Primary Synovial Sarcoma of Lung

  • Synovial sarcoma (SS) is a highly malignant tumor that accounts for 10% of all soft-tissue sarcomas. Primary SS arising from the lung is extremely rare, and the prognosis is poor. We report a case of pulmonary SS presenting with a mass lesion invading the right upper and middle lobes, extending to the mediastinum and the chest wall. After tru-cut biopsy, surgical resection was performed. The final diagnosis was SS (biphasic type) based on histological and immunohistochemical findings. There are no guidelines for optimal treatment due to the rarity of these tumors. Current treatment includes surgery and adjuvant chemotherapy and/or radiotherapy.

한우(韓牛)의 척추측마취(脊椎側痲醉) (A Technique for Paravertebral Anesthesia in Korean Cattle)

  • Paravertebral anesthesia for operation of abdominal wall in Korean cattle were effectively accomplished with the following technique. Any problems in surgical procedure were not observed under the block of ventral branches of the last thoracic nerve and the first and second lumbar nerves with the administration of preanesthetic. The site of injection for blocking of ventral branches of the thirteenth thoracic nerve were approximately 5cm lateral to the midline from the posterior edge of spinous process of the 13th thoracic vertebra and about 10ml of local anesthetic was injected immediately anterior to the transverse process of the first lumbar vertebra through thin site. The block of ventral branches of the first and second lumbar nerves were obtained by injecting 10ml of local anesthetic immediately below the posterior edge of transverse process of the 2nd and 4th lumbar vertebra, respectively.

  • Synovial sarcoma is a malignant soft tissue tumor that most commonly occurs in the extremities of young and middle-aged adults, in the vicinity of large joints. Although synovial sarcoma is frequently associated with joints, it may arise in unexpected sites, such as the mediastinum, heart, lung, pleura, or chest wall. Primary synovial sarcoma of the pleura is rare. To date, nearly 36 cases of primary synovial sarcoma of the pleura have been reported since Gaertner et al. published the first case in 1996. The oncologic characteristics, treatment, and prognosis for pleural synovial sarcomas are not well defined because of a paucity of data. However, a multimodal approach, including surgical resection, chemotherapy, and radiotherapy, has generally been suggested. We report the outcome of one patient with primary pleural synovial sarcoma treated with radical resection and adjuvant treatment.

흉복부 절개를 이용한 식도와 위에 발생한 거대 악성 위장관 간질 종양의 절제 (Excision of Malignant Gastrointestinal Stromal Tumor of Distal Esophagus and Stomach using Thoracoabdominal Incision)

  • 위장관 간질 종양은 위장관 전역에서 발생하며 그 중 위와 소장 등에 많이 생기며 식도에서도 발생한다. 44세 남성이 연하곤란과 체중감소를 주소로 내원하여 시행한 상부 위장관 내시경 검사 및 흉부 전산화 단층 촬영에서 식도 하부의 완전폐쇄, 위의 전방 편위 소견과 중앙부 괴사를 동반하는 최대 직경 15cm 정도의 저음영의 거대 종괴를 보여 흉복부 절개를 이용하여 하부 식도 및 위전적출술을 시행하였다 종양은 CDl17 (c-kit)와 CD34에서 양성을 보여 악성 위장관 간질 종양으로 확진되었으며 현재 외래 추적관찰 중이다.

  • 만성 폐동맥 색전증은 비교적 드문 질환으로 저산소증과 폐동맥 고혈압을 일으켜 결국 호흡부전 및 우심부전을 초래한다. 급성 폐동맥 색전증 환자들은 대부분 혈전 방지제, 혈전용해제 등의 내과적 치료에 잘 치료되나 만성 폐동맥 색전증의 경우 섬유화된 혈전이 폐동맥벽에 견고히 붙어있어 내과적 치료에는 별 효과가 없어, 수술적 치료를 고려할 수 있겠다. 본원에서는 만성 폐쇄성 폐질환 및 만성 폐동맥 색전증으로 진단 받고 타 병원에서 수 차례 입원 치료를 받아 오던 47세 남자환자를 간헐적인 완전 순환 정지를 이용하여 폐색전증에 대한 내막 절제술을 시행하여 좋은 결과를 얻었기에 보고하는 바이다

  • Synovial sarcoma is a malignant soft tissue tumor originated from primitive mesenchymal cell. It occurs primarily in the paraarticular regions, parapharyngeal regions and abdominal wall. We experienced a case of intrapulmonary synovial sarcoma which was the first case originated from the lung and confirmed postoperatively.

식도의 점믹하 해리;1례 보고 (Submucosal Dissection of the Esophagus)

  • Injury to the esophagus varies from a minor superficial tear to complete rupture of the esophageal wall. We have recently seen one healthy adult male who sustained submucosal dissection of the esophagus while endoscopy. The diagnosis has been made by esophagogram and chest computed tomogram. The therapy was conservative management and good prognosis without complications.

선천성 흉벽질환의 교정 (Surgical Treatment of Congenital Chest Wall Defects)

  • Fifty-nine cases of congenital chest wall defects experienced in the department of thoracic surgery of Seoul National University Hospital were analyzed and the relevant literatures were reviewed. They are 52 cases of funnel chest, 3 cases of pigeon breast, one case of superior sternal fissure, one case of costochondral incurvation, one case of Cantrell`s pentalogy, and one case of Poland`s syndrome. Funnel chest affected males more frequently than females by 44 to 8. All of the funnel deformities were corrected by Ravitch operation or its modification except one which was the first case of this series and was corrected by a sterno-turnover. Two cases required a mechanical ventilation for 3 days and 5 days respectively. Four minor complications which were two cases of skin wound infection and 2 cases of fluid accumulation were noted. Skin would infection was repaired by a secondary closure and fluid accumulation was treated by aspiration only. The result are all excellent without recurrence or reoperation. In 3 cases of pigeon breast, they were treated by subperichondrial resection of all of the involved costal cartilages and shortening their course with reefing sutures in the perichondrium with excellent result. The superior sternal fissure which was combined by a ventricular septal defect was treated by a simple wire closure with a good result. The costochondral incurvation was corrected by subperichondrial resection of deformed cartilages and a rib graft removed from the contralateral normal side. The Poland syndrome and the Cantrell`s pentalogy was already presented previously.

  • Elastofibroma dorsi is a benign soft tissue mass, not well-known because of its low incidence, and usually located between the chest wall and the inferomedial aspect of the scapula. This lesion is not true neoplasm but rather reactive hyperplasia of elastic fibers. It is mostly nontender mass, but occasionally causes snapping symptom. This tumor should be considered as a differential diagnosis of snapping scapula. The clinical diagnosis is made by magnetic resonance imaging and confirmed by pathologic findings. We present a case report of a female with elastofibroma dorsi, who had that the chief complaint was snapping scapula and palpable mass. We emphasize that snapping lesions located deep beneath the inferior tip of the scapula on the chest wall should arouse suspicion of an elastofibroma dorsi.

Chest Wall Reconstruction for the Treatment of Lung Herniation and Respiratory Failure 1 Month after Emergency Thoracotomy in a Patient with Traumatic Flail Chest

  • We report a case of delayed chest wall reconstruction after thoracotomy. A 53-yearold female, a victim of a motor vehicle accident, presented with bilateral multiple rib fractures with flail motion and multiple extrathoracic injuries. Whole-body computed tomography revealed multiple fractures of the bilateral ribs, clavicle, and scapula, and bilateral hemopneumothorax with severe lung contusions. Active hemorrhage was also found in the anterior pelvis, which was treated by angioembolization. The patient was transferred to the surgical intensive care unit for follow-up. We planned to perform surgical stabilization of rib fractures (SSRF) because her lung condition did not seem favorable for general anesthesia. Within a few hours, however, massive hemorrhage (presumably due to coagulopathy) drained through the thoracic drainage catheter. We performed an exploratory thoracotomy in the operating room. We initially planned to perform exploratory thoracotomy and "on the way out" SSRF. In the operating room, the hemorrhage was controlled; however, her condition deteriorated and SSRF could not be completed. SSRF was completed after about a month owing to other medical conditions, and the patient was weaned successfully.
