In this study, we investigate the use of a negative stiffness tuned inerter damper system to improve the performance of a base-isolated structure. The negative stiffness tuned inerter damper system consists of a tuned inerter damper connected in parallel with a negative stiffness element. To find the optimal parameters for the base-isolated structure with negative stiffness tuned inerter damper system, we develop an optimization method based on performance criteria. The objective of the optimization is to minimize the superstructure acceleration response ratio, while ensuring that the base displacement response ratio remains below a specified target value. We evaluate the proposed method by conducting numerical analyses on an eight-story building. The structure is modeled using both a simplified 3-degree-of-freedom system and a more detailed story-by-story shear-beam model. Lastly, a comparative analysis using time history analysis is performed to compare the performance of the base-isolated structure with negative stiffness tuned inerter damper system with that of the base-isolated structure and base-isolated structure with tuned inerter damper systems. The results obtained from the comparative analysis show that the negative stiffness tuned inerter damper system outperforms the tuned inerter damper system in reducing the dynamic seismic response of the base-isolated structure. Overall, this study demonstrates that the negative stiffness tuned inerter damper system can effectively enhance the performance of base-isolated structures, providing improved seismic response reduction compared to other systems.

In this study, we investigated students' responses to a discrepant event and an alternative hypothesis which had been presented in the conceptual change processes from the phlogiston theory to the oxygen theory, and compared them with scientists' responses. The data concerning scientists' responses to the discrepant event and the alternative hypothesis were gathered from the relevant literature on the history of science. Subjects were 148 eighth graders who possessed the target misconception about combustion through a preconception test. After having been presented with the discrepant event and the alternative hypothesis, students were asked to respond to the test of response to discrepant event. Although similar types of responses were obtained from both scientists and students, there was also a clear difference. Scientists tended to focus on explaining the problems of the discrepant event, whereas students tended to ignore and/or exclude the discrepant event in order to maintain their previous beliefs. Only a few students were also found to change their beliefs after having been presented with the alternative hypothesis.

This study presents code-compliant seismic details by addressing dry mechanical splices for precast concrete (PC) beam-column connections in the ACI 318-19 code. To this end, critical observations of previous test results on precast beam-column connection specimens with the proposed seismic detail are briefly reported in this study, along with a typical reinforced concrete (RC) monolithic connection. On this basis, nonlinear dynamic models were developed to verify seismic responses of the PC emulative moment-resisting frame systems. As the current design code allows only the emulative design approach, this study aims at identifying the seismic performances of PC moment frame systems depending on their emulative levels, for which two extreme cases were intentionally chosen as the non-emulative (unbonded self-centering with marginal energy dissipation) and fully-emulative connection details. Their corresponding hysteresis models were set by using commercial finite element analysis software. According to the current seismic design provisions, a typical five-story building was designed as a target PC building. Subsequently, nonlinear dynamic time history analyses were performed with seven ground motions to investigate the impact of emulation level or hysteresis models (i.e., energy dissipation performance) on system responses between the emulative and non-emulative PC moment frames. The analytical results showed that both the base shear and story drift ratio were substantially reduced in the emulative system compared to that of the non-emulative one, and it indicates the importance of the code-compliant (i.e., emulative) connection details on the seismic performance of the precast building.
Objectives: Cognitive impairments, ranging from mild to severe, adversely affect daily functioning, quality of life, and work capacity. Despite significant efforts in the past decade, more than 200 promising drug candidates have failed in clinical trials. Herbal remedies are gaining interest as potential treatments for dementia due to their long history and safety, making them valuable for drug development. This review aimed to examine the mechanisms behind the effect of Polygonum multiflorum on cognitive function. Methods: This study focused primarily on the effects of Polygonum multiflorum and its chemical constituents on cognitive behavioral outcomes including the Morris water maze, the passive avoidance test, and the Y maze, as well as pathogenic targets of cognitive impairment and Alzheimer's disease (AD) like amyloid deposition, amyloid precursor protein, tau hyperphosphorylation, and cognitive decline. Additionally, a thorough evaluation of the mechanisms behind Polygonum multiflorum's impact on cognitive function was conducted. We reviewed the most recent data from preclinical research done on experimental models, particularly looking at Polygonum multiflorum's effects on cognitive decline and AD. Results: According to recent research, Poligonum multiflorum and its bioactive components, stilbene, and emodin, influence cognitive behavioral results and regulate the pathological target of cognitive impairment and AD. Their mechanisms of action include reducing oxidative and mitochondrial damage, regulating neuroinflammation, halting apoptosis, and promoting increased neurogenesis and synaptogenesis. Conclusion: This review serves as a comprehensive compilation of current experiments on AD and other cognitive impairment models related to the therapeutic effects of Polygonum multiflorum. We believe that these findings can serve as a basis for future clinical trials and have potential applications in the treatment of human neurological disorders.
Advancements of medical care with the use of new medical technologies have been promoting public health, but they can also place a financial burden on individuals and the country. Since the introduction of the health insurance system in 1977, the South Korean government has been attempting to control medical expenses. Radiological examinations have been a major target for cost reduction. Meanwhile, the radiological report fee has been combined with the technical fee for a long time; hence, its value has not been independently evaluated, and was also decreased proportionally with examination cost reduction. However, in the revised medical fee estimation system for MRI, the report fee has begun to be calculated separately from the technical fee, significantly contributing to the evaluation of the radiologists' work value. In conclusion, radiologists need to know and actively participate in the process of determining the insurance cost of radiological examinations and reports to get a proper value of their professionalism.

Adequate nutritional support is crucial in preventing complications and improving outcomes in critically ill patients. Extracorporeal membrane oxygenation (ECMO) is a mode of supportive care for patients with respiratory and/or cardiac failure. ECMO patients frequently exhibit a hypermetabolic state characterized by protein catabolism and insulin resistance, which can lead to malnutrition. Nutritional therapy is a vital component of intensive care, but its optimal administration for ECMO patients is unknown. This case report aims to provide insights into effective nutritional management for critically ill patients undergoing ECMO therapy. The patient was a 72-year-old male with a history of gastric and lung cancer who underwent a lobectomy complicated by bronchopleural fistula, postoperative bleeding, pneumonia, and acute respiratory distress syndrome (ARDS). The patient's nutritional status was assessed indicating a high risk of malnutrition, using the modified Nutrition Risk in the Critically Ill (mNUTRIC) Score. Nutritional support was administered based on the recommendations of European Society for Clinical Nutrition and Metabolism (ESPEN) and the American Society for Parenteral and Enteral Nutrition (ASPEN), with energy requirements set at 25-30 kcal/kg/d and protein requirements set at 1.2-2.0 g/kg/day. The patient received parenteral nutrition until the enteral nutrition target amount was reached, with zinc supplements for wound healing. The study highlights the need for further research on proactive and effective nutritional support for ECMO patients to improve compliance and prognosis.

In contrast to chemical medicines, biopharmaceuticals exhibit reduced side effects and enhanced therapeutic efficacy. Antibody therapies have significantly advanced since the first monoclonal antibody's approval in 1986, now dominating the pharmaceutical market with seven out of the top 10 biopharmaceuticals. The bispecific antibody has a distinct capability to bind to two antigens simultaneously, unlike conventional monoclonal antibodies that target just one antigen. The notion of bispecific antibodies was initially introduced in 1960, and by 1997, the first symmetrical form of bispecific antibody was successfully produced. Subsequently, extensive research has been conducted on bispecific antibodies, leading to a significant milestone in 2014 when blinatumomab became the first FDA-approved drug to treat acute lymphocytic leukemia. Despite having a relatively shorter history compared to monoclonal antibodies, bispecific antibodies have proven their potential by targeting two antigens simultaneously, thereby rendering them highly effective as anti-cancer drugs. As of 2023, there are a total of 11 globally approved bispecific antibodies, with six of them receiving approval from FDA. In light of the rapidly expanding market for bispecific antibodies, this review article comprehensively explores the attributes, historical background, applications, and market status of bispecific antibodies. Additionally, it sheds light on the present trends in bispecific antibody development, drawing insights from 96 research articles and 105 clinical studies. Excitingly, we anticipate further progress in the development of bispecific antibodies and clinical trials on a global scale, with the aspiration of utilizing them not only in cancer treatment but also for addressing diverse medical conditions.
Background/Aims: This study aimed to investigate the timing and predictors of death during treatment among patients with multidrug/rifampin-resistant tuberculosis (MDR/RR-TB) in South Korea. Methods: This was a retrospective cohort study that included MDR/RR-TB cases notified between 2011 and 2017 in South Korea. Results: Among 7,226 MDR/RR-TB cases, 699 (9.7%) died at a median of 167 days (IQR 51-358 d) from the initiation of MDR-TB treatment. The cumulative proportion of all-cause death was 35.5% at 90 days and 52.8% at 180 days from treatment initiation. TB-related deaths occurred at a median of 133 days (IQR 32-366 d), which was significantly earlier than the median of 184 days (IQR 68-356 d) for non-TB-related deaths (p = 0.002). In a multivariate analysis, older age was the factor most strongly associated with death, with those aged ≥ 75 years being 68 times more likely to die (aHR 68.11, 95% CI 21.75-213.26), compared those aged ≤ 24 years. In addition, male sex, comorbidities (cancer, human immunodeficiency virus, and end stage renal disease), the lowest household income class, and TB-specific factors (previous history of TB treatment, smear positivity, and fluoroquinolone resistance) were identified as independent predictors of all-cause death. Conclusions: This nationwide study highlights increased deaths during the intensive phase and identifies high-risk groups including older people and those with comorbidities or socioeconomic vulnerabilities. An integrated and comprehensive strategy is required to reduce mortality in patients with MDR/RR-TB, particularly focusing on the early stages of treatment and target populations.
The high-speed railway system is mainly composed of tunnel, bridge, and viaduct to meet the straightness needed for keeping the high speed up to 400 km/s. Seismic fragility for the high-speed railway infrastructure can be assessed as two ways: one way is studying each element of infrastructure analytically or numerically, but it requires lots of research efforts due to wide range of railway system. On the other hand, empirical method can be used to access the fragility of an entire system efficiently, which requires case history data. In this study, we collect the 2004 MW 6.6 Niigata earthquake case history data to develop empirical seismic fragility function for a railway system. Five types of intensity measures (IMs) and damage levels are assigned to all segments of target system for which the unit length is 200 m. From statistical analysis, probability of exceedance for a certain damage level (DL) is calculated as a function of IM. For those probability data points, log-normal CDF is fitted using MLE method, which forms fragility function for each damage level of exceedance. Evaluating fragility functions calculated, we observe that T=3.0 spectral acceleration (SAT3.0) is superior to other IMs, which has lower standard deviation of log-normal CDF and low error of the fit. This indicates that long-period ground motion has more impacts on railway infrastructure system such as tunnel and bridge. It is observed that when SAT3.0 = 0.1 g, P(DL>1) = 2%, and SAT3.0 = 0.2 g, P(DL>1) = 23.9%.
