인공 무릎 관절의 3차원 운동 시뮬레이션 (Simulation of Three Dimensional Motion of the Knee Joint in Total Knee Arthroplasty)

  • Severe osteoarthrosis of the knee joint often requires total knee arthroplasty(TKA) to yield adequate knee function. The knee joint with TKA is expected ideally to restore the characteristics, however, this is not necessarily 1.ue in the clinical cases. In this study the motion of the intact joint and the joint after. TKA were investigated numerically using computer simulation. For active knee extension from 90 degrees of flexion to full extension, the intact knee joint exhibited anterior tibial translation near the full extension and it showed only rotation at other flexion angles. Physiologic external rotation of the tibia near full extension known as screw home movement was also noted in the analytical model. The analysis of the tibial insert of three different shapes (flat, semicurved, and curved types) demonstrated characteristic rotational and sliding motion as well as different contact forces.

An anomalous pseudoganglion associated with high division of sciatic nerve

  • Sciatic nerve (SN) is the thickest and longest nerve of the body. Deviations from the normal anatomical origin and level of bifurcation of SN have been frequently reported. In the present case, we are presenting a unique scenario of origin of terminal branches of the SN-tibial nerve (TN) and common peroneal nerve (CPN) in the pelvic region itself from divisions arising directly from the lumbosacral plexus. This variation was associated with origin of posterior femoral cutaneous nerve from the superior division of CPN with anomalous communicating branches between pudendal nerve and TN. The unique characteristics of the present case are the presence of 'pseudoganglion' found on the inferior division of TN. The present case stands out as the first of its kind to mention such pseudoganglion. Knowledge of some unusual findings like presence of pseudoganglion and intercommunications between nerves have clinical implications in anesthesiology, neurology, sports medicine, and surgery.

Comprehensive Analysis of Chicken Vessels as Microvascular Anastomosis Training Model

  • Background Nonliving chickens are commonly used as a microvascular anastomosis training model. However, previous studies have investigated only a few types of vessel, and no study has compared the characteristics of the various vessels. The present study evaluated the anatomic characteristics of various chicken vessels as a training model. Methods Eight vessels-the brachial artery, basilic vein, radial artery, ulnar artery, ischiatic artery and vein, cranial tibial artery, and common dorsal metatarsal artery-were evaluated in 26 fresh chickens and 30 chicken feet for external diameter (ED) and thicknesses of the tunica adventitia and media. The dissection time from skin incision to application of vessel clamps was also measured. Results The EDs of the vessels varied. The ischiatic vein had the largest ED of $2.69{\pm}0.33mm$, followed by the basilic vein ($1.88{\pm}0.36mm$), ischiatic artery ($1.68{\pm}0.24mm$), common dorsal metatarsal artery ($1.23{\pm}0.23mm$), cranial tibial artery ($1.18{\pm}0.19mm$), brachial artery ($1.08{\pm}0.15mm$), ulnar artery ($0.82{\pm}0.13mm$), and radial artery ($0.56{\pm}0.12mm$), and the order of size was consistent across all subjects. Thicknesses of the tunica adventitia and media were also diverse, ranging from $74.09{\pm}19.91{\mu}m$ to $158.66{\pm}40.25{\mu}m$ (adventitia) and from $31.2{\pm}7.13{\mu}m$ to $154.15{\pm}46.48{\mu}m$ (media), respectively. Mean dissection time was <3 minutes for all vessels. Conclusions Our results suggest that nonliving chickens can provide various vessels with different anatomic characteristics, which can allow trainees the choice of an appropriate microvascular anastomosis training model depending on their purpose and skillfulness.

근위 경골 절골술 후 한방 재활치료 프로토콜을 적용한 재활치료의 임상적 효과: 증례 보고 (A Rehabilitation Protocol for High Tibial Osteotomy in Korean Medicine: A Report of 3 Cases)

  • In this study, we propose a rehabilitation protocol involving Korean medicine for patients after high tibial osteotomy (HTO) and report its effectiveness. Three patients underwent HTO using the rehabilitation protocol involving Korean medicine. We estimated the outcome evaluating physical examination findings of the knee joint, numeric rating scale, pain disability index, Lysholm knee score, and walking state. After the treatment, we observed that knee movement and muscular strength were improved. In addition, the numeric rating scale, pain disability index, Lysholm knee score, and walking state of patients were improved. Therefore, a rehabilitation protocol involving Korean medicine can be effectively used for patients after HTO. The limitation of this study was the insufficient number of cases and the difficulty of follow-up because of the characteristics of this operation and the long periods of time that are needed for each phase of the protocol. Further studies are needed to design a rehabilitation protocol involving Korean medicine.

  • The aim of this study is to present the basic reference data of age and specific gait parameters for comparisons of the gait characteristics depended on amputation length of the Unilateral Trans-Tibial Prostheses. The basic gait parameters were extracted from 10 Adult, and 20 below knee(B/K) patients, 50 to 60 years of age using VICON 512 Motion Analyzer. The results were as follows; 1. The mean Cadence of the above knee(A/K) patients and below knee(B/K) patients were $87.77{\pm}8.64$ steps/min, to $99.84{\pm}11.14$ steps/min.(p<0.05) 2. The mean Walking Speed of the above knee(A/K) patients and below knee(B/K) patients were $0.84{\pm}0.15$ m/s, to $0.96{\pm}0.25$ m/s.(p>0.05) 3. The mean Stride Length of the above knee(A/K) patients and below knee(B/K) patients were $1.14{\pm}0.14\;m$, to $1.14{\pm}0.22m$.(p>0.05) 4. The mean maximal angles of joint on the hip flexion motion for different above knee(A/K) patients and below knee(B/K) patients were $34.75{\pm}10.18_{\circ}$, to $32.32{\pm}6.34_{\circ}$.(p>0.05) 5. The mean maximal angles of joint on the knee flexion motion for different above knee(A/K) patients and below knee(B/K) patients were $66.97{\pm}15.08_{\circ}$, to $52.65{\pm}9.21_{\circ}$. (p<0.05) 6. The mean maximal angles of joint on the ankle dorsi-flexion motion for different above knee(A/K) patients and below knee(B/K) patients were $14.41{\pm}4.82_{\circ}$, to $10.04{\pm}3.49_{\circ}$.(p>0.05) 7. 인 수준이 다른 육계사료에 Crude Phytase 첨가시 생산성에 미치는 영향 (Effect of Crude Phytase Supplementation on Performance of Broilers Fed Different Levels of Phosphorus)

  • An experiment was conducted to measure the effect of crude phytase supplementation on the growing performance, blood concentrations of some minerals and tibia characteristics of broiler chickens. Day-old 240 male broiler chickens (Avian) were randomly allotted to four treatments. There were six replicates per treatment, and ten chicks per replicate. Treatments consisted of two levels of crude phytase (0 and 600U/kg) made from Aspergillus ficuum and two levels of non-phytate P (0.45 and 0.35% NPP for the starter period, and 0.35 and 0.25% NPP for the grower period), making the experiment 2$\times$2 factorial. The starter period was from hatch to 21 d of age, and grower period was from 22 to 35 d of age. Feed intake and weight gain of chicks fed diet containing phytase were higher(P〈0.05) than those of chicks fed diets without phytase, however, no differences was found in feed/gain. mortality, and nutrient availabilities regarding the phytase supplementation. Chickens fed diets with low NPP and phytase excreted lower P than did birds fed diets containing normal NPP without phytase. The level of NPP and phytase did not affect N excretion. The Ca availability was increased by feeding low NPP diet. Dietary phytase increased the availabilities of P and Mg, but decreased those of Fe and Zn. There was interactions between dietary NPP level and phytase addition on mineral availability. Tibia was lighter and shorter in low NPP groups, and heavier in phytase treated groups. The tibial contents of Ca, P and Mg decreased in low NPP treated groups, but increased in phytase treated groups. The ash content of tibia of chickens fed diet with phytase was higher than that of birds fed diets without phytase. These data suggest that the crude phytase supplementation to broiler diets containing low NPP level improves growth performance and mineral availability and, reduces fecal P excretion.

족삼리 취혈방식 제안: 신경해부학적 특성을 기반으로 (Suggestion on Locating Method for ST36 Acupoint Based on Neuroanatomical Features)

  • Objectives : There are many variations in the ST36 acupoint location. The purpose of this article is to suggest a method of locating the ST36 acupoint. Methods : Based on the available research and the neuroanatomical characteristics of the underlying acupoint, we summarized the proper procedure for finding the ST36 acupoint. Results : ST36 is 3 B-cun inferior to ST35 and is vertically situated on the line that connects ST35 and ST41. The ST36 acupoint corresponds to the deep peroneal nerve, which is situated in the tibialis anterior muscle's back. The neurovascular bundles that are located on the interosseous membrane between the interosseous crests of the tibia and fibula include the deep peroneal nerve, anterior tibial artery, and anterior tibial vein. According to both classical and modern literature, this acupoint can be found horizontally between the two muscles, tibialis anterior and extensor digitorum longus. Conclusions : Based on a review of the literature and neuroanatomical features, we suggest that ST36 can be positioned horizontally between tibialis anterior and extensor digitorum longus. Additional imaging studies and clinical proof are required to determine ST36 acupoint.

경골 근위부 개방 절골술 시 개방부 압력과 동종 쐐기 골편의 최대압축하중 비교 (Comparison of Gap Pressure in Opening Wedge High Tibial Osteotomy versus Compressive Strength of Allogenous Wedge Bone Blocks)

  • 목적: 이 연구의 목적은 동종 쐐기 골편의 성상과 최대압축하중과의 관계를 알아보고 동종 쐐기 골편의 최대압축하중과 경골 근위부 개방 절골술 시 측정된 개방부의 압력을 비교하는 것이다. 대상 및 방법: 경골 근위부 개방 절골술을 시행하기 전 연구에 동의한 10명의 환자가 본 연구에 포함되었다. 수술 중 절골 부위를 8 mm에서 14 mm까지 개방시키면서 1 mm 간격으로 개방부의 압력을 측정하였다. 대퇴골, 경골 및 상완골에서 U자형 동종 쐐기 골편을 채취하여 골편의 높이, 폭, 단면적 및 피질골의 두께를 측정한 뒤에 골편이 파괴되기 직전의 최대압축하중을 측정해 골편의 성상과 최대압축하중과의 관계를 평가하였다. 경골 근위부 개방 절골술 시 별도의 신연장치 없이 개방부에 동종 쐐기 골편을 삽입할 수 있는지 평가하기 위해 경골 근위부 개방 절골술 시 측정된 개방부 압력과 동종 쐐기 골편의 최대압축하중을 비교하였다. 결과: 경골 근위부 개방 절골술 중 절골부를 많이 개방할수록 개방부 압력이 증가해 14 mm 개방하였을 때 개방부의 압력(평균 282±93 N, 최대 427 N)이 가장 크게 측정되었다. 동종 쐐기 골편의 최대압축하중은 평균 13,379±6,469 N (최소값 5,868 N, 최대값 29,130 N)으로 측정되었으며 피질골의 두께(상관계수=0.693, p=0.002) 및 단면적(상관계수=0.826, p<0.001)과 유의한 상관관계를 가지고 있었다. 살균 방법에 따라 동결 건조(freeze-dried) 골편은 평균 13,406±5,928 N (최소값 5,868 N, 최대값 25,893 N), 동결(fresh frozen) 골편은 평균 13,348±7,449 N (최소값 5,916 N, 최대값 29,130 N)으로 측정되었다. 동종 쐐기 골편의 최대압축하중 최소값은 경골 근위부를 14 mm까지 개방하였을 때 측정된 개방부의 최대압력에 비해 13.7배 높았다(5,868 N vs. 427 N). 결론: 동종 쐐기 골편의 최소 압축 강도는 경골 근위부 개방 절골술 시 측정된 개방부의 최대 압력보다 충분히 큰 것으로 확인되었다. 따라서 경골 근위부 개방 절골술 중 별도의 신연장치 없이 동종 쐐기 골편을 개방부에 안전하게 삽입 가능한 것으로 판단된다.

Characteristics of Leg and Ankle in Taxi Drivers

  • For professional drivers, there is a possibility to have musculoskeletal disorders on ankle joint due to repetitive pedaling operation. Therefore, this study have focused to examine ankle active range of motion (AROM), dorsiflexor strength, and pressure pain threshold (PPT) of tibialis anterior muscle (TA) in taxi drivers compared to a age-matched control group. Thirty male taxi drivers with at least 10 years of driving experience and thirty male sedentary workers were evaluated for ankle AROM, dorsiflexor strength, and PPT of TA. Multiple independent t-tests were used to identify significant differences between two groups. For the results, taxi drivers had significantly less AROM in dorsiflexion and greater AROM in external tibial rotation compared to the control group. Also, dorsiflexor strength and PPT of TA in taxi drivers was significantly lower than in the control group. This study indicates that the repetitive ankle movements associated with driving have an effect on ankle AROM, dorsiflexor strength, and PPT of TA and may lead to work-related musculoskeletal disorders on ankle. Professional drivers may need to be educated to prevent a potential musculoskeletal disorders associated with repetitive movement.
