Maximal Oxygen Consumption in the Secondary School Boys (남자 중 . 고둥학생의 최대 산소 성취량)

  • Maximal oxygen consumption measurements were performed on 15 middle school boys (age: mean 14.0, range: $13{\sim}16$ years) and 14 high school boys (age: mean 17.4, range: $16{\sim}19$ years). General body build was greater in the high school boys and absolute values of body height, body weight, skinfold thicknesses, maximal oxygen uptake, and maximal pulmonary ventilation followed the same trend. Considered on the basis of body build, however, the values of high school boys were not always greater than those of middle school boys. The following results were obtained. 1. Maximal oxygen consumption in middle school boys was 2.11 l/min., 53.7ml/kg b. weight, 13.9 ml/cm body height, and 63.7 ml/kg LBM. In high school boys the values were: 2.86 l/min., 52.7 ml/kg b.wt., 17.5 ml/cm b. height, and 57.9 ml/kg LBM. Thus, middle school boys were superior to high school boys on body weight and lean body mass basis. They were also superior to the European boys of the same age. 2. The ratio of maximal oxygen uptake to resting value was 9.7 in middle school boys, and 10.8 in high school boys. 3. Maximal pulmonary ventilation in middle school boys was 58.0 l/min., and 84.0 l/min. in high school boys. The ratio of maximal ventilation to resting value was the same as oxygen uptake, namely, 9.7 in middle school boys and 10.7 in high school boys. 4. Ventilation equivalent in middle school boys was 27.5 and 29.3 in high school boys. These values represent values of untrained male subjects. 5. Maximal heart rate in high school boys reached to 193 beat/min. and is 2.9 times that of resting heart rate. 6. Maximal oxygen pulse in high school boys was 16.6 ml/beat and was same as that of untrained subject. 7. Correlation between body weight and maximal oxygen consumption in middle school boys was r=0.570, and r=0.162 in high school boys. Correlation between lean body mass in middle school boys was r=0.499, and r=0.158 in high school boys. Interrelation between body weight and maximal pulmonary ventilation was poor. 8. The differences between trained and untrained subjects were discussed.

  • The quality of service delivery by governments to their citizens is a subject of concern in the contemporary society. E-governance is a critical aspect that is transforming government operation and service delivery to citizens and other bodies through application of information technology. This research explores the state of e-governing focusing on nations that are developing with special attention to Pakistan. The difficulties and benefits encountered are identified. The results are vital for implementers of e-governing systems in these countries. The methodology applied entails a secondary research that involves analysis and synthesis of literature relating the research topic. The results reveal that Pakistan has made incredible steps in setting up e-governance systems with growth in internet use and access of data from a digital platform. The findings reveal that e-government is associated with multiple benefits including enhanced quality of services, cost efficiency in service provision, enhance transparency and elimination of corruption, provide the basis for eradication of poverty, boost economic stability of a country, and provide room for direct democracy. The research also found that developing countries experience challenges in form of financial constraints, poor ICT infrastructure, illiteracy on e-government, political consensus constraints, legal obstacles, social and cultural constraints. E-government has the capability to transform the quality of governance provided by governments, and policymakers and implementers should address the constraints that act as a hindrance to its implementation.

  • Visual changes and speed changes in curved sections of highways are more dangerous than straight sections According to the enquetes conducted by Coalition for Transportation Culture in 1999, about 78% of drivers are dissatisfied with traffic guide and safety signs, with lack of visibility being the largest reason with about 27.9% of respondents indicating it as reason for dissatisfaction. In particular, the lack of visibility during nighttime or bad weather not only threatens driver safety due to poor delivery of information, but also affects the service level of highways. Because of this a new delineation must be installed and managed to enhance driver visibility. In this research, an optimal delineation system to enhance traffic safety is presented. In this research drivers effectively obtained information on highway alignment on the curved sections using a retro-reflection type delineation system and a newly-developed internal lighting delineation system to improve safety on the highways. A statistical comparison was conducted and analysis was done for the delineation systems that enhanced visibility through primary and secondary enquaetes. As a result, inside-lighting delineator will be selected in terms of safety at the curve sections. The inside-lighting delineator was more effective than the retro-reflection delineator on visibility, the necessity of reduction of speed and will reduce the hazard at curve sections. It is anticipated that when a delineation system based on this research is installed, a reduction in the number the number and severity of traffic accidents on curved sections will be reduced. In addition this system will more effectively provide drivers with information about highway alignment.

A clinical report of hybrid telescopic double crown denture with friction pin in a failed double crown denture case (실패한 이중관 국소의치에서 하이브리드 텔레스코픽 이중관 국소의치를 이용한 임상증례)

  • In removable partial dentures, many types of retentive systems have been studied and applied in clinical treatment. One of those systems is the double crown denture system which is widely used in European countries such as Germany and Sweden. Telescopic double crown dentures have several advantages such as convenience in maintaining oral hygiene, enabling to transfer occlusal force along the long axis of the abutment, and secondary splinting effect between the abutments which leads to higher clinical performance compared to conventional removal partial dentures. In this clinical case, the patient was initially restored with a maxillary hybrid telescopic double crown denture with friction pin using remaining natural teeth as abutments. After 7 years, due to lack of recall check-up and poor oral hygiene, the abutment teeth were affected by periodontitis and 4 out of 5 of the abutment teeth had to be extracted. 3 additional implants were placed and the original abutment tooth with the inner crown was maintained. The mandible had fixed prostheses including implants but nevertheless, with strategic implant placement, the patient adapted well and was satisfied with the new maxillary tooth-implant combined double crown denture.

Establishing cleft services in developing countries: Complications of cleft lip and palate surgery in rural areas of Indonesia

  • Background Cleft treatment is frequently performed in Indonesia, mostly in charity missions, but without a postoperative protocol it is difficult to establish the risks and complications of cleft treatment. The present study was designed to give an overview of current cleft lip and palate treatment strategies in Indonesia and to assess the complication rates during and after surgery. Methods This prospective study evaluated anesthetic, intraoperative surgical, and short-term postoperative complications in patients undergoing primary, secondary, or corrective surgery for cleft lip and palate deformities. The population consisted of 98 non-syndromic cleft patients. The main anesthetic complication that occurred during general anesthesia was high blood pressure, whereas the main intraoperative surgical complication was excessive bleeding and the main early postoperative complication was extremely poor wound hygiene. Results In this study, there were no cases of perioperative or postoperative mortality. However, in 23 (23.4%) of the 98 operations performed, at least one perioperative complication related to anesthesia occurred. The intraoperative and early postoperative complications following cleft lip and/or palate were assessed. There was a significant difference in the complication rate between procedure types (χ2=0.02; P<0.05). However, no relationship was found between perioperative complications related to anesthesia and the occurrence of postoperative complications (χ2=1.00; P>0.05). Nonetheless, a significant difference was found between procedure types regarding perioperative complications and the occurrence of postoperative complications (χ2=0.031; P<0.05). Conclusions Further evaluation of these outcomes would help direct patient management toward decreasing the complication rate.

Arteriovenous Fistula Formation with Prosthetic Graft Using the Vena Comitantes as a Venous Outflow (동반정맥을 정맥유출로 이용한 인조혈관 동정맥루 조성술)

  • Background: Arteriovenous fistula formation is not always easy to perform in hemodialysis patients because of poor preservation of veins due to repeated venipuncture and cannulation. We analyzed the patency rate and complications of prosthetic arteriovenous fistulas using the vena comitantes as a venous outflow in the antecubital fossa, which are protected from venipuncture. Material and Method: Between January 2006 and June 2008, 12 patients underwent prosthetic arteriovenous fistula formation using the vena comitantes as a venous outflow. Arterial inflow was via the brachial artery and the graft was placed in a loop fashion. The male-to-female ratio was 7 : 5 and the mean age was $59{\pm}14$ years. Six patients had diabetes mellitus and 10 patients had hypertension. Result: There were no complications, such as a graft infection or bleeding. Five patients showed postoperative stenosis at an average of 3 months. The primary patency rate was 75.0, 65.6, and 52.2% at 3, 6, and 12 months, respectively. All the patients with stenosis were able to continue hemodialysis after intervention therapy. The secondary patency rate was 100% at 12 months. Conclusion: Creation of a prosthetic arteriovenous fistula using uninjured vena comitantes resulted in a good patency rate and this vein may become a substitute for inappropriate superficial veins.

Development and Verification of A Module for Positioning Buried Persons in Collapsed Area (붕괴지역의 매몰자 위치측위를 위한 모듈 개발 및 검증)

  • Due to disasters such as earthquakes and landslides in urban areas, persons have been buried inside collapsed buildings and structures. Rescuers have mainly utilized detection equipment by applying sound, video and electric waves, but these are expensive and due to the directional approaches onto the collapsed site, secondary collapse risk can arise. In addition, due to poor utilization of such equipment, new human detection technology with quick and high reliability has not been utilized. To address these issues, this study develops a wireless signal-based human detection module that can be loaded into an Unmanned Aerial Vehicle (UAV). The human detection module searches for the 3D location for buried persons by collecting Wi-Fi signal and barometer sensors data transmitted from the mobile phones. This module can gain diverse information from mobile phones for buried persons in real time. We present a development framework of the module that provides 3D location data with more reliable information by delivering the collected data into a local computer in the ground. This study verified the application feasibility of the developed module in a real collapsed area. Therefore, it is expected that these results can be used as a core technology for the quick detection of buried persons' location and for relieving them after disasters that induce building collapses.

Predicting Mortality in Patients with Tuberculous Destroyed Lung Receiving Mechanical Ventilation

  • Background: Patients with acute respiratory failure secondary to tuberculous destroyed lung (TDL) have a poor prognosis. The aim of the present retrospective study was to develop a mortality prediction model for TDL patients who require mechanical ventilation. Methods: Data from consecutive TDL patients who had received mechanical ventilation at a single university-affiliated tertiary care hospital in Korea were reviewed. Binary logistic regression was used to identify factors predicting intensive care unit (ICU) mortality. A TDL on mechanical Ventilation (TDL-Vent) score was calculated by assigning points to variables according to ${\beta}$ coefficient values. Results: Data from 125 patients were reviewed. A total of 36 patients (29%) died during ICU admission. On the basis of multivariate analysis, the following factors were included in the TDL-Vent score: age ${\geq}65$ years, vasopressor use, and arterial partial pressure of oxygen/fraction of inspired oxygen ratio <180. In a second regression model, a modified score was then calculated by adding brain natriuretic peptide. For TDL-Vent scores 0 to 3, the 60-day mortality rates were 11%, 27%, 30%, and 77%, respectively (p<0.001). For modified TDL-Vent scores 0 to ${\geq}3$, the 60-day mortality rates were 0%, 21%, 33%, and 57%, respectively (p=0.001). For both the TDL-Vent score and the modified TDL-Vent score, the areas under the receiver operating characteristic curve were larger than that of other illness severity scores. Conclusion: The TDL-Vent model identifies TDL patients on mechanical ventilation with a high risk of mortality. Prospective validation studies in larger cohorts are now warranted.

Effects of Heat-treatments on Discharge Characteristics of TiFe1-xNix Alloy Electrodes for Ni/MH Secondary Battery (Ni/MH 2차전지용 TiFe1-xNix 합금전극의 방전특성에 대한 열처리의 영향)

  • The effects of heat-treatment on discharge characteristics of $TiFe_{1-x}Ni_x$ alloy were investigated. The content of Ni in alloy was varied from x = 0.1 to 0.6 by each 0.1 increment. Discharge capacity change of each alloy with C/D cycles was measured. With increasing Ni-content initial discharge capacity was increased. but at x = 0.6 it was deceased again. With increasing C/D cycles discharge capacity was rapidly decreased in the alloy of high Ni-content. In order to investigate the effects of heat-treatment on cycle life, $TiFe_{0.5}Ni_{0.5}$ alloy having maximum initial discharge capacity was heat-treated at various temperatures in the range of $700{\sim}900^{\circ}C$ and tested. The loss of initial discharge capacity was appeared at all temperatures. but cycle characteristics of the alloy was improved. The electrodes heat-treated for 1 hour in the range of $700{\sim}850^{\circ}C$ showed good recovery of discharge capacities through repeated cycles, and from SEM observation results these were considered as 1 hour in the range of $700{\sim}850^{\circ}C$ showed good recovery of discharge capacities through repeated cycles, and from SEM observation results these were considered asbeing due to increased electrode strength and small loss of porosity during heat-treatment. The electrode heat-treated for 1 hour at $900^{\circ}C$ showed poor discharge characteristics because of low porosity.

A Study on the Skin Disease of Sanghanron 《傷寒論》 and Geumgweyoryak 《金匱要略》 (《상한론(傷寒論)》, 《김궤요략(金匱要略)》의 피부증상(皮膚症狀) 및 질환(疾患)에 대한 연구(硏究))

  • Objective : The purpose of this study is to investigate the skin disease in Sanghanron"傷寒論" and Geumgweyoryak"金匱要略". Methods : We conducted a study on the original text paragraphs of Sanghanron and Geumgweyoryak containing the skin disease and analysis of Woo, Oh, Sung, Zhang, etc. We drew a parallel between skin disease from Sanghanron and Geumgweyoryak and matching diagnoses from western medicine. Results : The results were as follows. 1. In Sanghanron and Geumgweyoryak pruritus was related to psychogenic pruritus and similar to skin disease caused by decrease of sweating, dry skin in cholinergic urticaria, atopic dermatitis and eczema, etc. in western medicine. 2. In Geumgweyoryak ichthyosis was caused by woman's disease, after menopause, chronic disease and malnutrition and related to winter itch and xerotic eczema in western medicine. 3. In Sanghanron Goose bumps, keratosis follicularis, acute and chronic urticaria and cold urticaria were occurred because of poorly managed fever care and side effect of antibiotic. 4. In Geumgweyoryak red face was similar to face flushing and side effects, which is after drug and laser treatment on face, in western medicine. 5. In Geumgweyoryak urticaria was, in a broad sense, skin disease with pruritus and secondary infection resulted from scratch. Skin diseases with pruritus are similar to urticaria, dermatitis herpetiformis, lichen planus, atopic dermatitis, contact dermatitis, psoriasis, nummular eczema, lichen simplex chronicus, prurigo nodularis, neurodermatitis and internal disease with pruritus in western medicine. 6. In Geumgweyoryak spots in the skin were caused by fever and similar to allergic purpura and SLE symptom in western medicine. 7. In Geumgweyoryak bullous disease with yellowish discharge was similar to eczema, pustule, atopic eczema, etc. in western medicine. 8. In Geumgweyoryak repeated inflammation of oral cavity, genitals, eyes and skin was similar to Behcet's disease in western medicine. 9. In Sanghanron and Geumgweyoryak boil was similar to abscess and acute lymphadenitis in western medicine caused by oily foods and infections. 10. In Geumgweyoryak swelling and boil were occurred in a poor health. Damage from metalic material could cause a convulsion and was similar to tetanus in western medicine. Conclusion : We analyzed the original text paragraphs of Sanghanron and Geumgweyoryak and explanations about skin disease. As a results, we found out etiology, pathogenesis, treatments of the skin disease in Sanghanron and Geumgweyoryak. Further we compared with western medicine to develop better understanding of the skin disease.
