Purpose: To determine the relationship between anatomical variation at A-com. artery area an hemodynamically induced aneurysm-mimicking findings on 3D-TOF MRA clinically a experimentally. Method: Sixty-two patients who had no aneurysm at A-com artery on DSA were evalua with MRA. MRA was performed with 1.5T MR machine(Vision, Siemens). Scan parameters o MRA included TR/TE/FA=30/6.4/25$\psi$$\circledR$˙¡, 512*192 of matrix with MIP technique. The occurrence of signal defect at shoulder area of bifurcated A-com artery on MRA was evalua for the relationship between the symmetricity of bilateral ACA and the patency of A-c artery. DSA images were acquired at both ICA and VA. To analyze hemodynamical facto of signal defect, experimental studies of MRA and DSA were peformed with elastic silic phantom using conducting pulsatile pump. We also compared the results with those o computational fluid dynamics(CFD).
To evaluate clinical visualization of intracranial aneurysms between 3D-TOF MRA with 3.0T MR and CTA with 16-slices MDCT. In a prospective series, 12 patients with 16 aneurysms were studies with 3D-TOF MRA and CTA. MRA were performed on a GE Signa 3.0 T system (Signa VH/i, GE) with 25/3/20 (TR/TE/FA). CTA were peformed on a 16 slice MDCT (Sensation 16, Somatom, Siemens) with IV shooting of 80 ml iodinated contrast mediumat antecubital vein at a rate of 3.5 ml/sec. Four among 12 patients underwent DSA for surgery. Size, shape, neck and parent vessel of aneurysms were evaluated for comparison of visualization and detectability of aneurysms.
Objective : Distinguishing between an infundibulum and a true aneurysm is clinically important. This study aimed to evaluate whether using source image based new three-dimensional rotational angiography (S-n3DRA) can increase the rate of aneurysm detection and improve distinction between a true aneurysm and an infundibulum. Methods : Twenty-two consecutive patients with 23 lesions, were evaluated by time-of-flight (TOF) magnetic resonance angiography (MRA), S-n3DRA, and digital subtraction angiography (DSA). The data were retrospectively and independently reviewed by two neurointerventionists, and the diagnoses based on TOF MRA, S-n3DRA, and DSA were compared. The diagnostic efficacy (interobserver agreement and diagnostic performance) of S-n3DRA was compared with that of TOF MRA. Results : S-n3DRA showed higher interobserver agreement (κ=0.923) than TOF MRA (κ=0.465) and significantly higher accuracy than MRA in distinguishing an aneurysm from an infundibulum (p=0.0039). Conclusion : Compared to MRA, S-n3DRA could provide better screening accuracy and information for distinguishing an aneurysm from an infundibulum. Therefore, S-n3DRA has the potential to reduce the need for DSA.
Aneurysm-mimicking findings were frequently visualized due to hemodynamical causes of dephasing effects around area of A-com artery during magnetic resonance angiography(MRA) and these kind of phenomena have not been clearly known yet. We investigated the hemodynamical patterns of dephasing effect around area of the A-com artery that might be a cause of false intracranial aneurysms on MRA. For experimental study, We used hand-made silicon phantoms of the asymmetric A-com artery as like a bifurcation configuration. In a closed circulatory system with UHDC computer driven cardiac pump system. MRA and fast digital subfraction angiography(DSA) involved the use of these phantoms. Flow patterns were evaluated with axial and coronal imaging of MRA(2D-TOF, 3D-TOF) and DSA of Phantoms constructed from an automated closed-type circulatory system filled with glycerol solution [circulation fluid(glycerol:water = 1:1.4)]. These findings were then compared with those obtained from computational fluid dynamic(CFD) for inter-experimental correlation study. Imaging findings of MRA, DSA and CFD on inflow zone according to the following: a) MRA demonstrated high signal intensity zone as inflow zone on silicon phantom; b) Patterns of DSA were well matched with MRA on trajectory of inflow zone; and c) CFD were well matched with MRA on the pattern of main flow. Imaging findings of MRA. DSA and CFD on turbulent flow zone according to the following: a) MRA demonstrated hyposignal intensity zone at shoulder and axillar zone of main inflow; b) DSA delineated prominent vortex flow at the same area. The hemodynamical causes of signal defect, which could Induce the false aneurysm on MRA, turned out to be dephasing effects at axilla area of bifurcation from turbulent flow as the results of MRA, DSA and CFD.
This study investigated the accuracy of magnetic resonance angiography (MRA) and computed tomography angiography (CTA) in terms of reflecting the actual vascular length. Three-dimensional time of flight (3D TOF) MRA, 3D contrast-enhanced (CE) MRA, volume-rendering after CTA and maximum intensity projection were investigated using a flow model phantom with a diameter of 2.11 mm and area of $0.26cm^2$. 1.5 and 3.0 Tesla devices were used for 3D TOF MRA and 3D CE MRA. CTA was investigated using 16 and 64 channel CT scanners, and the images were transmitted and reconstructed by volume-rendering and maximum intensity projection, followed by conduit length measurement as described above. The smallest 3D TOF MRA measure was $2.51{\pm}0.12mm$ with a flow velocity of 40 cm/s using the 3.0 Tesla apparatus, and $2.57{\pm}0.07mm$ with a velocity of 71.5 cm/s using the 1.5 Tesla apparatus; both images were magnified from the actual measurement of 2.11 mm. The measurement with the 16 channel CT scanner was smaller ($3.83{\pm}0.37mm$) than the reconstructed image on maximum intensity projection. The images from CTA from examination apparatus and reconstruction technique were all larger than the actual measurement.
Intracrnial 3D TOF MR angiography was performed in 30 normal volunteers with both 1.5 and 3.0 T MRI system with high resolutions. Used Voxel sizes were $0.39{\times}0.39{\times}0.2$(1.5 T) and $0.19{\times}0.19{\times}0.35$(3.0 T), respectively. High image quality and depiction of small vessel branches were equality demonstrated with 1.5 T and 3.0 T HR TOF MRA(p<0.05). Intracranial high resolution TOF MRA with 1.5 T and 3.0 T provides high diagnostic information with having merits and demerits in depiction of vascular branches.

We will present various MR findings of hyperacute ischemic stroke with our own experiences in the management of the patients according to the findings. 대상 및 방법: A total of 441 patients were underwent 'acute stroke MR' imaging protocol between Mar. 2001 and Jun. 2003. The protocol included initial T2-weighted image (WI), diffusion WI (DWI, b=2000), time-of-flight (TOF) MR angiography (MRA), and pefusion WI(PWI), and follow-up T2WI, DWI, TOF MRA, and neck vessel contrast-enhanced MRA obtained three to five days after the insult. Among them, we retrospectively reviewed the MR findings and clinical courses of 193 patients with anterior circulation territorial infarction. Those ICA and MCA lesions were divided into six and five groups respectively according to the level and mechanism of the occlusion. PWI findings can be another factor in the management planning.

In this study we evaluated that flow rate changes affect the (time of flight) TOF image and contrast-enhanced (CE) in a three-dimensional TOF angiography. We used a 3.0T MR System, a nonpulsatile flow rate model. Saline was used as a fluid injected at a flow rate of 11.4 cm/sec by auto injector. The fluid signal strength, phantom body signal strength and background signal strength were measured at 1, 5, 10, 15, 20 and 25-th cross-section in the experienced images and then they were used to determine signal-to-noise ratio and contrast-to-noise ratio. The inlet, middle and outlet length were measured using coronal images obtained through the maximum intensity projection method. As a result, the length of inner cavity was 2.66 mm with no difference among the inlet, middle and outlet length. We also could know that the magnification rate is 49-55.6% in inlet part, 49-59% in middle part and 49-59% in outlet part, and so the image is generally larger than in the actual measurement. Signal-to-noise ratio and contrast-to-noise ratio were negatively correlated with the fluid velocity and so we could see that signal-to-noise ratio and contrast-to-noise ratio are reduced by faster fluid velocity. Signal-to-noise ratio was 42.2-52.5 in 5-25th section and contrast-to-noise ratio was from 34.0-46.1 also not different, but there was a difference in the 1st section. The smallest 3D TOF MRA measure was $2.51{\pm}0.12mm$ with a flow velocity of 40 cm/s. Consequently, 3D TOF MRA tests show that the faster fluid velocity decreases the signal-to-noise ratio and contrast-to-noise ratio, and basically it can be determined that 3D TOF MRA and 3D CE MRA are displayed larger than in the actual measurement.
Purpose: To determine the value of the appearance of the high signal intensity halo sign for detecting carotid intraplaque hemorrhage (IPH) on maximum intensity projection (MIP) of time-of-flight (TOF) MR angiography (MRA), based on high signal intensity on magnetization-prepared rapid acquisition with gradient-echo (MPRAGE) sequencing. Materials and Methods: A total of 78 carotid arteries in 65 patients with magnetization-prepared rapid acquisition gradient-echo (MPRAGE) positive on carotid plaque MR imaging were included in this study. High-resolution MR imaging was performed on a 3.0-T scanner prior to carotid endarterectomy or carotid artery stenting. Fast spin-echo T1- and T2-weighted axial imaging, TOF, and MPRAGE sequences were obtained. Carotid plaques with high signal intensity on MPRAGE > 200% that of adjacent muscle on at least two consecutive slices were defined as showing IPH. Halo sign of high signal intensity around the carotid artery was found on MIP from TOF MRA. Continuous and categorical variables were compared among groups using the Mann-Whitney test and Fisher's exact tests. Results: Of these 78 carotid arteries, 53 appeared as a halo sign on the TOF MRA. The total IPH volume of patients with a positive halo sign was significantly higher than that of patients without a halo sign ($75.0{\pm}86.8$ vs. $16.3{\pm}18.2$, P = 0.001). The maximum IPH axial wall area in patients with a positive halo sign was significantly higher than that of patients without a halo sign ($11.3{\pm}9.9$ vs. $3.7{\pm}3.6$, P = 0.000). Conclusion: High signal intensity halo of IPH on MIP of TOF MRA is associated with total volume and maximal axial wall area of IPH.
목적: SAAV sequence를 이용하여 동시에 획득한 동맥과 정맥의 두 MRA 영상을 Color Mapping으로 동맥과 정맥을 구분하여 한 영상에 나타냄으로써 AVM 이나 DAVF 등과 같은 혈관질환의 임상적 진단 및 치료에 도움을 주고자 하였다. 대상 및 방법: 일반적으로 MRA 영상은 사전 포화방법(presaturation)을 이용한 2D TOF 기법을 통하여 동맥과 정맥을 구분하여 영상을 획득한다. 이러한 일반적인 사전포화방법을 응용한 SAAV 기법은 일정영역을 미리 포화(saturation)시킨 후 포화영역의 위, 아래의 영역을 한번에 영상화하여 동맥과 정맥의 구분된 영상을 한번에 획득할 수 있다. 마산 삼성병원의 0.3T MRI system(Megfinder, AILab. Korea)에서 SAAV sequence를 이용하여 정상적인 피험자로부터 목 부위의 동맥과 정맥 혈관영상을 동시에 얻었다. 이들의 각 2D 영상을 Color Mapping으로 조합한 후 Maximum Intensity Projection(MIP) 기법을 통해 3D Artery-Vein Color Mapping(AVCM) MRA 영상으로 재구성하였다. 3명의 피험자에 대한 SAAV MRA data를 256$\times$256$\times$64(resolution: 0.89$\times$0.89$\times$2㎣)로 획득하였다.
