Changes in the Medical Cost and Practice Pattern according to the Implementation of per Diem Payment in Hospice Palliative Care (완화의료 일당정액수가제 시행에 따른 진료비와 진료행태의 변화)

  • Background: As of July 2015, per diem payment was changed from fee for service Therefore, this study aims to analyse changes in medical charges and medical services before and after enforcement of the palliative care, targeting palliative care wards in a general hospital, and provide basic data needed for development of per diem payment. Methods: The subjects of the study were a total of 610 cases consisting of 351 patients of service fee who left hospital (died) from July 2014 to June 2016 and 259 ones of per diem payment at Chosun University Hospital in Gwangju Metropolitan City. Results: The results are summarized as follows. First, after the palliative care system was applied, benefit medical service charges and insurance increased significantly (p<0.001). As benefit medical service charges increased, benefit private insurance payment increased significantly (p<0.001). Second, after the per diem payment was applied, total private insurance payment to medical institutes decreased significantly (p=0.050) and non-benefit also decreased significantly (p=0.001). Conclusion: It is suggested that additional rewards in the obligatory palliative care items should be continuously remedied and monitored to provide good quality hospice palliative care.

  • The purpose of this paper is to determine success factors important to electronic payment system and also to review its environments in order to present development direction of electronic commerce in Korea. A field survey including interviews and questionnaire surveys is made in 12 experts who were identified as having research experiences and/or development experiences of electronic payment systems, 35 merchants who are doing business over the Internet, and 96 netizens who were identified as a consumer group. In this study, we reviewed four dimensions affecting the acceptance of electronic payment systems such as safety, security, convenience, and efficiency. Finally, we suggested a total of 32 factors important to electronic payment systems and classified all factors into four dimensions.

A Study on the Determination of Optimum Level of Payments in the Direct Payment Program for Aquaculture Extruded Pellets (양식업 배합사료 직접지불제의 적정 지원수준 결정에 관한 연구 : 직접지불제의 생산 및 소득효과 분석을 중심으로)

  • This study is aimed at investigating optimum level of payments in the direct payment program for aquaculture extruded pellets both theoretically and practically by analyzing the effects on production and income in accordance with Green Box conditions of WTO Agreement on Agriculture. In particular, by analyzing both effects on production and income, it evaluates and compares payments affecting to the level of production and income, respectively. Analysis results indicate that the optimum level of payments in the direct payment program for aquaculture extruded pellets is determined by the growth rate of farmed fish, farming period, price and volume of extruded pellets, and additional amount of cost increase and decrease. Suppose that growth rates of farmed fish by extruded pellets(EP) and raw fish - based moist pellets(MP) are the same, it reveals the optimum level of payment should be lowered from the current level of payment. However, when the growth rate of farmed fish by EP is lower than that by MP, the optimum level of payment should be raised from the current level and total amount of payments by area should be increased as well.

TREE FORM CLASSIFICATION OF OWNER PAYMENT BEHAVIOUR

  • Contracting is said to be a high-risk business, and a common cause of business failure is related to cash management. A contractor's financial viability depends heavily on how actual payments from an owner deviate from those defined in the contract. The paper presents a method for contractors to evaluate the punctuality and fullness of owner payments based on historical behaviour. It does this by classifying owners according to their late and incomplete payment practices. A payment profile of an owner, in the form of aging claims submitted by the contractor, is used as a basis for the method's development. Regression trees are constructed based on three predictor variables, namely, the average time to payment following a claim, the total amount ending up being paid within a certain period and the level of variability in claim response times. The Tree package in the publicly available R program is used for building the trees. The analysis is particularly useful for contractors at the pre-tendering stage, when contractors predict the likely payment scenario in an upcoming project. Based on the method, the contractor can decide whether to tender or not tender, or adjust its financial preparations accordingly. The paper is a contribution in risk management applied to claim and dispute resolution practice. It is argued that by contractors having a better understanding of owner payment behaviour, fewer disputes and contractor business failures will occur.

Calculation of the Costs and Optimal profits per Inpatient-day of the Geriatric Hospitals (노인병원의 재원환자 1인당 일평균 원가 및 적정이윤 계산)

  • It has been asserted that per diem payment system should be introduced, in place of the current fee-for-service system, for payment of the inpatient services of the geriatric hospitals, Based on the assentation, this study aims at calculating costs and profits per inpatient-day of the geriatric hospitals, and thereby at contributing to the managerial improvement from the both sides of the Government and the hospitals. Relevant data of the three months, May to August, 2002 were collected from the five geriatric hospitals, and per inpatient-day costs and profits were calculated for the three disease groups. Major results and conclusions are as follow : Firstly, total costs per insured inpatient-day of the geriatric hospitals are 65, 389 won for dementia (including optimal profit of 3,858 won), 69,730 won for stroke (including optimal profit of 4,117 won), and 70,085 won for other diseases (including optimal profit of 4,134 won). Secondly, the amount of the non-insured costs per inpatient-day occupies 34.5% of the total costs for dementia, 30.3% for stroke, and 30.1% for other diseases. Thirdly, the total amount of the per inpatient-day costs calculated including the optimal profits is, on the average, higher by 12% than the present price level calculated for the current fee-far-service system. This implies that the present price level should rise by 12% when the current fee-far-service payment system be maintained, and Finally, introduction of a sliding-scale payment system should be considered for the inpatient medical management fees for the length of stay over six months or more that are being cut in the claim examination process by the insurance corporation.

The Effect of Inefficient Management on Debt Ratio in Public Institutions

  • This study investigated the determinants of debt ratio in public institutions. For this purpose, we analyzed the impact of inefficient management as internal factors on debt ratio. In this paper, inefficient management included total costs, payment, and employee benefit. The results of this study are as follows. First, we find that there is a significant positive relation between total costs and debt ratio. This result means that the higher total costs, the higher debt ratio. Second, we find that there is not a significant relation between payment and debt ratio. And we also find that there is not a significant relation between employee benefit and debt ratio. These results are empirical results that can be answers about some concerns that inefficient management of public institutions worsen debt ratio.

Study on optimal treatment payment by cost accounting in the artificiality kidney center in medical institutions (의료기관 인공신장실의 원가계산에 의한 적정수가에 관한 연구)

  • This study is to research cost accounting practice and to analyze propriety of patients' medical payment in artificiality kidney center. The researched cost datum of the year 2012 are as follows. - Hemodialysis medical treatment was reimbursed as much as 158,001 won in case of health insured patients, but payed-off as much as 135,810 won. - The average figure of the total hospitals and clinic center is 1,603,303 won, and one time cost of hemodialysis treatment is 154,487 won. Optimal treatment pay are suggested as follows. First, Regardless of the notified classification from MOHW(Ministry of Health and Welfare), 136,000 won of fixed price payment classification needs to be reclassified by patients, severity and tobe rearranged by fixed price payment system of hospitals. Second, Fixed payment code notified by the Ministry of Health and Welfare is recommended to be simplifies and to reflect according to contents of the medical treatment rendered to patients. Third, Establishment of artificial kidney center has to be risk managed because of its huge investment. Fourth, Cost analysis model has to be maintained as basis together with appropriate application of conversion index model mixed with SGR model.

  • Purpose The purpose of this study is to investigate which decision factors continue to use the service based on the online simple payment service. In this study, we will examine how to activate the service from the continuing factors through the dual-model theory consisting of personal dedication and constraint dedication which are frequently used in relation marketing. Design/methodology/approach In this study, we investigated the activation of online simple settlement service using factors affecting the preference of the simple payment service based on the dual-model of dedication-based mechanism and constraint-based mechanism. Findings This study confirmed that customer satisfaction and convenience under the dedication -based mechanism were found to be linked to the preference of the simple payment service. Switching cost under the constraint - based mechanism showed that the customer had no binding ability to continue using the service of the current provider. Likewise, preference was found to have a direct effect on the actual frequency of use of the simple payment service, the number of uses compared to the total number of online payments, and willingness to use more.

  • Advances in information and communication technology have introduced self-service technologies (SSTs) to service companies, changing the way they serve. In addition, as SSTs are expanding for low cost and high efficiency, opportunities for technology to interact with consumers have increased. Restaurant managers have the advantage of the SSTs win-win strategy, which allows consumers to freely receive the products or services they want, but some consumers have low satisfaction of the SSTs, especially elder people and the blind. This study examines the effect of unmanned order payment technology on service quality evaluation. For this, we interviewed a total of 12 consumers in their 10s to 50s with the experiences of using unmanned order payment technology, applying the Grounded Theory Method(GTM). Through a review of the research participant interview materials, totally 84 concepts were derived and the concepts were analyzed to derive 15 subcategories and 10 upper categories. Finally, a paradigm model of the effect of unmanned order payment technology on service quality assessment was derived. It also found the importance of consumers' sense of obligation to use unmanned order payment technology, recognition of advantages (e.g., time savings), recognition of shadow labor, and recognition of whether the technology usage process is in the service quality assessment stage.

A Study on Intention to Adopt Digital Payment Systems in India: Impact of COVID-19 Pandemic

  • Digitalization and digital transformations have metamorphized the face of Financial Inclusion globally, more so, in cash obsessed economies like India. The purpose of our study is to empirically analyze the users' intention to adopt digital payment systems, post Demonetisation, during the COVID-19 pandemic in India. The conceptual framework for the study is based on the Unified Theory of Acceptance and Use of Technology (UTAUT) adoption model with added operationalized constructs of Perceived Risk and Stickiness to use Cash. A total of 326 respondents were surveyed using a pre-tested questionnaire during the Nationwide Lockdown 3.0 in India. These responses were analyzed using Partial Least Squares - Structural Equation Modelling (PLS-SEM) technique. The findings of the study revealed that performance expectancy and facilitating conditions directly influence the intention of individuals to use digital payment systems, whereas the effect of perceived ease of use on digital payment systems is mediated through the attitude towards the digital payment systems during COVID-19 pandemic situation. Implications of the proposed adoption model are discussed. This will enable the other developing economies to formulate a digital ecosystem, that is here to stay even after the pandemic.
