Rabbit submandibular salivary gland replantation

  • Objectives: To test the feasibility of submandibular salivary gland (SMG) replantation techniques and the survival of the replanted glands. Such a study can provide a rationale for later allotransplantation procedures, along with implementation of conventional and advanced immunosuppression therapy. Materials and Methods: Six SMG replantations were performed in New Zealand white rabbits. One week postoperatively, $^{99m}Tc$ scintigraphy was performed and the uptake ratio and salivary excretion fraction were calculated. Two to four weeks later, submandibular glands were excised, fixed, and stained with H&E for histomorphometric evaluation. Results: Intraoperatively, all glands showed patent blood perfusion except gland 5. Positive tracer uptake and saliva excretion were documented by scintigraphy. On excision, all of the glands except glands 4 and 5 looked viable, with a red color and patent pedicles. Gland 4 was infected and filled with creamy pus, while gland 5 looked pale and necrotic. Histologically, glands 1, 2, 3, and 6 had preserved normal glandular tissue with slight variations from the contralateral normal glands, as their parenchyma was composed of mildly atrophic acini. Conclusion: Four out of six replanted SMGs successfully survived. The glands maintained good viability and function. Such success depends on safe harvesting, short anastomosis time, and strict control of infection.

치아 삭제의 다른 자가 평가 방법 후 개선에 대한 비교 연구 (A comparative study of the improvement after different self-assessment methods of tooth preparation)

  • 목적: 이 연구의 목적은 세 가지 자가 평가 방법에 따라 치의학전문대학원 학생들의 치아 삭제 능력의 향상 정도를 비교하는 것이다. 연구 재료 및 방법: 경북대학교 치의학전문대학원의 2학년 48명 학생을 대상으로 3개의 실험 그룹으로 나누었다. 금관(gold crown)제작을 위한 좌측 하악 제1대구치의 치아삭제를 수행하였다. 세 가지 자가 평가 방법(시각적, 디지털, putty index 자가 평가 그룹)을 사용하여 자가 평가를 수행하고, 치아 삭제를 다시 수행했다. 구강 스캐너를 사용하여 각 치아(삭제된 치아 및 삭제되지 않은 치아)를 스캔하고 standard tessellation language (STL) 파일 형식으로 데이터를 저장하였다. 삭제된 치아와 삭제되지 않은 치아의 STL 파일은 3 차원 분석 소프트웨어(Geomagic control X)를 사용하여 중첩되었다. 그리고 삭제량을 측정했다. 통계적 분석에서, 삭제량의 모든 값은 Wilcoxon signed rank 및 Kruskal-Wallis 테스트로 분석하였다(α = 0.05). 결과: 세 가지 자체 평가 방법은 통계적으로 유의한 차이를 나타냈다(P < 0.001). putty index 자가 평가 그룹은 디지털 자가 평가 방법보다 교육 후 오차의 감소가 더 컸다. 결론: 이 연구의 한계 내에서, 학생들은 세 가지 자기 평가 방법에 따라 치아 삭제 능력의 개선에 유의한 차이가 있음을 보였다.

Effects of 1 year of training on the performance of ultrasonographic image interpretation: A preliminary evaluation using images of Sjogren syndrome patients

  • Purpose: This study investigated the effects of 1 year of training on imaging diagnosis, using static ultrasonography (US) salivary gland images of Sjögren syndrome patients. Materials and Methods: This study involved 3 inexperienced radiologists with different levels of experience, who received training 1 or 2 days a week under the supervision of experienced radiologists. The training program included collecting patient histories and performing physical and imaging examinations for various maxillofacial diseases. The 3 radiologists (observers A, B, and C) evaluated 400 static US images of salivary glands twice at a 1-year interval. To compare their performance, 2 experienced radiologists evaluated the same images. Diagnostic performance was compared between the 2 evaluations using the area under the receiver operating characteristic curve (AUC). Results: Observer A, who was participating in the training program for the second year, exhibited no significant difference in AUC between the first and second evaluations, with results consistently comparable to those of experienced radiologists. After 1 year of training, observer B showed significantly higher AUCs than before training. The diagnostic performance of observer B reached the level of experienced radiologists for parotid gland assessment, but differed for submandibular gland assessment. For observer C, who did not complete the training, there was no significant difference in the AUC between the first and second evaluations, both of which showed significant differences from those of the experienced radiologists. Conclusion: These preliminary results suggest that the training program effectively helped inexperienced radiologists reach the level of experienced radiologists for US examinations.
