Primary Perivascular Epithelioid Cell Tumor of the Lung: A Case Report

  • Perivascular epithelioid cell tumors (PEComas) are very rare mesenchymal neoplasms, composed of histologically and immunohistochemically distinctive cells that form a sheet-like appearance around vessel lumens. Although most are benign, a malignant subset does exist, complicating clinical diagnostic efforts. Proper evaluation and management are thus essential for long-term patient survival. Herein, we present a rare case of a benign pulmonary PEComa that was diagnosed postoperatively in a 45-year-old woman.

  • 광역동치료는 흉부외과 영역에서 조기 식도암, 기관 및 기관지 종양, 위장관 점막 내 종양 및 바레트식도 등에서 광감작물질의 광파장 및 광선의 암세포에 대한 선택적이고 직접적인 독성 효과를 이용한 비수술적 치료로 안전하고 효과가 있으며, 기타 진행된 폐암이나 식도암에서 보존적 요법으로도 이용될 수 있다고 여겨지고 있다. 그러나 광역동치료 후 광독성에 의한 피부 독성이 흔하여, 직사 광선의 차단 및 약물 요법이 병행되어야 한다. 저자들은 식도암 수술 후 흘곽 내에 거치된 위장에 발생한 원발성 위암에 대한 광역동치료(Photodynamic tharapy)로 좋은 결과를 얻었기에 문헌 고찰과 함께보고하고자 한다.

  • Primary intrathoracic neoplasms occur frequently, and the most frequent intrathoracic tumors are of pulmonary or bronchogenic origin. Second in frequency are mediastinal tumors, and least frequency or comparatively rare are the neoplasms arising from the wall of the thorax and extending into the thoracic cavity. Of all intrathoracic tumors, fibroma is extremely rare. Recently, we have experienced a huge rapid growing intrathoracic fibroma originating from thoracic wall, which is removed successfully and confirmed histopathologically. Authors present one case of fibroma and discussion with a brief review of the relevant literatures.

  • Primary epithelial-myoepithelial carcinoma (EMC) of the lung is an extremely rare neoplasm that originates from submucosal bronchial glands and has been found in the salivary glands, breast tissue, and sweat glands. However, only a few cases in the respiratory tract have been identified. In the literature, most pulmonary EMCs have been reported to have developed endobronchially although a few EMC cases have been presented as intraparenchymatous tumors. We have identified a case of primary EMC that developed in the peripheral lung parenchyma.

  • A 71-year-old male patient visited Yeungnam University Hospital with abnormal chest computed tomography (CT) findings. Chest CT revealed multiple lung nodules and a posterior mediastinal tumor, the diagnosis of which was confirmed surgically. Magnetic resonance imaging (MRI) of the abdomen showed multiple small nodules, which were diagnosed as cavernous hemangioma in the liver based on the pathology results of the mediastinal and lung masses in combination with MRI findings. Cavernous hemangiomas are benign tumors that can occur throughout the body, mainly in the skin and subcutaneous tissue. The liver is the most common internal organ containing hemangiomas, whereas they are very rarely found in the lungs or mediastinum.

Video-Assisted Thoracic Surgery Thymectomy: Transpleural Approach

  • There are several types of minimally invasive approaches for thymectomy, of which the transpleural approach by video-assisted thoracoscopic surgery is particularly useful. In this approach, thymectomy is performed from either side of the thoracic cage. Thoracic surgeons should be familiar with the principles of the procedure, the anatomy of the region, and surgical strategies for successful thymectomy. The details of transpleural thymectomy are discussed herein.

  • 식도, 위, 폐 그리고 구강에 동시 발생한 다발성 원발암에 대해 보고하고자 한다. 65세 남자 환자가 구강 내 종괴에 대해 상피세포암을 진단받고 광범위 절제술을 시행 받았다. 수술 후 정확한 병기 결정을 위해 시행한 검사에서 식도암, 위암 그리고 폐암이 추가로 확인되었다. 저자들은 상기 세가지암에 대하여 한 차례 수술로 근치적 절제술을 시행하여, 만족할 만한 결과를 얻었기에 이에 대해 보고하는 바이다.

  • Mediastinal lymph node dissection is an important part of lung cancer surgery that provides accurate nodal staging and may improve survival outcomes. The minimally invasive approach, such as video-assisted thoracic surgery (VATS) lobectomy for patients with non-small cell lung cancer, has become a standard operation worldwide. VATS mediastinal lymph node dissection should be thorough and accurate to ensure the completeness of lung cancer surgery. Herein, the author describes techniques for VATS mediastinal lymph node dissection.

  • 데스모이드는 재발빈도가 높은 양성 종양이다. 본 저자들은 흉강 및 복강에 걸쳐 있는 거대한 데스모이드를 충분한 절단면을 확보한 후 절제하고 간좌엽절제술, 좌폐하엽 쐐기절제술, 부분적 심막절제술 후 우심막을 이용하여 심막재건술을 시행하였고 횡격막을 부분 절제한 후 마렉스 메쉬로 횡격막 재건술을 시행하였다.

  • Pulmonary malakoplakia is a rare lung lesion more frequently found in immunocompromised patients than in immunocompetent individuals. In this study, we report the challenging case of a young immunocompetent patient with an irregular pulmonary nodule with peripheral cysts who, after undergoing surgery, was diagnosed with malakoplakia. Due to the rarity of the disease and the similarity of this condition to malignant neoplasms, cytopathological or histopathological examinations are necessary for the correct diagnosis. A description of pulmonary malakoplakia with peripheral cysts has not been previously published in the literature.
