Study of matching user operation name and operation classification code (ICD-9-CM) (Through OCS program use facilitation at operating room) (사용자 수술명과 수술분류 code (ICD-9-CM) 일치율 향상에 관한 연구 (수술실 OCS program 사용 활성화를 통하여))

  • Background : The necessity of unify and standardize codes used at hospital has been emphasized since OCS (Order Communicating System) was adopted. Therefore, the purpose of this study were to standardize operation code by continuous training of the ICD-9-CM code that is used as standard code in OCS program at operating room. Method : In 400 operation data, operation code entered in OCS program at operating room was compared to operation name recorded in medical record. In addition, a matching rate between input data of operation code by medical record department and computing input data of operation code in 3,710 cases was compared for each department. User operation name and operation code were matched and major diagnosis by operation department and operation name were also matched. Results : User operation name was reflected in operation classification code in detail, and operation code entered on user was registered. Input rate and matching rate of operation code were gradually improved after improvement activity. In particular, a matching rate was high at ophthalmology where operation name is segmented. Plastic surgery and orthopedics with a lot of emergency operation and comprehensive operation name show low input rates. Conclusions : As the medical field makes progress in computerlization, awareness of information exchange and sharing becomes higher. Among codes to classified medical institution, codes related to surgical operation are all different by user of hospital and department. Computerlization and standardization is essential. And when efforts of standardization continue in alliance with individual hospital and institution, initiative of preparing medical policy data at a national level will be accelerated.

  • Background : in order to adapt to changes of the medical environm interests that is drawn in ambulatory surgery are increased as a method of approaching a patients' satisfaction and cost-effective management. The purpose of this study is to a assess the operation which is able to perform through ambulatory care unit, to identify the problem in ambulatory surgery, and to increased the opportunity of ambulatory surgery with safety. Methods : Between May 13th, 1998 and June 30th, 2000, we performed surgical treatment through ambulatory care unit, and evaluate the results of them. The sorts of operation, duration of stay in the hospital, total cost of treatment, satisfaction of patients and safety if anesthesia were assessed. Results : We performed ambulatory surgery without serve complications and the patients were satisfied with surgical treatment through ambulatory care unit. In comparison of ambulatory and admission surgery, there was a reduction of cost to 16.7~25.3% in ambulatory surgery. Also, the duration of admission was 2 days shorter than admission surgery. Conclusions : According to our results, the surgical treatment through ambulatory care unit is safe and useful method that increase the quality of medical service, satisfaction of patients and reduce the cost of treatments.

Primary Laparoscopic-Assisted Endorectal Pull-Through for Hirschsprung's Disease (히르슈슈프룽병의 일차성 복강경 보조 Endorectal Pull-Through 술식의 임상적 고찰)

  • The application of laparoscopic techniques for the surgical management of Hirschsprung's disease is the recent trend. We described the surgical technique and postoperative long-term outcomes of the one-stage, laparoscopic-assiseted endorectal pull-through operation for Hirschsprung's disease. The technique uses three to four small abdominal ports. Laparoscopic mobilization of the sigmoid colon and rectum is performed and marginal artery-preserving colon pedicle is prepared. The rectal mobilization is performed using a transanal endorectal sleeve technique. The anastomosis is performed 0.5~1 cm above the dentate line. The age at surgery ranged from 6 days to 4 years. The average operative time was 144 minutes. Almost all of the patients passed stool and flatus within 36 hours of surgery. The average hospital stay after surgery was 6.5 days. Among 42 patients, 32 patients older than 3 years old were evaluated for function on defecation. All 32 patients have been continent, of those who needed laxatives were 11 (34.3%) due to constipation and overflow incontinence. Four children (12.5%) have remained dependent on laxatives. Laparoscopic-assisted endorectal pull-through operation for Hirschsprung's disease appears to be safe, provides the less pain, shorter time to full feeding, shorter hospital stay, and excellent cosmetic outcomes. Helping patients and parents ensure the quality of life, they should be provided with counseling, education, and longer-term follow-up care.

Design of digital nuclear power small reactor once-through steam generator control system

  • The once-through steam generator used in the small modular reactor needs to consider the stability of the outlet steam pressure and steam superheat of the secondary circuit to achieve better operating efficiency. For this reason, this paper designs a controllable operation scheme for the steam pressure and superheat of the small reactor once-through steam generator. On this basis, designs a variable universe fuzzy controller, first, design the fuzzy control rules to make the controller adjust the PI controller parameters according to the change of the error; secondly, use the domain adjustment factor to further subdivide the input and output domain of the fuzzy controller according to the change of the error, to improve the system control performance. The simulation results show that the operation scheme proposed in this paper have better system performance than the original scheme of the small reactor system, and controller proposed in this paper have better control performance than traditional PI controller and fuzzy PI controller, what's more, the designed control system also showed better anti-disturbance performance in lifting experiment between 100% and 80% working conditions. Finally, the experimental platform formed by connecting the digital small reactor with Matlab/Simulink through OPC(OLE for Process Control) communication technology also verified the feasibility of the proposed scheme.

Analysis of Train Operation Obstacle Using Number of Failures and Delay Time of Electric Door System (전기식 도어시스템의 고장건수 및 지연시간을 활용한 열차운행장애 분석)

  • This paper analyzes functions of component parts of D-Urban Railway's door system along with operation obstacle risks on frequency(the number of occurrences/year) and severity(delay time/the number of occurrences). Based on this, the paper presents improvements and current system's problems after obstacle risks of EMU and door system are appled. The obstacle of door system causes corrosion of main parts such as DCU due to heat problem of operation environment, problems of maintenance methods and deterioration. DCUs on PCBs with more than 50% pattern corrosion cause problems. Even though the number of door system's obstacle occurrences for the last 5 years is 42, along with 104 minutes of operation obstacle, EMU operation obstacle risk is low(Level 1), which indicates there is limit in matrix of railway risks presented by the standard of railway safety management system. Therefore, it is necessary to have railway risk matrix suitable for the field. Finally, the paper deducts the obstacle risks through frequency and severity. Since 2017 when the risks of EMU and door system's obstacle, that of EMU has been 24(47% reduced) and that of door system has been average 9.5 per year(23% reduced).

  • Bogie(The Launch vehicle transporter) is the special land transportation vehicle for the LV (Launch vehicle) which is used for not only the transportation-means of LV from LVAB(Launch Vehicle Assembly Building) to LP(Launch Pad) but also one of principal tools for launch operation sequence. Due to the multi-role of Bogie, The technical requirements of bogie are very complicated. Especially launch operation has performed with not only bogie but also many ground support equipments. So, it is impossible to establish technical requirements of bogie without whole understanding of launch operation. This paper was made for arranging whole launch operation sequence and technical requirements of bogie based on the this sequence.

  • In digital economy, information systems are recognized as the most critical survival factor of companies. Companies must launch and expand business based on information systems either developed newly or adopted as ERP. However, successful information systems operation shall face many realistic problems. Especially most small and medium enterprises experience many difficulties in systems operation or even give up operating information systems due to unexpected operational problems. This paper suggests a framework for effective information systems operation for small and medium enterprises. The framework is designed through case studies, consisting of service framework, auditing framework, and risk management framework. It is expected that the framework contributes to successful information systems operation in small and medium enterprises.

SPCBC: A Secure Parallel Cipher Block Chaining Mode of Operation based on logistic Chaotic Map

  • Several block cipher modes of operation have been proposed in the literature to protect sensitive information. However, different security analysis models have been presented for attacking them. The analysis indicated that most of the current modes of operation are vulnerable to several attacks such as known plaintext and chosen plaintext/cipher-text attacks. Therefore, this paper proposes a secure block cipher mode of operation to thwart such attacks. In general, the proposed mode combines one-time chain keys with each plaintext before its encryption. The challenge of the proposed mode is the generation of the chain keys. The proposed mode employs the logistic map together with a nonce to dynamically generate a unique set of chain keys for every plaintext. Utilizing the logistic map assures the dynamic behavior while employing the nonce guarantees the uniqueness of the chain keys even if the same message is encrypted again. In this way, the proposed mode called SPCBC can resist the most powerful attacks including the known plaintext and chosen plaintext/cipher-text attacks. In addition, the SPCBC mode improves encryption time performance through supporting parallelized implementation. Finally, the security analysis and experimental results demonstrate that the proposed mode is robust compared to the current modes of operation.

Development of 6kW ZVS Boost Converter by 4-Parallel Operation (4-병렬 제어 기법을 적용한 6kW 영전압 스위칭 승압형 컨버터 개발)

  • This paper presents development of 6kw ZVS(Zero Voltage Switching) boost converter by 4-parallel operation. To realize a high capacity converter with 6 kw, 4-parallel operation of 1.5kW unit module is proposed in this paper. To meet high ratio input to output voltage, isolated type booster converter is designed. To achieve ZVS operation of 4-switches of full bridge and protect a voltage overshoot caused by switch turn-off, simple active-clamp circuit is applied to the primary side. For parallel operation of 4-modules, master-slave control method is proposed to achieve input current sharing of 4-unit converter modules accurately. For performance tests, simulation is carried out. Also, load and experimental tests of the developed booster converter, 230Vdc/6kW, are carried out under various conditions. For field tests, the developed converter is applied for boosting a battery power to high DC_link voltage for a VSI inverter which starts a micro-turbine(MT) installed in vehicle and it's performance is verified through high speed motoring a MT up to tens of thousands of rpm.
