  • ERp29는 ER chaperone의 한 종류이지만 아직까지 세포내에서 그 기능은 명확하지 않다. 배양 갑상선세포 (FRTL-5)에서 ERp29의 발현은 TSH (thyroid stimulating hormone) 의존적으로 조절되며, ERp29가 과발현된 세포에서는 ERp29-ER chaperone가 긴밀한 관계를 유지하여 분비단백질인 Thyroglobulin의 분비량이 약 30% 증가되었다.

  • Objective: Graves' disease encompasses hyperthyroidism and diffused goiter associated with auto-antibodies to the thyroid stimulating hormone(TSH) receptors. In clinical environment, treatments of Graves' disease have many side effects such as recurrence and hypothyroidism. We've studied the effects of Sipyukmiyukieum on DNA synthesis, cAMP synthesis, and MHC-class II expression of FRTL-5 thyroid cells were studied. Methods: DNA synthesis was investigated by using BrdU staining and cAMP synthesis by ELISA kit, and expression of $interferon-{\gamma}$ activated MHC class II by Flow cytometer. Results: After introduction of Sipyukmiyukieum, significant inhibition of DNA synthesis. cAMP synthesis, and expression of $interferon-{\gamma}$ activated MHC class II of FRTL-5 thyroid cells was observed. Conclusions: Judging from these results, Sipyukmiyukieum has potential as a potent herbal treatment for inhibiting the enlargement of goiter, synthesis of abnormal thyroidal hormones, and autoimmuine responses of Graves' disease.

일 지역 치매의심 노인군에서 갑상선관련 호르몬, 인지기능, 우울증, 주관적 기억저하의 연관성 (Thyroid Hormones, Cognitive Impairment, Depression and Subjective Memory Complaint in Community-Dwelling Elders with Questionable Dementia in Korea)

  • Objectives It was the aim to examine the association of the thyroid-related hormones with cognitive function, depression, and subjective memory impairment in community-dwelling elders with questionable dementia. Methods The sample consisted of 399 community residents with 'questionable dementia' aged 60 or over in whom serum thyroid-related hormones [thyroid stimulating hormone (TSH) and thyroxine] had been assayed. Cognitive impairment was defined using the Korean version of the Consortium Establish a Registry for Alzheimer's Disease. Depression was diagnosed using the Korean version of Geriatric Depression Scale and subjective memory complaint (SMC) was checked using the subjective memory complaints questionnaire (SMCQ). Age, gender, education, and the presence of apolipoprotein E {\varepsilon}4 were included as covariates. Results There was a significant positive association between verbal fluency test (VFT) score and serum TSH levels (p = 0.01). There was a significant negative association between SMCQ total score and word list memory test (WLMT)(p = 0.002) or word list recall test (WLRT) score (p = 0.013). Conclusions Lower serum TSH levels were associated with semantic memory (VFT), and we found that SMC was associated with episodic memory (WLMT and WLRT) in this sample.

개심술 후 갑상선 호르몬치 변화에 대한 연구 (Alterations in Thyroid Hormone Levels After Open Heart Surgery)

  • 갑상선 호르몬은 체내에서 기초대사량을 증가시키고, 교감신경 흉분작용을 나타내며,심장에 대해서는 심근수축력, 심박수, 심근 산소소모량 등에 영향을 미친다. 개심술과 같은 대수술시나 심한 전신질환이 있을 때 흔히 갑상선 호르몬치의 변화를 보이게 되는 바 본 연구에서는 인공 체외순환이 갑상선 기능에 어떤 영향을 미치는지, 그리고 변화된 갑상선 기능이 개심술후 회복에 영향을 미치는 지에 대해 알아보고자 하였다. 1995년 4월부터 1996년 2월까지 전남대학교 의과대학 흉부외과학교실에서 인공 체외순환 하에 개심 술을 받은 20명의 환자를 대상으로 갑상선 기능을 평가하였다. 대상 환자 모두 술전에 갑상선 질환을 앓은 병력이나 갑상선 기능에 영향을 미칠 수 있는 어떤 형태의 시술, 검사, 및 약물치료를 받은 적은 없었다. 대상 환자를 두 그룹으로 나누었으며, 술후 회복에 특별한 문제가 없었던 환자군(Group 1, n=10), 술후 합병증(호흡부전, 저심박출증, 다장기 기능부전 등)이 발생한 환자군(Group 2, n=10)으로 강심제를 고용량으로 3일 이상 유지하였거나, 4일 미상 중환자실 체류를 필요로 한 경우로 하였다. 갑상선 기능의 평가는 말초\ulcorner액에서 측정한 triiodothyronine(73), thyroxine(74), free thyroxine(n4), 및 thyroid stimulating hormone(TSH) 농도를 기준으로 하였다. 인공 체외순환이 갑상선 호르몬 변화에 미치는 영 향을 알기 위해 술전 검사치와 비교분석하였으며, 갑상선 호르몬의 변화가 술후 예후에 미치는 영향을 알아보기 위해 두 그룹간의 검사치를 비교 분석하여 보았다. 혈청 T)는 인공 체외순환 후 10분에 최저 치(30.05*17.5ng/dl, p(0.001)로 감소하였다가 점차 증가하였으나 전체적으로 술전에 비해 의미 있게 낮았으며(p<0.05), 혈청 T4, fT4, TSH는 다소간의 변동을 보여, 체외순환을 사용한 개심술이 갑상선 기능에 변화를 초래하며, 모두 그룹간의 비교에서도 혈청 T3는그룹 1에 비해 그룹2에서 의미 있게 낮았으며(P<0.05), 술후 4일째 그룹 1에서는 정상수준으로 회복되었으나 그룹 2에서는 정상수준으로 회복되지 못한 점으로 미루어 이처럼 변화된 갑상선 기능이 술후 환자의 회복에 영향을 미침을 알 수 있었으며, 이는 집중 치료 중인 가사 상태의 환자에서 흔히 볼 수 있는 "euthyroid sick syndrome"과 유사한 양상이었다. 이상의 결과로 인공 체외순환이 갑상선 기능을 변화시키며, 갑상선 기능의 변화가 술후 환자의 예후에 영향을 미칠 수 있음을 시사하였다. 앞으로 더욱 정선되고 선택된 경우에 대한 많은 연구와 더불어 다음 단계로 갑상선 호르몬(73)의 투여가 실제로 술후 환자의 회복에 관련이 있는 지의 연구가 필요하리라 사료된다.요하리라 사료된다.

PTU(6-n-propyl-2-thiouracil)로 유발된 Rat 갑상선 기능저하증에 미치는 烏梅의 효과 (Effects of Mume Fructus on the Rat Hypothyroidism Induced by PTU(6-n-propyl-2-thiouracil))

  • Objectives : The aim of this study was to evaluate the effect of aqueous extracts of Mume Fructus(MF) on the 6-n-propyl-2-thiouracil(PTU)-induced rat hypothyroidism.Methods : Aqueous extracts of MF(yield = 19.38%) were administered, once per day for 42 days from 2 weeks before starting of PTU treatment as an oral dose of 300 and 150 ㎎/㎏(body weight), and hypothyroidism was induced by daily subcutaneous treatment of PTU 10 ㎎/㎏ for 28 days. The changes in the body weight, thyroid gland weights, liver weight, serum levels of thyroid hormone-thyroid stimulating hormone(TSH), tri-iodothyronine(T3) and thyroxine(T4), total cholesterol, low density lipoprotein(LDL), high density lipoprotein(HDL) and triglyceride (TG), aspartate aminotransferase(AST), alanine aminotransferase(ALT), liver antioxidant defense system-lipid peroxidation, H2O2, superoxide dismutase(SOD) and catalase(CAT) were examined with histopathology of thyroid glands and liver.Results : Results were compared with LevoT40.5 ㎎/㎏ treated rats. MF extracts recovered from the decreases in the body weight, liver weight, T3and T4, TG, liver CAT activities as results of PTU treatment. And MF extracts recovered from the increases of thyroid gland weights, TSH, HDL contents, liver H2O2, AST as results of PTU treatment. In addition, these PTU-induced histopathological changes in thyroid glands and liver related to hypothyroidism were dramatically decreased by treatment of both different dosages of MF extract, respectively.Conclusions : According to the above results, it is suggested that MF extracts have advantageous effects on the thyroid hormone productions with beneficial effects on the hypothyroidism related liver injuries mediated by the modulation on the antioxidant system.

Thyroid Function after Postoperative Radiation Therapy in Patients with Breast Cancer

  • Objective: The aim of this study was to assess thyroid function in breast cancer patients exposed to therapeutic external beam radiation. The focus was on possible progressive changes and any relationships between the incidence of primary hypothyroidism, the time required to become hypothyroid, and factors such as chemotherapy, hormonotherapy and immunotherapy. Materials and Methods: Seventy females undergoing 3D conformal and IMRT radiation therapy for breast cancers were enrolled in a non-randomized prospective study. The patients was divided into two groups: those after mastectomy or breast conserving surgery (BCS) were irradiated to a scar of the chest wall/breast and the ipsilateral supraclavicular and the axillary areas (supraclavicular radiotherapy group - SC-RT group - 32 patients) and the control group receiving adjuvant chest wall/breast RT only (BCT group - 38 patients).The total doses were 50.0 to 70 Gy in 5 to 7 weeks. The median follow-up term was 24 months (range, 1-40 months). Thyroid function was evaluated by measuring thyroid stimulating hormone (TSH), free thyroxine (fT4), and free triiodothyronine (fT3) levels. The minimum, maximum and mean thyroid gland doses for 20 Gy (V20) were calculated for all patients. Results: Statistically significant results were obtained for the SC-RT group. Two yearsa fter the end of RT the chance of an event was increased in 6% of the population (p=0.009) in the SC-RT group. In the BCT group no significance was noted. No statistically significant differences were found for V20, chemio-, immunotherapy and hormonotherapy or Ki67 values (p=0.12). No significant results were obtained for development of hypothyroidism and clinical factors (age, thyroid volume, treatment modalities). Conclusion: Radiotherapy is associated with a higher incidence of thyroid toxicity in breast cancer patients. Routine thyroid function monitoring should be recommended in such cases.

호르몬수면상실이 에너지와 대사에 미치는 영향 (The Effect of Sleep Loss on Energy and Metabolism)

  • The release of hormones and the metabolism of human body are controlled by the circadian rhythm related to sleep-wake cycle. Growth hormone, prolactin, thyroid stimulating hormone, cortisol, glucose, and insulin-secretion rates fluctuate according to the sleep-wake cycle. In addition, sleep is related to the appetite regulation and carbohydrate and other energy metabolism. Hypocretin (orexin), an excitatory neuropeptide, regulates waking and diet intake, and the poor sleep increases diet intake. The short sleep duration increases one's body mass index and impairs the function of the endocrine and metabolism, causing increases in the risk of glucose intolerance and diabetes. The poor sleep quality and sleep disorders have similar impact on the metabolic function. In short, the sleep loss and the poor quality of sleep have a detrimental effect on the endocrine and energy metabolism. The improvement of sleep quality by the future research and appropriate clinical treatment would contribute to the decrease of the metabolic diseases such as diabetes.

인삼Saponin의 산가수분해물이 Epididymal Adipose Tissue의 지방대식에 미치는 영향 (Effect of Acid Hydrolyzates of Ginseng Saponins on Lipid Metabolism in Rat Epi didymal Adipose Tissue)

  • Studies were carried out to clarify the effect of ginsenoside-Rbl -Rbr and acid hydrolyzatps of ginsenoside-Rbl, -Rb2 (HRbl, HRbf) on lipolysis and lipogenesis induced by epinephrine, glucagon, ACTH (adrenocorticotrophic hormone), TSH (thyroid-stimulating hormone) and insulin in rat adipose tissilr. HRbl , HRb2 slightly inhibited lipolysis induced by epinephine. glucagon and TSH. ACTH-induced lipolysis in fat tissue slices was significantly inhibited by ginsenoside -Rbl, -Rb2, HRbl and HRb2, particulary HRb2. None of ginsenoside-Rbl, -Rb2, HRbl and HRb2 accelerated insulin-stimulated lipogenesis in fat calls. Among ginseng products, extract powder (freeze dried), extract powder (spray dried), red ginseng powder inhibited ACTH-induced lipolysis in adipose tissue slices, but red ginseng extract not affect them.

초음파검사로 진단된 갑상선결절의 융복합 위험요인 (Convergence Risk Factors for Thyroid Nodules Diagnosed Ultrasound)

  • 최근 초음파검사의 접근이 일반화되면서 한국에서는 갑상선암발생율의 연간변화율이 24.5%로 상승하였다. 이에 고식적 갑상선암의 위험요인인 전리방사선과 요오드식이 이외의 갑상선암의 위험요인을 파악하고자 하였다. 2011년 1월부터 2013년 3월까지 종합검진센타에서 초음파검사 후 미세침흡인검사로 진단된 양성결절 260명과 악성 결절 151명의 여자를 대상으로 나이, 혈액학적 수치, 비만지수, 허리둘레, 폐경여부, 유방결절 유무, 자근근종 유무, 지방간 유무를 비교하였다. 연구 결과 갑상선암 그룹에서 비만인 경우, 유방의 결절과 자궁근종이 있는 경우가 많았으며 갑상선자극호르몬 수치가 양성결절그룹보다 높게 나타났다. 향후 갑상선 및 갑상선암에 영향을 끼치는 요인들을 정립시키기 위한 연구가 지속적으로 필요할 것이다.
