Pneumococcal Vaccination Rate and Related Factors in Older Adults in Republic of Korea (한국 노인의 폐렴구균 백신 접종률 및 관련 요인)

  • Objectives: This study aimed to identify the vaccination rate for pneumococcus among aged 65 or older, and at the same time to determine the reasons for vaccination or non-vaccination. Methods: The population of this study was aged 65 or older, with a total of 1,150 subjects to be analyzed and a computer assisted telephone interviewing was used. The survey included pneumococcal vaccination status, reasons of vaccination, sources of information on vaccination, reasons for not vaccination, and other related factors. Results: The vaccination rate for pneumococcus among aged 65 or older was 56.2 percent, lower than 69.0 percent in the U.S. (2017), and 69.2 percent in the U.K. (2019). Among the factors related to the pneumococcal vaccination, the groups with the high vaccination rate were women, low age groups, residents of urban areas, people without a job, people with high education, medical insurance subscribers, married people, and people who have family members. In addition, the groups with high vaccination rates were those with high awareness, those who received recommendations from doctors, those who had vaccination records, those who believed in vaccination effects, and those who saw public service advertisements. Conclusions: In the future, it is necessary to develop alternatives to accurately manage vaccination records for adults who are not eligible for state support, and regular adult vaccination rates should be calculated so that they can be used as evidence for the country's infection control policy.

Issue Analysis on the Deregulation of Telemedicine in the Digital Convergence Era (디지털 융복합시대에 원격의료 규제 완화에 관한 쟁점 분석)

  • The purpose of this study is to analyze various issues that may arise from the recent deregulation of telemedicine implemented by the government, propose initiative preparations for the implementation of telemedicine policies, and suggest implications for the basic conditions and direction of the deregulation of telemedicine. Recently, deregulation policy cases in telemedicine include that designation of a special zone for regulatory freedom in Gangwon Province, allowing physician-patient telemedicine and telephone counseling and prescription cases in the COVID-19 crisis. There are four main issues that could arise with the deregulation of telemedicine: safety, effectiveness, differences in access to health care and the economic industry, and legal responsibility. As a initiative preparation, this study proposed a pilot project for telemedicine and enhanced support for face-to-face care complementary tools, vitalizing remote co-operation, policy model that do not disrupt the medical delivery system, and legal maintenance. In conclusion, as a suggestion of basic premise and direction in the implementation of telemedicine deregulation, the implementation of initiative measures to address issues concerning telemedicine, the review and regulation of conditions to be considered in the implementation of telemedicine, and the establishment of a close communication and cooperative sturcture with medical providers.

Systematic review for economic benefit of poison control center (중독관리센터의 경제적 효과에 대한 체계적 고찰)

  • Purpose: The purpose of this study was to conduct a systematic review to investigate the socio-economic benefits of the poison control center (PCC) and to assess whether telephone counseling at the poison control center affects the frequency of emergency room visits, hospitalization, and length of stay of patients with acute poisoning. Methods: The authors conducted a medical literature search of the PubMed, EMBASE, and Cochrane Library databases. Two reviewers evaluated the abstracts for eligibility, extracted the data, and assessed the study quality using a standardized tool. Key results such as the cost-benefit ratio, hospital stay days, unnecessary emergency room visits or hospitalizations, and reduced hospital charges were extracted from the studies. When meta-analysis was possible, it was performed using RevMan software (RevMan version 5.4). Results: Among 299 non-duplicated studies, 19 were relevant to the study questions. The cost-benefit ratios of PCC showed a wide range from 0.76 to 36 (average 6.8) according to the level of the medical expense of each country and whether the study included intentional poisoning. PCC reduced unnecessary visits to healthcare facilities. PCC consultation shortened the length of hospital stay by 1.82 (95% CI, 1.07-2.57) days. Conclusion: The systematic review and meta-analysis support the hypothesis that the PCC operation is cost-beneficial. However, when implementing the PCC concept in Korea in the future, it is necessary to prepare an institutional framework to ensure a costeffective model.

A Qualitative Study on Librarians' Recognition of the Joint Utilization of National Authority Data (국가전거데이터 공동활용에 대한 사서들의 인식에 관한 질적 탐구)

  • The purpose of this study was to conduct interviews with librarians who have experience in establishing local authority data by participating in the national authority sharing system of the National Library of Korea and to understand librarians' recognition and support for the joint utilization of national authority data. For this purpose, a total of 10 librarians who participated in the national authority sharing system project were interviewed by telephone using semi-structured questionnaires. Through this, it was possible to investigate the benefits, difficulties, utilization plans, revision plans of headings, and opinions on necessary support. The results of this study showed that the participants recognized that the joint utilization of national authority data provides the basis for the authority work of the local library and brings about the efficiency of the authority work, but they recognized the difficulty of modifying, selecting, creating new data, lacking knowledge, and lacking support system. The necessary support for the joint utilization of national authority data was provided with education and manuals related to authority, provision of rules related to authority that fully consider the position of the institution, budget and manpower support for system development and maintenance, establishment of communication channel and council, system and data advancement, and incentive to participating libraries. Based on the results of the study, the method and direction for the future operation of the joint utilization of national authority data were presented.

Korean Medicine Doctor's Perception in Korean Medicine Clinical Practice Guideline and Korean Medicine Cliniical Pathways (한의 표준임상진료지침 및 한의 표준임상경로에 대한 한의사의 인식)

  • Objectives : This study is aimed to survey Korean Medicine Doctors's perceptions in Korean Medicine Clinical Practice Guideline and Standard Korean Medicine Clinical Pathways for its spread(to increase utilization in clinical sites). Methods : The research population was set at 14,831 Korean medical institutions registered with the Health Insurance Review and Assessment Service for sampling representative of Korean Medicine Doctors, and the final 2,007 subjects were selected in consideration of the research period and budget. This survey was conducted based on a telephone survey, and in some cases, a fax or e-mail survey was also conducted together. Six questions were asked about the perception of 'Korean Medicine Clinical Practice Guideline and Standard Korean Medicine Clinical Pathways', and three questions about the characteristics of the respondent. Results : The rate of recognizing Korean Medicine Clinical Practice Guideline was 36.1%. Those who worked at Korean medicine hospitals, were under 39 years of age, and had less than 11-20 years of experience as an Korean medical doctor were more likely to be aware of it. Regarding the experience of using Korean Medicine Clinical Practice Guideline, the rate of 'not used in the past and not used now' was very high at 82.9%, but the intention to use it in the future was high at 60.7%. About the Korean Medicine Clinical Pathways, 79.9% of respondents answered that they did not know. 80.6% of respondents recognized the need for the development of clinical manuals such as Korean Medicine Clinical Practice Guideline and Standard Korean Medicine Clinical Pathways. Conclusion : Their low awareness of Korean Medicine Clinical Practice Guideline seems to affect its low utilization. Therefore, active education and public relations are required in the future.

Clinical study on recurrent Bell's palsy: A retrospective observational study (벨 마비 재발 환자에 대한 임상적 고찰: 후향적 관찰연구)

  • Objectives: This study is designed to statistically analyze characteristics, treatment effect and prognosis of patients who were treated for the recurrent Bell's palsy Methods: This study retrospectively analyzed the medical records and telephone questionnaire of 82 patients treated with the recurrent Bell's palsy at the OO Korean Medicine hospital from August 01, 2018 to July 31, 2021. Results: The lower the EQ-VAS® and HB scale before and after treatment, the higher the satisfaction with the treatment. The average of the number of days and duration of outpatient treatment for male was longer than that for female. The average number of days and duration of outpatient treatment in the group that received combined treatment was longer than that of patients who received Korean Medicine only. The higher the satisfaction of treatment, the higher the willingness to receive treatment in the event of recurrent facial palsy. HB scale after treatment was the lowest in their 40s, and the average value of HB scale after treatment tended to increase as age increased except for those in their 40s. Conclusion: This study showed that the lower the HB scale after treatment, the lower the EQ-VAS®, the higher the treatment satisfaction, and the higher the probability receiving treatment if the facial paralysis recurs. Those in their 10s to 40s showed excellent results after treatment. Comparatively, those in their 50s and above showed a little effect after treatment. Gender, affected side, history of facial palsy, season of onset, age, hypertension and diabetes had no statistically significance with recurrent Bell's palsy prognosis.

  • Purpose: The study aimed to identify the stress levels and their solutions in MZ(Millennials + generation Z) nursing students by using photovoice through the medium of photography. Methods: The study was conducted from November to December 2022 among freshmen to senior nursing students at a major university. Participants were recruited by telephone, email, and social media. Originally, four participants were recruited for each year of nursing school, but it resulted in a total of 19 participants: three freshmen students, nine sophomore students, four junior students, and three senior students. The participants were asked to take pictures of things in their surroundings that bothered them and things that helped them overcome the bothering things and to have group discussions in groups of 4-5 to analyze the results. Results: The results of the study revealed the following themes: 'Difficulties and worries as a nursing student', 'Striving to go beyond yourself', 'Unhealthy factors', 'Ways to relieve stress', and 'Meaning of stress'. For each of the themes, there were five subcategories under 'difficulties and worries as a nursing student', four under 'fighting with oneself', seven under 'ways to relieve stress', five under 'unhealthy factors', and two under 'meaning of stress', resulting in 21 subcategories. Conclusion: Based on the results of the study, firstly, there is a need to educate university students about the factors that contribute to unhealthy behaviors and how to deal with stress in a healthy way. Secondly, it is necessary to provide programs that allow students to have various experiences.

The Effects of Online Coaching Program on Improving Eating Habit Behaviors (식습관 개선을 위한 온라인 코칭프로그램의 개발 및 효과성 검증)

  • The purpose of this study was to investigate the effects of the online coaching program on improving eating habit behaviors. The online coaching program was based on the model of habit behaviors change process(Tak et al., 2019) and the success factors on improving eating habit behaviors(Choi et al., 2020), and previous research showing the effects of habit change. Participants were 20 adults, selected through telephone interviews over the age of 19 in the metropolitan area. They were divided into the experiment and control groups, and participated one-on-one coaching program. Data of 17 people were used for the final analysis, and weight, dietary habits, eating behaviors, dietary self-efficacy, self-regulation, and life satisfaction were measured at two times(pre and post). Results showed that there were significant interaction effects between time interval and groups for all the dependent variables except self-regulation and life satisfaction. In other words, there were significant changes in weight, dietary habits, eating behaviors, dietary self-efficacy in the experimental group between time interval, whereas there were no significant changes for the control group. Based on these results, the significance and practical implications, limitations of this research, and suggestions for further research were discussed.

Incidence of postoperative pain after using single continuous, single reciprocating, and full sequence continuous rotary file system: a prospective randomized clinical trial

  • Background: Extrusion of debris is a major factor that results in postoperative pain during root canal treatment with various instruments and instrumentation techniques. Therefore, instrumentation techniques that extrude minimal debris into the periapical area while reducing pain are desirable. This study aimed to compare the incidence of postoperative pain and intake of analgesic medication (frequency and quantity) after endodontic treatment of mandibular posterior teeth using two single files and full-sequence continuous rotary systems with different kinematic motions. Methods: Thirty-five of 105 patients were assigned equally to three groups according to the instrumentation system used: ProTaper Next (PN) X2, 25/06 (Dentsply, Maillefer, Ballaigues, Switzerland), One Shape (OS), #0.25/06 (Micro Mega, Besancon, France), and Wave One Gold (WG), Red - #0.25, 0.07 (Dentsply, Maillefer, Ballaigues, Switzerland). Five specialists were included in this study design; each professional prepared 21 teeth, and randomly selected 7 per instrument system. The VAS sheet ranging from 0 to 10 was used to record the initial and postoperative pains at 24, 48, and 72 h, and 7th day after single visit endodontic treatment in mandibular premolars and molars with a diagnosis of asymptomatic irreversible pulpitis with or without apical periodontitis. Postoperatively, an analgesic, ibuprofen 400 mg was administered for intolerable pain at a dose of 1 tablet for 6 h. The patients were asked over the telephone regarding postoperative pain at intervals of 24, 48, and 72 h, and 7th day using a visual analogue scale. Result: There were no statistically significant differences among the PN, OS, and WG systems (P > 0.05) with regard to the incidence of postoperative pain at any of the four time points assessed. Conclusion: The intensity of postoperative pain, frequency, and analgesic intake were similar across all three types of instrument systems; however, the reciprocating single file (WG) was associated with less postoperative pain than the full sequence continuous rotary file.

In addition, researches for home appliance control and automobile control using smartphone are conducted. As such, smartphone users can enjoy a more convenient life, but by hacking smartphones, tapping texts and conversations on smartphones, tracking location through spy apps, DDoS attacks using smartphones, and malicious apps When a message is received at a specific telephone number when using a micropayment, the corresponding text message is transmitted to a remote server, thereby increasing the risk of leakage of personal information and the like. Therefore, in this paper, we define the risk factors of the smartphone that are caused by the internal and external environmental, physical, contents (apps) of the smartphone through the smartphone that we use in real life, We propose a method to check vulnerability of smartphone security solution such as CC evaluation and the most effective response technique for each risk of smartphone by defining the technique.
