Esthetic improvement in the patient with one missing maxillary central incisor restored with porcelain laminate veneers

  • This article describes esthetic improvement in a patient with a missing maxillary left central incisor. Space analysis of the anterior dentition showed that minor tooth rearrangement was needed. Optimal space distribution for restorations was attained by orthodontic treatment. Through transforming tooth shape with porcelain laminate veneers, the maxillary left lateral incisor was transformed into central incisor and the maxillary left canine into a lateral incisor. The maxillary right central incisor was also restored for esthetic improvement. In a case of changing a tooth shape with porcelain laminate veneers, pre-treatment evaluation, space analysis and diagnostic wax-up are important factors.

유치조기발거후 보극장치물이 치궁발육에 미치는 영향 (Longitudinal Study on Effects of Dfntal Arch Growth in the Case of Using a Space-Maintaier After Primary Tooth Extraction)

  • The author observed the changes of dental arch dimensions and compared spae-maintainer group with non-space-maintainer group after primary tooth extraction. The results were as follows 1) The rate of growth of dental arch of expeirmental group in intercanine width and in inter-1st molar width was more rapid than that of the control group. 2) Arch length is almost no dimensions but it was slightly decreased in lower arch. 3) Dental arch growth and tooth eruption were stimulated by space-maintainer appliance, so it is the best way to replace a new appliance at least once a year. 4) It is thought that satisfactory conclusions in this observation must be researched successively until the mixed dentition completes.

Acupuncture Therapy and Herbal Medicine Accelerating Temporal Space Abscess after Tooth Extraction: A Case Report

  • Numerous oral and maxillofacial surgeons have found facial space infections after tooth extraction. Most of these infections can be managed easily, but some of them could be life-threatening. Among the facial infections, temporal space infections are rare. Most temporal space infections could be observed as secondary to maxillary third molar infections, maxillary sinusitis, and maxillary sinus fractures. Note, however, that there are insufficient studies on temporal space abscess due to mandibular second molars, especially with acupuncture. A 74-year-old female came to our hospital with severe trismus and facial swelling on the right temporal, buccal, posterior auricular, and cervical regions. The patient had undergone extraction of tooth #47 secondary to dental caries by a general dentist about a month ago. After the dental procedure, the patient had been treated with acupuncture therapy around the right temporomandibular joint area at the oriental medicine clinic. We performed emergency incision and drainage under general anesthesia and started antibiotic treatment with IV ampicillin/sulbactam 3 g every 24 hours and vancomycin 1 g every 24 hours for 5 days. The patient's symptoms subsided and ultimately disappeared. Temporal space abscess after mandibular molar extraction is quite rare. In this case, the spreading mechanism against gravity is considered to be acupuncture therapy.

  • Impaction of tooth is a situation in which an unerupted tooth is wedged against another tooth or teeth or otherwise located so that it cannot erupt normally. The supernumerary tooth is also called as hyperdontia and defined as the condition of having additional tooth to the regular number of teeth. The most common supernumerary tooth is a mesiodens, which is a mal-formed, peg-like tooth that occurs between the maxillary incisors. The supernumerary tooth is commonly impacted but they are frequently impacted on maxilla. Ectopic impaction of supernumerary tooth on mandibular condyle, coronoid process, ascending ramus, and pterygomandibular space is very rare condition. In this case, we report a case of impacted supernumerary tooth on mandibular sigmoid notch without definite pathologic change.

흰쥐의 악골에 동종 이식한 태아 치아싹의 장기간 발육 (THE LONG-TERM GROWTH OF HOMOGENEOUS EMBRYO TOOTH GERM TRANSPLANTED INTO THE MAXILLA OF A RAT)

  • 치아우식증이나 치주질환으로 치아를 상실한 경우 여러 가지 방법으로 수복 치료할 수 있으나 그 중에서 임플란트와 치아 이식에 대한 관심이 높아지고 있다. 최근에는 치아 이식에 대한 성공률을 높이기 위해서 치아를 형성시키고 발육시키는 과정에 관한 많은 연구가 이루어지고 있다. 치아싹 이식은 생체 내와 생체 외에서 연구되고 있고, 생체 내 이식은 쥐와 생쥐, 고양이, 개 등 여러 동물에서 시행되고 있다. 이러한 생체 내 이식은 대부분 구강 외에서 시행되고 있으며 구강 내 이식에 대한 연구는 드물다. 본 연구는 악골 내에 이식한 치아싹이 발육되고 석회화되는지 관찰하기 위하여 성숙한 흰쥐의 상악 제 1 구치를 발치하고 그 발치와에 임신 13.5일 된 태아쥐에서 모상기의 하악 제 1 구치의 치아싹을 이식하고 2, 6 개월 후 희생하여 방사선학적 그리고 조직학적으로 관찰하여 다음과 같은 결과를 얻었다. 12 개월과 6개월 동안 악골 내에 이식한 치아싹은 석회화된 치아 조직이 형성 되었고 치아 조직에서 상아질과 백악질, 치수 조직이 관찰되었으며 법랑질 공간 주변은 상피로 둘러싸여 있었다. 2. 6개월 동안 구강 상피 하방에 위치한 치아 조직은 상피로 둘러싸여 있었고 주위에 치주인대 및 결합조직이 관찰되었다. 3. 이식한 시간이 경과함에 따라 치아 형성이 진전되었으나 치아 조직은 크기가 작았고 형태학적으로 완전하지 못하였다.

보철치료를 위한 치아 이동 (Adjunctive orthodontic therapy for prosthodontic treatment)

  • Adjunctive orthodontic therapy may be required to create appropriate space, to address misalignment and tooth size discrepancy for better function and esthetics. The desired interproximal alveolar contour and gingival embrasure form can be developed during treatment. Various applications of temporary anchorage devices for pre-prosthetic tooth movement allowed clinicians to achieve high efficiency, shorter treatment time, and less discomfort of patient. Biomechanical considerations for the periodontal status of the affected teeth are required to successfully control the vertical and horizontal space. Hence, the interdisciplinary approaches have an essential role in maximizing the favorable treatment outcome. In particular, pivotal Clinical decisions such as whether to open or close the space should be made by consensus of the involved dentists. This article presents the orthodontic treatment approaches for prosthodontic works including mesiodistal and vertical space regaining specially for cases of unrestored teeth over an extended period of time.

대학병원 응급실로 내원한 치과 응급환자에 관한 임상적 연구 (A clinical study on the dental emergency patients visiting an University Hospital emergency room)

  • Introduction: In today's society, the rapid and appropriate care of the dental emergency patients is much more important. So, a retrospective study on the characteristics of emergency dental injuries and diseases will be very meaningful. Materials and Methods: This retrospective clinical study was carried by reviewing the radiographic films and emergency chart of 11,493 patients who had visited the emergency room of Hallym Sacred heart Hospital and were treated in the Department of Oral and Maxillofacial Surgery from January 2006 to December 2010. Results: The male to female ratio was 1.9:1. The highest monthly incidence was observed in May (10.4%) and June (8.9%) and the peak age distribution was the first decade (56.0%), followed by the second decade (16.0%). Trauma was the most common cause in dental emergency patients, followed in order by toothache, odontogenic infection, temporomandibular joint (TMJ) disorder and oral hemorrhage. Soft tissue injury was most prevalent in the trauma group, followed by tooth injury and facial bone fractures. In the tooth injury group, tooth fracture (56.7%) showed the highest incidence followed in order by tooth subluxation (18.2%), tooth concussion (16.9%), tooth avulsion (11.5%) and alveolar bone fractures (3.7%). In the facial bone fracture group, mandibular fractures (81.8%) showed the highest incidence followed in order by maxilla fractures (15.7%), nasal bone fractures (9.0%), zygomaticomaxillary complex fractures (5.4%), orbital bone fractures (2.5%). In mandibular bone fractures, the most common location was the symphysis (70.1%), followed in order by the mandibular angle (33.0%), mandibular condyle (22.8%) and mandibular body (13.6%). In the infection group, a submandibular space abscess (46.2%) was most common followed in order by a buccal space abscess (17.4%), canine space abscess (16.9%) and submental space abscess (12.3%). TMJ dislocation (89.3%) showed the highest incidence in the TMJ disorder group, followed by TMJ derangement (10.7%). In the other group, a range of specific symptoms due to post operation complications, trigeminal neuralgia, chemical burns and foreign body aspiration were reported. Conclusion: For the rapid and appropriate care of the dental emergency patients, well-organized system should be presented in oral and maxillofacial surgery. And it is possible under analysis of pattern and the variation of the dental emergency patients.

Comparison of clinical and histological characteristics of orthodontic tooth movement into recent and healed extraction sites combined with corticotomy in rats

  • Objective: This study was performed to investigate the rate of tooth movement and histological characteristics of extraction sockets those were subjected to corticotomy. Methods: A split-mouth randomized controlled trial experiment was designed. Thirty-two adult, male Wistar rats were divided into 2 groups: healing extraction socket (H) and recent extraction socket (R); these groups were randomly classified into 4 subgroups (0/7/21/60 days). The first maxillary molar was extracted on 1 side and 2 months were allowed for complete bone healing; then, the corresponding molar was extracted on the other side and surgical intervention was performed at the mid-alveolar point of the first maxillary molar. Ten grams of continuous force was applied. The outcomes measured were rate of tooth movement, percentage of periodontal space and histological evaluation. The rate of tooth movement was calculated as the measured distance divided by the duration of molar movement. Histomorphometric evaluations were performed on the second and third maxillary molars. The Wilcoxon signed rank test was used to compare differences between the two groups. Results: There were no significant differences in the rates of tooth movement between H and R groups at any of the 4 time points. The histological appearance and percentage of periodontal space between the R and H groups also demonstrated no significant differences. Conclusions: The rates of orthodontic tooth movement into recent and healed socket sites did not differ between the groups. Histological analysis of tooth movement revealed regional acceleration during every time period.

구개측 매복된 상악 견치의 교정적 치험례 (ORTHODONTIC TREATMENT OF THE PALATALLY IMPACTED MAXILLARY CANINE)

  • An impacted tooth is defined pathologically as a tooth that remains under the mucosa of inside bone without eruption of the crown after a specific period of eruption. Clinically, the term includes those teeth, even before eruption period, that are not expected to erupt due to shape, position and alignment of tooth and lack of space. Canine is prone to impaction more than other teeth because it has the longest time to develop and a complex route from the place of formation to the site of eruption. The impaction incidence of maxillary canine is repoted 0.92$\sim$3.3% (Ferguson, 1990). In 1995 Orton reported that the incidence was 0.92$\sim$2.2% and palatal impaction was more frequent than labial impaction(85%:15%). In 1969 Johnston presented it was more common to woman than to man(3:1). The etiology includes systemic disease such as endocrine disorder, cleidocranial dysostosis, irradiation, Crouzon syndrome, ricketts, facial hemihypertrophy and hereditary and local problems such as ectopic position of the tooth, distance of tooth from its place of eruption, malformation of the tooth, presence of supernumerary teeth, trauma of tooth germ, infection of tooth germ, displacement of tooth germ or tooth by a neoplasm, ankylosis, overretention of deciduous predecessor, lack of space for the tooth in the dental arch and mucosal barrier due to gingival fibrosis. The maxillary canine is especially important as it has the longest root, provides guidance for lateral movement of the mandible and masticatory function and assumes an important role esthetically as it is located at mouth angle. If left untreated, it may cause migration and external, internal resorption of adjacent teeth, loss of arch length, formation of dentigerous cyst or tumors, infection and referred pain as well as malposition of the tooth. Therefore, periodic examination of the development and eruption of the maxillary canine is especially important in a growing child. This case study presents the results of treatment of palatally impacted maxillary canine utilizing surgical exposure and orthodontic tooth movement on patients visiting SNUDH dept. of pediatric dentistry.

매복 하악 견치의 자가이식술을 이용한 치험례 (Autotransplantation of impacted mandibular canine)

  • Autotransplantation is the transplantation of embeded, impacted, or unerupted tooth, into extraction socket or surgically prepared socket in the same individual. Clinically, successful autotransplantation must show radiolucent space(periodontal ligament space) between transplanted tooth and supporting bone, lamina dura, no root resorption, no ankylosis, no inflammatory change, and physiologic tooth mobility. It is important that procedure is atraumatic, and the instruments should not contact the root surface during procedure. We performed autotransplantation of impacted mandibular canine that transversely located beneath the apices of the mandibular incisors with uncompletely developed apex. In radiographs and clinical evaluation, this transplant showed successful clinical finding except irregularity of mesial root surface after 14 months. It is conclued that transplantation of canine with $\frac{1}{2}{\sim}\frac{3}{4}$ root development provides a good chance of pulp survival, limited risk of root resorption and ensures sufficient final length, and is thus recommended.
