Hypogonadal men with moderate-to-severe lower urinary tract symptoms have a more severe cardiometabolic risk profile and benefit more from testosterone therapy than men with mild lower urinary tract symptoms

  • Purpose: To analyze data from an observational, prospective, cumulative registry study in 805 hypogonadal men stratified by mild or moderate-to-severe lower urinary tract symptoms (LUTS) according to International Prostate Symptom Score. Materials and Methods: A total of 412 men underwent testosterone therapy (TTh) with injectable testosterone undecanoate, 393 men served as untreated controls. Measures of urinary function, anthropometric and metabolic parameters were performed at least twice per year. Results: Data from 615 men with mild LUTS (253 treated, 362 untreated) and 190 with moderate-to-severe LUTS (159 treated, 31 untreated) were available. During a follow-up period of 8 years a significant improvement of LUTS was noted for all TTh-patients whereas the control-groups showed deterioration or fluctuation around initial values. Despite advancing age, TTh fully prevented worsening of symptoms. In parallel, a considerable improvement of anthropometric parameters, lipids and glycemic control, blood pressure, C-reactive protein, and quality of life was found. Moderate-to-severe LUTS was associated with worse cardiometabolic risk profile at baseline as well as worse cardiovascular outcomes during follow-up in comparison to mild LUTS. Effect size of TTh was more pronounced in men with moderate-to-severe than with mild LUTS. Conclusions: Correcting hypogonadism by TTh is highly effective and safe for improving LUTS in hypogonadal men. TTh may also improve cardiometabolic risk and major adverse cardiovascular events.

A Literature Review of Clinical Studies on Korean Medicine Treatment on Tension-type Headache: Focused on Domestic Case Reports (긴장형 두통의 한의학적 치료에 대한 임상연구 동향 분석 : 국내 증례보고를 중심으로)

  • Objectives : This study was conducted to analyze the Korean Medicine treatment on Tension-type Headache (TTH) in Korean clinical studies. Methods : Articles were searched from four Korean databases(KISS, RISS, OASIS, ScienceON). Study type, publication year, demographic information of participants, intervention type and details, outcome measurements and treatment results of selected articles were analyzed. Results : 17 articles were selected among those published until January 15th 2023. 'Cheonghyulgangki-tang', acupuncture at 'GV20', moxibustion at 'CV12', pharmacopunture at 'GB20', 'ByeolGab' were most frequently used in treatment on TTH. 'Numeric Rating Scale(NRS)' and 'Visual Analog Scale(VAS)' were most frequently used in outcome measurements. All of the articles exhibited that Korean Medicine treatments improved symptoms of TTH. Conclusion : This study showed that various Korean Medicine treatments for TTH were effective. This finding could be used in clinical practice and build the basis for the direction of clinical practice guidelines in future, Nevertheless, this study has few limitations. Further systematic reviews and meta analyses of randomized controlled trials are required for more evidence on Korean Medicine.

A Review of Chuna Manual Therapy for Tension Type Headache: Focusing on Pubmed and Korean Literature (긴장성 두통의 추나치료에 대한 임상 고찰: Pubmed와 국내문헌 중심으로)

  • Objectives : The purpose of this study was to review the effectiveness of Chuna manual therapy for the treatment of tension type headache(TTH). Methods : All processes were independently carried out by three investigators. Literature search was performed in 3 databases(pubmed, OASIS, NDSL) from their inception to May 2016. Searched reports was twice excluded for title, abstract and body. And then, data extract and analysis was done before assessing risk of bias by Cochrane Handbook. Results : 11 RCT were included. Generally, Fascia Chuna therapy and Chuna spine & joint manipulation therapy were used for TTH. Except for 1 report, Chuna manual therapy was shown to be effective in treating TTH. In assessing risk of bias, because of the characteristic of intervention, blinding of participants was high risk of bias in most reports. Conclusions : Chuna manual therapy was shown to be effective in treating TTH. In korea, better designed trials with high quality is needed from now on.

The Effect of the Pain on the TMJ and Masticatory Muscles to Tension-type Headache (측두하악관절과 저작근의 통증이 긴장성 두통에 미치는 영향)

  • Generally, Tension-Type Headache(TTH) patients exhibit muscle pain, but can also have TMJ pain, which includes mouth opening limitation or joint sounds. The purpose of our study is to observe the clinical pain characteristics between TTH patients with muscle pain and TMJ pain. One hundred sixty-seven patients were diagnosed with TTH according to the questionnaires based on the International Headache Society's proposal on the diagnostic criteria of TTH. The patients were classified into three group; arthralgia group (18 patients), myalgia group (50 patients) and arthromyalgia group (99 patients). TTH patients with pericranial muscle pain were classified in the myalgia group. TTH patients with temporal region pain were classified in the arthralgia group. TTH patients with both types of pain were classified in the arthromyalgia group. The parameters in the diagnostic criteria such as quality, intensity, laterality of pain, and aggravation due to physical activities were compared among the three groups. 1. There were no significant differences in the quality of pain among the three groups. 2. There were no significant differences in the intensity of pain among the three groups. 3. There were no significant differences in the laterality of pain among the three groups. 4. A higher percentage of patients in the arthromyalgia group experienced headaches that were aggravated due to physical activity (p=0.03) compared to the other groups. The results of this study show that TTH patients with both arthralgia (TMJ pain) and myalgia (pericranial muscle pain) are more aggravated by physical activity than TTH patients with either one.

Interrelationship of Tension-type Headache and Oral Parafunction (긴장성 두통과 이갈이 습관의 상호관계)

  • This study was designed to confirm the association between the pain of Tension-Type Headache(TTH) patients and their Oral Parafunction(OPF). Patients with TTH(n=58) visited the Department of Oral Medicine, Kyung Hee University Dental Hospital during two months were recruited to this study. 20 patients with OPF(bruxism, clenching, bruxism & clenching) are the experimental group and 38 patients without OPF are the control group. Both groups were asked to answer the questionnaire based on the diagnostic criteria of TTH(IHS). Then it was analyzed statistically. This study was designed to confirm the association between the pain of Tension-Type Headache(TTH) patients and their Oral Parafunction(OPF). Patients with TTH(n=58) visited the Department of Oral Medicine, Kyung Hee University Dental Hospital during two months were recruited to this study. 20 patients with OPF(bruxism, clenching, bruxism & clenching) are the experimental group and 38 patients without OPF are the control group. There was no difference in sex(p=0.91) and age(p=0.73) between two groups. 2. In the experimental group, dull pain was presented more frequently than in the control group. But, there was no difference between two groups(p=0.69). 3. In the experimental group, severe pain was presented more frequently than in the control group. But, there was no difference between two groups(p=0.40). 4. In the experimental group, pain shown bilaterally was presented more frequently than in the control group. But, there was no difference between two groups(p=0.52). 5. In the experimental group, pain was more increased by physical activities than in the control group. But, there was no difference between both groups(p=0.74). The pain of TTH patients with OPF was presented to be non-dull pain frequently and more bilaterally and severely, also increased more by physical activities than the pain of TTH patients without OPF. But, there was no significant difference between two groups. Therefore, it is considered that the pain of TTH is not influenced by OPF.

  • Dinuclear Ni(II)-thiophene halides, which contain linear bridging thienylenes, trans,trans-[$(PR_3)_2$(X)Ni-Y-Ni(X)$(PR_3)_2$] {X = Cl, Br; $H_2Y$ = 5,5'-dichloro-2,2'-bithiophene ($H_2bth$); $H_2tth$ = 5,5"-dichloro-2,2':5',2''-terthiophene ($H_2tth$)} were prepared by the oxidative addition of dihalobithiophene ($H_2bth$) or dihaloterthiophene ($H_2tth$) to [$Ni(COD)_2$] in the presence of tertiary phosphines. Subsequent reactions of $NaN_3$ with the dinuclear Ni(II)-thiophene chlorides gave the corresponding Ni(II)-azido complexes, trans,trans-[$(PR_3)_2(N_3)$Ni-Y-Ni$(N_3)(PR_3)_2$], whose reactivity toward trimethylsilyl pseudohalides such as trimethylsilyl isothiocyanates and cyanides was investigated. In addition, the reaction of trans-[$BrNi(PEt_3)_2-C_4H_2S-C_4H_2S$-CHO], a thienyl Ni(II) complex containing a terminal aldehyde group, with phosphonium ylide was examined.

Pattern of Pain on Temporomandibular Joint-Muscle Area in Tension-type Headache Patients (긴장성 두통 환자에서의 측두하악부 관절-근육통의 양상)

  • Although most cases of tension-type headache(TTH) are myogenic headache, in some cases, arthralgia appears, such as the disorder of the temporamandibular joint. This study is designed to compare the clinical patterns of arthralgia to those of myalgia, when both symptoms are co-existing. Among 167 patients who visit our clinic during a certain period, whose chief complaint was TTH, 18 patients were the arthralgia group, 50 patients were the myalgia group, and 99 patients who have both arthralgia and myalgia were the arthromyalgia group. Three groups were asked to answer the questionnaires about their age, gender, and oral parafunction such as bruxism or clenching. Then we gathered statistics on the data from the accomplished questionnaires. 1. There was statistical significance in age among the three groups(p=0.02). 2. There was no statistical significance in sex and oral parafunction among the three groups. 3. There was statistical significance the myalgia group was older than the arthromyalgia group(p<0.03). The results of the study show that as patients become older, TTH with myalgia happens more frequently than TTH with arthralgia and myalgia does.

Effects of Scalp Myofascial Technique and Manual Therapy on Pain and Quality of Life in Tension Type Headache Patients : Six Month Follow-up Results (두피근막이완기법과 도수치료가 긴장성두통 환자의 통증과 삶의 질에 미치는 영향 : 6개월 추적연구)

  • Purpose : We aimed to validate a new manual therapy to treat tension type headache(TTH) by applying myofascial release techniques to the scalp and to examine the changes in the quality of life and the headache characteristics after treatment and at the 6-month follow-up. Methods : Thirty patients were recruited in this study and were assigned to two groups through simple random sampling. Fifteen patients were assigned to the manual therapy group (MT) and 15 patients to the scalp myofascial release technique (SMT) group. However, five patients from the MT group and one from the SMT group were excluded. Therefore, 24 patients with TTH (10 males, 14 females) participated in the study. Patients underwent either MT or SMT. The procedures were performed by a physical therapist twice per week for 4 weeks. The quality of life [using the brief pain inventory (BPI) and the headache impact test (HIT)], and the frequency, duration, and intensity of the headache [on a visual analog scale (VAS)] were assessed before and after the treatment, and at the follow-up. Results : After 4 weeks of SMT, the frequency (p<.001), duration (p<.05), and intensity (p<.001) of the headache and the quality of life (HIT; p <.001, BPI; p<.001) significantly improved in the patients with TTH. The improvement in these parameters remained significant even after 6 months of follow-up. Similarly, After 4 weeks of MT, the frequency (p<.05), duration (p<.05), and intensity (p<.01) of the headache, and the quality of life (HIT; p<.05, BPI; p<.001) significantly improved in the patients with TTH. The improvement in these parameters remained significant even after 6 months of follow-up. There was no significant difference in these parameters between the two groups. Conclusion : It has been suggested that MT using the SMT can be used as a non-invasive treatment to treat the frequency, duration, and intensity of the TTH, and to improve the quality of life.

  • Porcine transforming growth factor-$\beta$1 (TGF-$\beta$1) was expressed in Escherichia coli using cDNA of TGF-$\beta$1 and glutathione S-transferase (GST) fusion vector pGEX-1$\lambda$T. An ApoI-Tth111I fragment of cDNA which correspond to the amino acid residues from 123 to 390 of the precursor TGF-$\beta$1 was inserted into EcoRI-Tth111I digested pGEM#-l$\lambda$T and the recombined plasmid was named pGET-12. Gene products from the cloned regions of the recombinant plasmids pGET-12 was not detected in soluble fraction of cell free extract but detected in insoluble fraction. The solubilization of insoluble gene product was achieved by the treatment of N-laurylsarcosine. Molecular weight of partially purified proteins determined by electrophoresis was same as expected from cloned fragment. The ELISA test results of the purified proteins showed that immunologically detectable epitope was preserved in recombinant protein.

Design of Food Management System Using NFC Tag

  • As society advances and consumers' perception of food safety changes, the demand for the safety of refrigerated and distributed foods is spreading. In this paper, we build an ICT system that can collect the TTH data from the NFC tag, store it, analyze the food safety level using it. This service platform consists of the temperature monitoring NFC tag attached to individual packaged food, the smartphone with NFC sensor, and the monitoring server with a big data system. Consumers and administrators in this system can easily identify and predict the safety level of individual packaged foods in real time using the provided app on smartphones. In the field of food safety, the use of new technologies such as ICT can create new value by combining with existing food industry in addition to creating new market by new service.
