Objective : To evaluate the relationship between the dose of Clodronate and serum level of alkaline phosphatase (ALP), calcium (Ca), and phosphate (PO4) during orthodontic tooth movement MaterialS and MethodS: A total of 18 sex-matched Wistar rats (weight=180~230g, mean age=8 weeks) were allocated into the 2.5mM Clodronate (2.5C) group, 10mM Clodronate (10C) group, or control group (n=6 for each group). After the application of a nickel-titanium closed coil spring (force of 60g) between the upper central incisors and first molars (UFM), 2.5C, 10C, or saline was injected every third day into the subperiosteum of the alveolar bone adjacent to UFM for the experimental and control groups. The animals were sacrificed 17 days later. Trunk blood was quickly collected into a heparinized tube and centrifuged at 2,000 rpm for 20 min. The plasma was used for the biochemical assays of the serum level of ALP, Ca, and PO4. Kruskall-Wallis test and Mann-Whitney test with Bonferroni correction were performed for the statistical analyses. Results : Dose-dependent increase in the level of ALP (P<0.01) and decrease in the level of Ca (P<0.001) were observed among the control, 2.5C, and 10C groups. Although there was no significant difference in PO4 between the 2.5C and 10C groups, the 10C group showed a significantly higher level of PO4 than the control group (P<0.01). Conclusion : Since Clodronate induced significant dose-dependent change in the serum level of ALP, Ca, and PO4 during orthodontic tooth movement, orthodontists should consider these biochemical markers not only as a diagnostic tool for bone turnover rate but also as a monitoring tool for orthodontic tooth movement.

Objective: The purpose of this study was to compare the three dimensional changes of tooth movement using four different types of maxillary molar distalization appliances; pendulum appliance (PD), mini-implant supported pendulum appliance (MPD), stainless steel open coil spring (SP) and mini-implant supported stainless steel open coil spring (MSP). Methods: These experiments were performed using the Calorific $machine^{(R)}$ which can simulate dynamic tooth movement. Computed tomography (CT) images of the experimental model were taken before and after tooth movement in 1 mm thicknesses and reconstructed into a three dimensional model using V-works $4.0^{TM}$. These reconstructed images were superimposed using Rapidform $2004^{TM}$ and the direction and amount of tooth movement were measured. Results: The mean reciprocal anchor loss ratio at the first premolar was 17 - 19% for the PD and SP groups. The appliances using mini-implants (MPD or MSP) resulted in less anchorage loss (7 - 8%). On application of a pendulum appliance or MPD, distalization was obtained by tipping rather than by bodily movement. Furthermore, the maxillary second molar tipped distally and bucally. But on application of MSP, distalization was achieved almost by bodily movement. Conclusions: Regarding tooth movement patterns during molar distalization, stainless steel open coil spring with indirect skeletal anchorage was relatively superior to other methods.
The purpose of this study was to investigate the alveolar bone turnover in diabetic rat, and to compare the alveolar bone turnover during tooth movement in diabetes with that in normal control Eighty Male Sprague-Dawley strain rats(8th week) were divided into normal control(N), normal-tooth movement (N-tm), diabetes(D), and diabetes-tooth movement(D-tm) groups. Eighteen days before the start of the experiment, diabetes was induced with a single injection of streptozotocin 50 mg/kg of body weight in citrate buffer as vehicle via the tail vein. Maxillary first molars of rats were moved mesially by 40 grams of the closed coil spring. Experimental animals were sacrificed after 1d, 3d, 7d, and 14d experimental period, and the alveolar bone around the maxillary first molars were assayed biochemically for acid phsophatase(ACP) and tartrate-resistant acid phosphatase (TRAP) as bone resorption markers, and alkaline phosphatase(ALP) and osteocalcin(OC) as bone formation markers. TRAP and OC concentration in serum and alveolar bone of D group were lower than those in N group, and especially OC concentration decreased mote following diabetes prolonged, which showed the decreased skeletal and alveolar bone resorption and formation potential in diabetic rats. In N-tm group compared with N group, alveolar bone ACP and TRAP concentrations were highest at 1d and 3d(p<0.01), decreased after then, and showed lowest at 14d, and alveolar bone OC concentration was higher at 3d, 7d, and 14d(p<0.001) and showed a tendency of peak level at 7d. which showed the peak of concentration of bone resorption markets at 1d-3d and those of bone formation markers at 7d. In D-tm group compared with N group, alveolar bone ACP and TRAP concentrations were higher at 3d, 7d and 14d(p<0.001), and tended to reach peak value at 7d and persisted through 14d, and alveolar bone ALP and OC concentration increased but not different from that of N group. The amount of tooth movement in D group were greater than that of N group at all experimental period. Those results were suggested that during diabetes, the alveolar and skeletal bone undergo low bone turnover and the mote amount of tooth movement, hut because the peak time of alveolar bone resorption activity was delayed and sustained in longer period of tooth movement and alveolar bone formation activity is lower than that of normal tooth movement, the periodontal space is supposed to be larger doting tooth movement.
Bone remodeling is a complex process regulated by various mediators. Cytokines are known to be associated with the mechanically induced response in orthodontic tooth movement. In particular, IL-$1\beta$ stimulates bone resorption and induces osteoclast proliferation. The purpose of this study was to identify and quantify IL-$1\beta$ in human gingival crevicular fluid(GCF), and to investigate the changes in its level during orthodontic tooth movement. Twelve patients(mean age of 19.2 years) were used as the subjects. An upper canine of each patient haying treatment lot distal movements served as the experimental tooth, whereas the contralateral was used as the control. The GCF of compression and tension side of the experimental teeth and the GCF of mesial side of control teeth was taken from the each subject immediately before activation, and at 1, 24, and 168 hr after initiation tooth movement. IL-$1\beta$ amount was detected by ELISA. The concentration of IL-$1\beta$ was higher in experimental group than in the control group after treatment. Its level was elevated after initiation of tooth movement and it was the highest level at 24 hr in compression side of experimental group. But there was no significant change in control group. The results indicate that the change in IL-$1\beta$ level in GCF is associated with orthodontic tooth movement.
Author came to the following conclusion and made the following report as a result of corticotomy that treated in department of orthodontics and oral surgery, Tokyo Dental College. 1. In the respect of oral surgery, corticotomy is able to operate under local anesthesia. This operation is very simple and there is little clinical discomfort after operation. 2. In the respect of orthodontics, tooth movement is 2-3 times rapid than common orthodontic treatment in adult and clinical problem such as pain, root resorption are slight. Especially, tooth movement by differential force, rapid expansion in adult and unilateral expansion which was difficult, came to possible. 3. Corticotomy shorten the treatment time in preoperative orthodontic treatment of developmental abnormality of jaw, application to cleft lip & palate, orthodontic treatment before prosthetics. Recent technological advance have greatly expanded the application of invisible orthodontic treatment using clear thermoplastic materials. However, the final outcomes using clear aligner system do not achieve the level of final goal frequently, which results in case refinement, midcourse correction, or fixed orthodontic treatment. Therefore, mechanical properties of thermoplastic materials should be considered to improve the quality of outcomes. The purposes of this special article were to evaluate the force and stress depending on the materials, deflection and thickness of thermoplastic materials and to evaluate the mechanical properties of thermoplastic materials after repeated loading. Thickness and amount of deflection rather than products and materials showed the largest effect on force and stress. In all products, at least 159 gf of force was required for more than 1.0 mm deflection or when materials with 1.0 mm thickness were deflected. Orthodontic forces delivered by thermoplastic materials depend on the materials, thickness, amount of activation, and intra-oral condition. Proper thickness of thermoplastic materials and deflection level of tooth movement should be decided for the efficient and physiologic tooth movement.

Corticotomy was introduced as a surgical procedure to shorten orthodontic treatment time. Corticotomy removes the cortical bone that strongly resists orthodontic force in the jaw and keeps the marrow bone to maintain blood circulation and continuity of bone tissues to reduce risk of necrosis and facilitate tooth movement. In the 21st century, the concept of regional acceleratory phenomenon was introduced and the development of the skeletal anchorage system using screw and plate enabled application of orthopedic force beyond conventional orthodontic force, so corticotomy has been applied to more cases. Also, various modified methods of minimally invasive techniques have been introduced to reduce the patient's discomfort due to surgical intervention and complications after surgery. We will review the history of corticotomy, its mechanism of action, and various modified procedures and indications.
Optimal orthodontic treatment could be possible when a orthodontist can predict and control tooth movement by applying a planned force system to the dentition. The moment to force(M/F) ratio at the bracket, has been shown to be a primary determinate of the pattern of tooth movement. As various n/F ratios are applied to the bracket on the tooth crown, strain distribution in periodontium can be changed, and the center of rotation in tooth movement can be determined. It is, therefore, so important in clinicalorthodontics to know the strain distribution in a force system of a M/F ratio. The purpose of this study was to analyze the strain distribution in orthodontic force system by strain gauge attached to tooth root, and to evaluate the usage of the method. For this study, an experimental upper anterior arch model was constructed, where upper central incisors, on the root surface of which, 8 strain gauges were attached, were implanted In the photoelastic resin, as in the case of 4mm midline diastema. Three types of closing of upper midline diastema closure were compared : 1. with elastomeric chain(100g force) in no arch wire, 2. elastomeric chain in .016“ round steel wire, 3. elastomeric chain in .016”x.022“ rectangular steel wire. The results were as follows. 1. Strain distributions on labial, lingual, mesial and distal root surface of tooth were able to be evaluated with the strain gauge method, and the patterns of tooth rotation were understood by presuming the location of moment arm. 2. Extrusion and tipping movement of tooth was seen in closing in no arch wire, and intrusion and bodily movement was seen with steel arch wire inserted.

Tooth movement facilitated by corticotomy and distraction osteogenesis, a new paradigm in orthodontics, was discussed. Molar uprighting was thought to be either difficult or nearly impossible. In this study, a section of cortical bone, which may act as resistance to tooth movement in alveolar bone, was removed. Active bone deposition was also made possible in the tension side This forms the main concept of tooth movement facilitated by corticotomy and distraction osteogenesis. Molars in two cases were uprighted at such a speedy rate as we could not imagine in conventional tooth movement. which lead to reduction of the total treatment period. And molar uprighting was possible without side effects. for example, supraversion oif the tooth or anterior anchorage loss. These were the superior aspects to conventional orthodontics.
Purpose: The aim of this study was to evaluate the degree of achievement of anterior tooth movement using a thermoplastic clear aligner by superimposing pretreatment, setup, and post-treatment models and to analyze the factors that influence it. Materials and Methods: Forty patients who underwent clear aligner treatment at the Department of Orthodontics were included in the study. The pretreatment, setup, and post-treatment models were superimposed, and the achievement rates were evaluated. We analyzed the factors that caused changes in achievement rate on each condition. Result: The average achievement rate of total tooth movement was 57.65%. Mesiodistal and anteroposterior displacements were achieved better than vertical displacement and rotation. In each displacement, posterior, mesial, and extrusive movements were better achieved than the rotation. Spacing was better corrected than crowding, especially in the mesiodistal direction. Most of the tooth movements were achieved better when using 3-step thicknesses of aligners were used. Conclusion: The treatment results of clear aligners may differ depending on the type of tooth movement, age and other treatment conditions. Clinicians should consider unfavorable conditions based on the results of this study.
