The relationship between past smoking period and tooth loss in Korean elderly (한국 노년층의 과거흡연기간과 상실치아의 연관성)

  • The purpose of this study was to evaluate the relationship between past smoking period and tooth loss in Korean elderly based on the data the Sixth Korea National Health and Nutrition Examination Survey (2013-2015). The study subjects were 4,037 elderly people (1,737 men and 2,300 women) over 65 years old. The statistical analyses were conducted by applying complex sample analysis technique using SPSS/WIN 22.0(${\alpha}=0.05$). The average number of tooth loss in Korean elderly was 11.14. The analysis result of the relationship of the tooth loss according to the past smoking period, the risk of tooth loss was more than twice higher in over 30 years past smoking period group than in under 10 years past smoking period group. In conclusion, the long term past smoking were associated with tooth loss in Korean elderly. Therefore, it seems to need a continuous oral health care through non-smoking policy and education.

Comparison of tooth movement and biological response in corticotomy and micro-osteoperforation in rabbits

  • Objective: The aim of this study was to evaluate the amount of tooth movement and histologic changes with different corticotomy designs and micro-osteoperforation in rabbits. Methods: The sample consisted of 24 rabbits divided into three experimental groups (triangular corticotomy [TC] and indentation corticotomy [IC] with flap, and flapless micro-osteoperforations [MP]) and a control. A traction force of 100 cN was applied by connecting the first premolars to the incisors. The amount of tooth movement was measured. Kruskal-Wallis test was used to assess differences in tooth movement between the groups. Micro-computed tomography, hematoxylin and eosin staining, and tartrate-resistant acidic phosphatase (TRAP) analysis were performed. Analysis of variance was applied to assess differences in TRAP-positive osteoclast count between the groups. Results: The amount of tooth movement increased by 46.5% and 32.0% in the IC and MP groups, respectively, while the bone fraction analysis showed 69.7% and 8.5% less mineralization compared to the control. There were no significant intergroup differences in the number of TRAP-positive osteoclasts. Conclusions: The micro-osteoperforation group showed no significant differences in the amount of tooth movement compared to the corticotomy groups, nor in the TRAP-positive osteoclast count compared to both corticotomy groups and control.

The effect of drug holiday before tooth extraction on the development of medication-related osteonecrosis of the jaw in cancer patients receiving intravenous bisphosphonates

  • Objectives: Drug holidays are suggested to reduce the formation of osteonecrosis in patients under intravenous (IV) bisphosphonates (BPs) therapy. The objectives of this study are to evaluate the incidence of medication-related osteonecrosis of the jaw (MRONJ) following tooth extraction in cancer patients using IV BP, and to assess the effect of drug holiday on the development of MRONJ. Patients and Methods: A manuel search of the patient folders of Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, Hacettepe University was undertaken to identify cancer patients who used IV BPs and had at least one tooth extraction between 2012 and 2022. Patents' age, sex, systemic condition, the type of BP used, duration of BP used, number of tooth extraction, duration of drug holiday, localization of tooth extraction and incidence of MRONJ were recorded. Results: One hundred nine teeth were removed from 57 jaws in 51 patients. All tooth extractions were performed under perioperative antibiotic prophylaxis and with primary wound closure. The incidence of MRONJ was 5.3%. Stage 1 MRONJ developed in 3 patients (only one had a drug holiday). The median duration of drug holiday was 2 months. No significant difference between the patients with and without a drug holiday and MRONJ development was found (P=0.315). The mean age of patients developed MRONJ was 40.33±8.08 years. A statistically significant difference was found between age and MRONJ development (P=0.002). Conclusion: The effect of a short-term drug holiday on the development of MRONJ may be limited because BPs remain in bone tissue for a long time. Drug holidays should be applied with the approval of an oncologist with other preventive measurements.

A study on the error of tooth size measurements (치아크기 계측오차에 관한 연구)

  • Precise and accurate measuring of tooth size is essential procedure in orthodontic diagnosis. The purpose of this study was to evaluate the reproducibility and accuracy of tooth size measurements with regard to tooth and investigator. Five investigators measured the size of one side of teeth, from central incisor to first molar, on 20 sets of casts that show a moderate degree of crowding. Mesio-distal width was measured with digital vernier calipers in each tooth and this procedure was repeated three times at two weeks interval. To obtain a standard measurement for each tooth, dental casts were cut into individual tooth, and its width was measured with micrometer. The difference between the measurement from dental cast and the standard measurement was defined as the measurement error. Through various statistical analyses, following results were obtained. 1. The reproducibility did not show significant differences with regard to tooth or investigator. 2. The measurement error showed some difference with regard to tooth and investigator. 3. The magnitude of the measurement error showed increasing tendency from anterior to posterior teeth with maximum value in the first molar. 4. While the measurements obtained on study casts generally showed smaller number compared to standard measurements, the direction of the difference showed variability according to tooth or investigator. 5. The measurement errors did not show significant correlations with the degree of crowding. The results of the present study indicate that the possibility of tooth size measurement error should be taken into consideration when diagnosing an orthodontic case.

Patient related and tooth related risk factor of tooth loss after periodontal surgical treatment - prospective study (치주수술 후 치아상실에 대한 환자관련, 치아관련 위험요소 - 후향적 연구)

  • Purpose: The purpose of this study is to analyze the prognostic factors related to tooth loss after 5 years of periodontal surgery in periodontal disease patients. Materials and Methods: From January to December 2017, 22 patients and 124 teeth who underwent periodontal treatment through periodontal surgery were targeted. At the time of treatment, the measured values were evaluated after recording the maximum probing depth, average periodontal probing depth, number of root, furcation involvement, pulp vitality, and prosthesis state on the day of periodontal surgery. Based on the initial records at the time of visit, patient-related factors were gender, age, smoking, tooth loss due to periodontal disease at the time of first visit, diabetes, and maintenance period. The influence of each factor on tooth loss was evaluated. Results: As a result of examining the influence of tooth-related factors on tooth loss, the maximum probing depth depth (P: 0.000), bone loss (P: 0.021) was found to have a significant effect on tooth loss. Conclusion: As a result of examining the influence of patient-related factors on tooth loss, any variables had no significant effect. Bone loss, maximum probing depth acted as statistically significant prognostic factors for tooth loss in patients who underwent periodontal surgery.

EFFICIENCY OF ULTRASONIC ROOT-END RETROGRADE CAVITY PREPARATION AND ITS INFLUENCE ON TOOTH STRUCTURE (초음파기구의 치근단 역충전와동형성 효율 및 치질에의 영향)

  • The purposes of this study were to evaluate the efficiency of cavity preparation and to determine the incidence of tooth crack when root-end retrograde cavity preparation was done with ultrasonics. 91 distobuccal root-ends of extracted human maxillary first molars were cut by 3 mm perpendicularly to the long axis of tooth using a slow speed diamond saw, retrocavities were prepared using a slow-speed no. 2 round bur as controls, and stainless steel ultrasonic tips of power settings of 1 through 10 as experimentals. Time consumed and the number of strokes used for the cavity preparation were measured and evaluated, and the incidence of tooth cracks was observed under a stereomicroscope. The results were as follows : For the retrograde cavity preparation, time and number of strokes used were decreased as the ultrasonic power setting increased (p<0.001). High power setting of ultrasonics induced significantly more tooth cracks than did the slow-speed bur or low- and medium power setting of ultrasonics (p<0.05). Teeth with previous crack induced significantly more tooth cracks than those without previous one when high power setting of ultrasonics were used for the retrograde cavity preparation (p<0.001). Teeth with initial apical canal size of no. 10 induced significantly more crack than did those with size of no. 15 when low power setting of ultrasonics were used for the retrograde cavity preparation (p<0.05).

Quantitative Ultrastructural Analysis of Endings Presynaptic to the Tooth Pulp Afferent Terminals in the Trigeminal Oral Nucleus

  • The ultrastructural parameters related to synaptic release of endings which are presynaptic to tooth pulp afferent terminals (p-endings) were analyzed to understand the underlying mechanism for presynaptic modulation of tooth pulp afferents. Tooth pulp afferents were labelled by applying wheat-germ agglutinin conjugated horseradish peroxidase to the rat right lower incisor, whereafter electron microscopic morphometric analysis with serial section and reconstruction of p-endings in the trigeminal oral nucleus was performed. The results obtained from 15 p-endings presynaptic to 11 labeled tooth pulp afferent terminals were as follows. P-endings contained pleomorphic vesicles and made symmetrical synaptic contacts with labeled terminals. The p-endings showed small synaptic release-related ultrastructural parameters: volume, $0.82{\pm}0.45{\mu}m^3$ ($mean{\pm}SD$); surface area, $4.50{\pm}1.76{\mu}m^2$; mitochondrial volume, $0.15{\pm}0.07{\mu}m^3$; total apposed surface area, $0.69{\pm}0.24{\mu}m^2$; active zone area, $0.10{\pm}0.04{\mu}m^2$; total vesicle number, $1045{\pm}668.86$; and vesicle density, $1677{\pm}684/{\mu}m^2$. The volume of the p-endings showed strong positive correlation with the following parameters: surface area (r=0.97, P<0.01), mitochondrial volume (r=0.56, P<0.05), and total vesicle number (r=0.73, P<0.05). However, the volume of p-endings did not positively correlate or was very weakly correlated with the apposed surface area (r=-0.12, P=0.675) and active zone area (r=0.46, P=0.084). These results show that some synaptic release-related ultrastructural parameters of p-endings on the tooth pulp afferent terminals follow the "size principle" of Pierce and Mendell (1993) in the trigeminal nucleus oralis, but other parameters do not. Our findings may demonstrate a characteristic feature of synaptic release associated with p-endings.

  • The fusion is an anomalous union of two or more tooth germ at some stage in the development process resulting in a decreased number of teeth in the dental arch. Fusion is common odontogenic anomaly but triplication of primary teeth is a rare phenomenon. We report a case of a 3 - year - old girl who visited our clinic for the pain on maxillary anterior tooth. The patient was diagnosed by the fusion of a the maxillary primary right central and lateral incisor with a supernumerary tooth and caries of dentine. And then, Endodontic treatment and composite resin restoration were performed on the triple teeth. After follow-up of 6 months period, there was no marked complications.

  • Non-circular gear has a good velocity ratio in high speed and heavy load without any slip, moreover, it can transmit various motion, using simpler structure than link and cam, automation mechainism. In case of designing and manufacturing non-circular gear. I suggest one of references in applying non-circular gear to industrial plant, and suitable range of application by pressure angle curvature and angle ratio
