수유부에서 두유 보충이 모유, 혈액 및 뇨 중 Isoflavone 농도에 미치는 영향 (The Effects of Soy Milk Supplement on Isoflavone Concentration of Breast Milk, Plasma and Urine from Breast Feeding Woman)

  • Soybeans have been a major protein source for many centuries in Korea. Soybeans contain phytochemicals which are isoflavones, biochemically active component. Isoflavone is a kind of phytoestrogen, structurally and functionally similar to estrogen. It has been reported that the breast milk and blood of breast feeding mothers who consume soy products contain isoflavones. This study was conducted to investigate the effects of soy milk supplement on the isoflavones (daidzein, genistein) concentration of breast milk, plasma and urine from breast feeding woman. Seventeen healthy women who delivered at Kyung Hee Medical Center were recruited. For the first 2 weeks after delivery, seventeen women ingested 400 ml (isoflavone 43.2 mg) of soy milk on the given time starting from the day of giving birth. For the next 2 weeks, soy milk ingestion was withdrawn. Dietary intake and anthropometric data were checked and breast milk, blood, and 24 hr urine samples were collected on the day of giving birth, the 14th (the last day of the supplement phase) and 28th (the last day of the withdrawal phase) day, respectively. HPLC analysis was used to measure the concentration of isoflavones. Dietary intakes of the subjects were inadequate for the Korean RDA regardless of soy milk supplementation. Especially, intakes of vit A, calcium, and iron were very low. The Anthropometric data such as LBM, TBW, PIBW, BMI checked on the day of 14th decreased and maintained their levels by the 28th day. Daidzein concentration in breast milk was not affected by soy milk supplementation. However, genistein concentration decreased by the 28th day (14th day: 0.89 $\pm$ 0.10 $\mu$g/ml, 28th day : 0.48 $\pm$ 0.07 $\mu$g/ml) (p < 0.05). Plasma daidzein and genistein concentrations were not changed by the 14th day and decreased by the 28th day (14th day: 49.64 $\pm$ 3.30 ng/ml, 26.72 $\pm$ 2.90 ng/ml, 28th day: 38.30 $\pm$ 4.40 ng/ml, 6.51 $\pm$ 0.50 ng/ml, respectively) (p < 0.05). Twenty four hour urine concentrations of daidzein and genistein significantly increased by the 14th day and decreased by the 28th day (14th day: 5.80 :t 0.3 mg/d, 4.17 $\pm$ 0.2 mg/d, 28th day: 6.72 $\pm$ 0.4 mg/d, 5.09 $\pm$ 0.5 mg/d, respectively) (p < 0.001). The rate of urinary recovery of daidzein was greater than that of genistein. The results of this study indicate that the supplement of dietary soy milk to the lactating women elevates the contents of isoflavone in the breast milk.

고지방식이 및 고탄수화물식이가 흰쥐의 혈청 Leptin 농도 및 지질함량에 미치는 영향 (Effect of High Fat and High Carbohydrate Diet on Serum Leptin and Lipids Concentration in Rat)

  • The purpose of this study was to investigate the effects of diet composition on serum leptin and lipids profiles in rats. At the baseline, seven 4-week-old Sprague-Dawley rats, male, were sacrificed and the remaining rats were divided into five groups and each group was fed one of the following five dietary regiments for 2 weeks and 6 weeks;the control diet AIN76(CAL, n=2l), high-carbonhydrate(rice)(HCR, n=2l), high-carbohydrate(flour)(HCF, n=2l), high-fat(corn oil)(HFO, n=2l), high-fat(beef tallow)(HFB, n=2l). Serum leptin was determined by a double antibody ELISA assay at the baseline(n=7), 6 week(n=35) and 10 week of age(n=70). At 6 weeks of age, the increase in the Food Efficiency Ratio(FER) was related to adipocyte hyperplasia in rats on HFB diets. The serum triglyceride, total cholesterol, and LDL-cholesterol increased significantly in HFB group, and decreased in HFO group compared to control group. The HFC group showed significant increase in serum triglyceride level compared with control group. After 2 weeks and 6 weeks, noticeably high increases in epididymal adipose tissue fat cell mass and numbers were observed with the HFB fed group. Serum leptin levels increased as body weight increased over the period of time(4weeks; 1.50$\pm$0.13ng/ml versus 10weeks; 2.08$\pm$-.13ng/ml). And this result shows that there are 193% higher in rats fed high fat-beef tallow diet than the control diet. Serum leptin levels of the HFB group(4.01$\pm$0.39mg/ml) were significantly higher than that of the HFO(2.06$\pm$0.5613ng/ml), CAL(2.08$\pm$0.1313ng/ml), HCR(2.41$\pm$0.2113ng/ml) and HCF(2.80$\pm$0.4713ng/ml) at p<0.05. The serum leptin concentration was positively correlated with the amount of epididymal fat pads(r=0.47 p<0.01), serum triglyceride(r=0.49, p<0.001), tatal cholesterol(r=0.48, p<0.001), LDL-cholesterol(r=0.58, p<0.001), atherogenic index(r=0.67, p<0.001), and inversely correlated with HDL-cholesterol(r=-0.65, p<0.001). In conclusion, the changes in composition of dietary fat and carbohydrate intake could affect changes in concentration of serum lipids and leptin. Especially, the high-fat diet with animal fat source could increase circulating leptin level. (Korean J Nutrition 34(2) : 123-131, 2001)

창원에서 유행한 백일해의 임상적 특징 (Clinical Characteristics of Pertussis Epidemic in Changwon)

  • 목적: 백일해는 백신접종으로 예방이 가능한 질환이지만 세계적으로 증가하고 있고, 국내에서도 환자가 증가하고 있는 추세이다. 저자들은 최근 창원 지역에서 발생한 백일해 환자의 역학적인 특징과 임상 양상을 알아보고자 이 연구를 실시하였다. 방법: 2015년 7월부터 2016년 3월까지 호흡기 증상을 주소로 창원파티마병원 소아청소년과에 내원하는 환자 중 1주 이상의 기침을 하면서 발작적인 기침, 기침 후 구토, 흡기 시 '흡' 소리(whooping), 야간 기침이 동반된 환자들을 대상으로 호흡기 검체를 채취하여 백일해에 대한 배양, 중합연쇄반응검사를 실시하였고, 혈액의 항백일해 독소 항체검사(anti-pertussis toxin of ELISA kit, IBL International GmbH)를 실시하였다. 기저에 질환이 있거나 최근 1년 안에 DTaP 혹은 Tdap 백신 접종을 한경우에는 제외하였다. 결과: 연구 기간 동안 대상 환자 50명 중 진단된 환자는 37명이었다. 이 중 중합연쇄반응검사에서 양성인 환자는 2명이었고 혈청검사에서 양성인 환자는 37명이었다. 환자의 연령은 생후 1개월부터 15세였다. 항생제 사용 후 모든 환자가 합병증 없이 회복되었다. 결론: 2015년 7월부터 2016년 3월까지 창원 지역에 백일해의 유행이 있었으며, 이 연구 결과는 향후 국내에서 발생하는 백일해 역학 연구에 도움이 될 것으로 판단한다.

제 2형 당뇨모델 OLETF 랫드에서 동충하초의 항당뇨 효과 (Anti-diabetic Effects of Isaria tenuipes in OLETF Rats as an Animal Model of Diabetes Mellitus Type II)

  • 본 연구는 동충하초(Isaria tenuipes)를 유전적 당뇨 질환 동물모델인 OLETF 랫드에서 4주간 경구 투여하여 항당뇨 효과에 대한 연구결과로써, OLETF 랫드 동충하초(Isaria tenuipes) 투여군은 당뇨대조군과 비교하여 투여 4주에는 사료 섭취량과 체중이 증가하였다. 이는 동충하초(Isaria tenuipes)를 섭취한 OLETF 랫드는 고지방식 사료만 섭취한 OLETF 랫드에 비해 당뇨 유발이 개선된 결과로 판단된다. 혈중 포도당 농도, 인슐린 농도 및 인슐린 저항성 지표인 HOMA-IR 수치가 용량 의존적으로 감소하였다. 또한 GLUT4 유전자 발현을 측정한 결과 대조군에 비해 동충하초(Isaria tenuipes) 투여군의 mRNA 및 단백질이 용량 의존적으로 증가하는 경향을 보였다. 따라서, 동충하초(Isaria tenuipes)의 섭취가 제2형 당뇨에서 인슐린의 감수성이나 저항성에 영향을 미치고, 혈당을 강화시키는 것으로 사료되므로 당뇨병환자의 혈당저하에 기여할 것이라 기대한다. 이에 본 연구는 천연물질을 이용한 당뇨병의 예방 및 치료에 동충하초(Isaria tenuipes)를 사용할 수 있는 이론적 토대를 제공할 것으로 생각된다.

Genome-wide hepatic DNA methylation changes in high-fat diet-induced obese mice

  • BACKGROUND/OBJECTIVES: A high-fat diet (HFD) induces obesity, which is a major risk factor for cardiovascular disease and cancer, while a calorie-restricted diet can extend life span by reducing the risk of these diseases. It is known that health effects of diet are partially conveyed through epigenetic mechanism including DNA methylation. In this study, we investigated the genome-wide hepatic DNA methylation to identify the epigenetic effects of HFD-induced obesity. MATERIALS AND METHODS: Seven-week-old male C57BL/6 mice were fed control diet (CD), calorie-restricted control diet (CRCD), or HFD for 16 weeks (after one week of acclimation to the control diet). Food intake, body weight, and liver weight were measured. Hepatic triacylglycerol and cholesterol levels were determined using enzymatic colorimetric methods. Changes in genome-wide DNA methylation were determined by a DNA methylation microarray method combined with methylated DNA immunoprecipitation. The level of transcription of individual genes was measured by real-time PCR. RESULTS: The DNA methylation statuses of genes in biological networks related to lipid metabolism and hepatic steatosis were influenced by HFD-induced obesity. In HFD group, a proinflammatory Casp1 (Caspase 1) gene had hypomethylated CpG sites at the 1.5-kb upstream region of its transcription start site (TSS), and its mRNA level was higher compared with that in CD group. Additionally, an energy metabolism-associated gene Ndufb9 (NADH dehydrogenase 1 beta subcomplex 9) in HFD group had hypermethylated CpG sites at the 2.6-kb downstream region of its TSS, and its mRNA level was lower compared with that in CRCD group. CONCLUSIONS: HFD alters DNA methylation profiles in genes associated with liver lipid metabolism and hepatic steatosis. The methylation statuses of Casp1 and Ndufb9 were particularly influenced by the HFD. The expression of these genes in HFD differed significantly compared with CD and CRCD, respectively, suggesting that the expressions of Casp1 and Ndufb9 in liver were regulated by their methylation statuses.

뇨중 대사체 정량에 의한 capsaicinoid의 생화학적 섭취지표 개발 연구 (Capsaicin Intake Estimated by Urinary Metabolites as Biomarkers)

  • 최근 고추섭취량과 위암과의 양의상관도를 보이는 역학조사 결과가 발표되었는데 이런 자료를 신뢰하기 위해서는 향신료로 쓰이는 고추의 섭취량이 정확히 측정되어야 할 것이다. 이에 고추의 매운맛 성분 capsaicin의 인체대사산물을 확인하고 이를 응용하여 생리학적으로 의미있는 섭취량을 측정하려는 취지에서 실험을 계획하였다. 먼저 동물실험에서 관찰된 capsaicin의 대사체들을 소변에서 HPLC로 동시에 검출하는 방법을 고안하고 이들 대사체의 신속한 검색을 위해, 이전부터 사용하던 유기용매 추출을 지양하고 효소 glucuronidase 처리한 소변희석액을 바로 HPLC에 주입하는 방법을 사용하였다. 20대의 젊은 여성 5-6명을 대상으로, 이틀 간의 capsaicin 고갈식이 이후 capsaicin이 포함되지 않은 식사시 capsacin의 제공원으로서 고추장을 섭취하게 하였다. 60g씩 사홀 고추장을 섭취한 실험시 여섯명의 참가자 중 네명에게서 섭취를 시작한지 3일 째 4-hydroxy-3-methoxybenzoic acid가, 한 명에게서 3일 째 capsaicin이 검출되었다. 80g씩 나흘을 섭취한 2차 실험시는 세 명의 피실험자에게서 4-hydroxy-3-methoxybenzoic acid가 나흘째 소변에서 검출되었다. 2차 실험에서 나흘째 검출된 4-hydroxy-3-methoxybenzoic acid의 총량은 실험기간 중 섭취한 capsaicinoids 총량의 85% 이상이었다. 따라서 쥐 등을 이용한 capsaicinoid 대사 실험에서는 4-hydroxy-3-methoxybenyl alcohol이 주대사물질이었으나 인체에서는 그와 달리 4-hydroxy-3-methoxybenzoic acid가 주대사산물이라고 여겨진다. 그러나 1, 2차 실험식이를 충실히 따른 두명의 소변에서는 4-hydroxy-3-methoxybenzoic acid를 포함한 어떤 대사체도 찾을 수 없었기 때문에 개인간의 흡수 대사 차이가 크게 기여한다고 추정되며 생리적으로 의미 있는 capsaicin 섭취의 지표 물질은 인체를 대상으로 처음 얻어진 이 결과에 기반으로 한 계속 실험을 통해 선정되리라 기대한다.

성인여성의 요실금에 관한 기초 연구 (A Study on urinary Incontinence of Adult Women : Preliminary Study)

  • The purpose of this study was to identify the incidence of urinary incontinence in adult women and to identify factors related to life style and sexual intercourse that were related to incontinence. The sample consisted of 1,065 women living in Seoul or one of five provinces. Data for this study were collected from January 16 to June 23 using structured questionnaires. The Urinary Symptom Questionnaire developed by Jackson and a demographic questionnaire were used to collect the data. The data were analyzed using frequency, percentage with an SPSS/PC$^{+}$ program. The major finding are as follows : 1. The distribution of age of subjects was as follows : 20-29(10.1%), 20-39(17.8%), 40-49(27.3%). 50-59(22.3%), 60-69(12.9%), 70-79(6.1%). 80-89(2.8%), more than 90(0.6%). The frequency of normal delivery, 0(155%), 1-2(36.0%), 3-4(29.2%), 5-6(13.0%). more than 7(6.3%). The rate of subjects with menopause was 40.8%. 2. It was reported that 50.7% of the subjects experienced urinary incontinence with stress, mixed. and urgency incontinence being 49.8%, 43.4%, 6.8% respectively. 3. The lower urinary symptoms and incidence with urinary incontinence were as follows : Daily frequency. 22.3%, nocturia. 40.8%, urgency, 71.2%, bladder pain 47.8%, unexplainned incontinence, 32.4%, noctural incontinence, 16.1%, and frequency of incontinence, 37.7%. In term of quantity of incontinence, drop/pants damp, 29.5%, dribble/pants wet, 20.5%, flood or soaking through to outer clothing, 1.7%, and flood or running down legs or onto floor, 0.2%. 4. The symptoms related to sexual intercourse and incidence of urinary incontinence were as follows : dry vagina, 39.1%, sex life trouble, 10.8%, pain during sexual intercourse, 27.4%, and urine leakage during sexual intercourse. 8.8%. 5. Life style problems related to urinary incontinence were as follows : fluid intake restriction, 20.0%, affected daily task, 24.5%. avoidance of placess and situations, 35.0%, interference in physical activities, 30.6%, interference in relationships with other people. 19.0%. interference in relationships with husband/companion, 8.1%, and time after attack of urinary symptoms, 76.9%. In term of the feeling about the rest of their lifves the women reported : perfectly happy, 11.3%. pleased, 16.9%. mostly satisfied, 20.2%, mixed feelings, 21.0%, mostly dissatisfied, 21.0%, very unhappy 8.5%, and desperate. 1.0%. In conclusion. this study was a preliminary study to provide nursing practices guidelines for incontinence in adult women. Nurse working with adult women should develop and provide adequate care for these women.n.

선학초(짚신나물) 경구투여시 항암효과 탐색 및 약물 대사효소의 변화 (The Anticancer Effects and Drug Metabolic Enzyme Change by Oral Intake of Agrimonia Pilosa Ledeb)

  • Objective : This research was aimed to investigate the anti-tumor effect, safety, mechanism and metabolizing enzyme of Agrimonia pilosa LEDEB(APL) in female C57B/L mouse. Methods : At first, to evaluate the anti-tumor activity of APL, we divided into four groups, normal, control, APL100(100mg/kg), APL150(150mg/kg). LLC obtained American Type Culture Collection was used. LLC had been inoculated to induce tumor. To measure the anti-tumor effect of APL, we calibrate tumor size and weight. To study for mechanism of anti-tumor in APL, we used western blotting and to know metabolizing enzyme in APL we used to real-time PCR. Results : APL100, APL150 inhibited tumor growth after medicine injected. APL did not only induced caspase-dependent apoptosis in LLC-bearing mouse tumor. In APL100, it were decreased 72% in CYP3A11. In APL150, it were decreased 62%, 75% in CYP3A11 and MRP1a respectively. Conclusion : These results suggests that APL has some anti-tumor effects in female C57B/L mouse tumor. APL should be careful use with other drugs related with CYP3A11 or MRP1a.

농촌 노인의 스트레스와 주관적 구강건조감의 관련성: 예비조사 (Relationship between stress and subjective oral dryness in the elderly in a rural region: a pilot study)

  • Objectives: Stress is a psychological reaction to stimuli such as anxiety or threat felt by a person either physically or mentally when placed in a difficult situation. Although a relationship between stress and dry mouth has been reported, it remains understudied. The purpose of this study was to investigate the association between stress and subjective dry mouth among the elderly living in a rural region. Methods: A total of 214 people aged ${\geq}60years$ living in a rural area were recruited with an informed consent for a cross-sectional analysis. Information about stress and subjective dry mouth was obtained by an interview survey with a constructed questionnaire. Data on other potential confounding factors (including oral factors) such as socio-demographic data, health-related behaviors, the number of remaining teeth, and subjective chewing ability were also gathered at the same time. Logistic regression models were used to analyze the relationship of stress and dry mouth using SPSS. Results: The Crude odds ratio (OR) of stress awareness affecting subjective dry mouth was 2.59 (95% confidence interval: 1.43-4.68). After adjusting for sex, education, income, smoking, and alcohol intake, the adjusted OR was 2.52 (95% confidence interval: 1.30-4.87) which was statistically significant. Conclusions: Elderly people who were stressed had an approximately 2-fold increase in experiencing subjective dry mouth when compared to their stress-free counterparts.

수면 무호흡증 관리를 위한 섬유센서 기반의 슬립 모니터링 스마트 웨어 개발에 관한 연구 (A Study on the Development of Sleep Monitoring Smart Wear based on Fiber Sensor for the Management of Sleep Apnea)

  • 본 연구에서는 수면무호흡증이 다양한 합병증을 유발하는 질병으로서, 이를 위한 표준화된 수면다원검사가 고가이며 측정이 불편함을 고려하여, 환자가 익숙한 환경에서 검사를 받을 수 있도록 스트레치 섬유센서를 이용한 무구속 상태의 웨어러블 모니터링 시스템을 개발하고자 한다. 또한, 의복과 일체화된 디자인으로 제품화하여 그 유용성을 제안하고자 한다. 성인 남성용 롱 슬리브 티셔츠 형태로 착용형 수면무호흡 모니터링 스마트 의류를 개발하여, 수면무호흡 진단 지표 중 섬유형 센서를 기반으로 측정하는 호흡수(breathing rate)측정용 섬유센서, 산소포화도(oxygen saturation), 호흡기류(airflow)를 실시간 측정하였다. 최적의 조합비 샘플 4가지를 통한 gauge factor를 측정한 결과, gauge factor 20.3을 확인하였고, 3회 숨을 크게 내쉴 때 호흡유량이 최대 2048ml였다. 산소포화도 69.45%로 최소 측정 가능 산소포화도 70% 정도의 결과를 나타내었다. 세탁 후 인장에 따른 센서 성능 평가 결과는 전체적으로 gauge factor 18 수준의 값으로, 세탁에 대한 내구성이 입증되었다. 본 연구를 통하여 수면무호흡증 환자의 수면효율이 떨어지고 여러 합병증을 동반하는 문제에 대해, 가정에서 쉽게 사용할 수 있는 착용형 수면무호흡 모니터링 스마트 의류를 개발하여 호흡수, 호흡유량, 산소포화도의 3가지 지표를 측정 가능함을 확인하였다.
