Chest Wall Tuberculosis: Clinical Features and Treatment Outcomes

  • Background: A diagnosis and treatment of chest wall tuberculosis (CWTB) is both difficult and controversial. The aim of this study was to collect information on the optimal treatment for CWTB. Methods: The clinical features, radiographic findings, and treatment outcomes of 26 patients, who underwent surgery and were diagnosed histopathologically, were retrospectively analyzed. Results: The most common presenting symptom was a palpable mass found in 24 patients (92.3%). In all patients, CT revealed a soft tissue mass that was accompanied by a central low density, with or without peripheral rim enhancement. The sensitivity and specificity of the bone scintigram for bone involvement were 87.5% and 100%, respectively. CWTB was diagnosed preoperatively by aspiration cytology and smear for acid-fast bacilli in five out of 11 patients. Twenty-three patients (88.5%) underwent a radical excision and three underwent incision/drainage or an incisional biopsy. The duration of antituberculous medication was 7.5${\pm}$3.98 months with a follow-up period of 28.2${\pm}$26.74 months. Among the 20 patients who completed their treatment, nine received chemotherapy for six months or less and 11 received chemotherapy for nine months or more. Two patients had a recurrence four and seven months after starting their medication. Conclusion: A 6 month regimen may be appropriate for CWTB patients who have undergone a complete excision.

Traumatic Hemothorax Caused by Thoracic Wall and Intrathoracic Injuries: Clinical Outcomes of Transcatheter Systemic Artery Embolization (흉벽 및 흉곽 내 장기 손상으로 인한 외상성 혈흉: 전신 동맥 색전술의 임상 결과)

  • Purpose We aimed to assess the clinical efficacy of transcatheter arterial embolization (TAE) for treating hemothorax caused by chest trauma. Materials and Methods Between 2015 and 2019, 68 patients (56 male; mean age, 58.2 years) were transferred to our interventional unit for selective TAE to treat thoracic bleeding. We retrospectively investigated their demographics, angiographic findings, embolization techniques, technical and clinical success rates, and complications. Results Bleeding occurred mostly from the intercostal arteries (50%) and the internal mammary arteries (29.5%). Except one patient, TAE achieved technical success, defined as the immediate cessation of bleeding, in all the other patients. Four patients successfully underwent repeated TAE for delayed bleeding or increasing hematoma after the initial TAE. The clinical success rate, defined as no need for thoracotomy for hemostasis after TAE, was 92.6%. Five patients underwent post-embolization thoracotomy for hemostasis. No patient developed major TAE-related complications, such as cerebral infarction or quadriplegia. Conclusion TAE is a safe, effective and minimally invasive method for controlling thoracic wall and intrathoracic systemic arterial hemorrhage after thoracic trauma. TAE may be considered for patients with hemothorax without other concomitant injuries which require emergency surgery, or those who undergoing emergency TAE for abdominal or pelvic hemostasis.

  • Although gastric cancer is quite common in Korea, the treatment outcome is relatively favorable compared to those in western countries. However, there are currently no Korean multidisciplinary guidelines for gastric cancer. Experts from related societies developed guidelines de novo to meet Korean circumstances and requirements, including 23 recommendation statements for diagnosis (n=9) and treatment (n=14) based on relevant key questions. The quality of the evidence was rated according to the GRADE evidence evaluation framework: the evidence levels were based on a systematic review of the literature, and the recommendation grades were classified as either strong or weak. The applicability of the guidelines was considered to meet patients' view and preferences in the context of Korea. The topics of the guidelines cover diagnostic modalities (endoscopy, endoscopic ultrasound, and radiologic diagnosis), treatment modalities (surgery, therapeutic endoscopy, chemotherapy, and radiotherapy), and pathologic evaluation. An external review of the guidelines was conducted during the finalization phase.

  • Objectives: This study investigated quality among three herb medicine extract granules(DSGOST) which were made from different companies to check quality control of herb medicine extract granules. Methods: we selected three DSGOST extract granules which were made from different companies. And we experimented extract granules by method from K.P(Korean Pharmacopoeia), K.H.P(Korean Herbal Pharmacopoeia) of KFDA. Results: In qualitative analysis of DSGOST, we indentified Akebiae Caulis (木通), Asari Herba Cum Radix (細辛), Evodiae Fructus (吳茱萸) in three different DSGOST extract granules. In quantitative analysis of DSGOST, Medication A,B,C contained similar content of Paeoniflorin & Glycyrrhizic acid. However Medication B contains especially lowest value of Cinnamic acid & total Decursin. Conclusions: Herb medicine extract granules have different contents of ingredients although those were made by same prescription. And these differences may influence medicinal effect to patients. So we need to make system of quality control with various research of quantitative & qualitative analysis about herb medicine extract granules.

  • The purpose of this study is to review the literature on Danggwisayeokgaohsuyusaenggang-tang. The classic literature review was carried out using "http://www.theqi.com". The journal search was performed using MEDLINE, EMBASE, National Digital Science Links(NDSL), OASIS, Korean studies Information Service system(KISS), RISS, China National Knowledge Infrastructure(CNKI), Japan Science and Technology information Aggreator, Electronic(J-STAGE) from search engine's opening day to November 2013. Searching key words were the various combination of "Danggwisayeokgaohsuyusaenggang-tang", "Danggwisayeok", "Danggwi", "ohsuyu", "saenggang". The inclusion criteria was all kinds of journals except for review, essay, and experiment study. 40 classic literatures and 37 journals(1 chinese randomized controlled trial, 25 chinese case studies, 1 Korean case study, and 10 japanese case studies) were selected finally. The literatures and journals reported that Danggwisayeokgaohsuyusaenggang - tang was used to treat diverse types of diseases with coldness of hands and feet.

Relationship Between Soil Properties and Tip Burn of Chinese Chive Cultivated in Plastic Film House (시설재배 부추 잎끝마름증 발생에 영향을 미치는 토양특성)

  • Tip burn has been reported as one of the most serious physiological disorder in Chinese chives (Allium tuberosum Rottl.) cultivated in plastic film house. In this study, a physiography and chemical properties of 132 plastic film house soils were investigated to elucidate factors affecting tip burn symptom. Also influence of soil properties on tip burn was statistically determined by path analysis and association analysis including a chi-square test or logistics analysis. Probability distribution of inorganic aqueous species, such as ammonia (g) was calculated using MINTEQ program. Soil order and chemical properties, especially pH, exchangeable calcium and inorganic nitrogen, showed a significant relationship with tip burn of Chinese chives. Tip burn symptoms occur mainly in an alkaline soil classified as Alfisols. Result of linear regression and path analysis exhibited that formation of ammonia (g) from soil solution depend upon soil pH and were associated with ammonium resulting from soil organic matter or nitrate. These results indicate that tip burn symptom of Chinese chives is directly affected by ammonia gas originated from alkaline soil condition.

Regulatory Effect of Scutellariae Radix on the Proinflammatory Cytokine Production and Abnormal T-Cell Activation in Vitro in Pristane-Induced Lupus Mice

  • Scutellaria baicalensis is known as a herbal medicine with anti-inflammatory and anti-oxidative activities. However, effect of Scutellaria baicalensis on lupus pathogenesis that is characterized by overproduction of proinflammatory cytokines and abnormalities in regulation, function, and interaction of immune cells remains unclear. We investigated effects of Scutellariae radix methanol extract (SBMeOH) on the production of proinflammatory cytokines and abnormal activation of T cells in vitro in pristane-induced lupus BALB/c mice. These results demonstrated that SBMeOH significantly decreased the LPS-stimulated production of $TNF-{\alpha}$, IL-6, and IL-10 by splenic and peritoneal macrophages and IL-6 and IL-10 by splenocytes from pristane-induced lupus mice. SBMeOH significantly downregulated the Con A-stimulated overproduction of IL-6, IL-10, and $IFN-{\gamma}$ by splenocytes from pristane-induced lupus mice. Also, SBMeOH significantly attenuated the Con A-induced expression of CD4+ T cells and CD69+CD4+ T cells but not CD8+ T cells in pristane-induced lupus mice. Our findings indicate that SBMeOH may ameliorate lupus pathogenic inflammation and autoimmunity via downregulation of proinflammatory cytokine production and abnormal activation of T cells.

Removing Malodor Using Photocatalyst and Infrared (광촉매와 적외선을 이용한 악취저감)

  • Recently, people interest in environmental pollution and attempt to improve the indoor air quality contaminated with various pollutants since it is very important to construct healthy and comfortable living environment. In the current study, we used the technology that has first received the certification of green technology for improving the removal efficiency of malodor causing substances. This green technology is a new technology to increase the reactivity of the odorous substances with OH radicals for oxidation reaction by using an infrared lamp in the existing air purification system. Comparing the efficiency of the green technology with the infrared lamp to that of the existing technology of air cleaner, there was a difference in the decomposition efficiency depending on the initial concentrations and speciation of the odorous substances. The removal efficiencies of contaminants were enhanced by 16.9 and 13.2% at low and high concentrations, respectively.

  • The objective of this study is to offer some standards for the classification of Sasang Constitutions according to age groups through analyzing the characteristics of their body shape and comparing with their frontal silhouettes. The subject of this study were 192 female and 167 male patients who aged from 17 to 64 in Pusan. They were treated with Sasang Constitutional medicine. 5 Heights, 7 widths of their body were measured with modified IBS-2000. Collected 12 anthropometric data was analyzed by T-test and ANOVA. Width is more suitable than height in estimating Sasang Constitutions regardless of gender differences. W2_1, W2_2, W2_4, W2_5, W2_51 are suitable to estimate Sasang Constitutions regardless of gender differences and age groups. in case of female, H45 in 33${\sim}$64 age group is suitable to estimate Sasang Constitutions. In case of male, H12, W2_1 in 17${\sim}$32 age group and H01, W2_0, H23 in 33${\sim}$64 age group are suitable to estimate Sasang Constitutions. All Sasang Constitutions with the exception of Taeyangin female have to consider age groups, when we estimate Sasang Constitutions. From the above results, we have to consider not only gender differences but also age groups to classify Sasang Constitutions from body shape point of view.

  • The government wants to apply the construction cost estimating method based on historical data published in the first six months of 2004. Construction companies, however, require the proposed cost estimation model, to be improved which makes it difficult to predict a reasonable construction costs. This paper presents an improved historical data selection model after analyzing the problem of previous method throughout comparing contracted unit prices of reinforced concrete works selected by the previous model to market prices. The model which can select more feasible data would assist participates such as general contractors and sub-contractors to earn a proper profits.
