  • Nowadays, demand for human resource for the e-learning industry is rapidly increasing along with the expansion of e-learning market capacity. However, there arc numerous difficulties in expansion and industrialization of e-learning due to insufficient supply of human resources to meet the demand. Therefore, the goal of this study is to present various policy measures that can supplement the supply of e-learn ing manpower. Overall contents of this study focus on presenting the long-term directions for fostering of human resources for e-learning industry. Among these, role of government policies for fostering of human resources for e-learning industry is being particularly emphasized because e-learning industry is still at its infant stage.

Analysis of Korean Childcare Studies (국내 영유아보육 논문의 연구동향 분석)

  • The purpose of present study was to analyze a tendency in childcare studies published in Korea. Specifically, this study investigated the amount of childcare studies and analyzed the main topic of childcare studies. Total 1,406 studies which include the term 'bo-yook' and/or 'tak-ah' as their keywords were gathered from internet database, and finally 973 studies were analyzed. First, the results from this study showed that childcare studies had gradually increased since 1990 and approximately one hundred studies had been published per year after 2004. The growing tendency of childcare studies might reflect social and political concern about childcare in contemporary Korean society. Second, childcare studies were mainly conducted within the topic of childcare policy, childcare worker, actual condition in childcare service, and childcare quality. Lastly, special types of childcare such as infant childcare, handicapped childcare, extended-time childcare, and after-school childcare were rarely studied. From these results, several future direction for childcare studies were suggested.

  • In this study, the nutritional composition of colostrum milk obtained from 22 mothers delivering perterm infants and 17 mothers delivering term infants was analyzed. The energy content of colostrum was yielded based on nitrogen, lactose, and lipid data. Preterm milk is more close the nutritional needs of the preterm infant than dose other breast milk. The results are as follows : 1) Protein concentration was significantly higher, but carbohydrate and lipid concentration were lower in preterm milk than fullterm milk. Energy content yielded from total nitrogen, lactose and lipid concentration was higher in the fullterm milk than preterm milk. 2) Calcium was higher in the preterm milk than fullterm colostrum. Phosphate was lower in fullterm milk than pretem milk. Calcium/Phosphate ratio were 2.61/1 in the preterm and 2.06/1 in the fullterm milk. Magnesium was higher in the preterm milk than fullterm milk. Zinc, Copper and Manganese concentrations were tended to lower in the preterm milk than fullterm milk.

  • Tracheal agenesis is an extremely rare and typically lethal congenital disorder. Approximately 150 cases have been described since 1900, and very few cases of survival have been reported. We describe tracheal reconstruction with external esophageal stenting in a patient with Floyd's type II tracheal agenesis. Neither long-term survival nor survival without mechanical ventilation for even a single day has previously been reported in patients with Floyd's type II tracheal agenesis. The infant in the present case survived for almost a year and breathed without a ventilator for approximately 50 days after airway reconstruction using external supportive stents.

  • Majority of sick full term newborns have adequate adrenal cortical function in response to stress. Acutely ill neonates with a basal cortisol level less than $15{\mu}g/dL$ (414 nmol/L) suggest adrenal insufficiency and require function testing of adrenal function. In premature infant, immaturity of hypothalamic-pituitary adrenal axis (HPA axis), may limit the ability to increase cortisol production in response to stress. The response to low dose ACTH and CRH appears to be useful as an additional test of adrenal function. CRH stimulation has been used increasingly in neonates. The ACTH and CRH stimulated cortisol response of more than $17{\mu}g/dL$ (469 nmol/L) indicates a normal response.

Management of Persistent Pulmonary Hypertension in Preterm Infants

  • Persistent pulmonary hypertension of the newborn (PPHN) is a consequence of the failure of a decrease in the elevated pulmonary vascular resistance after birth. Pulmonary vasodilators, including inhaled nitric oxide (iNO), have been the mainstream of targeted therapy for PPHN, but no drugs have been proven to be effective in preterm infants with PPHN. The fetus remains hemodynamically stable despite lower arterial oxygen tension and pulmonary blood flow as compared to full-term newborns. This adaptation is due to the lower oxygen requirement and high oxygen-carrying capacity of fetal circulation. The immature lungs of preterm infants are more vulnerable to reactive oxygen species, and the response of pulmonary vascular dilatation to blood oxygen tension is blunted in preterm infants. Recently, iNO has been reported to be effective in a selected group of preterm infants, such as those with prolonged preterm rupture of membrane-oligohydramnios-pulmonary hypoplasia sequence. PPHN in preterm infants, along with maximum supportive treatment based on fetal physiology and meticulous assessment of cardiovascular function, is in dire need of new treatment guidelines, including optimal dosing strategies for pulmonary vasodilators.

Practice for preterm patent ductus arteriosus; focusing on the hemodynamic significance and the impact on the neonatal outcomes

  • Hemodynamically significant preterm patent ductus arteriosus (PDA) affects mortality; comorbidities such as necrotizing enterocolitis, intraventricular hemorrhage, and bronchopulmonary dysplasia; and adverse long-term neurodevelopmental outcomes in preterm infants, particularly in very low birth weight infants. However, recent studies have indicated that there is no consensus on the causal relationship between PDA and neonatal outcomes, the benefit of PDA treatment, the factors guiding the need for treatment, and optimal treatment strategies. Such uncertainty has resulted in wide variations in practice for treating preterm PDA between units, regions, and nations. Nowadays, there has been a paradigm shift to more conservative treatment for preterm PDA, and suggestions regarding selective management of preterm PDA considering risk factors and hemodynamic significance are increasing. Neonatologist-performed echocardiography and advances in modalities to assess hemodynamic significance such as biologic markers and near-infrared spectroscopy also help improve the efficacy of selective treatment of preterm PDA.

Traumatic Posterior Fossa Subdural Hematoma in a Neonate: A Case Report

  • Although traumatic posterior fossa subdural hematoma (TPFSH) in neonates immediately after birth is extremely rare, it can pose a serious clinical problem in the neonatal period. Here, the author presents the case of a 3-day-old male infant who underwent emergency surgical treatment of TPFSH with a favorable outcome. Debate continues about surgical versus conservative treatment of TPFSH in neonates. The clinical symptoms, extent of hemorrhage, early diagnosis, and prompt and appropriate surgery are the most important factors in the treatment of TPFSH in neonates. Therefore, neurosurgeons should establish treatment strategies based on the newborn's clinical condition, the size and location of the TPFSH, and the potential of the hematoma to cause long-term complications.

Recurrent hemolytic uremic syndrome caused by DGKE gene mutation: a case report

  • Atypical hemolytic uremic syndrome (aHUS) is a rare disease characterized by microangiopathic hemolytic anemia, thrombocytopenia, and acute kidney injury without any association with preceding diarrhea. Dysregulation of the complement system is the most common cause of aHUS, and monoclonal humanized anti-C5 antibodies are now recommended as the first-line treatment for aHUS. However, if the complement pathway is not the cause of aHUS, C5 inhibitors are ineffective. In this study, we report the second reported case of aHUS caused by DGKE mutations in Republic of Korea. The patient was an 11-month-old infant who presented with prodromal diarrhea similar to typical HUS, self-remitted with conservative management unlike complement-mediated aHUS but recurred with fever. While infantile aHUS often implies genetic dysregulation of the complement system, other rare genetic causes, such as DGKE mutation, need to be considered before deciding long-term treatment with C5 inhibitors.
