Job Stress and Mental Health of Female Household Head Workers (여성가구주 근로자의 직무 스트레스와 정신건강)

  • Purpose: The purpose of this study is to identify job stress and psychological health of female household head workers and investigate the relationship between them. Methods: This study was cross-sectionally designed. We analysed the secondary data extracted from the 2nd and 3rd Korean Working Condition Survey. 4,807 female employees were included in the final analysis. $x^2$ test and multiple logistic regression using IBM SPSS 23.0 were performed. Results: 46.7% of female household head workers were in poor psychological health. In the area of job stress-related characteristics affecting the level of psychological health, it was found that female household head workers had a higher likelihood of becoming psychologically unhealthy due to organizational relationships and inadequate compensation factors. Conclusion: Considering the fact that the number of female household head workers is gradually expanding as a breadwinner, it is necessary to find a way of paying attention to their job stress and psychological health in order to support them.

Association of Sleep Duration and Quality of Sleep with Depression in Middle School Students: Based on the Korean Children and Youth Panel Survey (중학교 청소년의 수면시간과 수면의 질이 우울에 미치는 영향: 한국아동·청소년패널조사를 중심으로)

  • Purpose: The purpose of this study was to examine the relationship between sleep duration, quality of sleep and depression, and to identify the factors associated with depression in middle school students. Methods: This study used a cross-sectional design with secondary data, middle school student panel data from the Korean Children and Youth Panel Survey (2018), and data from a total of 2,590 students was used for analysis. For the statistical analysis, t-test, ANOVA, and hierarchical multiple regression were performed using SPSS ver. 26. Results: The mean depression score in adolescents was 17.99±6.38. Depression had significant differences according to sleep duration and quality. Adolescents with less than eight hours of sleep showed the highest depression, and poor sleepers showed higher depression scores than good sleepers. Gender, school achievement, school satisfaction, economic status, and exercise time were found to significantly relate to depression. After controlling for general characteristics, depression explained 17.2% of the variance in quality of sleep (β=-.15, p<.001) and sleep duration (β=-.04, p=.022). Conclusion: These findings indicate that it is necessary to develop intervention strategies to enhance the quality of sleep and appropriate sleep duration for preventing depression in adolescents.

  • The tracheocutaneous fistula(TCF) may develop infrequently as a complication after tracheostomy. Prolonged tracheostomy tube dependence increases the risk of TCF developing, and in growth of stratified squamous epithelium lines the furrow connecting the tracheal mucosa and the skin, accounting for persistence of the fistulous tract. Such fistulas are a nuisance and create nursing and social problems including poor hygiene, aspiration, difficulty with speech, and depletion of pulmonary reserve. Surgical closure has generally been successful by primary closure, fistulectomy with primary closure, and closure by secondary intention following excision of the tracheocutaneous fistula. No large series compares the efficacy of these techniques and each has its own merits. Recent literature has purposed to minimizing complications. For ten years, from January 1985 to December 1994, the authors experienced 25 cases of TCF which were analyzed in respect to incidence and interval of cannulation, duration between decanulation and fistular closure, precedent disease, closure methods, and complications of TCF repair.

Chordae Tendineae Approximation Technique for Severe Tricuspid Regurgitation with Severe Leaflet Tethering Using a Totally Endoscopic Beating-Heart Strategy: A Case Report

  • Untreated severe tricuspid regurgitation (TR) is associated with poor outcomes. Functional TR occurs secondary to dilatation of the annulus and tethering of the leaflets. Ring annuloplasty alone can correct most cases, but is insufficient in cases of severe annular dilatation due to severe leaflet tethering. In such cases, a tricuspid edge-to-edge technique may be an option. However, stitching of the leaflet tips alone is likely to result in tearing of the leaflets. Approximation of the durable chordae tendineae is considered helpful for this problem. Herein, we present the case of a 39-year-old man who had undergone open-heart surgery for acute type A aortic dissection 13 months earlier. A right mini-thoracotomy approach with a beating-heart strategy was used, which did not require unnecessary pericardial adhesiolysis and dissection. This technique had the advantage of reducing the operation time and the risk of bleeding. To summarize, we present a case of tricuspid valve repair in a high-risk patient with severe leaflet tethering that was successfully managed using these methods.

  • As complementary metal-oxide semiconductor (CMOS) is scaled down to achieve higher chip density, thin-film layers have been deposited iteratively. The poor film uniformity resulting from deposition or chemical mechanical planarization (CMP) significantly affects chip yield. Therefore, the development of novel fabrication processes to enhance film uniformity is required. In this context, high-pressure deuterium annealing (HPDA) is proposed to reduce the surface roughness resulting from the CMP. The HPDA is carried out in a diluted deuterium atmosphere to achieve cost-effectiveness while maintaining high pressure. To confirm the effectiveness of HPDA, time-of-flight secondary-ion mass spectrometry (ToF-SIMS) and atomic force microscopy (AFM) are employed. It is confirmed that the absorbed deuterium gas facilitates the diffusion of silicon atoms, thereby reducing surface roughness.

Correlation of red cell distribution width and left atrial enlargement in Maltese dogs with myxomatous mitral valve disease in Republic of Korea

  • Myxomatous mitral valve disease (MMVD) is a degenerative disease of the valve leaflets, causing left atrial dilatation and eccentric hypertrophy of the left ventricle by hemodynamic instability. Red cell distribution width (RDW) is a hematologic parameter that indicates the variation of red blood cell volume and size, reflecting anisocytosis. Human studies have found that anisocytosis is associated with poor prognosis in heart disease patients, and recent veterinary studies have also confirmed that the increase in RDW is associated with high mortality in MMVD patients. Medical records of 37 Maltese dogs with MMVD were retrospectively reviewed. When comparing RDW among the MMVD stage groups, there was a significant difference between stage B1, B2 and C. A significant and strong correlation between RDW and the left atrial-to-aortic ratio was identified. RDW was significantly correlated with the reticulocyte count independent of hematocrit, and the reticulocyte count exhibited a significant increase at stage C. This suggests that the congestive heart failure secondary to MMVD could be a contributory factor leading to an elevation in RDW. In conclusion, elevated RDW may associated with left atrial enlargement and progression of MMVD.

Clinical Nurses' Perception on Barriers to Research Utilization (임상간호사의 연구활용 장애에 대한 인식)

  • Purpose: Although previous studies reported that actual application of research findings into the clinical setting was still poor, research utilization is a major component for implementation of Evidence-based Nursing Practice (EBNP). This study was conducted to identify the barriers to research utilization in clinical nurses. Methods: A descriptive survey design was used. Participants were 392 clinical nurses who were interviewed using a structured questionnaire. Results: Priority factors of barriers were setting, communication, research, and nurses. Five of the top 10 barriers were related to 'setting.' The first barrier was difficulty in the comprehension of statistical analyses. Nurses who worked in secondary hospitals and regularly read research articles perceived significantly lower barriers to research utilization compared with other nurses. There were statistically differences in research utilization among the groups by level of searching skills related to research documents and degree of understanding of EBNP. Conclusion: The study findings identified the top barriers to research utilization and key factors for nursing administrators and educators to consider in developing strategies to facilitate the utilization of research findings in clinical settings.

  • Objectives: Transverse myelitis is a rare neurological disorder that is part of a spectrum of neuroimmunologic diseases of the central nervous system. A patient was hospitalized with intestinal pseudoobstruction(poor oral feeding, vomiting, abdominal pain, constipation) secondary to Transverse myelitis. We treated her with Oriental medical approach and obtained satisfactory result. Methods: The Herbal medicine(Bihwa-eum), Acupuncture, Moxibustion therapy were applied for improving the deficiency of energy of the spleen and stomach, regulate the flow of vital energy. Results: After treatments, Patient's vomiting frequency is decreased and abdominal pain is improved and oral feeding is available. She gains in weight continuously. Conclusion: The authors thought that Bihwa-eum is effective to vomiting and abdominal pain due to intestinal pseudoobstruction. The more study is needed.

  • Scalp defect management is complicated secondary to reduced laxity in the scalp and forehead area. For reconstruction of larger defects with exposed bone and loss of the periosteal layer, free flap reconstruction is one option for single-stage surgery, although the procedure is lengthy and includes the possibility of flap loss. We successfully performed a single-stage reconstruction of a large scalp defect using a combination of artificial dermis, split-thickness skin graft, and full-thickness skin graft following wide excision of a cutaneous angiosarcoma, and present our method as one option for the treatment of large oncologic surgical defects in patients who are poor candidates for free flap surgery.

Prevalence and Risk Factors of Unmet Healthcare Needs among Korean Adults with Hypertension (한국인 고혈압 환자의 의료접근성 및 미 충족 의료실태와 위험요인 분석)

  • Purpose: The purposes of this secondary analysis study was to examine prevalence, risk factors and unmet healthcare needs among adults with hypertension. Methods: A sample of 3,386 adults over the age of 40 with hypertension were drawn from the Korea Health Panel Study (2013). Using SPSS 22.0 version, descriptive statistics including frequency, percentage, chi-square and logistic regression were performed. Results: Results showed that 18.9% of the sample reported unmet healthcare needs with the most frequently cited one was financial burdens (43.2%). The reported experiences of unmet healthcare needs differed by gender, marital status, vision or hearing impairment, memory problem, impaired mobility, subjective health status, total family income, depressive episode and the difficulty in making decisions. The sample participants were more likely to report unmet healthcare if they had vision impairment, low income and perception that their health status as moderate to poor. Those without vision impairment were less likely to report unmet healthcare needs. Conclusion: The identified risk factors of unmet healthcare needs should be addressed which would enhance access both to health care and to resolution of unmet healthcare needs. Since visual ability seems to impact perception of unmet healthcare needs, it may be useful to find ways to address this factor.
