The Comparison with Salivary Image and Saliva Clearance Rate in Salivary Scintigraphy (타액선 신티그라피 검사에서 타액선 영상과 타액 제거율의 비교 연구)

  • Purpose: The aim of study is to find a correlation between Salivary clearance rate using saliva and blood and Secretion rate and Excretion rate using Salivary gland Scan images. Materials and Methods: Salivary Scan and Stimulate clearance of $^{99m}Tc$-pertechnate was performed in 20 patients with moderate function(group 1), 9 patients with severe function glands (group 2), 3 patients with non function (group 3) and normal 6 controls. Salivay clearance rate was compare with Secretion rate and Excretion rate of Salivary glands' ROI. Result: Stimulate salivary clearance of normal controls was 18.4 ml/min, salivary clearance of group 1 was 10.1 ml/min, salivary clearance of group 2 was 10.4 ml/min and salivary clearance of group 3 was 2.3 ml/min. Significant difference was found between normal controls and group 2,3 (p<0.05, p<0.05). Secretion rate and Excretion rate of normal controls was 21.6%, 24.6%, Secretion rate and Excretion rate of group 1 was 17.6%, 24.0%, Secretion rate and Excretion rate of group 2 was 8.8%, 13.9% and Secretion rate and Excretion rate of group 3 was 5.6%, 2.9%. Significant difference was found between normal controls and group 2,3 (p<0.05, p<0.05). Conclusions: Stimulate salivary clearance using saliva and blood and Secretion rate and Excretion rate using Salivary gland Scan images accord well together.

Usefulness of New GAGG Scintillation Detector for Gamma Camera : A Monte Carlo Simulation Study (GAGG 섬광체 물질을 적용한 감마카메라 영상의 유용성 평가: 몬테카를로 시뮬레이션 연구)

  • In this study, we evaluated image quality for new Gadolinium Aluminum Gallium Garnet (GAGG) scintillation material based on the Geant4 Application for Tomographic Emission (GATE) simulation tool. The gamma camera detectors with GAGG and NaI scintillation were designed. In particular, we modeled modified body phantom by National Electrical Manufacturers Association International Electrotechnical Commission to evaluate the simulated images. To analysis the image performance, the contrast to noise ratio (CNR) and coefficient of variation (COV) were used by drawn the region of interests, respectively. Based on the CNR and COV results, the CNR value for GAGG material is higher approximately 17 % than NaI material. In addition, the COV value for GAGG material is lower approximately 17 % than NaI material. In conclusion, we confirmed the performnace of GAGG based gamma camera is useful to improve the image quality for the nuclear medicine instrumentation.

  • Juvenile polyposis is an uncommon condition characterized by the development of multiple juvenile polyps predominantly in the colon but also in the rest of the gastrointestinal tract. Patients with juvenile polyposis commonly present with rectal bleeding, diarrhea, abdominal pain, anemia, prolapse of the polyp. We experienced a juvenile polyposis in a 7 year-old male patient with protein losing enteropathy who was diagnosed by $^{99M}Tc$-human serum albumin abdominal scintigraphy, colonoscopy, and small bowel series. Proctocolectomy with ileostomy was performed and then protein losing enteropathy was resolved.

A Case of Crigler-Najjar Syndrome Type 2 Diagnosed Using Genetic Mutation Analysis (유전자 검사로 진단된 제2형 Crigler-Najjar 증후군 1예)

  • Crigler-Najjar syndrome is a rare inherited disease associated with unconjugated hyperbilirubinemia. It is inherited via an autosomal recessive pattern and is caused by mutation in one of the five exons of the bilirubin uridine-diphosphoglucuronate glucuronosyltransferase (UGT1A1) gene. The synthesis of inactive isoforms of bilirubin uridine-diphosphoglucuronate glucuronosyltransferase (B-UGT) results in unconjugated hyperbilirubinemia. A 13-year-old boy with jaundice for 4 months was admitted to our hospital. He had unconjugated hyperbilirubinemia with no evidence of infection, hemolysis, or structural abnormalities on abdominal ultrasonography or 99mTc-DISIDA scan. The authors identified a missense mutation of Tyr486Asp in the fifth exon of the UGT1A1 gene and diagnosed the patient with Crigler-Najjar syndrome type II. This is the first reported case of Crigler-Najjar syndrome in a Korean child, and it is also the first reported case of a genetic mutation leading to Crigler-Najjar syndrome in Korea.

  • A pulmonary thromboembolism (PTE), which is a sudden blockage in a pulmonary artery, usually due to a blood clot, is rare in children. The clinical presentation is often subtle or masked by the underlying clinical condition and the condition must be suspected during clinical testing. Although the choice of treatment depends on the clinical presentation, anticoagulation is the mainstay of therapy for children with PTE. We report the case of a healthy 1-month-old boy who presented with hemoptysis without hemodynamic instability. He was diagnosed based on chest computed tomography with angiography and 99mTc macroaggregated albumin lung perfusion scintigraphy and treated with low-molecular-weight heparin.

  • In order to evaluate the exposure to the radiologic technologists from patients who had been administrated with radiopharmaceuticals, we measured the spatial dose rates at $5{\sim}300\;cm$ from skin surface of patients using an proportional digital surveymeter, 1.5(PET scan) and 4hr(bone scan) after injection. In results, the exposure to the technologists in each procedure was small, compared with the dose limits of the medical workers. However, the dose-response relationships in cancer and hereditary effects, referred to as the stochastic effects, have been assumed linear and no threshold models ; therefore, the exposure should be minimized. For this purpose, the measurements of spatial dose rate distributions were thought to be useful.

Radionuclide Penogram in Diagnosis of Vasculogenic Impotence (혈관성 발기부전의 진단을 위한 방사성핵종 음경촬영술)

  • Yasculogenic impotence is produced by abnormalities of vascular blood supply or drainage, and is the most common cause of various organic impotences. An increasing awareness of vascular causes of impotence has resulted from development of various diagnostic tests, but precise measurement of penile blood flow is difficult. Radionuclide penogram has been introduced recently to diagnose vasculogenic impotence. Forty-one impotent patients and 12 normal men were studied with radionuclide erection penogram using Tc-99m pertechnetate and an intracavernous injection of papaverine. We defined arteriogenic impotence as arterial index less than 0.66, and venogenic impotence as venous index greater than 0.09. By this criteria, the false positive ratio in normal men was 17%, and the false negative ratio in radically cystectomized patients was 0%. Side effects were small purpura of the penile shaft and dull pain during injection of papaverine. The radionuclide erection penogram was noninvasive and gave a dynamic evaluation of the arterial supply, venous drainage, and blood flow in the corporeal bodies. This method should be considered as a valuable adjunct to evaluate patients with vasculogenic impotence.

Spect Assessment of Regional Cerebral Perfusion Abnormality in Head Injury (두부외상 환자에서 HMPAO-SPECT를 이용한 국소 뇌혈류 변화의 평가)

  • Patterns of abnormality in regional cerebral perfusion and its relation to clinical severity was evaluated with 32 head injury patients using $^{99m}Tc-HMPAO$ single photon emission tomography (SPECT). The findings were compared with computed tomography (CT) done within 48 hours of each SPECT study. The initial SPECT study was done within 7 days of injury in 16 cases, between 1 week and 2 months in 12, and after over 2 months in 4. Nineteen of the patients underwent followup SPECT and CT after a mean interval of 1 to 2 months. The initial SPECT showed abnormalities in 96% (31/32) of the patients while CT showed abnormal findings in only 81% (26/32). There were a total of 54 supratentorial SPECT lesions in all. Ninity percent (49/54) of these were of regional hypoperfusion, while 5 lesions showed focal hyperperfusion. The lesions were most often localized in the frontal and temporal lobes. Fifty five percent (30/54) were areas not detected as a lesion on CT. Cerebellar diaschisis was observed in 50% (16/32) of the patients. The degree of perfusion abnormality was quantified by the product of differential activity and a size factor. Correlation between the degree of perfusion abnormality and the clinical severity (Glasgow coma scale) failed to show statistical significance (p=0.053). The amount of change in the degree of perfusion abnormality on follow up SPECT was compared to the amount of change in clinical severity. Perfusion abnormality showed a tendancy to improve in most patients, and the degree of improvement showed significant correlation with the amount of clinical improvement (p < 0.01).

Early Detection of Bone Metastasis in Malignancy With whole Body Bone Scan (전신골격주사(全身骨格走査)를 이용(利用)한 골전이(骨轉移)의 조기발견(早期發見))

  • Bone scans with $^{99m}Tc$-MDP (methylene diphosphonate) was analysed in 112 patients with various type of biopsy proven malignant tumor who visited Seoul National University Hospital from March 1979 to August 1979. The results were as follows; 1. Of the 112 cases, bone scans were positive in 61 cases (54.1%), while only 27 cases (24.1%) were positive in roentgenogram. 2. Of the 61 cases with positive bone scan, bone metastases were found in 25 cases by roentgenogram. Of the 51 cases with negative bone scan, bone metastasis was found in only 1 case by roentgenogram. 3. In comparison of bone scan and roentgenogram by number of regions, 137 regions were positive in bone scan or roentgenogram. Of these, 46 regions (33.5%) were both bone scan and roentgenogram positive, 89 regions (65.0%) were bone scan positive roentgenogram negative, and 2 regions were bone scan negative roentgenogram positive. 4. Bone scan and roentgenographic findings had significant correlation with the presence of bone pain but no significant correlation with the elevated level of serum alkaline phosphatase, acid phosphatase, Ca, and P. From above result, we found that bone scan was more sensitive than roentgenogram in early detection of bone metastasis in malignant tumors.

Radionuclide Peritoneal Scintigraphy in Patients with Ascites and Pleural Effusion (방사성핵종 복막촬영술을 이용한 복수에 동반된 수흉의 감별 진단)

  • Simultaneous presence of ascites and pleural effusion has been documented in patients with cirrhosis of the liver, renal disease, Meigs' syndrome and in patients undergoing peritoneal dialysis. Mechanisms proposed in the formation of pleural effusion in most of the above diseases are lymphatic drainage and diaphragmatic defect. But sometimes, hepatic hydrothoraxes in the absence of clinical ascites and pleural effusion secondary to pulmonary or cardiac disease are noted. It is not always possible to differentiate between pleural effusion caused by transdiaphragmatic migration of ascites and by other causes based soly on biochemical analysis. Authors performed radionuclide scintigraphy after intraperitoneal administration of $^{99m}Tc-labeled$ colloid in 23 patients with both ascites and pleural effusion in order to discriminate causative mechanisms responsible for pleural effusion. Scintigraphy demonstrated the transdiaphragmatic flow of fluid from the peritoneum to pleural cavities in 13 patients correctly. In contrast, in 5 patients with pleural effusion secondary to pulmonary, pleural and cardiac diseases, radiotracers fail to traverse the diaphragm and localize in the pleural space. Ascites draining to mediastinal lymph nodes and blocked passage of lymphatic drainage were also clarified, additionaly. Conclusively, radionuclide peritoneal scintigraphy is an accurate, rapid and easy diagnostic tool in patients with both ascites and pleural effusion. It enables the causes of pleural effusion to be elucidated, as well as providing valuable information required when determining the appropriate therapy.
