A Study on the Expression of Connexin 43 in the Experimental Tooth Movement of Rat (백서의 실험적 치아이동시 connexin 43의 발현에 관한 연구)

  • Bone remodeling in response to force requires coordinated actions of osteoblasts, osteoclasts, osteocytes, and periodontal ligament cells. Coordination among these cells may be mediated, in part, by cell-to-cell communication via gap junctions. This study was designed to evaluate the expression of gap junction, connection 43 In periodontal tissue during the experimental movement of rat's incisors, by LSAB(labelled streptavidine biotin) immunohistochemical staining for connexin 43. Twenty seven Sprague-Dawley rats were divided into a control group(3 rats), and 6 experimental groups(24 rats) where 75g of force was applied from helical springs across the maxillary incisors. Rats of experimental groups were sacrificed at 12 hours, 1, 4, 7, 14 and 28 days after force application, respectively. And the tissues of a control group and experimental groups were studied immunohistochemically. The results were as follows : 1. In control group, the expression of connexin 43 was rare in gingiva, dentin, cementum, periodontal ligament, and bone cells. 2. In experimental group, the expression of connexin 43 was increased in pulp, periodontal ligament, osteoblasts, and osteoclasts, comparing to that in control. And it was rare in gingiva, dentin, and odontoblasts regardless of the duration of force application, which was not different from that of control group. 3. The expression of connexin 43 in pulp of experimental group began to increase in 4-day after force application and got to the highest degree at 7-day. And it decreased after 14-day to be similar to that of control group at 28-day. 4. The expression of connexin 43 in periodontal ligament was noted in small capillaries adjacent to alveolar bone, showing higher intensity of immunolabelling after 4-day And it was stronger in the pressure side than in tension side of periodontal ligament. After 7-day, decrease in connexin 43 expression was observed. 5. The expression of connexin 43 in alveolar bone began to increase 1-day, reached to the highest degree at 4-day, and decreased at 7-day. And the expression in osteoclasts was more than that in osteoblasts or osteocyte at 7-day.

Experimental brush wear pattern and cariostatic effect of Biscover (Biscover의 잇솔질에 따른 마모양상과 항우식 효과)

  • Objective: The aim of this study was to investigate the experimental brush wear pattern of a light cured surface sealant, Biscover (Bisco, Schaumburg, IL), and to evaluate its cariostatic effect. Methods: Caries-free human premolars were used for the Biscover coating group (n = 90) and the control group (n = 10). The Biscover coating group was randomly assigned to nine subgroups of 10 each and the control group was assigned to two subgroups of 5 each according to the number of brushing strokes. An experimental 3-body wear test was conducted under different strokes of wear test. Tooth-brushing was accomplished with movement of each brush head set at a frequency of 100 rpm under a force of 1.5N, Surface roughness was tested before, and after Biscover coating, and after brushing. Then, each of the 10 teeth of both groups were placed in artificial caries inducing solution for 7 days. All tooth surfaces were assessed using scanning electron microscopy. Results: Biscover coated surfaces showed a smoother texture than enamel surfaces. The roughness was increased after experimental brushing and after 10,800 brushing strokes, the whole layer of Biscover wore out. However, teeth in the Biscover coating group had a cariostatic effect in cariogenic conditions. Conclusions: We suggest that white lesions in orthodontic patients can be suppressed by topical applications of Biscover.

The vertical location of the center of resistance for maxillary six anterior teeth during retraction using three dimensional finite element analysis (상악 6전치부의 후방견인시 저항중심의 수직적 위치에 관한 3차원 유한요소법적 연구)

  • The delivery of optimal orthodontic treatment is greatly influenced by clinician's ability to predict and control tooth movement by applying well-known force system to dentition. It is very important to determine the location of the centers of resistance of a tooth or teeth in order to have better understanding the nature of displacement characteristics under various force levels. In this study, three dimensional finite element analysis was used to measure the initial displacement of the consolidated teeth under loading. The purpose of this study was to define the location of the centers of resistance at the upper six anterior segment. To observe the changes of six anterior segment, 200gm, 250gm, 300gm, and 350gm forces at right and left hand side each were imposed toward lingual direction. For this study, two cases, six anterior teeth and six anterior teeth after corticotomy, were reviewed. In addition, it was reviewed the effects of changes on the location of the center of resistance in both cases based on different degree of forces aforementioned. The results were that : 1. The instantaneous center of resistance for the six anterior teeth was vertically located between level 4 and level 5, which is, at 6.76mm, $44.32\%$ apical to the cementoenamel junction level. 2. The instantaneous center of resistance for the six anterior teeth after corticotomy was located vertically between level 4 and level 5, that is, at 7.09mm $46.38\%$ apical to the cementoenamel junction level. 3. Changes of force showed little effect on the location of the center of resistance in each case. 4. It was observed that the location of the instantaneous center of resistance for the six anterior teeth after corticotomy was changed more than the six anterior teeth without corticotomy to the apical part, and the displacement of the consolidated anterior teeth moved further in case of the consolidated teeth after corticotomy.

The moment generated by the torque of the orthodontic rectangular wire : Three-dimensional finite element analysis (교정용 각형선재에 부여된 torque가 브라켓에 발생시키는 모멘트에 관한 유한요소법적 분석)

  • The purpose of this study was to investigate the ideal clinical torque(In the SWA rectangular wire, the torque by the angle between the plane part and twisted part to move the tooth) of the orthodontic rectangular wire which produce the proper labiolingual movement of the single tooth during finishing stage of the orthodontic treatment. The clinical torque is the sum of the play and the active torque which generates the moment at the bracket. The play is calculated by the formula and the active torque is calculated by the computer aided three-dimensional finite element method. The finite element model was consist of the three brackets which formed a row and 3 kinds of orthodontic rectangular wire(stainless steel, TMA, NiTi) which inserted in brackets. Both sides of the model were twisted and the moment generated in the center bracket was calculated. The sizes of seven wires which were used commonly were .016'X.022', .017'X.022', .017'X.025', .018'X.025', .019'X.025', .020'X.025', .021'X.025'. In 018' bracket, 016'X.022', .017'X.022', .017'X.025' wires were inserted and in 022' bracket, all the sizes of wires except .016'X.022' were inserted and tested. The following conclusions could be drawn from this study. 1. The moments generated on the same size of the wires by the same active torque were equal regardless of the bracket slot size. 2. The moments were increased with the size of the wires. The moment generated on the .021'X.025' wire was about 1.75 times as large as that on the .016'X.022' wire regardless of the material. 3. The moments were increased in the order of the NiTi, TMA stainless steel. The moment of the TMA wire was 0.35 times as small as that of the stainless steel wire and the moment of the NiTi was0.16 times as small as that of the stainless steel wire. 4. The moment was decreased as the interbracket distance was increased. 5. To get a desired moment with the specific size and material of the wire on the specific bracket slot, the formula and the results were displayed.

THE CLINICAL APPLICATION OF BUCCAL ACRYLIC APPLIANCES (Buccal acrylic appliance의 임상적 적용)

  • Removable appliance has many utilities in pediatric dentistry in the fields of space maintainance or occlusal guidance. In contrast to maxilla, many problems have occurred with mandible in using removable appliance such as deteriorated retention, inconvenience and patient's unwillingness to wear. It is probably due to adapting several retentive clasps at buccal side of the tooth, where there is actually little or no undercut especially in young children having the erupting teeth Moreover, acrylic base plate of conventional removable appliance is located at lingual parts, which leads to the conflict with alveolar crest undercut and sublingual soft tissues. Moreover, young children frequently do not follow the dentist's direction to elevate tongue properly during the impression taking process. Throughout this study, the position of clasps and acrylic baseplate was changed opposite to the traditional Hawley acrylic appliance in order to make a buccal acrylic appliance with buccal base plate and lingual clasp into primary dentition and early mixed dentition. This study was performed for the purpose of evaluating the buccal acrylic appliance in regards of patient compliance, retention and efficiency compared with traditional lingual acrylic appliance along with its merits and demerits. Buccal and lingual acrylic appliance were applied to 5 cases respectively which required space maintaining and tooth movement, and drew a comparison in relevance to patients' compliance, combination with the tongue's function and clinical effectiveness. According to this study, in every case buccal appliance was proved superior to lingual appliance in terms of retention and patients' compliance. Moreover, buccal appliance has shown to be similar to lingual appliance with respect to clinical effectiveness. Throughout this study, it was thought that, if removable appliance is to be applied to mandible, several drawbacks related to conventional lingual appliance shall be resolved. Conclusively, the buccal acrylic appliance is thought to be a good alternative in designing a mandibular appliance in young children and recommended to clinical applications.

Finite element analysis of effectiveness of lever arm in lingual sliding mechanics (Lingual sliding mechanics의 lever arm 효과에 대한 유한요소분석)

  • Objective: The aim of this study was to conduct three-dimensional finite element analysis of individual tooth displacement and stress distribution when a posterior retraction force of 200 g was applied at different positions of the retraction hook on the transpalatal arch (TPA) of a molar, and over different lengths of the lever arm on the maxillary anterior teeth in lingual orthodontics. Methods: A three-dimensional finite element model, including the entire upper dentition, periodontal ligaments, and alveolar bones, was constructed on the basis of a sample (Nissan Dental Product, Kyoto, Japan) survey of Asian adults. Individual movement of the incisal edge and root apex was estimated along the x-, y-, and z-coordinates to analyze tooth displacement and von Mises stress distribution. Results: When the length of the lever arm was 15 mm and 20 mm, the incisal edge and root apex of the anterior teeth was displaced lingually, with a maximum lingual displacement at the lever arm length of 20 mm. When the posterior retraction hook was on the root apex, the molars showed distal displacement. When the length of the lever arm was 20 mm, anterior extrusion was reduced and the crown of the canine displaced toward the buccal side, in which case, the retraction hook was on the edge, rather than at the center, of the TPA. Conclusions: The results of the analysis showed that when 6 anterior teeth were retracted posteriorly, lateral displacement of the canine and lingual displacement of the incisal edge and root apex of the anterior teeth occur without the extrusion of the anterior segment when the length of the lever arm is longer, and the posterior retraction hook is in the midpalatal area.

Occlusal Analysis of the Subjects with Chewing Side Preference Using the T-Scan II System (T-Scan II 시스템을 이용한 습관적 편측저작자들의 교합 분석)

  • While orofacial pain or various dental factors are generally considered as the primary cause of unilateral chewing tendency, there exist several studies indicating that dental factors did not affect the preferred chewing side. The aim of this study was to examine difference of occlusal scheme between the subjects with and without chewing side preference. The difference between the chewing and non-chewing sides in the unilateral chewing group was investigated as well. Computerized, T-Scan II system was used for occlusal analysis. 20 subjects for the unilateral chewing group (mean age of $25.25{\pm}2.84$ years) and 20 subjects for the bilateral chewing group (mean age of $27.00{\pm}5.07$ years) were selected by a questionnaire on presence or absence of chewing side preference and those with occlusal problem or pain and/or dysfunction of jaw were excluded. T-Scan recordings were obtained during maximum intercuspation and excursion movement. The number of contact points, relative occlusal force ratio between right and left sides, tooth sliding area and elapsed time throughout the maximum intercuspation were calculated. Elapsed time for excursion was also investigated. The results of this study shows that the unilateral chewing group had the smaller average tooth contact areas compared with those of the bilateral group (p<0.005). In the unilateral chewing group, the contact areas of non-chewing side are smaller than those of chewing side (p<0.005). The contact areas on their preferred sides were not significantly different with those of right or left side of the subjects without chewing side preference. There was no significant difference in the elapsed time during maximum intercuspation and lateral excursion, the sliding areas and relative of right-to-left occlusal force ratio between the two groups. From the results of this study, it is likely that individuals prefer chewing on the side with more contact areas for efficient chewing.

Study of Disclusion Time during Mandibular Eccentric Movement in Myofascial Pain Syndrome Patients by T-Scan II, Computerized Occlusal Analysis System (컴퓨터 교학분석기인 T-Scan II를 이용한 측방운동시 구치부 이개시간에 관한 연구)

  • Temporomandibular disorders(TMD) is a collective term which is embracing a number of clinical problems that involve the masticatory musculature, the TMJ and associated structures, or both. Myofascial pain, which is a kind of masticatory muscle disorder of TMD, is the sensory, motor, and autonomic symptoms caused by myofascial trigger points. There has been some controversies regarding etiologies of TMD and MFP. Especially the issue of occlusal conditions has been a critical issue for long time. Despite much efforts, the results of studies regarding occlusal conditions were contradictory. These controversies might be mostly due to various factors resulting from the complex nature of TMD, however, inaccurate and inappropriate study design, selection criteria, methodologies also play significant roles. Recently, a computerized occlusal analysis system, T-Scan II which made it possible to reveal quantifiable time data and relative force data for analyzing occlusion, was introduced. Some authorities suggested that the concept of disclusion time and prolonged disclusion time of posterior tooth and MFP are related using T-Scan II. But the previous studies which used T-SCAN II are not reliable for they did not provide accurate diagnostic criteria of MFP. Morever they did not compare with controls, and had many other problems. The purpose of this study was to evaluate the relationship between MFP and prolonged disclusion time of posterior tooth, which is one of the occlusal factors of TMD, by selecting 30 subjects as the study group through strict criteria and comparing them with 38 controls using T-SCAN II, computerized occlusal analysis system. The results, statistically analyzed, are summarized as follows: 1. Cronbach ${\alpha}$ coefficient of repeated measurements of disclusion time was 0.92. 2. There were no statistically significant differences at repeated measured disclusion time of both side between control and study group. 3. There was no statistically significant diffefence in the disclusion time between right and left side. From the results above, we can suggest that there was no relationship between MFP and disclusion time, so irreversible treatments leading to the reduction of disclusion time for treating MFP would not be appropriate. However more controlled, large scaled study, which consider various occlusal factors, and quantification of symptoms using Helkimo index would be necessary in the future.

  • 1958년(年) Fullmer와 $Lillie^9$에 의(依)하여 최초(最初)로 보고(報告)된 Oxytalan 섬유(纖維)는 산(酸)에 내성(耐性)이 강(强)한 섬유(纖維)로 치근막(齒根膜)에 많이 출현(出現)하고 기계적(機械的) 자극(刺戟)에 의(依)하여 배열(配列), 주행(走行) 및 수적(數的)인 변화(變化)를 가져온다. 저자(著者)는 교정력(矯正力)을 이용(利用)하여 치아(齒牙)를 이동(移動) 시킨후(後) 치근막내(齒根膜內) Oxytalan섬유(纖維)의 수(數), 주행(走行) 및 형태(形態)의 변화(變化)를 실험적(實驗的)으로 관찰(觀察)한바 있어 이를 보고(報告)하는 바이다. 본연구(本硏究)에 사용(使用)된 실험동물(實驗動物)로는 체중(體重) 60gram내외(內外)의 자성백서(雌性白鼠) 15마리를 택(澤)하였다. 각동물(各動物)은 Ether마취후(麻醉後) 교정용(矯正用) 고무줄편(片)을 상악우측(上顎右側) 제1구치(第一臼齒)와 제2구치(第二臼齒) 사이에 삽입(揷入)하여 24, 48, 72시간(時間) 간격(間隔)으로 관찰(觀察)하였다. 동물(動物)을 도살후(屠殺後) 상악골(上顎骨)을 적출(摘出)하여 10%중성(中性) 호루마린에 고정후(固定後) 3%의산(蟻酸)으로 탈회(脫灰)하였다. 파라핀 포매후(包埋後) 근원심적(近遠心的)으로 $4{\sim}6{\mu}$의 절편(切片)을 만들어 Hematoxylin-eosin 및 Aldehyde fuchsin염색(染色)을 시행(施行)하여 경험(鏡險)한 결과(結果)는 하기(下記)와 같다. 1. 치아이동후(齒牙移動後) 교원성섬유(膠原性纖維) 및 Oxytalan섬유(纖維)들의 배열(配列)에 있어 뚜렷한 차이(差異)를 보였다. 2. 치아이동후(齒牙移動後) 사주섬유(斜走纖維)는 염박측(壓迫側)에서는 치아장축(齒牙長軸)에 수직(垂直)되게, 견인측(牽引側)에서는 평행(平行)되게 주행(走行)하고 있었다. 3. 48시간군(時間群)에서 세포증식(細胞增殖)이 심(甚)하였다. 4. Oxytalan섬유(纖維)는 치아(齒牙)들에 교정력(矯正力)을 가(加)한후(後) 견인(牽引) 염박(壓迫) 양측(兩側) 공(共)히 수(數)가 증가(增加)하였다. 5. 염박측(壓迫側)에서 Oxytalan섬유(纖維)는 속(束)을 형성(形成)하며 치아장축(齒牙長軸)에 평행(平行)되게 주행(走行)하였다. 6.견인측(牽引側)에서는 하나 또는 두세개의 섬유(纖維)들이 산발적(散發的)으로 치아장축(齒牙長軸)에 평행(平行)되게 하였다. 7. Oxytalan섬유(纖維)의 수(數)는 염박측(壓迫側)에서 보다 견인측(牽引側)에서 더 변화(變化)가 많았다.

Effects of Estrogen and Progesterone on the Proliferation and Activity of Osteoblastic cells Abstract (에스트로젠과 프로게스테론이 골모세포의 증식과 활성에 미치는 영향)

  • Biomechanical reactions of tooth movement are the combination of bone formation and resorption, in which many paracrine factors are involved. The sex hormone is one of the paracrine factors and the sex hormonal level of an adult female vanes according to the body condition, e.g. mensturation, pregnancy, postmenopause, etc. Although the exact mechanism is not clarified yet, estrogen and progesterone are known to regulate the function of osteoblast. Again osteoblast is reported to affect the function of osteoclast. The purpose of this study is to determine the influence of the female sex hormone, estrogen and progesterone, on the cell proliferation and activity of HOS and ROS17/2.8 cell line. The observed results were as follows. 1. Estrogen inhibited HOS cell proliferation and promoted ROS17/2.8 cell proliferation. 2. Estrogen increased the activity of alkaline phosphatase of HOS cell and reduced the activity of alkaline phosphatase of ROS17/2.8 cell. 3. Progesterone inhibited the proliferation of HOS and ROS17/2.8 cell, but had no influence on the activity of alkaline phosphatase. 4. Estrogen and progeterone did not have any particular effects on the activity of super oxide, nitric oxide and gelatinase of HOS and ROS17/2.8 cell.
