심근경색으로 좌심실 벽 운동장애를 가진 환자들에서 관상동맥 우회술 후 장기생존율 (Long-term Survival after CABG in Patients with Abnormal LV Wall Motion after MI)

  • 배경: 좌심실 벽 운동장애는 관상동맥 우회술 후 장기생존율에 영향을 줄 수 있다. 이 연구는 심근경색증 후 발생한 좌심실 벽 운동장애가 관상동맥 우회술 후 장기생존율에 어떠한 영향을 주는가를 알아보았다. 대상 및 방법: 관상동맥 우회술 후 9년이 넘은 환자들 133예(남/여, 92/41)를 대상으로, 심근경색 후 좌심실 벽 운동장애가 있는 환자 56예(남/여 42/14, 평균연령 $59.2\pm9.2$세)와 좌심실 벽 운동장애가 없는 환자 77예(남/여 50/27,평균연령 $58.0\pm7.6$세)로 나누어 비교 분석하였다. 대부분의 환자들(l12/133, $84.2\%$)에서 체외순환 하에 좌측 속 가슴동맥과 하지 큰 두렁정맥을 이용하여 수술하였고 대동맥 차단 상태에서 근위연결 및 원위연결을 시행하는 방법으로 수술하였다. 걸과: 좌심실 벽 운동장애가 있는 환자들의 좌심실 구혈률은 평균 $48.7\pm13.2\%$로 좌심실 벽 운동장애가 없는 환자들(평균$57.1\pm10.1\%$)보다 감소되어 있었다(p=0.0001). 운동장애가 없는 환자군에서 평균 $135.1\pm18.0$개월의 추적으로 5년, 10년, 13년의 생존율은 각각 $85.7\pm4.0\%,\;76.2\;4.9\%,\;57.2\pm10.3\%$였고, 좌심실 운동장애가 있는 환자군에서 평균 $122.8\pm22.7$개월의 추적으로 5년, 10년, 13년의 생존율은 각각 $80.4\pm5.3\%,\;58.7\pm7.3\%,\;11.9\pm7.9\%$이었다(p=0.1). 심근경색에 의한 좌점실 벽 국소 운동장애가 있는 환자의 장기생존율에 영향을 미치는 인자는 좌심실 구혈률과 외래 치료였다. 다변량 분석에서 좌심실 벽 운동장애가 있는 환자군의 장기생존율은 외래 치료를 한 환자에서 우수하였고 좌심실 벽 운동장애가 없는 군의 장기생존율은 여성에서 우수하였다. 결론: 심근경색 후 좌심실 벽의 운동장애가 있는 경우 장기 생존율은 운동장애가 없는 경우보다 떨어지는 경향을 보이며, 그런 환자들에서 수술 후 외래 치료가 장기 생존에 매우 중요하다고 생각한다.

  • The desmoid tumor has been reported as the most common histologic subtype of soft tissue sarcoma occuring in chest wall and it known to be highly recurrent. The treatment of choice is a radical wide resection including a safe margin of uninvolved structures around the grossly visible tumor. We report a case of chest wall reconstruction using Marlex sandwich and latissimus dorsi musculocutaneous flap after wide resection of desmoid tumor on the chest wall.

Surgery for Partial Anomalous Pulmonary Venous Connections: Modification of the Warden Procedure with a Right Atrial Appendage Flap

  • Background: Surgical repair of a partial anomalous pulmonary venous connection (PAPVC) to the superior vena cava (SVC) may be complicated by sinus node dysfunction or SVC obstruction. We modified the Warden procedure by using a right atrial auricular flap to decrease the occurrence of these complications. Methods: Between February 2005 and July 2012, 10 consecutive patients underwent a modified Warden procedure to correct PAPVC. The median patient age was 5.7 years. Eight patients (80%) had an atrial septal defect. To surgically correct the PAPVC, we made a U-shaped incision on the right atrial appendage and sutured the flap to the posterior wall of the SVC. The anterior wall was reconstructed with various patch materials. Results: No early or late deaths occurred, nor did any patient require early or late reoperation for SVC or pulmonary venous obstruction. No new arrhythmias appeared during follow-up, which was complete in all patients (mean, 29.5 months). Conclusion: Our modification of the Warden procedure seems to be effective and safe. This technique may lower the risk of SVC obstruction, pulmonary venous obstruction, and sinus dysfunction.

  • 스파르가눔증의 주요 발생 원인은 뱀이나 개구리의 생식이다. 스파르가눔증의 흔한 임상 증상은 복벽, 흉벽, 대퇴부, 음낭 등에 나타나는 이동성 피하 결절이며, 가장 확실한 진단 및 치료는 외과적으로 충체를 적출하는 것이다. 저자들은 70세 남자 환자의 흉막에 발생한 스파르가눔증을 치험하였기에 문헌 고찰과 함께 보고하는 바이다.

  • Langerhans 세포 조직구증(LCH, Langerhans\` Cell Histiocytosis)은 Langerhans\` cell histiocyte의 이상 증식을 특징으로 하는 원인 불명의 질환이다. 이 질환은 eosinophilic granuloma, Hand-Sch ller-Christian씨 병, Letterer-Siwe병을 포함하는 것으로 과거에는 histiocytosis X로 불리던 질환이다. 피부, 림프절, 골, 골수 및 체내 모든 조직과 기관을 침범할 수 있으나 국내에서 흉벽에서 발생된 예는 보고된 증례가 많지않다. 18개월 된 남자 환아에서 흉벽의 늑골에서 기원하여 골용해 소견을 동반한 종괴가 있어 수술적 절제한 후 LCH로 확진된 증례가 있어 문헌고찰과 함께 보고하는 바이다

Adventitial Fibroblast Abormality in Thoracic Aortic Aneurysms and Aortic Dissections

  • Background: Development of thoracic aortic aneurysms and aortic dissections (TAAD) is attributed to unbearable wall tension superimposed on defective aortic wall integrity and impaired aortic repair mechanisms. Central to this repair mechanisms are well-balanced and adequately functional cellular components of the aortic wall, including endothelial cells, smooth muscle cells (SMCs), inflammatory cells, and adventitial fibroblasts. Adventitial fibroblasts naturally produce aortic extracellular matrix (ECM), and, when aortic wall is injured, they can be transformed into SMCs, which in turn are involved in aortic remodeling. We postulated the hypothesis that adventitial fibroblasts in patients with TAAD may have defects in ECM production and SMC transformation. Materials and Methods: Adventitial fibroblasts were procured from the adventitial layer of fresh aortic tissues of patients with TAAD (Group I) and of multi-organ donors (Group II), and 4-passage cell culture was performed prior to the experiment. To assess ECM production, cells were treated with TNF-${\alpha}$ (50 pM) and the expression of MMP-2/MMP-3 was analyzed using western blot technique. To assess SMC transformation capacity, cells were treated with TGF-${\beta}1$ and expression of SM ${\alpha}$-actin, SM-MHC, Ki-67 and SM calponin was evaluated using western blot technique. Fibroblasts were then treated with TGF-${\beta}1$ (10 pM) for up to 10 days with TGF-${\beta}1$ supplementation every 2 days, and the proportion of transformed SMC in the cell line was measured using immunofluorescence assay for fibroblast surface antigen every 2 days. Results: MMP-3 expression was significantly lower in group I than in group II. TGF-${\beta}1$-stimulated adventitial fibroblasts in group I expressed less SM ${\alpha}$-actin, SM-MHC, and Ki-67 than in group II. SM-calponin expression was not different between the two groups. Presence of fibroblast was observed on immunofluorescence assay after more than 6 days of TGF-${\beta}1$ treatment in group I, while most fibroblasts were transformed to SMC within 4 days in group II. Conclusion: ECM production and SMC transformation are compromised in adventitial fibroblasts from patients with TAAD. This result suggests that functional restoration of adventitial fibroblasts could well be a novel approach for the prevention and treatment of TAAD.

Outpatient Drainage Therapy with a Thoracic Vent for Traumatic Pneumothorax due to Bull Attack

  • Outpatient drainage therapy is generally indicated for spontaneous pneumothoraces. A 63-year-old man, who had been attacked by a bull sustaining injuries on the right side of his chest, was referred to the emergency room with dyspnea. His chest X-ray showed a small pneumothorax. The next day, a chest X-ray demonstrated that his pneumothorax had worsened, although no hemothorax was identified. Outpatient drainage therapy with a thoracic vent was initiated. The air leak stopped on the third day and the thoracic vent was removed on the sixth day. Thoracic vents can be a useful modality for treating traumatic pneumothorax without hemothorax.

  • 전신저혈압은 흉부대동맥질환의 스텐트 그라프트 치료에 전통적으로 많이 이용되어 왔으나, 혈관확장제를 이용한 혈압강하는 심박출량을 증가시킬 수 있으며 이로 인해 스텐트 그라프트의 전개 시 바람자루효과가 발생할 수 있다. 빠른 심실유도는 자동제어방식으로 일시적인 심박출량의 정지를 유도하여 심정지 지속시간이 제어 가능하고, 스텐트 그라프트 전개 시 대동맥 손상을 최소한으로 줄일 수 있다는 장점이 있다. 본원에서는 흉부대동맥궁 하방에 발생한 주머니동백자루 환자에게 빠른 심실유도하에 Valiant Captivia 스텐트 그라프트를 이용하여 효과적으로 혈관내 스텐트 그라프트 시술을 하였기에 문헌 고찰과 함께 보고하는 바이다.

휴벽에 발생한 원발성 연골육종 (A case of the primary chondrosarcoma of the chest wall)

  • A 36 years old male patient underwent en bloc excision of the primary chondrosarcoma arising from 8th, 9th and 10th ribs of the chest wall. The tumor was 11.5 x 7.5 x 6.5 cm in size and was 320 gram in weight. After en bloc excision of the tumor, the defect of the chest wall was reconstructed with pedicle muscle flap and using compression dressing for 2 weeks, there happened no respiratory problem. And so we made a case report and review of literature.

Post-traumatic Intramuscular Hemangioma of the Chest Wall

  • Intramuscular hemangioma originated in chest wall is a rare benign tumor, with no relevant reports in Korea. In most cases, the tumor is discovered before the age of 30 years and it is reported that trauma operates as the initiation factor. It is essential to concern the clinical suspicion and conduct a CT scan for diagnosis. The principle of treatment is surgical excision with clear resection margin. The authors of this study report a case of surgical excision for post-traumatic intramuscular hemangioma of the chest wall with review of literature.
