Evaluating the Performance of the Emergency Medical Services Index

  • Background: In 2006 Emergency Medical Services Index (EMSI), which summarizes the performance of regional emergency medical services system, was developed. This study assesses the performance of the EMSI to help determine whether EMSI can be used as evaluation tool. Methods: To build a composite score of the EMSI from predefined 24 indicators, 3 normalized values were calculated for each indicator, the normalized values of each indicator were weighted using 4 weighting methods, and the weighted values were aggregated into the final composite score using 2 aggregation schemes. The performance of EMSI was evaluated using 3 criteria: discrimination, construct validity, and sensitivity. Discrimination was the proportion of regions that did not include the overall median rank in the 5th to 95th percentiles rank interval, which was calculated from Monte Carlo simulation. Construct validity was a correlation among the alternative EMSIs. Sensitivity of EMSIs was evaluated by total shift of quartile membership and changes of 5th to 95th percentile intervals. Results: The total discrimination performance of the EMSI was 50.0%. Correlation coefficients between EMSIs using standardized values and those using rescaled values ranged from 0.621 to 0.997. Variation of the quartile membership of regions ranged from 0.0% to 75.0%. The total change in the 5th to 95th percentile intervals ranged from -19 to +17 places. Conclusion: The results suggested that the EMSI could be used as a tool for evaluating quality of regional EMS system and for identifying the areas for quality improvement.

Assessment of the Best Available Technology to Establish the Industrial Wastewater Effluent Limitations Guidelines (처리기술 수준에 근거한 배출허용기준 설정을 위한 BAT 도입 연구)

  • BAT (Best Available Technology economically achievable) assessment is indispensible to effective management policy of industrial wastewater discharging facilities such as setting effluent limitations guidelines for individual industry based on the wastewater characteristics and treatment efficiency, making effluent limitations guidelines for new sources, and calculation of the reducible amount of emission load conforming to a total emission general plan and enforcement plan. This study reviewed foreign cases of BAT assessment and condition of domestic wastewater discharging facilities, and suggested BAT assessment guideline suitable for the domestic conditions. The major assessment factors and flow are; (1) Identification of pollution source characteristics, (2) Identification and survey on technology, (3) Selection of candidate technology, (4) Assessment of legislation adaptation, (5) Assessment of environmental factor, (6) Assessment of economical acceptability, (7) Total weighting value factor assessment on technological factor, and (8) Total assessment and final BAT selection. The result of this study can be utilized as a major policy data and a base of industrial wastewater management system.

A Study on the Budget Allocation to Public Health Programs Using Matrix Delphi Technique (매트릭스 구성 델파이법을 이용한 공공보건사업 예산배분 연구)

  • This study was conducted to get a resonable set of budget allocation to public health programs. Matrix Delphi technique was used to obtain the logic of study results and eventually to form a human model which could predict opinion of professionals on budget allocation. Thirty-two professionals in academic and governmental area responded to Delphi survey. Questionnaire was developed using matrix formation, and the matrix was formed by 6 decision criteria on budget allocation and 26 public health programs. The decision criteria are as following: size of problem(morbidity), severity of problem, social equity, importance of prevention, technical feasibility and efficiency of programs. Severity of problem dropped out of the model because it had significant correlation with the size of problem. A total score of each program was obtained by weighting the relative importance of each criteria which also were given by survey respondents. These total scores indicate that the most important public health program is vaccination for infants and children in terms of budget allocation. Monitoring communicable diseases, mental health program, and anti-smoking program are the next. In addition, respondents were asked of the desirable budget size of each program. The result was rearranged by multiple regression model using the scores of each decision criteria. In this process, the current budget size of central government was provided to the respondents, and included in the model. h set of desirable budgets modified using tile model was obtained. Considering the current size of budget, tile results of the model is very different from that of the total score. Managing dementia is ranked the first. Health promotion program for the elderly, rehabilitation of the disabled and monitoring communicable diseases are the next. The need to increase the budget of vaccination for the infants and children was not found as so high. The matrix structure in Delphi survey gave us the precise basis to make optimal decision, and made it possible to develop an opinion predicting model. However the plentifulness and diversity of professional opinions were not fully obtained due to the limited number of decision criteria.

Survival Analysis of Patients with Brain Metastsis by Weighting According to the Primary Tumor Oncotype (전이성 뇌종양 환자에서 원발 종양 가중치에 따른 생존율 분석)

  • Purpose: This study was performed to retrospectively analyze patient survival by weighting according to the primary tumor oncotype in 160 patients with brain metastasis and who underwent whole brain radiotherapy. Materials and Methods: A total of 160 metastatic brain cancer patients who were treated with whole brain radiotherapy of 30 Gy between 2002 and 2008 were retrospectively analyzed. The primary tumor oncotype of 20 patients was breast cancer, and that of 103 patients was lung cancer. Except for 18 patients with leptomeningeal seeding, a total of 142 patients were analyzed according to the prognostic factors and the Recursive Partitioning Analysis (RPA) class. Weighted Partitioning Analysis (WPA), with the weighting being done according to the primary tumor oncotype, was performed and the results were correlated with survival and then compared with the RPA Class. Results: The median survival of the patients in RPA Class I (8 patients) was 20.0 months, that for Class II (76 patients) was 10.0 months and that for Class III (58 patients) was 3.0 months (p<0.003). The median survival of patients in WPA Class I (3 patients) was 36 months, that for the patients in Class II (9 patients) was 23.7 months, that for the patients in Class III (70 patients) was 10.9 months and that for the patients in Class IV (60 patients) was 8.6 months (p<0.001). The WPA Class might have more accuracy in assessing survival, and it may be superior to the RPA Class for assessing survival. Conclusion: A new prognostic index, the WPA Class, has more prognostic value than the RPA Class for the treatment of patients with metastatic brain cancer. This WPA Class may be useful to guide the appropriate treatment of metastatic brain lesions.

  • This study treats the criteria, considering the fuzziness occurred by optimization design. And we applied two weighting methods to show the relative importance of criteria. This study develops multi-objective optimization programs implementing plain stress analysis by FEM and discrete optimization design uniformaly. The developed program performs a sample design of 10-member steel truss. This study can carry over the multi-objective optimization based on total system fuzzy-genetic algorithms while performing the stress analysis and optimization design. Especially, when general optimization with unreliable constraints is cannot be solve this study can make optimization design closed to realistic with fuzzy theory.

Effect of Dietary Protein and Cysteine Levels on Cadmium Toxicity in Rats (식이 단백질과 cysteine 수준이 흰쥐의 Cadmium 중독에 미치는 영향)

  • This study was performed to investigate the effect of dietary protein and cysteine levels on cadmium toxicity in rats. Seventy-two male rats of Sprague-Dawley strain weighting 171$\pm$3g were blocked into 12 groups according to body weight, and were raised for 30 days. cadmium chloride was given at levels of 0 or 400ppm, protein at levels of 7, 15 and 40%, and cysteine was added(total dietary cysteine contents : 0.45%) to diet or not. The results are summarized as follow. Food intake, weight gain, food were lower than those of cadmium free group. But, these were increased with increasing dietary protein level and cysteine addition. Fecal cadmium excretion was remarkably increased in high protein (40%) groups. Thus, cadmium retention rates were decreased in high protein groups. Metallothionein concentrations in liver and kidney were increased in cysteine addition, and cadmium administration. Especially, these were remarkably increased in cadmium and cysteine added groups. Urinary calcium excretion was increased with cadmium administration, but urinary protein excretion and creatinine clearance were not changed in these animal. In conclusion, food intake, weight gain and organ weights were decreased with administration. Cadmium toxicity was alleviated by increasing fecal cadmium excretion, while cysteine addition increased metallothionein concentrations in liver and kidney. From these results, it was shown that cadmium toxicity was alliviated by synergistic effect of high protein level and cysteine addition.

  • Arrhythmia has recently emerged as one of the major causes of death in Koreans. Premature Ventricular Contraction (PVC) is the most common arrhythmia that can be found in clinical practice, and it may be a precursor to dangerous arrhythmias, such as paroxysmal insomnia, ventricular fibrillation, and coronary artery disease. Therefore, we need for a method that can detect an abnormal heart beat and diagnose arrhythmia early. We extracted the features corresponding to the QRS pattern from the subject's ECG signal and classify the premature ventricular contraction waveform using the features. We modified the weighting and bias values based on the error back-propagation algorithm through learning data. We classify the normal signal and the premature ventricular contraction signal through the modified weights and deflection values. MIT-BIH arrhythmia data sets were used for performance tests. We used RR interval, QS interval, QR amplitude and RS amplitude features. And the hidden layer with two nodes is composed of two layers to form a total three layers (input layer 0, output layer 3).

Application of a Multiobjective Technique for Optimum Operation of Pumps and Reservoirs in Service Water Transmission Systems (다목적 분석 기법을 이용한 상수도 송수계의 펌프와 배수지의 연계 최적 운영)

  • A multiobjective analysis technique was applied for the optimum operation of pumps and reservoirs in service water transmission systems. Three major objectives were identified and assessed on the normally operating service water transmission systems. They are, 1) stability of pump operation; 2) economic point of view in minimizing the energy cost for pumping; 3) reliability in meeting the stochasticaly varying demands. The measures of these objectives were required times of pump on-offs in stability, required total energy cost in economics, and minimum required storage during the operating horizon in reliability. In order to find the best meeting solution to the decision maker, a set of non-dominated solutions which show the tradeoff relationships between the considering objectives were generated. The DM selects the best solution from this explicit tradeoff relationships using his heuristic decision rules or experience. The theory was verified by applying to the Kumi Service Water System. A combined technique of the ${\varepsilon}-constraint$ and the weighting methods was used to generate the nondominated solutions, and the dynamic programming algorithm was applied to find the optimal solution for the discretized multi-objective analysis problems.

  • The multi-reservoir operation problem for efficient utilization of water resources involves conflicting objectives, and the problem can be solved by varying weight coefficient on objective functions. Accordingly, decision makers need to choose appropriate weight coefficients balancing the trade-offs among multiple objectives. Although the appropriateness of the weight coefficients may depend on the total amount of water inflow, reservoir operating policy may not be changed to a certain degree for different hydrological conditions on inflow. Therefore, we propose to use fuzzy Data Envelopment Analysis (DEA) to rank the weight coefficients in consideration of the inflow variation. In this approach, we generate a set of Paretooptimal solutions by applying different weight coefficients on Coordinated Multi-reservoir Operating Model. Then, we rank the Pareto-optimal solutions or the corresponding weight coefficients by using Fuzzy DEA model. With the proposed approach, we can suggest the best weight coefficients that can produce the appropriate Pareto-optimal solution considering the uncertainty of inflow, whereas the general DEA model cannot pinpoint the best weight coefficients.

Application of FMECA with Stochastic Approach to Reliability-Centered Maintenance of Electric Power Plants in Korean Power Systems (RCM 수립을 위해 발전설비의 고장확률을 고려한 확률론적 FMECA 평가 기법)

  • Preventive maintenance can avail the generation utilities to reduce cost and gain more profit in a competitive supply-side power market. So, it is necessary to perform reliability analysis on the systems in which reliability is essential. In this paper, RCM (Reliability -Centered Maintenance) analytical method is adopted using real historical failure data in Korean power plants. Therefore, the reliability -based Probability model for predicting the failures of components in the power plant is also established, and application to FMECA(Failure Mode Effects and Critical Analysis) consideration of failure probability, Based on the weighting ranking of generating equipments which status to be probability estimation by FMECA. The FMECA is an engineering analysis and a core activity performed by reliability engineers to review the effects of probable failure modes of generating equipments and assemblies of the power system on system performance. The results of this paper show that application of FMECA with stochastic approach to the preventive maintenance can efficiently avail decreasing the cost on maintenance and hence improve the total benefit.
