The Effect of Pulmonary Rehabilitation in Patients with Chronic Lung Disease (만성 폐질환 환자에서의 호흡재활치료의 효과)

  • Background : It is known that pulmonary rehabilitation improves dyspnea and exercise tolerance in patient with chronic lung disease, although it does not improve pulmonary function. But there is a controversy whether this improvement after pulmonary rehabilitation is due to increased aerobic exercise capacity. We performed this study to evaluate the effect of pulmonary rehabilitation for 6 weeks on the pulmonary function, gas exchange, exercise tolerance and aerobic exercise capacity in patients with chronic lung disease. Methods : Pulmonary rehabilitations including education, muscle strengthening exercise and symptom-Umited aerobic exercise for six weeks, were performed in fourteen patients with chronic lung disease (COPD 11, bronchiectasis 1, IPF 1, sarcoidosis 1 ; mean age $57{\pm}4$ years; male 12, female 2). Pre- and post-rehabilitaion pulmonary function and exercise capacity were compared. Results: 1) Before the rehabilitation, FVC, $FEV_1$ and $FEF_{25-75%}$ of the patients were $71.5{\pm}6.4%$. $40.6{\pm}3.4%$ and $19.3{\pm}3.8%$ of predicted value respectively. TLC, FRC and RV were $130.3{\pm}9.3%$, $157.3{\pm}13.2%$ and $211.1{\pm}23.9%$ predicted respectively. Diffusing capacity and MVV were $59.1{\pm}1.1%$ and $48.6{\pm}6.2%$. These pulmonary functions did not change after pulmonary rehabilitation. 2) In the incremental exercise test using bicycle ergometer, maximum work rale ($57.7{\pm}4.9$) watts vs. $64.8{\pm}6.0$ watts, P=0.036), maximum oxygen consumption ($0.81{\pm}0.07$ L/min vs. $0.96{\mu}0.08$ L/min, P=0.009) and anaerobic threshold ($0.60{\pm}0.06$ L/min vs. $0.76{\mu}0.06$ L/min, P=0.009) were significantly increased after pulmonary rehabilitation. There was no improvement in gas exchange after rehabilitation. 3) Exercise endurances of upper ($4.5{\pm}0.7$ joule vs. $14.8{\pm}2.4$ joule, P<0.001) and lower extremity ($25.4{\pm}5.7$ joule vs. $42.6{\pm}7.7$ joule, P<0.001), and 6 minute walking distance ($392{\pm}35$ meter vs. $459{\pm}33$ meter, P<0.001) were significantly increased after rehabilitation. Maximum inspiratory pressure was also increased after rehabilitation ($68.5{\pm}5.4$ $CmH_2O$ VS. $80.4{\pm}6.4$ $CmH_2O$, P<0.001). Conclusion: The pulmonary rehabilitation for 6 weeks can improve exercise performance in patients with chronic lung disease.

  • There is the uncertain period of the construction in case of the products meeting the quality standard of KS M 6080, the quality is degraded because of the abrasive loss of the paint caused by vehicle tires as the traffic amount increases and of the loss and detachment of the glass beads providing the retroreflective function. The abrupt degradation of visibility causes the high frequency of traffic accidents at night and increases the traffic accident rate. Additional supplementary construction induces the direct material and construction costs. As the more cost induction effect than the direct cost, the traffic jam caused by the additional construction increases the indirect social costs such as time cost and vehicle cost. Hence, the study is concerned with performing the abrasion resistance test based on the EN 1436 standard to check and improve the quality of various road marking materials resulting in improving the durability of road marking materials. However, even though the difference in the durability lifetime of resins(binders) is bibliographically or theoretically clear, there was no difference in the durability lifetime (retroreflectivity aspect) of the road marking paint using these binders. The reason is that the bonding of beads was very insufficient or that the cross density caused by crack or freshness was low. Moreover, the measured wet retroreflectivity was distributed as the Rw3 or higher class in average on the basis of EN 1436 but was very insufficient on the basis of the minimum wet threshold retroreflectivity with 100mcd/($m^2{\cdot}lx$) managed overseas.

Effect of Temperature on the Biological Attributes of the Brown Lacewing Micromus angulatus (Stephens) (Neuroptera: Hemerobiidae) (갈고리뱀잠자리붙이의 생물적 특성에 미치는 온도의 영향)

  • This study was performed to investigate the biological characteristics of the brown lacewing Micromus angulatus (Stephens) at four different temperatures (15, 20, 25, and $30^{\circ}C$) and $70{\pm}10%$ relative humidity under a photoperiodic regime of 16:8(L:D) h. The developmental threshold and effective accumulative temperature from egg to pupa for M. angulatus were $9.6^{\circ}C$ and 270.3 day-degrees, respectively. The developmental period of egg, larva, and pupa at $25^{\circ}C$ was 4.4, 5.5, and 6.9 days. respectively. The longevity of an adult female M. angulatus was 34.9 days. The oviposition period for M. angulatus was 28.7 days, in which it laid a total number of 515.2 eggs during its life span. The maximum number of eggs laid by a female in a day was 54.8. Daily consumption by M. angulatus at $25^{\circ}C$ was 18.9 $1^{st}$ instars, 47.2 $2^{nd}$ instars, 57.7 $3^{rd}$ instars, and 91.0 adults of the glasshouse potato aphid, Aulacorthum solani. Therefore, M. angulatus could be a promising biological control agent against aphids.

Effects of tianeptine on symptoms of fibromyalgia via BDNF signaling in a fibromyalgia animal model

  • Previous reports have suggested that physical and psychological stresses may trigger fibromyalgia (FM). Stress is an important risk factor in the development of depression and memory impairments. Antidepressants have been used to prevent stress-induced abnormal pain sensation. Among various antidepressants, tianeptine has been reported to be able to prevent neurodegeneration due to chronic stress and reverse decreases in hippocampal volume. To assess the possible effect of tianeptine on FM symptoms, we constructed a FM animal model induced by restraint stress with intermittent cold stress. All mice underwent nociceptive assays using electronic von Frey anesthesiometer and Hargreaves equipment. To assess the relationship between tianeptine and expression levels of brain-derived neurotrophic factor (BDNF), cAMP response element-binding protein (CREB), and phosphorylated cAMP response element-binding protein (p-CREB), western blotting and immunohistochemistry analyses were performed. In behavioral analysis, nociception tests showed that pain threshold was significantly decreased in the FM group compared to that in the control group. Western blot and immunohistochemical analyses of medial prefrontal cortex (mPFC) and hippocampus showed downregulation of BDNF and p-CREB proteins in the FM group compared to the control group. However, tianeptine recovered these changes in behavioral tests and protein level. Therefore, this FM animal model might be useful for investigating mechanisms linking BDNF-CREB pathway and pain. Our results suggest that tianeptine might potentially have therapeutic efficacy for FM.

Evaluation of yield and growth responses on paddy rice under the extremely high temperature using temperature gradient field chamber (온도구배야외챔버를 이용한 고온에서의 벼 생육반응 및 수량성 평가)

  • The effect of elevated temperature on temperate paddy rice will be significant for dependable food supply in East Asia. Using temperature gradient field chamber (TGFC), which was designed to make the horizontal air temperature gradient by $0^{\circ}C$ to $3^{\circ}C$ higher than outside, we examined the measurement to understand the effects of extremely high temperature on paddy rice. In particular, the data of the year 2016, the worst heat wave in over 22 years, was analyzed in this study. The rice height in the relatively warmed condition was rapidly increased during early growth stage. However, the average grain weight and number of spikelet per panicle in the warmed chamber condition were gradually declined with increasing air temperature averaged for 40 days after first heading in each chamber. In particular, the grain yield was more dramatically decreased by the raising temperature because the percent ripened grain was quickly dropped as getting over the threshold temperature for pollination. Therefore, the surplus photosynthetic product by such lower grain filling rate may disturbed the decreases of the NDVI (Normalized Difference Vegetation Index) and SPAD chlorophyll values after first (normal) heading. In addition, the late-emerging head grain were appeared. However, this yield was too small to recover the normal yields decreased by extremely high temperature condition. Our result represented that the warmed condition in 2016 would be the critical limit for the stable yield of temperate paddy rice.

A Study on Rational Improvement of the Pension Income and Retirement Income Taxation (연금소득 및 퇴직소득 세제의 합리적 개선방안에 관한 연구)

  • [Purpose] The purpose of this study is to point out the issue of pensions and retirement income tax which were amended from 2012 to 2016 and present a rational and intellectual improvement to this issue. [Methodology] This study was carried out a literature survey about the tax laws of pension income and retirement income. In order to confirm the differences between the case-by-case tax burden, calculated the effect of tax burden by presenting the examples in the case of retirement income. [Findings] It is necessary to apply the differential multiples in accordance with the seniority when leaving the company rather than applying the five times or 12 times in the tax base as the current retirement income tax. Also, It should be given the flexibility of pension income to ease the requirements of inevitable temporary receipt of a pension income. In addition, it is needed to expand separate taxation threshold which is current 12 million won. Finally, the annual limit of tax incentives for IRP should be increased significantly more than the current 7 million won. [Implications] The improvement of the study is to offer suggestions on the revision of retirement income and pension income taxation to the tax authorities or National Pension Service and retirement pension providers. The result of this study is expected to be used as reference to develop policy options on the legislative process.

The Effects of Bee Venom Acupunture Therapy on Shoulder Pain Patients in Stroke Sequelae (봉약침이 중풍후유증으로 인한 견비통에 미치는 치료 효과에 관한 연구)

  • Objective : The aim of this study is to investigate the curative effect of Bee Venom Acupuncture Therapy for pain and limited R.O.M (range of motion) of shoulder in stroke patients. Methods : The subjects of this study were 6 patients with shoulder pain in stroke sequelae. Routine Oriental Medical programs (Acupunture, moxibustion, herbal medicine and physical therapy) were maintained for each subject throughout this study. Single subject ABABAB design was adopted. Each period was 4 days as a rule. Only during the treatment period, Bee Venom Acupuncture Therapy was provided as intervention at the acupoints of LI15(Gyeonu), TE14(Gyeollyo), GB21(Gyeonjeong), LI14(Bino). The change of pain was measured with a Visual Analogue Scale(VAS). The pain threshold was measured using pressure algometer at the same acupoints where Bee Venom Acupuncture Therapy was provided. And the R.O.M of shoulder joint (flexion, extension, abduction, adduction, external rotation, internal rotation) was measured as well. Analysis was performed by Bayesian analysis using WinBUGS for the comparison of treatment(Bee Venom Acupuncture Therapy) and non-treatment. Results : The median overall improvement for difference in VAS was -2.219(-3.213, -1.175), for difference in external rotation of shoulder R.O.M was 9.992(-2.298, 18.49), for difference in tenderness score of LI14(Bino) by pressure algometer was 5.05(0.6283, 7.762). 95% credibility intervals being shown in brackets. However, the median overall improvement for difference in the other measurements was not significant. Conclusion : This study suggests that Bee Venom Therapy may be applicable to decrease pain and improve R.O.M of shoulder in hemiplegia patients with stroke. Further elaborated single subject designs need to be accumulated to confirm the effects of Bee Venom Acupunture Therapy on shoulder pain in patients with stroke sequelae.

Short-Term Storage and Cryopreservation of Abalone (Haliotis discus hannai) Sperm

  • In present study, attempts were made to preserve abalone (Haliotis discus hannai) sperm in liquid form at low temperature, to evaluate the effect of various diluents in short-term storage on sperm, and cryopreservation procedures were optimized for the cryoprotectants as well as freezing rates, in terms of the motility and survival rate, and the ultrastructural changes of sperm after short-term storage and cryopreservation were observed. The abalone sperm reached maximum motility until about 4min after activation. The motility was constant for about 16min, after which it dropped gradually, and about 50min later all motility ceased. Threshold activation of sperm was found in 40% artificial seawater (ASW), and motility increased as the concentration of ASW increased. In Hanks balanced salt solution without calcium (Ca-Free HBSS, 300 and 400 mOsmol/kg) and 10%, 20%, and 30% ASW the sperm was immotile, and motility once again restored incompletely only in HBSS of 300 and 400 mOsmol/kg, 20% and 30% ASW after 100% ASW was added. Sperm motility was extended following 20 days of cold storage only in 70% and 100% ASW. A high motility index of 3.5-4.5 was observed for the first 8 days in 70% and 80% ASW. In other diluents sperm motility was constant less than 10 days, and the motility index was obviously lower than that of sperm in 70% and 100% ASW. After 20 days of cold storage survival rates of 10.2%-20.7% were obtained in ASW and 300 mOsmol/kg HBSS, and that in 400 HBSS (65.3%) was significantly higher than others. The constant period of sperm motility stored in 70% ASW was longer obviously than that in 100% ASW after 6 days of storage, and the time to maximum motility of sperm stored in 70% increased gradually, while the difference in which of sperm in 100% ASW was not significant. The sperm plunged into liquid nitrogen all died except that sperm using 15% glycerol as cryoprotectant restored 10.4% of motility. The highest motility index (3.4) was obtained with 5% glycerol and freezing procedure: $50^{\circ}C$/min from $20^{\circ}C$ to $-80^{\circ}C$.

Quantitative Analysis of Artifactual Perfusion Defects due to the Cutoff Frequencies of Reconstruction Filters in Tc-99m-MIBI Myocardial SPECT Images (Tc-99m-MIBI 심근 SPECT에서 재구성필터의 차단주파수에 의한 인위적 관류결손의 정량적 평가)

  • Tc-99m-MIBI (Sestamibi) myocardial SPECT along with TI-201 tomographic Imaging has demonstrated wide application and high image quality sufficient for the diagnosis of myocardial perfusion defect, which consequently reflects regional myocardial blood flow. The qualitative values of myocardial SPECT with Tc-99m-MIBI as well ds the quantitative cases depend in some degree on the reconstruction techniques of multiple projections. Filtered backprojection (FBP) Is the common standard method for reconstruction rather than the complicated and time-consuming arithmetic methods. In FBP it is known that the distribution of radioactivity in reconstructed transverse slices varies with the selected litter parameters such as cutoff frequencies and order (Butterworth case) The cutoff frequencies used in clinicAl practice partially remove and decrease the true radioactive distribution and alter the pixel counts, which lead to underestimation of true counts in specific myocardial regions. In this study, we have investigated the effect of cutoff frequencies of reconstruction filter on the artifactually induced perfusion defects, which are often demonstrated near inferior and/or inferoseptal cardiac walls due to the intense hepatic uptake of Tc-99m-MIBI. A computerized method for Identifying the relative degree of artifactual perfusion defect and for comparing those degrees along with the relative amount of hepatic uptake to myocardium was developed and patient images were studied to observe the quantitative degree of underestimation of myocardial perfusion, and to propose some reasonable threshold of cutoff frequency in the diagnosis of perfusion defect quantitatively. We concluded that from the quantitative viewpoint cutoff frequencies may be used as high as possible with the sacrifice of homogeneity of image quality, and those frequencies lower than the common 0.3 Wyquist frequency would reveal severe degradation of radioactive distribution near inferior and/or inferoseptal myocardium when applying Butterworth or low pass filter.

  • Urban flooding threatens human beings and facilities with chemical and physical hazards since the beginning of human civilization. Recent studies have emphasized the integration of data and models for effective urban flood inundation modeling. However, the model set-up process is tend to be time consuming and to require a high level of data processing skill. Furthermore, in spite of the use of high resolution grid data, inundation depth and velocity are varied with building treatment methods in 2-D inundation model, because undesirable grids are generated and resulted in the reliability decline of the simulation results. Thus, it requires building generalization process or enhancing building orthogonality to minimize the distortion of building before converting building footprint into grid data. This study aims to develop building generalization method for 2-dimensional inundation analysis to enhance the model reliability, and to investigate the effect of building generalization method on urban inundation in terms of geographical engineering and hydraulic engineering. As a result to improve the reliability of 2-dimensional inundation analysis, the building generalization method developed in this study should be adapted using Digital Building Model(DBM) before model implementation in urban area. The proposed building generalization sequence was aggregation-simplification, and the threshold of the each method should be determined by considering spatial characteristics, which should not exceed the summation of building gap average and standard deviation.
