국소 침윤마취법을 이용한 족지조직 유리 피판술 (Free Toe Tissue Transfer using Infiltration Method of Local Anesthetic Agent)

  • This study was designed to introduce free toe tissue transfer using infiltration method of local anesthetic agent. Four toe pulp tissues were transferred to reconstruct finger tip defect in four patients who were not suitable for general anesthesia. Two flaps taken from the lateral side of the great toe was used for reconstruction of thumb defect and two flaps from the medial side of the second toe for resurfacing of the index and fifth finger. Flap sizes were various from $2.0{\times}2.0\;cm^2$ to $1.6{\times}4.0\;cm^2$. Anesthesia was induced by infiltration of 2% lidocaine hydrochloride (with 1:100,000 epinephrine) with dilution of normal saline in same volume unit, as like as in ordinary digital block. All vessels were anastomosed within 2 cm of distance from the proximal margin of the defect. Whole operative procedures were carried out by one team. All flaps were successfully taken without complication. The average operation time was 4 hours 10 minutes. The amount of anesthetic agent used in whole operative procedures was roughly 4 mL in the toe, 8 mL in the finger, and 12 mL totally. In conclusion, free toe tissue transfer using infiltration method of local anesthetic agent would be good strategy for finger tip reconstruction in the patient not suitable for general anesthesia.

드롭 랜딩에서 발끝자세가 무릎관절 운동역학에 미치는 영향 (Effect of Toe Headings on the Biomechanics of Knee Joint in Drop Landing)

  • The purpose of this study was to investigate the effect of the toe headings on the biomechanics of knee joint in drop landing in an attempt to find the potential risk of non-contact anterior cruciate ligament (ACL) injury. Seventeen male college students ($20.5{\pm}1.1$ yrs; $175.2{\pm}6.4$ cm; $68.8{\pm}5.8$ kg), having no neuromuscular injury within an year, participated in this study. Three different toe headings such as toe-in (TI), neutral (N), and toe-out (TO) positions were tested. Motion capturing system consisting of eight high speed cameras and two force platforms were used to collect three-dimensional motion data and ground reaction force data during landing. Results indicated joint angles and peak joint moments were significantly affected by the toe headings. TI position produced larger valgus angle due to reduce knee distance in addition to higher flexion and valgus moment than other positions, which was somewhat vulnerable to the potential risk of non-contact ACL injury. TO position caused the largest internal rotation angle with smaller joint moments. Therefore, it is recommended that athletes need to land on the ground with neutral toe-heading position as possible in order to minimize the potential risk of non-contact ACL injury.

여대생의 발 유형과 선호 구두 형태와의 관계 (Relationship of Types of Foot Shape and Favorite Shoes Shape of Female University Students)

  • The objective of this paper was to analyse the foot type of female university students by the direct and indirect measure of foot and the measures of foot outline. The results were as follow: 1. The length and the width of foot were small and ankle slender and the modification of the big toe slight. 2. The analysis on the 16 measure items revealed five factors; the first factor was related to the width and girth of foot, the second one to the length of foot, the third one to the transformation of the little toe, the fourth one to the height of foot, the fifth one to transformation of the big toe. 3. The grouping on the shape of foot revealed three types. The inline angle of foot was the smallest in average in the type 1, while it appeared to be middle in the other types. The wider foot shapes were grouped as type 2 because their inline and outline angles of foot were the biggest in average. In the type 3, the big toe was curved a lot, since it was the largest in average. 4. From the naked eye measuring method, classification from which toe was popped-out told that Egyptian foot was 30.9%, Greek foot 27.9%. On the other hand, the instrumental method showed that Egyptian foot was 57.3%, Squared foot 35.3% and Greek foot 7.4%. 5. The result from the analysis about the traits of foot shape by the degree of the arch formation of bottom foot, showed that 33.8% had the normal foot, 66.2% slightly flat foot or middle one. There was no one with rigid flat foot. 6. The shoes size they declared were that 240cm was 33.3%, 235cm 18.1% and 245cm 18.1%. The shape of their shoe heel were flat heel(34.8%), cuban heel(25.5%), and narrow straight heel(19.9%); and that of the shoe toe were round toe(25.4%), oval toe(20.4%) and squared toe(20.4%). 7. The discomforting parts when they wear shoes were the little toe(35.4%), the rear of foot(13.9%), and the width of foot(13.9%), which was related to their dislike of shoe with the pointed toe.

실험계획법을 이용한 전륜 토우각의 최적설계 및 조종 안정성능 평가 시뮬레이션 (Optimum Design of Front Toe Angle Using Design of Experiment and Dynamic Simulation for Evaluation of Handling Performances)

  • At the initial design stage of a new vehicle, the chassis layout has the most important influence on the overall vehicle performance. Most chassis designers have achieved the target performances by trial and error method as well as individual knowhow. Accordingly, a general procedure for determining the optimum location of suspension hard points with respect to the kinematic characteristics needs to be developed. In this paper, a method to optimize the toe angle in the double wishbone type front suspension of the four-wheel-drive vehicle is presented using the design of experiment, multibody dynamic simulation, and optimum design program. The handling performances of two full vehicle models having the initial and optimized toe angle are compared through the single lane change simulation. The sensitive design variables with respect to the kinematic characteristics are selected through the experimental design sensitivity analysis using the perturbation method. An object function is defined in terms of the toe angle among those kinematic characteristics. By the design of experiment and regression analysis, the regression model function of toe angle is obtained. The design variables which make the toe angle optimized ae extracted using the optimum design program DOT. The single lane change simulation and test of the full vehicle model are carried out to survey the handling performances of vehicle with toe angle optimized. The results of the single lane change simulation show that the optimized vehicle has the more improved understeer tendency than the initial vehicle.

고성능 경량 TCP/IP를 이용한 소프트웨어 기반 TCP/IP 오프로드 엔진 구현 (Implementation of a TCP/IP Offload Engine Using High Performance Lightweight TCP/IP)

  • 최근 이더넷 기술은 기가비트급의 대역폭을 넘어서 10 기가비트급으로 빠른 속도로 발전하고 있다. 이러한 고속 네트워크 환경에서는 호스트 CPU가 운영체제내의 TCP/IP를 처리하는 기존의 방식은 호스트 CPU에 많은 부하를 야기하며, 그 결과 실제 수행되어야 할 사용자 응용 프로그램에 충분한 컴퓨팅 파워를 제공하지 못한다. 이러한 문제점의 해결을 위해 네트워크 어댑터에서 TCP/IP를 처리하도록 하는 TCP/IP Offload Engine(TOE)이 연구되고 있다. 본 논문에서는 TOE를 위한 고성능의 경량 TCP/IP를 구현하였으며, 이를 임베디드 시스템에 실제 적용하여 검증 및 실험을 수행하였다. 본 논문에서 구현한 고성능의 경량 TCP/IP는 기존 TCP/IP의 기본적인 기능들인 흐름제어, 혼잡제어, 재전송, 지연 ACK, Out-of-Order 패킷처리 등을 지원한다. 또한 본 논문에서 구현한 고성능의 경량 TCP/IP는 기가비트 이더넷 MAC에서 하드웨어적으로 지원하는 TCP segmentation offload(TSO), Checksum offload(CSO), 인터럽트 coalescing 기능 둥을 이용하도록 구현하였다. 그리고 데이타를 전송할 때, 호스트 사용자 메모리에서 네트워크 어댑터의 메모리로 데이타를 복사하는 부하를 제거하였다. 또한 재전송해야 할 경우를 대비해 전송한 데이타에 대한 복사본을 네트워크 어댑터의 메모리에 저장하는 방법을 개선하여 지연시간 및 대역폭 성능을 향상시켰다. 본 논문에서 구현한 고성능의 경량 TCP/IP를 이용한 소프트웨어 기반 TOE는 6% 이하의 호스트 CPU 사용률과 453Mbps의 최대 대역폭을 보인다.

  • 본 연구는 TOE(Technology-Organization-Environment) 모델을 기반으로 원격근무 솔루션(RWS)의 기술적, 조직적, 환경적 측면을 고려해 위드 코로나 시대 이용자들의 지속사용 의도를 설명할 수 있는 연구 모형을 제안하였다. 연구 모형 및 가설 검증을 위해 국내 원격근무 솔루션 사용자를 대상으로 온라인 설문을 실시하였고 부분최소자승법(Partial Least Square, PLS)을 이용해 411개 수집 데이터를 분석하였다. 분석 결과, 기능성과 보안성은 생산성 향상과 만족도에 모두 유의한 양의 상관관계가 있음이 밝혀졌다. 또한 조직 준비성은 생산성 향상에 긍정적인 영향을 끼치나 만족도에는 영향을 주지 못하는 것으로 드러났다. 정부지원은 원격근무 솔루션의 지속사용의도에 긍정적인 영향을 미치는 것이 확인되었으나 건강에 대한 우려와 지속사용의도와의 상관관계는 유의하지 않은 것으로 나타났다. 마지막으로 생산성 향상과 만족도, 지속사용의도 간의 상관관계들은 모두 유의한 것으로 확인되어 총 11개 가설 중 9개 가설이 지지되었다.

용접토우부의 그라인딩에 의한 피로강도 증대효과에 대한 연구 (A Study on the Fatigue Strength Improvement using Weld Toe Burr Grinding)

  • While it is known that the weld toe grinding method may give 3.4 times of fatigue strength improvement, this improvement may significantly vary according to weld bead shapes and loading modes. Although tremendous interest have been given in improving fatigue strength improvement for ship structures, quantitative results are yet in need. In this context, a series of fatigue tests is carried out for a type of test specimen that are typically found in ship structures. Weld burr grinding is carried out using a electric grinder in order to remove surface defects and improve the weld bead profile. The test results are compared with the same type of test specimen without applying the fatigue improvement technique in order to obtain a quantitative measure of the fatigue strength improvement. On the other hand, both hot spot stress and structural stress methods are employed to compare the effectiveness of the two methods in evaluating the fatigue strength improvement of welded structures.

  • Recently, as the global warming by fossil fuels become social issues. the interest of renewable energy producing system is increasing rapidly. Among these, wind turbines are most highlighted because of its economic competitiveness. The tower occupying about $20\%$ of overall turbine costs, is one of the main components of wind turbine. Tower access door located to base part of the tower, is used to enter the tower. This is the main structural weak point because of door hole, weldment, etc. In this study, by FEM, we retrieved the maximum van Mises stress at door location and carried out fatigue analysis using stresses at weld toe locations of tower access door part.

  • Orthotropic steel bridge decks are potentially liable to cause fatigue cracks due to weld defects, residual stresses, and in-plane or out-of-plane stresses. In particular, the cracks propagated through deckplate in longitudinal rib to deckplate joints occur at weld toe and weld root due to stress concentrations. Numerical parametric studies are performed to show the Influence of the parameters on the stress concentration at the connection between the longitudinal rib and the deckplate. The parameters include root gap, toe angle $\theta$, toe radius $\rho$, and weld penetration. This study provides a fundamental point for the improvement of fatigue resistance and the estimation of the fatigue crack propagation in wekded joint details.

발가락 벌림 보조기가 양하지 뇌성마비 아동의 보행에 미치는 영향 (The Immediate Effects of the Toe Spreader on the Gait of Children With Spastic Diplegic Cerebral Palsy)

  • 본 연구는 발가락 벌림 보조기(toe spreader)가 긴장성 발가락 굽힘 반사(Tonic Toe Flexion Reflex: TTFR)가 있는 양하지 뇌성마비 아동의 보행에 미치는 영향을 알아보고자 하였다. 12명의 TTFR이 있는 양하지 뇌성마비 아동을 대상으로 같은 날에 맨발과 발가락 벌림 보조기를 착용한 상태에서 보행분석을 실시하였다. 시간 거리 보행변수로 활보장(step length), 보장(step length), 보폭(step width), 발가락 외전각도(toe out angle), 활보시간(stride), 입각기(stance phase), 유각기(swing phase), 분속수(cadence), 보행속도(speed)를 측정하였다. 발가락 벌림 보조기 착용 시 시간 거리 보행변수는 통계적으로는 유의한 차이가 없었다(p>.05). 양하지 뇌성마비 아동에서 TTFR을 억제하기 위한 발가락 벌림 보조기의 즉시적인 효과는 유의하지 않다고 볼 수 있으며, 추후연구로 장기간 일상생활에서의 지속적인 착용을 통한 연구가 필요하다고 본다.
