조기 교정치료의 목적은 현재 존재하는 문제점들을 바로잡고 발생 가능한 문제점들을 차단하며 상황이 점점 악화되는 것을 미리 방지하는 것이다. 근기능적인 영향이 치열과 안모성장에 미치는 영향은 정상 치열구조에 형태적 변이를 일으키고 이미 존재하는 부정교합을 가속화시키게 된다. 따라서 이런 근기능적인 영향이 부정교합에 미치는 영향을 조기에 차단하여 부정교합이 악화되는 것을 예방하고 부가적으로 일어날 수 있는 문제점들을 막고자 하였다. 근기능치료는 1960년대 구강 습관을 치료하는 것으로부터 촉진되어 현재까지 많은 장치를 이용한 치료가 시행되고 있다. 최근 기존의 기능적 교정 장치가 가지고 있는 단점을 없애며 치아와 악골 배열과 관련된 근기능적인 습관을 조절할 수 있도록 혼합치열기의 부정교합을 가지고 있는 아동에게 적절한 장치로 Pre-orthodontic $TRAINER^{(R)}$가 소개되었다. Pre-orthodontic $TRAINER^{(R)}$는 $6{\sim}10$세 영구 전치가 맹출한 혼합 치열기 어린이에서 특히 하악 전치부 밀집이나, 전치부 반대교합, 2급 부정교합, 과개 교합을 가졌을 경우 사용될 수 있다. 또 만성적인 구호흡이나 손가락 빨기 버릇, 잘못된 연하습관을 가진 어린이에게도 유용하게 사용될 수 있다. 본 증례는 단국대학교 소아치과에 교정치료를 하기 위해 내원한 환자들 중에서 본격적인 교정치료 시작 전 습관조절을 위해 Pre-orthodontic $TRAINER^{(R)}$를 이용하여 만족할 만한 결과를 얻었기에 보고하는 바이다.
Lesch-Nyhan syndrome은 purine 대사장애로 인해 나타나는 질환이다. 유아기부터 전신근육의 강직, 발육 저하가 나타나고 손발의 불수의적 운동이나 불규칙적인 운동이 보이며 정신지체, 강박적 자해행위가 나타난다. 자해행위는 대개 1세 전후에 나타나지만 간혹 10대 후반에 나타나기도 한다. 입술, 특히 아랫입술과 혀, 협점막, 손, 손가락 등을 깨물고 입술, 혀, 협점막 등이 손상되거나 심할 경우 절단되기도 한다. 나이가 들면서 자해 행위가 점점 심해지고 상처 부위를 통한 2차 감염 가능성이 있다. 자해행위로 인한 주기적인 연조직 손상은 심할 경우 구강암으로 이행되기도 한다. 이와 같은 자해행위를 억제하기 위해 약물치료, 장치치료, 발치, 외과적 수술 등 여러 가지 방법이 시도되고 있다. 본 증례는 자해행위로 인한 입술손상을 주소로 내원한 Lesch-Nyhan syndrome 환자들을 대상으로 발치 대신 보존적 치료를 위해 가철성, 고정성 장치를 사용한 결과 입술외상의 빈도를 줄이고 심미적으로 양호한 결과를 얻을 수 있었기에 보고하는 바이다.
Lesch-Nyhan 증후군은 purine의 대사장애를 보이는 질환이다. 신생아에서 대략 1:100,000 정도의 이환율을 보이는 X염색체 열성 유전 질환으로 남자에서 흔하며, purine의 대사에 관여하는 효소인 hypoxanthine guanine phosphoribosyl transferase(HGPRT)의 결손 또는 활성 감소 결과, 과도한 요산 형성 및 과요산혈증(hyperuricemia)이 일어난다. 이 질환은 임상적으로 정신지체, 무도증(choreoathetosis), 경련성 뇌성 마비, 심각한 자해 행위가 특징이다. 이 중 자해행위는 입술과 혀, 손가락을 깨무는 경우가 흔하고, 혀와 손가락이 완전히 절단될 수도 있다. 이 경우 자해로 인한 통증 뿐 아니라, 상처 부위로의 2차 감염 및 연조직 결손으로 인한 심미성이 문제가 된다. 본 증례는, 상하악 유전치의 입술 깨물기 습관에 의해 하순이 심하게 손상된 Lesch-Nyhan 증후군 환아로서, mouth guard를 이용한 보존적인 방법과 소아정신과와의 협진을 통한 신경학적 약물치료로 자해에 의한 손상을 방지하여, 단기간에 비교적 만족할 만한 결과를 얻었기에 이를 보고하는 바이다.
This study was designed to analyze nasal respiratory patency and its correlation with skeletal components in growing children with anterior crossbite. The subjects consisted of 40 control patients, 24 nose breathers with anterior crossbite and 18 mouth breathers with anterior crossbite. The mean age was 11.4 years in the control group, 10.1 years in nose breathing group and 9.5 years in mouth breathing group. The results were as follows, 1. In anterior cross bite group, and nasal respiratory airflow rates (N.R.A.R.) was significantly lower than that of control group regardless of nasal decongestants application. 2. The N.R.A.R. of mouth breathers with anterior crossbite in male group was significantly lower than that of mouth breathers, but increased to the level of control group after nasal decongestants application. But in female group, the N.R.A.R. was significantly lower in mouth breathing group at both conditions. 3. Mouth breathing group showed smaller anterior vertical nasal cavity height (ANS-ANS'), lower upper anterior facial height ratios (N-sp'/N-Me) and higher maxillary occlusal plane ratios (OL-ML/ML-NL) than those of nose breathing group with anterior crossibte. 4. Items showing nasal height (ANS-ANS', PNS-PNS'), anterior upper facial height (N-sp') was were strongly correlated with N.R.A.R. at 150 pascal in inspiration. But item showing maxillary occlusal plane ratios (OL-ML/ML-NS) was negatively correlated with N.R.A.R. at 150 pascal in inspiration. 5. There were forward tongue position in mouth breathing group, but it was not significantly correlated with N.R.A.R. at 150 pascal in inspiration.
The purpose of this study is to provide the general history of fostering dentists in Japan and introducing their new roles. This research was conducted based on the government policy report on dentists and the information published by each educational institution. Based on the collected data, the official websites were used to represent the latest statistics of the institutions. The number of dentists in Japan has increased. The government established the National Examination for Dentists to guarantee the quality of dentists. After the standards for developing questions for the national examination were established in 1985, the contents of the examination have been appropriately improved by revising the standards every four years. This improvement has required dental students to study a variety of subjects for six years at dental school. Since dentists in Japan are required to respond to various demands from the nation; the Model Core Curriculum for Dental Education was developed to teach medical ethics and abilities to ensure that dentists conduct themselves professionally. Recently, the roles of dentists have been changing in Japan. When providing dental services to older patients over the age of 65, dentists and other dental professions focus on maintaining oral functions, such as saliva secretion, bite force, tongue movement, and masticatory/swallowing functions. However, oral function-related services for children are different. In addition to providing essential dental services, dental practitioners also provide special treatment, such as oral muscle training, myofunctional therapy, health guidance, and space retainers to the child patients with developmental insufficiency in oral functions. Dentistry in Japan has undergone numerous changes over the years and has continued to offer high-quality dental health services. Thus, information gained from the Japanese experience may be helpful to dental professions in other developed countries for planning oral health measures.
Cerebral palsy(CP) is one of the most common motor disease, due to brain injury during fetal and neonatal development which results in neuromotor paralysis and associated neuromuscular symptoms. Features of CP include motor disability due to the lack of muscle control, often accompanied by sensory disorders, mental retardation, speech disorders, hearing loss, epilepsy, behavior disorders, etc. There are increasing chances of treatment of dental patients with cerebral palsy, as the occurrence of CP is increasing with the decrease in infant mortality and an increase in immature birth and premature birth and also, there is a trend to pursue of higher quality of life. Reports on the relationship between CP and maxillofacial deformity are uncommon, but it is well known that the unbalance and discontrol of the facial muscles, lip, tongue and the jaws leads to malocclusion and temporomandibular joint disorders, and statistics show that class 2 relationship of the jaws and open bite is frequently reported. However, it is difficult to perform maxillofacial deformity treatment, which consists of orthodontic treatment, maxillofacial surgery and muscle adaptation training, due to difficulties in communication and problems of muscle adaptation caused by difficulties in motor control which leads to a high recurrence rate. This case report is to trearment of maxillofacial deformity in CP patient. A 26 year old female patient came to the department with the chief complaint of prognathism of the mandible and facial asymmetry. According to the past medical history, she was diagnosed as cerebral palsy 1 week after birth, classified as GMFC, classII accompanied with left side torticollis. The patient's intelligence was moderate, and there were no serious problems in communication. For two years time, the patient underwent lingual frenectomy, pre-operation orthodontic treatment and then bimaxillary orthognathic surgery to treat mandibular prognathism and facial asymmetry followed by rehabilitatory exercise of facial muscle. After 6 months of follow up, there was a good result. This is to report to the typical signs and symptoms of DFD in CP patient and the limitation of the usual method of the treatment of DFD in CP patient with literature review.
저자는 입술 및 혀에 대해 자해증상을 보이는 2명의 Lesch-Nyhan 증후군 환아에서 구강조직에 대한 자해행동을 예방하기 위해 soft mouth guard를 이용한 보존적 치료법과 전악발거의 침습적 방법을 이용하여 치료한 증례를 보고하는 바이다. Lesch-Nyhan 증후군 환자의 자해행동을 예방하기 위한 치료법은 아직 정립된 기준이 존재하지 않아 각 환자의 자해양상 및 정도에 따라 치료방법은 달라질 수 있다. 가능하다면 치아를 보존할 수 있는 장치 또는 BTX-A 제재 등을 이용한 보존적 방법을 먼저 시도해 보는 것이 좋다. 이 때 치료의 성공률을 높이기 위해서는 치과적 접근 이외에도 소아정신과와의 협진을 통한 정신심리학적 접근, 필요시 약물치료를 고려하는 것 등 여러 방향으로 접근하는 것이 더욱 바람직한 결과를 가져올 것으로 사료된다. 보존적 접근이 실패할 경우 더 이상의 조직상실과 전신적 감염 예방을 위해 극단적 방법이지만 전악발거를 고려해야 할 것이다.
