A Study on Methods of Acupuncture Points Combination and Hierarchy Concept of Acupuncture Prescription(鍼 處方) in Korean Medicine(韓醫學). (한의학(韓醫學) 침(鍼) 처방(處方)의 구성 방법 및 주차(主次) 개념에 관한 고찰)

  • Objectives: This study is to understand and develop Acupuncture treatment in Korean Medicine(韓醫學). Methods : Firstly, the definitions of Acupuncture Prescription(鍼 處方) and Methods of Acupuncture Therapies(鍼法) were clarified. Secondly, the results from the researches of previously existing Acupuncture Prescriptions(鍼 處方) were analyzed. Results & Conclusions : Acupuncture Prescription(鍼 處方) represents the results achieved from combining Acupuncture Points(腧穴) according to the symptoms of diseases. Methods of Acupuncture Therapies(鍼法) denote methodologies for proposing Acupuncture Prescriptions(鍼 處方) which were accomplished by combining Acupuncture Points(腧穴) based on a specific theory. The existing methods of combining Acupuncture Points(腧穴) derived from the analysis of Acupuncture Prescription (鍼 處方) have a meaning in suggesting a direction for acupuncture treatments. However, deficiencies still exist to restore the absence of universality and objectivity in acupuncture treatment. Researches on Methods of Acupuncture Therapies(鍼法) based on the principle are required accordingly. The concept of hierarchy exists in Acupuncture Prescription(鍼 處方) and it ranks Acupuncture Points(腧穴) as primary and secondary according to the indications of the Acupuncture Points(腧穴). Since there is the time difference in needling process, the concept of hierarchy is fulfilled through the needling procedure.

Analysis the Correlation of Filtration Rate of Herbal-acupunctures and Pain Score of Herbal Acupuncture Stimulation (약침제재별 여과속도와 약침 시술시 느끼는 통증의 상관성 분석)

  • Objectives : This study was designed to find out correlation of filtration rate of herbal-acupunctures and pain score of herbal acupuncture stimulation. Methods : Filtration rate of Hwangryunhaedoktang herbal acupuncture, Kidney yang deficiency number two herbal acupuncture, Hominis Placenta herbal acupuncture, Cervus Elaphus herbal acupuncture, BUM herbal acupuncture and Carthami Flos herbal acupuncture were measured at the korean institute of herbal acupuncture. And Hwangryunhaedoktang herbal acupuncture, Kidney yang deficiency number two herbal acupuncture, Hominis Placenta herbal acupuncture, Cervus Elaphus herbal acupuncture, BUM herbal acupuncture and Carthami Flos herbal acupuncture were injected at Xie points of the subjects. Results : Ranking of herbal acupunctures filtration rates is the same as ranking of pain score and duration time of herbal acupuncture. Correlation analysis result of herbal acupunctures filtration rates and herbal acupuncture VAS with a simple linear regression analysis is that correlation coefficient of Hwangryunhaedoktang herbal acupuncture, Kidney Yang Deficiency Number Two herbal acupuncture, Hominis Placenta herbal acupuncture, Cervus elaphus herbal acupuncture, BUM herbal acupuncture. Carthami Flos herbal acupuncture is 0.44, 0.53, 0.58, 0.76, 0.47 and 0.54. Correlation analysis result of herbal acupuncture's filtration rates and herbal acupuncture stimulation's duration time is that correlation coefficient of Hwangryunhaedoktang herbal acupuncture, Kidney Yang Deficiency Number Two herbal acupuncture, Hominis Placenta herbal acupuncture, Cervus Elaphus herbal acupuncture, BUM herbal acupuncture, Carthami Flos herbal acupuncture is -0.09, 0.11, -0.07, 0.28, -0.11 and -0.09. Conclusion : We found the correlation of filtration rate of herbal-acupunctures and pain score of herbal-acupuncture stimulation. This study help to control pain amount and pain duration time of herbal acupuncture treatment. And this study improve acupuncture theory of herbal acupuncture.

  • A comparative study of influence by area between acupuncture points on hand of original acupuncture system with those of hand acupuncture system was resulted as followes 1. The influence of acupuncture points on original acupuncture system was different from those on hand acupuncture system by meridian system even though on same area or point but was similar by meridian coordination even though on different area or point. 2. The influence of acupuncture points on hand acupuncture system was almost added by those of coordinated original acupuncture system point on those of hand acupuncture system point. 3. The influence of non coordinated hand acupuncture points with several original acupuncture points was according to those of up or down point on same meridian. 4. Tung;s acupuncture system had no corelationship with other two acupuncture systems, where it has no holistic system on original princeple and Extra-points had also no corelationship, where it has no meridian system.

Research Trends in Korean Journal of Acupuncture: Focus on Keywords Analysis (경락경혈학회지 연구동향 분석: 주요 키워드 분석을 중심으로)

  • Objectives : The study's goal was to look at the research trends of articles published in the Korean Journal of Acupuncture. Methods : The Korean Journal of Acupuncture's website yielded a total of 882 articles. The VOSviewer application was used to visualize research trends from keywords. Results : A total of 87 keywords were found and visualized based on the average year of publication. The relevant characteristics and trends of basic acupuncture research published in Korean Journal of Acupuncture were determined by a network analysis based on the co-occurrences and publication year of keywords. Acupuncture, acupoint, herbal acupuncture, electroacupuncture, moxibustion, and meridian were the most frequently used keywords. Conclusions : This bibliometric study will give you a broad picture of research trends in Korean Journal of Acupuncture. These data may help to establish a timeline for the advancement of acupuncture basic research.

Dong-Qi Therapy of Dong-Si Acupuncture to Movement System Impairment Syndrome of Lumbar Spine and Knee (요부.슬부 동태손상증후군에서의 동씨침 동기요법의 활용)

  • Objectives : The aim of this study was to show the application to add the Dong-Qi therapy of the Dong-Si acupuncture to exercise therapy of the movement system impairment syndrome(MSIS) and to determine the best acupuncture point for the Dong-Qi therapy. Methods : We reviewed Diagnosis and Treatment of Movement Impairment Syndrome and Movement System Impairment Syndromes Of The Extremities, Cervical and Thoracic Spines written by Sahrmann SA. to show the exercise therapy of the sort of MSIS. We reviewed complete works of Yang Wei Jie to show the acupuncture points of the Dong-Si acupuncture and the Dong-Qi therapy. Results : We showed the acupuncture point of the Dong-Si acupuncture by each type of MSIS based on the reference book of MSIS, the Dong-Si acupuncture. Also, we selected and tabulated the best possible acupuncture point of the Dong-Si acupuncture which could minimize a side effect of acupuncture during the therapeutic exercise by each type of MSIS. Conclusion : A specific acupuncture point of the Dong-Si acupuncture could be chosen for a specific MSIS therapeutic exercise. The best possible acupuncture point could be chosen when selecting an acupuncture point of the Dong-Si acupuncture.

Research Trends in Evidence Based Medicine on Acupuncture -Randomized Controlled Trial- (침 관련 근거중심의학의 연구 동향 - Randomized Controlled Trial을 중심으로 -)

  • Objectives : The purpose of this study is to review the recent research trends of evidence based medicine, especially human randomized controlled trials on acupuncture. Methods : The articles were collected by retrieving the database of Pubmed and Journal of Korean Acupuncture & Moxibustion Society. The retrieving period was from October 2003 to September 2008, and the search term was 'acupuncture'. The articles were classified according to their publication journals, countries, publication years, targeted diseases, types of acupuncture and types of control. Results : In total, 558 articles in Pubmed and 35 articles in Journal of Korean Acupuncture & Moxibustion Society were searched. The number of articles on acupuncture research has increased with higher rate since 2000, but the Qualitative development has not achieved the same amplitude. Studies in Korea were insufficient both in Qualitative and Quantitative aspects. In countries, China had the most papers, and in targeted diseases, pain diseases were most dominant. In the types of acupuncture, classical acupuncture, and in the types of control, conventional western treatment showed the highest frequency. Conclusions: We need to do many-sided and more high Quality researches on acupuncture. For that, well-designed randornized trials are absolutely necessary.

A comparison study of acupuncture sensation scale between real acupuncture and sham needle (거짓침 피부접촉부 형태에 따른 침감 차이 연구)

  • Objectives : Proper acupuncture stimulation is associated with a characteristic set of sensation usually referred to as 'De-Qi'. In order to develop the appropriate sham acupuncture, various sensations to each stimulation should be considered through analysis of the profiles of acupuncture sensation. It was therefore investigated to compare the acupuncture sensation scale (ASS) of two types of sham acupuncture to that of the real acupuncture. Methods : Ninety-four participants (mean age 26.4, range 26-49) were asked to complete five point-Likert scale ASS developed by Vincent et al. after real or two-kinds of sham acupuncture stimulation: blunted tip sham acupuncture (BT) and round tip sham acupuncture (RT). Needling was done at LI4 acupoint on non-dominant hand and stimulated for 30 seconds with real or two-kinds of sham needle. Finger withdrawal latency (FWL) of each group was also measured to evaluate the pain sensitivity to noxious heat stimuli. Results : BT acupuncture significantly less produced penetrating, numb, intense, hurting, pulling, shock, tingling, throbbing sensation than real acupuncture stimulation. RT acupuncture significantly less produced penetrating, burning, electric, numb, intense, hurting, pulling, aching, shasharp, shock, stinging, tingling, throbbing sensation than real acupuncture. Each group did not demonstrate the differences of pain sensitivity to noxious stimuli. Conclusions : These results indicated that types of tip of acupuncture produced different kinds of acupuncture sensation. Our finding provides a general information of sensations to two kinds of sham acupuncture for development of ideal placebo sham needle.

A Review of Acupuncture Treatment for Infertility

  • The purpose of this study was to analyze acupuncture treatment methods and acupoint used to treat infertility in the last decade. The data were retrieved from 2010.1 to 2021.8. using national and international electronic databases (PubMed, EMBASE, OASIS, NDSL, and KISS). "Infertility," "sterility," "acupuncture," "electroacupuncture," "bloodletting," "pharmacopuncture," were used as search terms. All studies were mainly concerned with acupuncture treatment of infertility, but the studies which were not related included; (1) infertility or acupuncture treatment; (2) animal studies; (3) review/meta/protocol or clinical study; comparative studies and case reports which were excluded from this review. Only clinical trials for acupuncture treatment of infertility were included in this review (n = 18). For infertility treatment studies using acupuncture, polycystic ovarian syndrome was the main cause of infertility, the most common acupoint used was CV4, CV3, CV6, SP6, BL23, and the treatment effect significantly improved in the group receiving acupuncture treatment compared with the control group, or the group taking Western medicine alone. In some studies, the group using acupuncture treatment did not show side effects. Acupuncture is an effective treatment for infertility.

Review and Analysis of Sham Acupuncture in Acupuncture Clinical Trials (침 임상연구에 사용된 거짓침의 분석)

  • Objectives : Recent well-designed randomized controlled trials(RCTs) and their meta-analysis have been published on the efficacy of acupuncture in different condition. In most of them, real acupuncture is compared with sham acupuncture including invasive and non-invasive sham methods. But it is not clear how active sham methods are. These results tend to lead the conclusion that acupuncture has no more effective than sham acupuncture. In order to investigate that sham acupuncture is appropriate as a control, we reviewed several acupuncture trials using different sham acupuncture as a control. Methods : We searched Cochrane researches of acupuncture, reviewed and analyzed 25 RCTs in 42 Cochrane reviews. And especially we compared the effect of acupuncture according to the type of sham acupuncture. Results : Invasive sham acupunctures are used in 12 RCTs and non-invasive types are used in the rest. The majority of studies(19 RCTs) fail to show effects beyond a sham acupuncture. Streitberger's sham needle is a validated sham acupuncture of non-invasive type that was used in 8 trials and also no significant group differences are shown except one trial. Conclusions : Acupuncture is a complex intervention. Clinical trials of acupuncture need to be reexamined and redesigned to remove several bias. Especially, sham acupuncture as a control might be investigated for physiological effects as well as validation test including patient-blinding and de qi sensation. Other research need to be investigated and developed for acupuncture trials.

Study protocol for clinical trial to Compare the Effectiveness of 'Individualized Acupuncture' with 'Standardized Acupuncture' in Korean patients with Knee Osteoarthritis

  • Background : One of the characteristics of acupuncture, a popular modality for treating musculoskeletal pain, is a plurality in diagnosis and treatment that can profoundly influence the treatment outcome. This multiplicity in treatment modality has to be considered in any research on the effectiveness of acupuncture. Many practitioners stress the necessity for individualized patient treatment, including acupuncture point selection and manipulation technique. However, the importance of individualization in acupuncture treatment, compared with standardization, has received little attention in clinical trials. The aim of the future study described here is therefore to compare the effectiveness of individualized acupuncture for knee osteoarthritis with standardized acupuncture and no acupuncture in patients with knee osteoarthritis. Methods : A total of 195 patients aged 50 years and over with knee pain, will be randomly divided into three treatment groups: individualized acupuncture, standardized acupuncture, and waiting list. Outcome data will be collected through patient.completed questionnaires before randomization, and at 4, 8 and 12 weeks after randomization. The questionnaires will be investigated demographic details as well as information on pain, movement and function of the affected knee, general health and quality of life. Discussion : This paper presents details on the rationale, design, and methods of the trial.
