Primary Extraskeletal Osteosarcoma in the Anterior Mediastinum: A Case Report and Review

  • Extraskeletal osteosarcoma (ESOS) is a malignant soft tissue neoplasm producing osteoid, without any continuity with the bone or periosteum. Primary ESOS presenting in the mediastinum is an extremely rare, yet aggressive malignant tumor associated with a poor prognosis. We report a case of primary ESOS arising from the thymus in a 63-year-old male patient.

  • Right heart failure is a relatively common complication after left ventricular assist device (LVAD) implantation. Severe right heart failure can be managed by temporary right ventricular assist device (RVAD) implantation. However, trans-sternal RVAD insertion requires a subsequent third sternotomy for cannula removal. Herein, we present a case of RVAD insertion via a left anterior mini-thoracotomy after LVAD implantation in a patient with alcohol-induced cardiomyopathy.

  • Here, we report the rare case of a 13-year-old girl with a congenital diaphragmatic hernia (also known as Bochdalek hernia), which was revealed to be an extralobar pulmonary sequestration that was treated using laparoscopic and video-assisted thoracic surgery sequestrectomy and repair of the diaphragm defect after detection of a supradiaphragmatic mass connected with the retroperitoneum. The patient showed no postoperative complications at a 1-month follow-up examination.

  • Thymomas are common anterior mediastinal tumors with a relatively favorable prognosis compared to that of other types of thoracic malignancies. However, thymomas that invade surrounding structures, such as the heart or vena cava, have been infrequently reported, and intracardiac thymomas are exceedingly rare. Treatment of invasive thymoma is difficult because the high rate of incomplete resection results in a high rate of recurrence. Herein, we present a rare case of a thymoma that originated in the right atrium and extended into the superior vena cava and brachiocephalic vein.

  • 인공심폐기를 사용하지 않는 관상동맥우회술에서 이식편의 근위부 문합을 상행대동맥에 시행하는 경우 상행대동맥의 조작으로 인한 공기나 대동맥조직, 죽종(atheroma) 등으로 인한 색전증과 대동맥 손상으로 유발되는 대동맥박리, 가성동맥류 등의 합병증을 유발할 수 있다 대동맥근부치환술 시 양측관상동맥의 재이식 시 도관을 사용하는 Cabrol 술식을 관상동맥우회술 시 적용하였다. 요골동맥이식편은 복재정맥과 다르게 혈관 내 판막이 없다는 이점을 고려하여 근위부 문합을 상행대동맥에 평행하게(side to side) 문합하여 하나의 근위부 문합으로 요골동맥이식편의 양측으로 혈류를 공급하는 Cabrol 술식을 이용하여 근위부 문합 수를 줄일 수 있으며 상행대동맥의 조작 수를 줄여 대동맥의 외상을 줄일 수 있다고 생각한다.

  • Penetrating chest trauma may result in significant intracardiac injury. A traumatic ventricular septal defect is a rare complication that requires surgical management, particularly if heart failure ensues. We report a case of delayed repair of an outlet-type ventricular septal defect and perforation of the aortic and pulmonary valve leaflets following a stab wound. This report highlights diagnostic and surgical considerations and also presents an opportunity to review the conotruncal anatomy, which may be relatively unfamiliar to many adult cardiac surgeons.

  • An 87-year-old man presented with a saccular aneurysm at the proximal descending thoracic aorta. As computed tomography revealed a shaggy aorta, we planned hybrid thoracic endovascular aortic repair (TEVAR) with embolic protection devices (EPDs) in both internal carotid arteries to prevent a cerebrovascular accident. We inserted an Emboshield NAV6 Embolic Protection System (Abbott Vascular, Abbott Park, IL, USA) into both internal carotid arteries before performing the TEVAR procedure. The patient was discharged from the hospital on postoperative day 4 without any neurological complications.

  • There are several types of minimally invasive approaches for thymectomy, of which the transpleural approach by video-assisted thoracoscopic surgery is particularly useful. In this approach, thymectomy is performed from either side of the thoracic cage. Thoracic surgeons should be familiar with the principles of the procedure, the anatomy of the region, and surgical strategies for successful thymectomy. The details of transpleural thymectomy are discussed herein.

  • Pectus excavatum is rare, but it is the most common type of sternal congenital disorder. There are many surgical methods to correct pectus excavatum such as the Ravitch method, Wada method, Silastic mold method, and the Nuss operation. We report a case of minimal invasive surgery for pectus excavatum using a polyvinyl alcohol sponge.
