노인주거관련연구의 한.미 .일 비교 ( I ) - 일본의 경우 - (Comparison about the Study of elderly housing in Korea , U.S.A. and Japan ( I ) -Focusing on Japan)

  • The purpose of this study was to investigate the difference in the trend of the study of elderly housing for the suggestion of the direction in Korea. This study was the first series about the study of elderly housing and analysed the 164 pieces of researches in Japan. The data were analysed classification with contents first, then examined the class in chronological order, and suggest the trend in the study of elderly housing in Japan. 1. The study of elderly facilities was reported the highest interest and was completed in the latter half of the 1980s except the payment elderly home in Japan. 2. Their second interest was the study of general elderly housing. Rural elderly facilities as a local elderly housing were researched frequently. 3. The study of housing for three generation was studied much in early 1980s, but decreased concerns these days. The study of housing for the elderly was studied the latter of 1980s and concerns were sustained. 4. Trend of the study of elderly housing was classified as follows; 1950s was the stage of unconsciousness, 1960s was the stage of sprout, 1970s was the stage of blossom, 1980s was the stage in full bloom and then the study was peak in 1986 and slowly decrease the concern. 5. In 1990s the study of elderly housing was moved into elderly facilities in a consumer's view and older people's life their family relation.

노인간호요양시설의 QMI 개발과 현장 실태분석 (Development of Nursing Home Quality Management Index & Application in the Field)

  • Purpose: Expecting a new system of nursing security for elderly in operation in 2007, a substantial expansion of nursing facilities for elderly and quality management of those facilities are imperative now. Therefore. specific quality management measures or schemes which could be used for the operation of small-scale nursing homes are in urgent need. The purpose of this study was to develop a Quality Management Index(QMI) to guide Nursing Home management. Method: This study developed a QMI for small-scale nursing homes, which focused on quality management in structural dimensions such as environment, staffs, local community network, atmosphere, and quality management in process dimensions such as nursing, communication, resident satisfaction. The QMI developed in this study is based on extensive review of literature and the actual experiences of nursing home operation. It consists of 7 dimensions, 39 components and 148 indexes, which have been verified by three times validity tests of experts group. Then the QMI was used to evaluate the quality management levels of nursing homes in Korea. Result: In sum, after analyzing all nursing homes in Korea operated by nurses, this study found that there were significant differences of quality management level among facilities in several dimensions such as environment, staffs, local community connections, communication. In dimensions such as nursing, atmosphere, resident satisfaction, however, little differences of quality management level among facilities were reported. It is considered that this result has to do with several factors like the size of facilities, operation expenses, operation forms, the disposition of professional human resources, and philosophy and management policy of the owners. As far as generalization is concerned, however, it needs to be noted here that the sample size for this study is not statistically big enough to generalize the results. Conclusion: Having Developed a QMI for small-scale nursing home with 7 dimensions, 39 components and 148 indexes, this study is expected to be used in developing more elaborated quality evaluation tools for nursing homes, and also function as a practical guide of quality management for those who are opening and managing nursing homes. I hope this quality index could lead to further development of a standardized quality management index, and eventually contribute to quality improvement of nursing homes.

노인공동주거시설 단위주호의 설계지침 중요도 분석 (Analysis on the Importance Degree of Design Checklist for the Elderly Housing Unit)

  • A rapid increase of the elderly population and changes in the social structure, family type and lifestyle bring us to the moment for considering the various aspects of supporting the elderly including the elderly housing facilities. Traditionally it is considered as a merit for a son to support parents in his own house. Hut recently the elderly increasingly want to live independently without support from their children. To satisfy these needs, new housing facilities for the elderly were developed by private non-profit foundations. A number of outstanding housing facilities for the elderly have been built in the last five years. These facilities show significant progress in housing for the elderly. Unfortunately, there u e no standard design checklists or guidelines for housing for the elderly available in Korea. The housing facilities were built based on foreign design guidelines or architect's experience. This research aims to develop an affordable design checklist that can meet the needs of the Korean elderly lifestyle. This study evaluated the senior residential facilities using Woo's checklist which consolidated the guidelines available here and abroad. As the result of this study, we presented the checklist subject in upper group $25\%, middle group, lower group $25\% distribution and 6, 5, 4, categories in order to importance. When designing the future senior residential facilities, a constraint can be made based on the scope and financial factors of the facility first and the determine how extensive the application of the checklist should be.

일본 유료 노인홈의 건물 내 층별 실구성의 특성 (Characters on building floor of Japanese elderly house)

  • It is predicted that Japan will be in super-aged society at 2010. Recently elderly house buildings with self-supporting life and personal care senior house types have supplied over the area. So Elderly house buildings in Japan were studied for characters of room arrangement, through documents and internet from Jan. 2007 to Apr. 2007. Five cases built since 2000 were examined. There were self-supporting life house type, personal care senior house type, convenient facilities, etc. There were not lots of facilities in the building. Generally multi-purpose dining room and general bath room, lots for car and bicycle or restaurant or kindergarten or home helper station or green food shop or day service or moving service or clinic were there. These facilities were shared with local community. The reason of not being many facilities in the building was that the houses were located at urban with good transportation and convenient facilities. The residents in the building were get the utmost of regional facilities and the local people did the facilities in it because scare facilities in the building and fluent facilities in region. So strong community was composed of the residents and local people, on the contrary.

노인요양시설 물리치료사의 클라이언트 관계경험을 통한 정체성 탐색 (Finding Identity through Client Relationship Experience of Physical Therapists in Elderly Care Facilities)

  • Purpose: This study was conducted to understand the client relationship experience of the physical therapists working at elderly care facilities and to lay the foundation for finding the identity of physical therapists. Methods: In-depth interviews were conducted on eight incumbent physical therapists who had experienced working in elderly care facilities for at least one year. Data collection was conducted between June and September 2017, and the interview took an hour to an hour and a half to complete. After an in-depth interview, it was arranged as a file and analyzed according to the four steps of the phenomenological research method of the Giorgi. Results: Data were collected and analyzed; 19 categories, 6 themes and 5 domains were derived. Six themes were presented according to the 5 domains; the subject of relationships included 'diversity elderly'; context of relationships included 'the world of elderly nursing homes I did not know'; influence of relationships included 'the physical therapist is heartbroken because of the tormenting elderly'; condition of relationships included 'a reality that is tied to work'; aspects of relationships included 'how to reestablishing relationships with the elderly' and 'recognized and identified as a physical therapist'. Conclusion: This study shows that physical therapists working in elderly care facilities generally tend to fulfill their responsibilities and interact with clients; we found that identity of physical therapists was primarily defined by relationship experiences with patients.

미국 치매노인시설의 거주환경디자인 특성의 내용분석 (Contents Analysis on the Characteristics of Living Environmental Design in Facilities for the Elderly with Dementia, USA)

  • The purpose of this study was to analyze environment-behavior concepts included in environmental design characteristics of facilities for elderly with dementia. Major method was contents analysis of actual environmental designs. For comparison between the environment-behavior concepts and actual cases, an analytical framework was developed. Eleven cases of facilities designed for people with dementia were analyzed. Those were published ones in periodicals in USA. The analytical data used included floor plans, texts, and visual materials including photographs. Under the framework for analysis, a total of 296 criteria were used to analyze the actual cases of environmental design. This study was expected to offer a knowledge base for better environmental design for elderly with dementia and to help establish guidelines for designing such facilities which can meet the cultural characteristics and traditional conditions of Korea.

경인지역 요양시설의 식사지원 서비스 실태 (Meal Services at Long-term Care Facilities in Kyung-In Area)

  • Purpose: This study was aimed at understanding meal services provided at long-term care facilities. Method: Interview survey was conducted using questionnaires at 254 facilities located in the Kyung-In area. Result: Of the residents, 37.4% were eating meals unassisted. Eating places included living rooms and residents' rooms in most facilities. Major noise source was television in 63.8%. Apron was applied to all elderly residents at mealtimes in 49.6% of the facilities. Half of the facilities used feeding utensils except for ordinary spoon and chopsticks. Of the facilities having individual prosthetic devices, dentures were applied before eating in 98%, glasses in 20.2% and hearing aids in 9.2%. Most facilities included the residents' favorite foods in menu: wheres, only 9.4% offered the menu which residents could choose. Conclusion: Standard guidelines and staff education for meal services need to be provided for elderly residents.

장기 보호 여성노인의 수발에 관한 연구 - 수발특성과 우울을 중심으로 - (Caregiving for the Long-Term Care Elderly Women - Focusing on Caregiving Characteristics and Depression -)

  • The present study was initially designed to figure out the general condition of care giving system for the elderly women who need long term care and the level of their depression according to the conditions of care. And This research is intented to present appropriate policy that could help the establishment of supporting system for the fragile elderly women.1 used the data from <2001 National Study on the Needs for the Long-Term Care Elderly> by Korea Health and Population Institute. The results are as follows: First, Two third of all the respondents had serious problems (2-9 activities limits) in Instrumental Daily Living Ability(DAL). Most respondents reported “low” in satisfaction level related to receiving care, meaning the elderly had negative perception for the care from the family. The elderly expected their children to be as the primary care giver and mostly wanted to live with them in the future. Second, The majority of the long term care elderly women haven't used community service facilities very often and said they are not likely going to use the facilities in the future. Third, The respondents reported high in depression level as to lower satisfaction with their children's support, poorer health condition, more reluctant to use service facilities due to the cost, and fewer friends and neighbors resources around them. Therefore I could say that negative factors for the elderly women's psychological health were having unsatisfactory relationship with intimate people, developing physical illness, being in economic difficulties. That is, receiving less help from close family members, shrinking social network, and experiencing economic hardship would have negative effects on elderly women's psychological health. In the basis of these results, I suggest that in the mean time we shouldn't overlook the importance of the private support when we develop the public elderly support system.

고령자용 소규모 그룹홈의 생활환경 실태 (Actual conditions on the Living Environment of Small-scale Group-homes for the Elderly)

  • To investigate the actual conditions on the living environment of group-homes for the elderly and their living environment, 4 group-homes were surveyed. As the results, conditions of location for the group-home of the elderly looked positive, on the other hand, safety for criminal prevention, accessability and convenient facilities were not on level satisfied. The average area of each space in the group-home indicated that the bed room was $11.21\;m^2$, the living room $28.13\;m^2$, the kitchen $23.59\;m^2$, and the bath room had an integrated type of bath room and toilet $7.63\;m^2$. The physical environments of each space in the group-home were investigated, In bed room, door, windows and illumination were relatively good but safety was extremely inferior. Living room mostly looked good except the doorsill which needs refurbishment. In kitchen, cooking table, windows, illumination, ventilator were somewhat satisfactory but gas-warning device should have been installed. In addition, in bath room and toilet, location, door, window, illumination and finished material were satisfactory. But the stepped difference between the bath room and other space, safety-bar, emergency bell and floor heating system were seriously deteriorated and must be improved for the safety of the elderly. Conclusively, living environment of group-home for the elderly is mainly satisfactory but it was necessary to take a consideration for their safety.

일본의 재가간호 및 방문간호 -새로운 개호보험제도의 실시를 앞두고- (A Study on Home Care and Home Visiting Nursing in Japan)

  • Japan has been prepared for aging society from 1970. In 1970, the percentage of distribution of population of 65 years old and over was 7.1%. It is similar to present percentile of the elderly in Korea. Therefore, it will be needed to study about home care and home visiting nursing in Japan at present. This study was aimed to prepare the fundamental documents for home care nursing in Korea and to know the background of new health care system of Long-term Care Insurance in Japan, by studying home care and home visiting nursing in Japan. With the continuing aging of the population, especially the increase in the number of latter stage elderly, it is predicted that there will be an increase in the number of the elderly who are bedridden and suffering senile dementia. To ensure that these people will be able to continue living in the communities and homes they are accustomed to, surrounded by their families and neighbors, Japan substantially improve and expand in-home services. There were also long-term effort to reach the level of services outlined in the Gold Plan and the New Gold Plan within the decade between FY 1991 and FY 1999 in the field of health care and welfare. Under this plan, the most noticeable change was occurred in home care, home was permitted as the field of care and visiting nursing was established in law. Through this 10- Year Strategy for Promotion of Health and Welfare Services for the Aged, many problems have been improved and solved, but some problems remained such as inadequate service supply and consumption of medical insurance for the elderly. Japan will be a society composed 25% of elderly people of total population in 2020, and it will be soon faced with a shortage of welfare and medical facilities and manpower. As for equalizing the benefits and cost burdens, and other future arrangements for health care and welfare, Long-term Care insurance system was established in 1994. This system will be enforced from April 2000 and use present facilities and services. To know home care and home visiting nursing in Japan, we need to consider present conditions well and to take notice of changes and measures to cope with an aging society continuously.
