Post-extraction pain in the adjacent tooth after surgical extraction of the mandibular third molar

  • Background: After tooth extraction, pain due to dry socket and pain in the adjacent tooth are common. The aim of this study was to retrospectively analyze pain in the adjacent tooth after surgical extraction of the mandibular third molar. Methods: Postoperative pain due to dry socket, pain in the adjacent tooth, and pain from other causes were present. Group A included patents with dry socket alone; group B included patients with pain in the adjacent tooth alone; and group C included patients with both. The duration of symptoms was recorded. In addition, the prognosis of pain was divided into the complete improvement, improvement, maintenance, deterioration, and complete deterioration groups. Results: A total of 312 mandibular third molars were extracted from 13, 60, and 10 patients in groups A, B, and C, respectively. The mean duration of symptoms was 5 days in group A and B and 15.2 days in group C. There were statistically significant differences in the duration of symptoms between groups A and C and groups B and C. Conclusion: Pain in the adjacent tooth after third molar extraction can be caused by inflammatory reactions and pressure on this tooth. The pain caused by pressure on the periodontal ligament and alveolar bone results from the cytokines released by osteoclasts, which are responsible for bone destruction. However, pain from periodontal ligament damage caused by excessive pressure may be misunderstood as pulpal pain. Unconscious parafunctional habits, such as clenching and bruxism, could also be associated with post-extraction pain.

청소년의 구강건강행태와 구강보건상태와의 관련요인 (Association between Oral Health Behavior and Oral Health Status in Adolescent)

  • Purpose : This study was conducted to improve cognitive oral healthcare care by analyzing the correlation between oral health behavior and oral health status Korean adolescents. The subjects were 65,528 youths in 798 schools monitored in the Twelfth Korea Youth Risk Behavior Web-based Survey in 2016. Methods : The general characteristics consisted of 9 items including sex, grade, and academic performance. The academic performance and economic status were restructured into "upper, middle, lower". Subjective health perception and subjective oral health perception were reorganized as "health", "normal", "no health". The sub-areas of oral health behavior include five times of daily brushing during the day yesterday. The number of brushing times was "0", "1-2", "3", "more than 4" After lunch at school, brushing was restructured as "always," "sometimes," "not." The oral health status subscale consisted of 6 items including tooth breakage and toothache at the time of eating. Result : Tooth fracture(43.7 %), pain in mastication(43.7 %), tooth tingling(47.4 %), gum pain and bleeding(45 %), pain in buccal mucosa(43.8 %) and halitosis(46.6 %) were rare in adolescent having healthy oral health status cognitively(P<.001). Nosymptom adolescents without dental sealant showed low tooth fracture(0.64 times), pain in mastication(0.67 times), tooth tingling(0.59 times), gum pain and bleeding(0.84 times), pain in buccal mucosa(0.76 times), and halitosis(0.90 times). Nosymptom adolescents without scaling showed low tooth fracture(0.88 times), pain in mastication(1.03 times), tooth tingling(0.82 times), gum pain and bleeding(0.64 times), pain in buccal mucosa(0.70 times) and halitosis(0.82 times). Conclusion : This study revealed that oral health status is correlated with oral health behavior. Development of educational media and program, and aggressive promotion required to establish oral health Korean adolescent.

Topical anesthesia for stainless steel crown tooth preparation in primary molars: a pilot study

  • Background: Placement of full-coverage restorations such as stainless steel crowns (SSCs) for pulpectomy treated primary molars is essential for successful outcomes. The tooth preparation process for SSCs can cause discomfort to gingival tissues since the crown should be seated 1 mm subgingivally. The purpose of this prospective trial was to compare the effectiveness of subgingival and transmucosal application of topical anesthetics on dental pain during SSC tooth preparation among 6- to 8-year-old children. Methods: A consecutive sample of 27 children, aged 6-8 years, who required an SSC after pulp therapy in primary molars were randomly divided into three groups. Group A received infiltration anesthesia before tooth preparation for SSC placement, whereas in Group B and C, only topical anesthesia was applied subgingivally and transmucosally. Wong-Bakers Faces pain rating scale (WBFPS) scores were recorded after tooth preparation. Faces, Legs, Activity, Cry and Consolability (FLACC) scores were evaluated by two blinded and calibrated investigators through video recordings of the patient during tooth preparation. Data were tabulated, and inter-group comparisons were performed using the Kruskal-Wallis and analysis of variance tests. Results: Out of the 27 participants, 48% were boys and 52% were girls, with an overall mean age of 6.83 years. Group A showed the least pain scores according to both the scales, followed by Group B and Group C. The pain intensity was statistically significant on both the pain scales with P = 0.003 for FLACC and P < 0.001 for WBFPS. Conclusion: Subgingival application of topical anesthesia reduced pain to a certain extent but not as effectively as infiltration anesthesia during SSC tooth preparation in primary molars. Transmucosal application of topical anesthesia did not reduce discomfort when compared to the other two interventions.

악관절 내장환자의 전치부 접촉에 관한 연구 (A Study on the Anterior Tooth Contacts of Patient with Internal Derangement of TMJ)

  • The aim of this study was to investigate the state of anterior tooth contacts in patient with internal derangement of TMJ. We have investigated the change of the number of tooth contacts after conservative treatment including stabilization spint in 83 patients with TMJ internal derangemnet who visited the Orofacial Pain Clinic, Dept of Oral Diagnosis, Seoul National University Dental Hospital. The obtained results were as follows : 1. The gender ratio of patients was 1:3.7(M:F) 2. The number of patients with bony changes in TMJ was 17(20.5%) 3. The number of patients with absence of incisor tooth contacts was 73(88%) 4. The number of Tooth contacts after treatment was decreased compared with that before treatment in both groups. However there was no significant difference between groups 5. There was the decrease in the number of tooth contacts in 12(76%) of 17 patients with bony changes in TMJ and in 23(34.8%) of 66 patients without bony changes in TMJ. 6. The ratio of patients who showed the decrease of the number of tooth contacts was higher in patients with bony changes in TMJ than patients without bony changes in TMJ. 7. After conservative treatments with stabilization splints in patients with TMJ internal derangement, the possibility of the decrease in the number of tooth contacts had no relationship with the presence of incisor tooth contacts. However, the incidence was higher in patients with bony changes in TMJ.

Osteomyelitis of the Mandibular Coronoid Process Mimicking a Temporomandibular Joint Disorder: A Case Report

  • Osteomyelitis of the jaw is an inflammatory process of the bone marrow that is caused by odontogenic local infection and trauma such as tooth extraction and fractures in the oral and maxillofacial region. The clinical signs include pain, swelling, pus formation, and limited mouth opening. Chronic osteomyelitis presents a diagnostic challenge because of the variability of symptoms across different disease stages and varying health conditions of the patients. This report presents a case of osteomyelitis that was misdiagnosed as a temporomandibular joint disorder (TMD) after tooth extraction. The patient was treated for inflammation after tooth extraction in the early stage; however, as the osteomyelitis progressed chronically, symptoms mimicked those of a TMD. The patient was finally diagnosed with osteomyelitis 6 months after tooth extraction. A review of this case and relevant literature revealed the necessity for a differential diagnosis of chronic osteomyelitis that mimics TMD symptoms.

Charcot-Marie-Tooth Disease 환자에 대한 한방치료 1례 (Clinical Study on the Case of Charcot-Marie-Tooth Disease Treated with Korean Medicine: A Case Report)

  • Objectives : The purpose of this study is to report the effect of Korean Medicine treatment on a patient with Charcot-Marie-Tooth disease. Methods : A 60-year-old woman who was diagnosed as Charcot-Marie-Tooth disease was admitted with both lower and upper limbs weakness, difficulty in walking, palm pain and neck pain. The patient was treated with acupuncture, electroacupuncture, herbal medicine, cupping therapy and physical treatment from 2th September 2013 to 14th November 2013. Improvement of the patient's symptoms was evaluated by numeric rating scale(NRS), visual analog scale(VAS), SF-36 bodily pain and patient global assessment(PGA). Results : After treatment, pain Intensity evaluated by VAS was significantly decreased(from 7.2 to 2). Increased SF-36 bodily pain score(from 10 to 67.5) showed that patient's quality of life has been improved. Conclusions : These results suggest that Korean medicine treatment may be effective in reducing the symptoms of Charcot-Marie-Tooth disease.

측두하악장애 유병률과 구강관련 악습관과의 연관성에 관한 연구 (A Study on Correlation between the Prevalence Rate of Temporomandibular Disorder and Oral Habits)

  • Purpose: This study is intended to survey male and female university students to see if there is correlation between temporomandibular disorder and oral habits known to cause temporomandibular disorder. Methods: A survey was conducted to 400 male and female university students attending two universities located in Daegu Metropolitan City and Gyeongsangbuk-do to examine if there is correlation between oral habits and temporomandibular disorder. Among total 400 questionnaire sheets, 334 (83.5%) sheets were used for final analysis except for those unreliably answered or containing error. Results: Female students showed a higher prevalence rate of temporomandibular joint pain than male students, and those having the habit of resting jaw on hand indicated a higher prevalence rate of temporomandibular joint pain, temporomandibular joint clicking, or trismus than normal students not having it. Also, those having the habit of tooth clenching showed a significantly higher prevalence rate of temporomandibular joint pain or temporomandibular joint clicking than normal students not having it. Conclusion: It can be concluded that the habit of resting jaw on hand is closely related with temporomandibular disorder like temporomandibular joint pain, temporomandibular joint clicking, or trismus, and the habit of tooth clenching is significantly correlated with such symptoms as temporomandibular joint pain and temporomandibular joint clicking.

근막동통으로 인한 하악 구치부 연관통의 임상증례 (A Case Report of Referral Pain on Mandibular Toothache Originated from Myofascial Pain)

  • 치아의 통증을 호소하는 환자 중 임상검사 및 방사선학적 검사소견에서 특기할 병적 소견이 관찰되지 않는 경우 비치성 치통을 의심해보아야 하며, 근육질환, 상악동염, 신경병성 통증, 신경혈관 질환 등의 원인으로 치통이 유발될 수 있다. 이러한 비치성 치통의 경우 진성 치통과 구별하기 위하여 철저한 병력 조사 및 전반적인 임상검사가 시행되어야 하며, 진단용 국소마취를 통하여 보다 정확한 진단을 확립할 수 있다. 이러한 비치성 치통의 가장 흔한 원인 중의 하나인 근막동통은 근육조직을 촉진 시 단단한 띠가 만져지는 것이 특징적이며 이것이 발통점으로 작용하여 근육이 뻣뻣한 느낌과 피로감, 연관통, 치아에 전이되는 통증, 근긴장성 두통, 통각과민 등의 증상을 유발할 수 있으며, 특히 교근은 상악 구치부 및 하악 구치부의 통증을 유발한다. 본 증례는 우측 교근부의 근막동통에 기인한 하악 우측 구치부의 치통 양상을 운동요법, 물리치료, 약물치료 등의 통상적이고 가역적인 근육질환의 치료법을 통해 증상의 호전을 보인 증례이다. 비치성 치통은 정확하게 진단되지 않을 경우 근관치료, 치주치료, 발치 등의 불필요한 치과치료가 시행될 수 있으며, 이러한 치료를 통하여도 환자의 통증은 경감되지 않기 때문에 치과치료가 시행되기 전에 반드시 정확한 감별진단이 필요하리라 사료된다.

샤르코 마리투스 병 환자의 족관절 통증에 대한 족관절 추나를 병행한 한의학적 치료 증례보고 (A Case Report on Ankle Pain Induced with Charcot Marie Tooth Disease treated by Traditional Korean Medicine Treatment with Chuna Manual Therapy)

  • Background : Charcot Marie Tooth disease can cause muscle weakness and foot deformity. Ankle pain induced by foot deformity affect patients' gait pattern and quality of life. Objectives : The purpose of this study is to evaluate the traditional Korean medicine treatment for ankle pain induced with Charcot-Marie Tooth Disease, especially Chuna manual therapy on ankle joints. Methods : One patient was treated with acupuncture, phamacopuncture, herbal medication and chuna manual therapy on ankle joints. To evaluate the pain of ankle, lower back and lower extremity, visual analog scale(VAS) was measured. Results : After treatment for 5 week, the pain of ankle joint was declined from VAS 6 to VAS 2. Conclusions : Traditional Korean medicine treatment including acupuncture, pharmacopuncture, herbal medication and Chuna manual therapy is effective for ankle pain with foot deformity. But further studies are required to prove the effectiveness of Chuna manual therapy on ankle joints.

Preliminary Study on the Age Estimation through a New Scoring System of Tooth Wear

  • The authors evaluated the degree of permanent tooth wear in order to make the basic date that are necessary to estimate the age. 10443 teeth of 389 individuals with known age were selected to estimate the degree of tooth wear. The obtained results were as follows : 1. The degree of tooth wear was decreased in males and females with aging. (P<0.001) 2. The degree of tooth wear in males in higher than that of females.(P<0.05) 3. The decision coefficient increased gradually from anterior teeth to posterior teeth.
