Nonconstrained Total Elbow Arthroplasty with or without Radial Head Resection (요골 두 절제 유무에 따른 비구속형 전 주관절 성형술)

  • Purpose : To evaluate the effect of radial head resection on longevity in nonconstrained total elbow arthroplasty, Materials and Methods : The results of 20 cases of radial head replaced Pritchard ERS and 18 cases of radial head excised Kudo elbow was followed for 6 to 16 years, 10.6 years in average. The rate of loosening, osteolysis, radial head subluxation, joint dislocation and valgus deformity were compared between the two groups. Results'There was no exaggerated cubitus valgus in Pritchard ERS group. However there developed one case of delayed subluxation of radial head occurred in 6 years postoperatively, one case of loosening, and one case of sponta- neous fracture of humeral shaft due to osteolysis. In Kudo elbow group,5 cases (27.8%) showed exaggerated valgus deformity with instability. There were one case of loosening and one case of delayed dislocation occurred in a year postoperatively. Conclusion : The most clear feature of Kudo elbow was exaggerated cubitus valgus. However, the ostolysis was developed in both groups and there was no difference in rate of the other complications between the two groups.

Clinical Result of Combined Radiotherapy and Hyperthermia Induced by 915 MHz Microwave and Ultrasound in Locally Advanced Malignant Tumors of Head and Neck (915 MHz 극초단파 및 초음파를 이용한 온열치료와 방사선치료 병합치료에 의한 두경부암의 치료성적)

  • Thirty five lesions of 35 patients with locally advanced malignant tumors of head and neck were received thermoradiotherapy with ultrasound and/or 915 MHz microwave. Most of all patients were failed with previous conventional therapeutic trial. Hyperthermia had been done immediately after radiotherapy, twice a week, $43^{\circ}C$ for one hour and radiotherapy had been done 5 fractions per week with a fraction size of 2 Gy up to total 30 to 60 Gy. Conclusions are as follows; 1) Total response rate (CR+PR) of thermoradiotherapy with microwave and ultrasound was 80%. 2) Tumor depth, minimum temperature of tumor center, number of heat fraction and irradiation dose were statistically significant factors affecting response. 3) Hyperthermia with microwave and ultrasound can be used efficiently to control locally advanced malignant tumors in head and neck whether previously received near tolerance dose of radiotherapy or not.

  • The anterior mediastinal tracheotomy(AMT) facilitates resection of stomal recurrence after total laryngectomy and tumors involving the cervicothoracic trachea and esophagus. An 81-year-old-man came to our clinic due to the progressive dyspnea during three months. He received the total laryngectomy five years ago. We diagnosed as Sisson type I stomal recurrence and then performed the wide excision, both selective neck dissection, sternal manubrium resection and AMT. Before surgery, we planned the pectoralis major myocutaneous flap. Unluckily we could not fulfill this procedure because of patient's medical status during anesthesia. The tracheocutaneous fistula was observed in the second postoperative day. He expired due to the huge bleeding from the wound. When AMT is performed, exact manipulation of major vessels and adequate flap are mandatory these elevate the feasibility of AMT.

  • Avascular necrosis of the femoral head is a debilitating disease that leads to destruction of tho hip joint in patients who are in the third, fourth, or fifth decade of life. The total societal cost is so high because these patients are generally young, hold jobs, and have a considerable life expectancy. Therefore, we have to delay or eliminate the need for total hip replacement as possible as we can. The purpose of this review is to provide a current perspective of avascular necrosis of the femoral head.

  • Renal cell carcinoma is the third most common metastatic tumor to the bone and soft tissues of the head and neck. The common sites of metastatic renal cell carcinoma in head and neck region are nasal cavity, paranasal sinuses, oral mucosa, gingiva, tongue, palate, lip as the favored site. The present paper deals with one patient with metachronous oral tongue, nasal cavity and suspicious brain metastases after 2 years of renal cell carcinoma nephrectomy. Also, the patient had history of total thyroidectomy for thyroid follicular carcinoma. Total excision of nasal cavity and tongue mass were performed. Therapeutic aspects are briefly reviewed in literature.

  • Sternal recurrence has been defined as a diffuse infiltrate of neoplastic tissue at the junction of the amputated trachea and the skin. The overall prognosis is poor, resulting from progressive tracheostomal obstruction or massive hemorrhage due to erosion of major vessels. Neither radiation therapy nor chemotherapy has demonstrated any efficacy in controlling these sternal recurrence. Surgery, especially mediastinal dissection, may benefit only an occasional patient. Recently authors experienced one case of mediastinal dissection for sternal recurrence after total laryngectomy for laryngeal cancer. We report our case with a brief review of literature.

A Simple Estimation of the Viscous Resistance of Ships by Wake Surveys

  • Several formulae have been proposed to estimate the viscous resistance of ships by wake surveys. Both the total head and the velocity should be measured. The integration of he total head loss shows over estimations of the resistance by about 10%. Therefore measurements of the velocity are required, which need much more works. A simple method is suggested in this paper to take accout of the velocity-defect from the measured total head. It gives reasonable estimations of the viscous resistance within the experimental accuracy. Experimental data of a low-drag body of revolution in the wind-tunnel and Series 60 model, CB=0.6 in the tank are used to verify the suggested formula.

  • This study was conducted for verification and systematization of estimation method about the headloss using CFD(Computational Fluid Dynamics). Head loss which happens between the inlet and outlet of in-line mixer can be a major factor for the design and construction. Also, this Case studies about the sensitivity related to the velocity in the piping system. As result, program's default calculation function was used to get each side's total pressure and the differential of each total pressure could be defined as head loss from in-line mixer. In the case of adopting pipe surface friction factor and geometry loss, Calculation residual can be much more reduced. It was found that residual of value between CFD method and field test ranged about 3 through 18 precent.

A Study on a Perfomance Analysis of the Centrifugal Pump Impeller using CFD (CFD에 의한 원심펌프 임펠러 성능해석에 관한 연구)

  • A commercial CFD code is used to calculate the 3-D viscous flow field within the centrifugal pump impeller. Design conditions are changed by impeller inlet(9.3mm, 12.2mm) and outlet breadth(6.65mm, 6.85mm). Numerical calculation was performed by changing flow rate from 8 to 26m$^{3}$/hr. Computation results shows that total head is increased at the larger inlet and outlet breadth than the others. And when the flow rate is increasing, the total head was decreased. The maximum efficiency of pump is shown at the design flow rate(16m$^{3}$/hr). In this study shows that the calculated results are good agreements with analysis results of design condition.

Effect of The Impeller Discharge Angle on the Performance of a Spurt Vacuum Pump

  • The spurt vacuum pump is widely used to transfer sludge and slurry, and to control flow rate in a variety of processing fields, such as the oil, chemical, and fiber industries. The efficiency of the pump depends on the design parameters of the impeller, such as the number of blades, and the blade angle. In this study, the effect of the configuration of the impeller discharge angle of a spurt vacuum pump, which influences total head, shaft power, and efficiency, was numerically investigated using the commercial code, ANSYS CFX ver. 16.1. In addition, the performance of the pump was evaluated on the basis of the correlations between the total head, pump efficiency, and pressure distribution.
