Hybrid Coronary Artery Revascularization for Takayasu Arteritis with Major Visceral Collateral Circulation from the Left Internal Thoracic Artery

  • Coronary arterial involvement in Takayasu arteritis (TA) is not uncommon. Herein, we describe a case of TA with celiac trunk and superior mesenteric artery occlusion combined with coronary artery disease. Bilateral huge internal thoracic arteries (ITAs) and the inferior mesenteric artery provided the major visceral collateral circulation. After percutaneous intervention to the right coronary artery, off-pump coronary artery bypass grafting for the left coronary territory was done using a right ITA graft and its large side branch because of its relatively minor contribution to the visceral collateral circulation.

  • Coronary artery disease has historically been a contraindication to lung transplantation. We report a successful combined bilateral lung transplantation and off-pump coronary artery bypass in a 62-year-old man. The patient had a progressive decline in lung function due to idiopathic pulmonary fibrosis and a history of severe occlusive coronary artery disease.

Long Segmental Reconstruction of Diffusely Diseased Left Anterior Descending Coronary Artery Using Left Internal Thoracic Artery with Extensive Endarterectomy

  • In coronary artery bypass grafting, a diffusely diseased left anterior descending coronary artery (LAD) is an obstacle to achieving complete revascularization, consequently leading to the possibility of a poor prognosis. Long segmental reconstruction with or without endarterectomy is a revascularization method for treating diffusely diseased coronary arteries. Herein, we report a successful case of long segmental reconstruction of a diffusely diseased LAD using a left internal thoracic artery onlay patch after endarterectomy.

  • 58세 남자 환자가 좌쇄골하동맥협착이 동반된 관상동맥 질환으로 하이브리드술식을 시행받았다. 쇄골하동맥에 대한 스텐트 삽입과 좌내흉동맥의 좌전하행지 문합을 포함한 심폐 바이패스 없이 관상동맥 우회술을 시행하였다. 수술 후 시행한 관상동맥 조영술과 컴퓨터 단층촬영(CT)상에서 이식편의 개방성은 양호하였고 스텐트의 삽입부위는 적절한 것으로 보였고 개방성도 양호하였다. 술 후 8일째 퇴원하였고, 현재 6개월째 외래 경과관찰 중이다.

관상동맥우회술시 부적절한 좌내흉동맥의 변형 활용에 대한 경험 (Reconstruction of Injured or Inadquate Left Internal Thoracic Artery in Cornonary Artery bypass Graft)

  • Use of the left internal thoracic artery(ITA) to bypass the left anterior descending(LAD) coronary artery has become the standard of care based on its superior graft patency, reduced cardiac events, and enhanced survival. But rarely we encountered with injury to the artery during harvesting which leads to loss of the merits of surgery. We reconstructed inadequate ITAa with other arterial conduits so proximal stump to be a blood source if possible. Maternal and method: Between January 1996 and March 1999, 12 patients received bypass with the reconstructed left internal thoracic artery grafts to left anterior descending artery because of an injury(n=8), short or small(n=4). Right or left ITA was used to LAD as a free graft(n=2). And the other 10 left ITAs were extended with radial artery(n=6), right ITA(n=3), saphenous vein(n=1). Composite "T" graft was made with other arterial conduits in these extended graft(n=5). Result: There was only one morbidity of minor would problem, and no mortality. The patency of extended graft to LAD was complete in 5 patients who received angiography during the period of 2wks to 2 years postoperative, but one of side branch of "T" graft occluded. All of these patients were well. Conclusion: Reconstructive extension with the use of other arterial conduit for the injured proximal ITA is warranted in any patients with acceptable results. acceptable results.

양측 내흉동맥을 이용한 관상동맥우회술의 조기 결과 (The Early Results of CABG with Bilateral Internal Thoracic Artery)

  • 배경: 관상동맥우회술 시 이용하는 내흉동맥은 그 장기 개통률이 다른 도판보다 우수하다고 알려져 있다. 또한 양측 내흉동맥을 이용하면 좋은 개통률로 보다 많은 관상동맥에 접근할 수 있다. 그러나 양측 내흥동맥의 박리로 수술조기 합병증이 증가할 것이라는 관점이 있어 보고자가 경험한 수술 조기결과를 보고하고자 한다. 대상 및 방법: 2001년 7월부터 2002년 5월까지 양측 내흉동맥을 이용하여 관상동맥우회술을 시행한 26예를 대상으로 하였다. 대상환자 중 당뇨병을 가진 8예와 당뇨병이 없는 18예를 비교하여 수술결과를 분석하였다. 또한 같은 기간에 단측 내흉맥을 이용하여 수술한 20예를 대상 환자군과 비교하여 그 수술결과를 분석하였다. 결과: 수술사망은 없었다. 수술 후 합병증은 창상감염으로 당뇨군과 정상군 각각 1예가 있었다. 당뇨군과 정상군 간에는 수술시간, 기관삽관시간, 출혈량, 강심제투여시간, 그리고 합병증 발생에서는 서로 차이가 없었다. 단측 내흉동맥군과 수술결과에 있어 차이가 없었다. 결론: 양측 내흉동맥을 이용한 관상동맥우회술의 수술 조기결과는 단측 내흉동맥을 이용한 수술결과와 차이가 없었고 양측 내흉동맥을 이용한 환자 중 당뇨병이 있는 경우에도 정상군과 수술결과의 차이는 없었다. 당뇨병 환자에 있어 양측 내흉동맥의 이용은 고려할 만하다고 생각한다.

  • 관상동맥우회술은 인공심폐기 및 심근보호의 안정성으로 인공심폐기사용하에 시행하는 것이 보편화된방법이다. 그러나, 좌전행지 및 우관상동맥에 병소가 위치하는 경우는 인공심폐기의 사용없이 심장이 박동하는 상태에서 관상동맥우회술을 시행하는 방법도 일부에서 시행되어왔다 저자들은 좌전행지 및 대각지에 협착을보인 환자에서 인공심폐기를 사용하지 않고 성공적으로 관상동맥우회술을 시행하였기에 보고하는 바이다.

Refractory Coronary Artery Spasm after Minimally Invasive Direct Coronary Artery Bypass Grafting

  • Postoperative coronary arterial spasm is a rare but potentially fatal complication. A 51-year-old male patient with a history of a reactive ergonovine stress test coronary angiogram developed refractory coronary artery spasm after undergoing minimally invasive direct coronary artery bypass grafting of the left anterior descending coronary artery. The patient was successfully managed with rapid implementation of intra-aortic balloon-pump counter pulsation and extracorporeal membrane oxygenation.

Redo-Coronary Artery Bypass due to Progression of the Celiac Axis Stenosis

  • We report a redo coronary artery bypass grafting (CABG) in a 55-year-old man. Angina recurred 7 years after the initial surgery. Coronary angiography showed all patent grafts except a faint visualization of the in situ right gastroepiploic artery (RGEA) graft, which was anastomosed to the posterior descending coronary artery, associated with celiac axis stenosis. Redo-CABG was performed at postoperative 10 years because of aggravated angina and decreased perfusion of the inferior wall in the myocardial single photon emission computed tomography. The saphenous vein graft was interposed between the 2 in situ grafts used previously; the right internal thoracic artery and RGEA grafts. Angina was relieved and myocardial perfusion was improved.

Coronary Artery Bypass Grafting in an Infant after an Arterial Switch Operation

  • Coronary artery bypass grafting (CABG) is rarely performed in infants because of its technical difficulty and unclear long-term results. A 90-day-old male infant weighing 3.5 kg who underwent an arterial switch operation (ASO) for transposition of the great arteries developed left coronary artery insufficiency despite augmentation and reimplantation of the left coronary button. On-pump beating heart CABG was performed using an internal mammary artery graft to revascularize the left anterior descending artery. Postoperative computed tomography angiography revealed that the graft was patent. At 7 months postoperatively, the patient weighed 8.5 kg, and echocardiography revealed good ventricular function. CABG can be an alternative treatment for post-ASO coronary complications in early infancy.
