Orbital wall restoration with primary bone fragments in complex orbital fractures: A preliminary study

  • Background: We have reported orbital wall restoration surgery with primary orbital wall fragment in pure blowout fractures using a combination of transorbital and transnasal approach in pure blow out fractures. However, this method was thought to be difficult to use for complex orbital wall fractures, since the sharp screw tip that fixate the maxillary wall increases the risk of balloon ruptures. In this study, we reviewed 23 cases of complex orbital fractures that underwent orbital wall restoration surgery with primary orbital wall fragment and evaluated the result. Methods: A retrospective review was conducted of 23 patients with complex orbital fracture who underwent orbital restoration surgery with primary orbital wall fragments between 2012 and 2019. The patients underwent orbital wall restoration surgery with primary orbital wall fragment with temporary balloon support. The surgical results were evaluated by the Naugle scale and a comparison of preoperative and postoperative orbital volume ratio. Complex fracture type, type of screw used for fixation and complications such as balloon rupture were also investigated. Results: There were 23 patients with complex orbital fracture that used transnasal balloon technique for restoration. 17 cases had a successful outcome with no complications, three patients had postoperative balloon rupture, two patients had soft-tissue infection, and one patient had balloon malposition. Conclusion: The orbital wall restoration technique with temporary balloon support can produce favorable results when done correctly even in complex orbital wall fracture. Seventeen cases had favorable results, six cases had postoperative complications thus additional procedure seems necessary to complement this method.

  • Harford는 구씨관기능의 측정에 impedance audiometry의 진단학적 가치를 설명한 바 있다. 고막에 천공이 없는 경우 중이강내의 과도한 음압은 tympanogram에 의해 간접적으로 구씨관기능부전을 나타내는 것은 주지의 사실이다. 구씨관기능이 정상인 경우 valsalva maneuver에 의해 고막은 외측으로 밀려나갈 것이며 이것은 balance meter needle의 운동을 야기시킬 것이다. 이에 저자들은 고막, 중이, 구씨관기능이 모두 정상소견을 보이는 사람을 대상으로 valsalva maneuver에 의해 야기될 수 있는 balance meter needle의 운동거리를 측정함으로써 정상인에서의 구씨관에 의한 중이강의 공기체적의 변동양을 조사하였다. 즉, electroacoustic bridge (Madsen ZO 70) with silicone rubber probe tip을 사용하여 고막, 중이, 구씨관기능이 모두 정상인 사람에서 valsalva maneuver에 의한 balance meter needle의 운동범위는 1.0~4.0(평균치 $1.6{\pm}0.06$)이었으며, 그 치가 1.0미만인 경우에는 어떤 정도의 구씨관기능의 저하 내지 부전을 진단할 수 있는 것으로 생각되어 impedance audiometry에 의한 구씨관기능검사의 의의를 고찰하며 보고하는 바이다

Three-dimensional numerical simulation of hydrogen-induced multi-field coupling behavior in cracked zircaloy cladding tubes

  • In the high-temperature and high-pressure irradiation environments, the multi-field coupling processes of hydrogen diffusion, hydride precipitation and mechanical deformation in Zircaloy cladding tubes occur. To simulate this hydrogen-induced complex behavior, a multi-field coupling method is developed, with the irradiation hardening effects and hydride-precipitation-induced expansion and hardening effects involved in the mechanical constitutive relation. The out-pile tests for a cracked cladding tube after irradiation are simulated, and the numerical results of the multi-fields at different temperatures are obtained and analyzed. The results indicate that: (1) the hydrostatic stress gradient is the fundamental factor to activate the hydrogen-induced multi-field coupling behavior excluding the temperature gradient; (2) in the local crack-tip region, hydrides will precipitate faster at the considered higher temperatures, which can be fundamentally attributed to the sensitivity of TSSP and hydrogen diffusion coefficient to temperature. The mechanism is partly explained for the enlarged velocity values of delayed hydride cracking (DHC) at high temperatures before crack arrest. This work lays a foundation for the future research on DHC.

Cyclic fatigue resistance of M-Pro and RaCe Ni-Ti rotary endodontic instruments in artificial curved canals: a comparative in vitro study

  • Objectives: To compare the flexural cyclic fatigue resistance and the length of the fractured segments (FLs) of recently introduced M-Pro rotary files with that of RaCe rotary files in curved canals and to evaluate the fracture surface by scanning electron microscopy (SEM). Materials and Methods: Thirty-six endodontic files with the same tip size and taper (size 25, 0.06 taper) were used. The samples were classified into 2 groups (n = 18): the M-Pro group (M-Pro IMD) and the RaCe group (FKG). A custom-made simulated canal model was fabricated to evaluate the total number of cycles to failure and the FL. SEM was used to examine the fracture surfaces of the fragmented segments. The data were statistically analyzed and comparisons between the 2 groups for normally distributed numerical variables were carried out using the independent Student's t-test. A p value less than 0.05 was considered to indicate statistical significance. Results: The M-Pro group showed significantly higher resistance to flexural cyclic fatigue than the RaCe group (p < 0.05), but there was no significant difference in the FLs between the 2 groups (p ≥ 0.05). Conclusions: Thermal treatment of nickel-titanium instruments can improve the flexural cyclic fatigue resistance of rotary endodontic files, and the M-Pro rotary system seems to be a promising rotary endodontic file.

활로 4징증의 근치수술후의 원격 방실전도 차단 (Complete A-V Block 3 Months after Total Correction of Tetralogy of Fallot)

  • There appears some conduction defects frequently after total correction of Tetralogy of Fallot. Common defect is right bundle branch block due to surgical intervention. We experienced complete A-V block which occured 3 months later after total correction of Tetralogy of Fallot in a 8 year old boy. The patient was completely free of any A-V block after the operation for 3 months, and sudden onset of A-V block with coupled premature ventricular contractions resulted him in shock state during the attack of severe bronchopneumonia for 4 days prior to the second visit. Emergency implantation of Cordis demand type temporary pacemaker was necessary to control the complete heart block with bradyarrhythmia and frequent ventricular fibrillation. Permanent cardiac pacemaker was implanted two weeks later as indicated with Cordis Stanicor lambda demand pacemaker, and the patient was discharged uneventfully on the 8th post implantation day with the heart rate of 72/min. Another 3 months after the implantation, the patient was transported to this hospital as dead on arrival after an accidental fall from a 2 meter height, and all possible cardiopulmonary resuscitation was performed for 60 minutes at the emergency room in vain. Autopsy was done to find out the cause of sudden death and the etiology of complete heart block. Microscopic focal infarctions with scar formation were noted along the course of conduction system in the interventricular septum, which might be the main cause of complete heart block during the attack of severe bronchopneumonia complicated with acute bacterial endocarditis. The tip of the pacemaker wire was slipped from the granulation scar at the apex of the right ventricular cavity, and this might be the direct cause of pacing failure and death.

경막외 유착용해술시 투여되는 8%와 10% 고장성 식염수농도의 제통효과 및 부작용의 비교 (Comparative Study for Analgesic and Adverse Effects of 8% and 10% Hypertonic Saline in Epidural Adhesiolysis)

  • Background: Epidural Adhesiolysis is an interventional pain management technique for patients with low back pain. In addition to local anesthetics and corticosteroid, hypertonic saline (NaCl) are used for the technique. Present study was aimed to compare analgesic and adverse effects of two different concentration of hypertonic saline in Epidural Adhesiolysis. Methods: Fifty-three subjects with low back pain with radiculopathy were assigned to one of two epidural adhesiolysis treatment groups: 8% (Group I, n=26) or 10% (Group II, n=27) hypertonic saline. 17 G epidural needle was inserted at sacral hiatus and catheter was advanced untill its tip was located at lesion site under fluoroscopic guidance. Subjects in all treatment groups received epidural corticosteroid and local anesthetic. And then, hypertonic saline injection via catheter were carried out daily for 3 days. Evaluation included assessment of pain relief (Numerical Rating Scale; NRS) at post-epidural adhesiolysis 1, 2, 3 days, 1 week, and 1, 3 months. We also looked for complications of epidural adhesiolysis at different concentration of hypertonic saline. Results: Statistical analysis demonstrated that NRS was not significantly different between two groups during 3 months after epidural adhesiolysis. There were no major differences of complications between two groups and disappeard after a few months without residual sequelae. Conclusions: We conclude that epidural adhesiolysis using 8% hypertonic saline is effective for relief of low back pain with proven lumbosacral fibrosis without any residual sequelae compared with 10%.

여산송씨일가묘 출토 남자 포의 가장자리 구성법 연구 (A Study of Sewing Methods Used for the Hems of Men's Coat Excavated from the Tombs of Yeosan Song Clan)

  • The purpose of this study is to examine how to sew the hem of men's coat which were excavated from the Yeosan Song Clan's tombs that are thought to be formed between the late 15th and the late 16th centuries, findings of the study can be summarized as follows. 1. Andan, or inside facing was used to prevent the hem from being worn or swollen or to make the hem properly arranged. It was generally used for single-layered and cotton-quilted coat for men. Deotdan was used mainly for men's coat having oblique cutting lines and jemuldan for those having straight cutting lines. In single-layed coat for men, the width of andan was widened a little as the times moved from the late 15th to the late 16th century, but in cotton-quilted ones, the width was not significantly changed. 2. Fixing the lining and the upper was used as a method to prevent the lining of men's coat from being pushed out or the hem of the clothes from being swollen. The method was generally used for double-layered, cotton and cotton-quilted coat for men. The double back-stitching was usually used for double-layered and cotton coat for men and the broad-stitching for Cotton-quilted ones. 3. Among men's coat, those whose siding was partially opened were less spare in the lower end than those whose siding was wholly opened. For this reason, the former clothes were more likely to have the tip of the siding easily torn. To solve this problem, overhand knotting 1, overhand knotting 2 and lotus-flower knotting strings were used for men's coat whose siding was partially opened. Among the three types, lotus-flower knotting was most elaborate in form. For men's coat, overhand knotting became gradually replaced by lotus-flower knotting as the times moved from the late 15th to the late 16th century.

  • 한반도에 자생하는 돌뽕나무(Morus tiliaefolia)의 임성과 재배학적 특성은 다음과 같다. 1. 종자형성률은 46∼80%이며 종자의 발아율은 19∼28%에 불과하다 2. 개화최성기(1994년)는 4월 28일∼5월 3일로서 산뽕나무보다는 늦고 개량뽕보다 빠르다 3. 춘기 동아의 탈포기는 4월 21일∼24일로서 산뽕나무와 개량뽕보다 늦다. 4. 잎은 대형으로 엽형지수 1.05의 폭광성을 나타내며 잎두께는 산뽕나무보다 두껍다. 5.성숙엽의 화학성분은 가용성 무질소물과 조회분이 많고 조단백질, 조지방, 조섬유가 적다. 6. 낮추베기때의 자세는 대부분 전개성이며 가지는 조장형이고 절간장이 길다 7. 추위견딜성과 뽕나무 동고병에 약하다.

덕트의 유로 최적화를 통한 소음저감 연구 (Study on Noise Reduction by Optimizations of In-line Duct Flow)

  • This paper was a study about noise reduction through flow stabilization in duel using experimental method and numerical analysis at the same time. To determine the fan's type three kinds of fans(axial fan, centrifugal fan, and axial fan with centrifugal type) was examined to investigate the suitability for duct. As a result, under the equal number of rotation 2000 RPM, performance of an axial fan with centrifugal type was the most superior by 55dBA at 4.3CMM among other fans. After this, analyzed the results of the numerical analysis to find out the optimum design of pitch angle such as $0^{\circ},\;10^{\circ},\;15^{\circ}\;and\;20^{\circ}$. The intensity of turbulence was low when pitch angle was $15^{\circ}$ and air volume became peak by 5.08 CMM. It was observed that axis component of velocity increased gradually when pitch angle increased from $0^{\circ}\;to\;20^{\circ}$. Finally, designed the shapes of D/S(Down Stream) in duct that agreed inlet angle($\delta$) of stationary blades with pitch angle($\beta$) of axial fan with centrifugal type and derived flow to duct medial, and changed the shape of motor-mount to reduce occurance of unstable vortex in tip of impeller, and embodied noise reduction and improvement of air flow rate through flow stabilization.

$CO_2$ 소화설비 방사시 정보저장장치의 저온손상에 관한 연구 (An Experimental Study on the Damage of the Data Process Equipment When $CO_2$ is Discharged)

  • 이산화탄소 소화설비는 가스계 소화설비 중 비용의 저렴함과 작동후의 청결함, 그리고 절연성등의 특성으로 인하여 통신기기설, 전산실, 전기실 등에 많이 사용되고 있다. 이산화탄소는 저장시 액체이다가 방출과 동시에 가스상태로 기화하는데 이때 온도가 급격히 내려간다. 이러한 냉각효과에 의하여 반도체장비나 정보저장장치에 손상을 줄 수 있다. 본 연구는 이산화탄소 방사시 구획내부의 온도분포와 손상정도를 규명하여 차후 연구의 기초자료를 제공하고 $CO_2$소화설비 설계나 관련규정의 정비에 도움이 되고자 시행했다. $CO_2$ 소화설비 방사 시 노즐부분의 순간방출온도는 $-82.53^{\circ}C$까지 내려갔다. 소화약제가 모두 방사되는 1분경의 실내평균온도는 $-40^{\circ}C$이며 이는 충분히 정보저장 장치류에 피해를 입힐 수 있는 온도이다. 플로피 디스켓을 설치하고 실험한 결과 70%의 고장율을 보였다. 컴퓨터를 넣고 실제 화재를 일으켜 실험한 결과, 특별한 고장이 없었으나, 작동으로 인한 내부의 열과의 온도차이에 의하여 수증기가 응결되고 녹아 흐르는 현상을 보여 Water Damage의 발생가능성을 발견할 수 있었다. 또한 저온지속시간(-5$^{\circ}C$기준)이 각 시나리오에서 평균 5분이상 나타났으며, 만약 실내의 단열과 밀폐조건이 더욱 좋은 경우라면 더 장시간 지속될 수 있다.
