.8% were boys. The dorsal part of the leg was involved most frequently. According to the severity classification, 15 children were grade I, 6 were grade II, and 18 were grade III. Among 24 patients, 13 were treated with skin graft and 3 were treated using a sural flap. Twelve patients developed complications, such as hypertrophic scarring, contractures, and deformities with significant bone loss. Conclusion: Various degrees of skin or soft tissue defects were caused in children by car tires. In this study, patients were often also had tendon or bone damage. Proper and timely initial treatments are needed to reduce the incidence of infection, the number of operative procedures, and the hospital stay.

A Novel Network Anomaly Detection Method based on Data Balancing and Recursive Feature Addition

  • Network anomaly detection system plays an essential role in detecting network anomaly and ensuring network security. Anomaly detection system based machine learning has become an increasingly popular solution. However, due to the unbalance and high-dimension characteristics of network traffic, the existing methods unable to achieve the excellent performance of high accuracy and low false alarm rate. To address this problem, a new network anomaly detection method based on data balancing and recursive feature addition is proposed. Firstly, data balancing algorithm based on improved KNN outlier detection is designed to select part respective data on each category. Combination optimization about parameters of improved KNN outlier detection is implemented by genetic algorithm. Next, recursive feature addition algorithm based on correlation analysis is proposed to select effective features, in which a cross contingency test is utilized to analyze correlation and obtain a features subset with a strong correlation. Then, random forests model is as the classification model to detection anomaly. Finally, the proposed algorithm is evaluated on benchmark datasets KDD Cup 1999 and UNSW_NB15. The result illustrates the proposed strategies enhance accuracy and recall, and decrease the false alarm rate. Compared with other algorithms, this algorithm still achieves significant effects, especially recall in the small category.

Diagnostic Radiology and Conservative Management of L1 Lumbar Spine with Compression Fracture (L1 요추 압박골절에 대한 진단방사선학 및 보존적 치료)

  • Diagnostic radiology and conservative management for S75 patient with L1 lumbar fracture by traffic accidents were discussed with references, and then the obtained results were as follows ; 1. Wedging compression fractures with 10% deformity was confirmed at anterior vertebral body of L1 lumbar spine through lateral plain X-ray film. 2. Irregular bony fractures were observed at anterior vertebral body of L1 lumbar spine by CT scans, anatomically T12-L1 sites showed highly frequency of injuries, Denis's fracture type was classified as multiple compression fracture at anterior column without abnormal middle and posterior column, also no Cobb's angle, and then Frankel's neurological classification was E grade. 3. Orthopaedic treatments were performed with conservative methods. With rest on the bed, anti-in-flammatory medication, electrolyte and nutritional solution, the pain diminished. 4. After 3 weeks, rehabilitation was worked with putting on polyethylene back corset, although pains remained slightly until after 8 weeks, thereafter the spine showed gradually stability.

The Study on matrix based high performance pattern matching by independence partial match (독립 부분 매칭에 의한 행렬 기반 고성능 패턴 매칭 방법에 관한 연구)

  • In this paper, we propose a matrix based real-time pattern matching method, called MDPI, for real-time intrusion detection on several Gbps network traffic. Particularly, in order to minimize a kind of overhead caused by buffering, reordering, and reassembling under the circumstance where the incoming packet sequence is disrupted, MDPI adopts independent partial matching in the case dealing with pattern matching matrix. Consequently, we achieved the performance improvement of the amount of 61% and 50% with respect to TCAM method efficiency through several experiments where the average length of the Snort rule set was maintained as 9 bytes, and w=4 bytes and w=8bytes were assigned, respectively, Moreover, we observed the pattern scan speed of MDPI was 10.941Gbps and the consumption of hardware resource was 5.79LC/Char in the pattern classification of MDPI. This means that MDPI provides the optimal performance compared to hardware complexity. Therefore, by decreasing the hardware cost came from the increased TCAM memory efficiency, MDPI is proven the cost effective high performance intrusion detection technique.

  • The aim of this study is to examine how time-use and activities are affected by work hours. To achieve this, we focused on the weekday time-use of full-time workers in Seoul Metropolitan Area(SMA). The long 'work hours' are under active discussions since it is related to the quality of life. However, many Social researcher thought that problem of Korean working hours is linked to quality of life in the abstract. Because activity connects time-use and quality of life, the key point is activity under time constraints. Therefore, travel patterns should be understood by time-use and activity patterns. This study composes trip-chains from travel data of 2010 Household Travel Survey(HTS). Grouping trip-chains by activity patterns, we could make sure that a few of activities after work is affected by a short free time. This study has potential implications for the policy of work hours and traffic problems in the evening, and will provide new geographical perspective related to measuring quality of life.

  • Inhabitants make a specific cultural landscape in various ways according to his culture. In this paper, I consider the characteristics of cultural landscapes distributed in Sangju. I divide Sangju-City Area owing to physical criteria(relief and drainage basin}, appreciate them according to human criteria(administrative district of Joseon Period, dialects, periodical market area} and classify into 6 small regions: (1) Nakyang Region is the traditionally central place based on alluvial plain marginal to Byeongseong stream and Dong stream, and on hills of Sabeol. (2) Nakdong Region is the traffic center which the water transport of Nakdong river and the land transport of Yeongnamdaero cross each other. (3) The upstream region of Byungseong stream is the agricultural region connected with Nakyang Region. (4) Jungmo Region is a intermontane basin where grape cultivation using its specific climate flourish. (5) Wharyeong Region is a mountainous sightseeing district based on the beautiful scenery and Gyeonwhueon's remains of Sogni mountains and Yongyu valley. (6) Hamnyeong Region is the district of Goryeonggaya realm where 3 whites(cocoon, dried persimmon, rice) are abundant.

Vision-Based Fast Detection System for Tunnel Incidents (컴퓨터 시각을 이용한 고속 터널 유고감지 시스템)

  • Our country has so large mountain area that the tunnel construction is inevitable and the need of incident detection that provides safe management of tunnels is increasing. In this paper, we suggest a tunnel incident detection system using computer vision techniques, which can detect the incidents in a tunnel and provides the information to the tunnel administrative office in order to help safe tunnel operation. The suggested system enhances the processing speed by using simple processing algorithm such as image subtraction, and ensures the accuracy of the system by focused on the incident detection itself rather than its classification. The system is also cost effective because the video data from 4 cameras can be simultaneously analyzed in a single PC-based system. Our system can be easily extended because the PC-based analyzer can be increased according to the number of cameras in a tunnel. Also our web-based structure is useful to connect the other remotely located tunnel incident systems to obtain interoperability between tunnels. Through the experiments the system has successfully detected the incidents in real time including dropped luggage, stoped car, traffic congestion, man walker or bicycle, smoke or fire, reverse driving, etc.

A COMPARATIVE STUDY OF RADIOGRAPHIC IMAGINGS IN PATIENTS WITH MAXILLOFACIAL FRACTURES (상안면부 골절에 관한 방사선학적 비교 연구)

  • The subjects of this study consisted of 58 patients with Le Fort fractures, who were admitted to Chonbuk National University Hospital from Jan. 1988 to Oct. 1995. The author classified the maxillofacial fractures by Le Fort classification and examined the. incidence of Le Fort fractures by age, sex and etiology. The purpose of this study was to compare the imagings of conventional radiograms with those of computed tomograms and to aid in the diagnosis of patients with Le Fort fractures. The obtained results were as follows: 1. The Le Fort fractures occured mainly in 3rd and 4th decades, but there was no significant difference between decades. A ratio of men to women was 5.5 : 1. The major etiologic factors were traffic accident(69%), assault(14%) and fall-down(12%). 2. The most common type of Le Fort fracture was type I(49.3%). The numbers of Le Fort II, ill fractures were 41.3%, 9.4% respectively. And 8.6% were midsagittal splitting fractures. 3. The discontinuity and haziness of the maxillary sinus were easily detected by Waters' view. In skull PI A view, it was difficult to observe fractures due to overlapping of the other structures, but the haziness of the maxillary sinus was sometimes observed. 4. In Le Fort fracture, the fracture pattern of orbit, the wall of maxillary sinus, nasal bone and pterygoid plate were more easily detected in computed tomograms than in conventional radiograms.
