A Case of Idiopathic Tracheal Stenosis Treated with Tracheal Resection After a Retrievable Stent Insertion (스텐트 삽입술 후 수술적 절제술로 치료한 특발성 기관 협착증 1예)

  • Idiopathic tracheal stenosis is a type of benign stenosis that possesses specific characteristics but is of unknown origin. It is a rare disease characterized by extensive fibrosis of a portion of trachea, and predominantly found in women. The lesion presents as circumferential fibrotic stenosis that usually occurs at upper trachea and the subglottic larynx, but lower trachea may also be involved. Diagnosis is made from the clinical characteristics accompanide by compatible pathologic features and by exclusion of other etiologies. Conservative management such as laser resection, dilatation and stent insertion can be tried initially, but surgical resection is recommended for definitive treatment due to frequent restenosis and maintenance problems of conservative approach. We report a case of idiopathic tracheal stenosis treated with tracheal resection and anastomosis followed by insertion of a retrievable stent for immediate relief of airway obstruction.

Apoptosis in experimentally infected chicks with avian infectious bronchitis

  • This experiment was performed to investigate apoptosis during undergoing pathogenesis of avian infectious bronchitis virus(IBV)-infected chicks. Sixteen days old chicks were infected with IBV, Massachusetts-41 strain(M-41, 10$^4$-10$^{5}$ EID$^{50}$ ) experimentally, they were autopsied to remove trachea, lung, kidney and cloacal bursa at 6hr, 12hr, 1 day, 3 day and 7 day post infection(PI) respectively for H-E and TUNEL staining. Grossly, mild serous, catarrhal exudate was observed in the trachea, nasal passages and sinuses nasal from 4 day PI. The cloacal bursa was swollen from 3 day PI. Histopathologically, the trachea was seen mild cellular infiltration, edema of the mucosa and submucosa, vascular congestion and mild hyperplasia of the epithelium from 6 hr PI and the changes were seen a little more severely on 7 day PI. It was observed that the cloacal bursa was getting more and more hyperplasia through the experiment. The nuclei degeneration were shown in the kidney on 7 day PI. No specific changes were seen in the lung. In TUNEL analysis, apoptotic cells showed sharp increasing at 12 hr PI and reaching a maximum on 1 day PI in the trachea, lung, kidney and cloacal bursa. And then apoptotic cells decreased gradually returning to a level of the control by 7 day PI in all the removed organs.

  • Foreign body in the trachea & bronchus is considered as one of the frequently observed accidental cases in the field of E.N.T. and many cases & analysis of them are reported by many authors. We have, recently, experienced three cases of the foreign bodies in the trachea & bronchus and removed successfully through bronchoscopy under general anesthesia. Case A : a kind of plastic reed (2.2CM$\times$0.8CM$\times$0.8CM) was removed from the carina of trachea of a 7 year old female. Case B: a nail (4.9CM$\times$0.3CM$\times$0.3CM) was removed from the right main bronchus of a 2 year old male. Case C : a kind of plastic reed (2.0CM$\times$0.8CM$\times$0.8CM) was removed from the right main bronchus of a 7 year old female.

  • Tracheal diverticulum is relatively rare. It results from congenital or acquired weakness of the tracheal wall. Most cases are asymptomatic, but when symptoms are present, they are usually nonspecific. A 54-year-old man complained of sputum lasting for several months. Chest computed tomography showed an air-containing cystic structure in the trachea. Fiberoptic bronchoscopy demonstrated ostium arising from the right posterolateral wall at the trachea. Reported herein is a case of eosinophilic bronchitis associated with tracheal diverticulum.

  • Benign tracheal tumors are less common than malignant tumors, but they are relatively more important because they are operable and curable. Neurilemmoma is originated from Schwann cells of nerve sheath, which are characterized by benign, solitary, and encapsulated mass. Tracheal neurilemmomas are extremely rare and reported only 21 cases in the world literature. Recently, we experienced a case of neurilemmoma which arose from the trachea of a 48-year-old female patient who complained of progressive dyspnea. The tumor mass was removed successfully through bronchoscopic and tracheal fissure approach. The final pathological diagnosis viewed under a microscope after H&E stain was a neurilemmoma in which Antoni type A and type B both existed.

  • Background and Objectives: Primary benign masses in subglottis and trachea are rare. Symptoms of tracheal obstruction are similar to those of bronchial asthma, chronic bronchitis, as well as malignant lesions. Materials and Methods: Eight patients with benign tracheal masses from April 1992 through June 2001, at otolaryngology-head and neck surgery. department of Seoul national university hospital were studied by retrospective medical record review. Results : They were 3 females and 5 males aged from 0 to 57 years. The pathologies of the intratracheal masses were lipoma. tuberculosis, pleomorphic adenoma, hemangioma(two case), reparative giant cell granuloma, epithelial inclusion cyst and nonspecific lymphadenopathy, respectively. The most characteristic symptoms were dyspnea and stridor, both inspiratory and expiratory. Five of them had been treated as bronchial asthma. Conclusion: For the management of patients with the subglottis and tracheal masses, it is important to establish secure airway. regardless of pathology of the masses. The diagnosis should be considered in any patient with asthma-like manifestation, especially who fails to respond to medical treatment. It is necessary to examine the airway thoroughly, and chest and simple cervical X-ray may contribute to the diagnosis of possible intratracheal mass.

The Effects of Scrophulariae Radix on the Injured Tracheal Tissue Induced $SO_2$ in Rats (현삼(玄蔘)이 $SO_2$로 손상된 흰쥐의 호흡기 조직에 미치는 영향)

  • Objective : This experiment was performed in order to study the effects of Scrophulariae Radix on the injured tracheal tissue induced So, in rats. Methods : Healthy adult male rats weighting about 250g were divided into 4 groups - the Normal group, the Control group, the group of Scrophulariae Radix administration for 5 days after $SO_2$, gas exposure(Sample I), and the group of Scrophulariae Radix administration for 10 days before and for 5 days after $SO_2$, gas exposure(Sample II). Results : In the trachea Control group, the lesion of the ciliated epithelium was severe and the mucus secretion of the respiratory tract was increased significantly. In the trachea of Sample I group, the lesion of the ciliated epithelium and the mucus secretion of the respiratory tract were decreased compared with Control group. In the trachea of Sample II group, the lesion of the ciliated epithelium and the mucus secretion of the respiratory tract were decreased compared with Control and Sample I group. conclusions : According to the above results, Scrophulariae Radix has significant effects on the injuried tracheal tissue caused by $SO_2$ in rats.

  • A 20 year old woman had developed stenosis at the lower part of the trachea, right main bronchus, and right upper lobe bronchus as a complication of endobronchial tuberculosis. The patient had complained of severe dyspnea. Tracheobronchial stenosis was so extensive that we did reconstruction of the trachea and right bronchus with resection of the lower trachea and right main bronchus and right upper lobectomy. She has been doing well without any respiratory symptoms or complications.

  • This experiment was performed in order to study the effects of Gyumsuyukgunjun on the injured tracheal tissue induced $SO_2$ in rats. Healthy adult male rats weighting about 250g were divided into 4 groups - the Normal group, the Control group, the group of Gyumsuyukgunjun administration for 5 days after $SO_2$ gas exposure (Sample I), and the group of Gyurnsuyukgunjun administration for 10 days before and for 5 days after $SO_2$ gas exposure (Sample II). The results were obtained as follows ; 1. In the trachea Control group, the lesion of the ciliated epithelium was severe and the mucus secretion of the respiratory tract was increased significantly. 2. Comparison of Tracheal Anastomosis by Interrupted and Continuous Technique of Absorbable polydioxanone (PDS) Suture Material in Rabbit (가토에서 흡수성 PDS 봉합사의 단속봉합술식과 연속봉합술식에 따른 기관문합의 비교)

  • Background: Tracheal resection and end-to-end anastomosis are the preferred treatment for tracheal tumor or tracheal stenosis. Absorbable suture material has been commonly used in case of tracheal anastomosis. But anastomosis technique is controversial. This experimental study compared between interrupted suture and continuous suture using Polydioxanone (PDS) absorbable suture material after tracheal resection and end-to-end anastomosis in rabbits. Methods : Twenty-four rabbits were used, divided into two groups of 12 animals each. The trachea was resected and then anastomosed with continuous 6-0 polydioxanone (PDS) (group 1), and intermittent 6-0 polydioxanone (PDS) (group 2). The animals were followed up for 6 months. Stenosis of the trachea was assessed at the time of death. Results : In two groups, anteroposterior diameter, transverse diameter, cross sectional area, and perimeter were greater in group 2 than in group 1 but, not significantly different. In addition, histologic findings were not different in two groups. Conclusions : This study suggest that stenosis after tracheal resection and end-to-end anastomosis using absorbable polydioxanone (PDS) suture material be not significantly different in both groups.
