The Value of Comparison with Four Dimension Time Resolved Imaging of Contrast Kinetics(TRICKS) MRA by Time of Flight(TOF) MRA (4차원 TRICKS 자기공명혈관조영술과 기존 TOF 자기공명혈관조영술의 비교 및 유용성)

  • To assess the clinical value of time resolved imaging of contrast kinetics(TRICKS) MRA by comparison with conventional time of flight(TOF) MR angiography. Both TOF-MRA and TRICKS-MRA were performed in 17 patients with cerebrovascular disease and in 6 patients with brain tumor. Among 17 cerebraovascular patients, digital subtraction angiography(DSA) data were also obtained in 11 patients. TOF-MRA showed good spatial resolution but short in temporal resolution. Although TRICKS-MRA showed somewhat low spatial resolution, it showed superior temporal resolution by distinguishing vessel and tumor in all patients. Also, from the analysis of vessel-tumor relationship, TRICKS-MRA showed better performance than TOF-MRA. TRICKS-MRA makes it possible to image arterial, capillary and venous phase sequentially with very speedy manner and therefore, the clinical use of this method is highly suggestive for future use.

Effects of Flow Rates and CS Factors on TOF MRA using Compressed Sensing (Compressed sensing을 이용한 TOF MRA 검사에서 Flow rate와 CS factor의 변화에 따른 영향)

  • This study aimed to measure the quantitative changes in images according to the use of compressed sensing in expressing the slow flow rate in TOF MRA test using magnetic resonance imaging. This study set different blood flow rate sections by using auto-injector and flow phantom and compared changes in the SNR, CNR, SSIM, and RMSE measurements by different CS factors between TOF with CS and TOF without CS. One-way ANOVA was performed to test the effect on the image induced by the increase of the CS factor. The results revealed that TOF MRA with CS significantly decreased scan time without significantly affecting SNR and CNR compared to TOF MRA with CS. On the other hand, the differences in SSIM and RMSE between TOF with CS and TOF without CS increased as the CS factor increased. Therefore, it is necessary to efficiently reduce scan time by adapting the CS technique while considering the appropriate range of the CS factor. Additionally, more studies are needed to evaluate CS factors and the similarity precision of images further.

  • The purpose of this study was to evaluate the possibility of utilizing MT pulse at CE-TOF-MRA in patients with cerebral infarction. MRA using time-of-flight(TOF) technique with varying offset frequencies (0, 600, 1,200, and 1,800 Hz) magnetization transfer were performed in 10 patients with cerebral infarction at 3.0T MR scanner. CE-TOF-MRA and TOF-SPGR in normal vessel shown decreased SNR and increased CNR. The highest CNR in narrowing vessel shown at CE-TOF-MRA using 600 and 1,200 Hz offset frequencies. CNR in stenosis vessel increased dependent on using offset frequencies. The occlusion was clearly shown, and the highest CNR in occlusion shown at CE-TOF-MRA using 1,800 Hz offset frequencies. There was no shape variation in narrowing vessel or no visualizing vessel.

Magnetization Transfer Contrast Angiography for Organized Thrombosed Intracranial Aneurysm in TOF MR Angiography: a Case Report

  • A 66-year-old woman was referred for treatment of incidental detection of two intracranial aneurysms. Time-of-flight MR angiography (TOF MRA) revealed two aneurysms at the M1 segment of the right middle cerebral artery, and clinoid segment of left internal carotid artery, respectively. On digital subtraction angiography, there was a saccular aneurysm on the left internal carotid artery, but the other aneurysm was not detected on the right middle cerebral artery. Based on comprehensive review of imaging findings, organized thrombosed aneurysm was judged as the most likely diagnosis. In the presented report, magnetization transfer (MT) pulse to TOF MRA was used, to differentiate aneurysm-mimicking lesion on TOF MRA. We report that MT technique could be effective in differentiating true aneurysm, from possible T1 high signal artifact on TOF MRA.

Evaluation of Image Quality for Compressed SENSE(CS) Method in Cerebrovascular MRI: Comparison with SENSE Method (뇌혈관자기공영영상에서 Compressed SENSE(CS) 기법에 대한 영상의 질 평가: SENSE 기법과 비교)

  • The object of this research is CS, which increases resolution while shortening inspection time, is applied to MRA to compare the quality of images for SENSE and CS techniques and to evaluate SNR and CNR to find out the optimal techniques and to provide them as clinical basic data based on this information. Data were analyzed on 32 patients who performed TOF MRA tests at a university hospital in Chung cheong-do (15 males, 17 females), ICA stenosis:10, M1 Aneurysm:10, and average age 53 ± 4.15). In the inspection, the inspection equipment was Ingenia CX 3.0T, Archieva 3.0T, and 32 channel head coil and 3D gradient echo as a method for equipment data. SNR and CNR of each image were measured by quantitative analysis, and the quality of the image was evaluated by dividing the observer's observation into 5 grades for qualitative evaluation. Imaging evaluation is described as being significant when the p-value is 0.05 or less when the paired T-test and Wilcoxon test are performed. Quantitative analysis of SNR and CNR in TOF MRA images Compared to the SENSE method, the CS method is a method measurement method (p <0.05). As an observer's evaluation, the sharpness of blood vessels: CS (4.45 ± 0.41), overall image quality: CS (4.77 ± 0.18), background suppression of images: CS (4.57 ± 0.18) all resulted in high CS technique (p = 0.000). In conclusion, the Compressed SENSE TOF MRA technique shows superior results when comparing and evaluating the SENSE and Compressed SENSE techniques in increased flow rate magnetic resonance angiography. The results are thought to be the clinical basis material in the 3D TOF MRA examination for brain disease.

The Research to Correct Overestimation in TOF-MRA for Severity of Cerebrovascular Stenosis (3D-SPACE T2 기법에 의한 TOF-MRA검사 시 발생하는 혈관 내 협착 정도의 측정 오류 개선에 관한 연구)

  • It is very important accurate diagnosis and quick treatment in cerebrovascular disease, i.e. stenosis or occlusion that could be caused by risk factors such as poor dietary habits, insufficient exercise, and obesity. Time-of-flight magnetic resonance angiography (TOF-MRA), it is well known as diagnostic method without using contrast agent for cerebrovascular disease, is the most representative and reliable technique. Nevertheless, it still has measurement errors (also known as overestimation) for length of stenosis and area of occlusion in celebral infarction that is built by accumulation and rupture of plaques generated by hemodynamic turbulence. The purpose of this study is to show clinical trial feasibility for 3D-SPACE T2, which is improved by using signal attenuation effects of fluid velocity, in diagnosis of cerebrovascular disease. To model angiostenosis, strictures of different proportions (40%, 50%, 60%, and 70%) and virtual blood stream (normal saline) of different velocities (0.19 ml/sec, 1.5 ml/sec, 2.1 ml/sec, and 2.6 ml/sec) by using dialysis were made. Cross-examinations were performed for 3D-SPACE T2 and TOF-MRA (16 times each). The accuracy of measurement for length of stenosis was compared in all experimental conditions. 3D-SPACE 2T has superiority in terms of accuracy for measurements of the length of stenosis, compared with TOF-MRA. Also, it is robust in fast blood stream and large stenosis than TOF-MRA. 3D-SPACE 2T will be promising technique to increase diagnosis accuracy in narrow complex lesions as like two cerebral small vessels with stenosis, created by hemodynamic turbulence.

  • The purpose of this study was to quantitatively correlate the change of flow velocity and signal voiding in TOF-MRA. We made our phantom to control the flow velocity, and changed the flow velocity in 16 steps from 8.0 to 127.3 mc/s. The TOF-MRA test was performed using a 3.0T MRI system and the signal intensity was measured by classifying the signal voiding length and image into the In flow, Mid flow, and Out flow. The length of signal voiding was the longest when the flow velocity was 127.3 cm/s and the signal intensity decreased with increasing flow velocity(p<0.05). In flow(-.547) and Mid flow(-.643) were negatively correlated with flow velocitys(p<0.05). In conclusion, it was confirmed that the increase in flow velocity was a major factor causing signal voiding in TOF-MRA. In the future, this study will provide basic data when studying sequences and parameters to reduce signal voiding in models with a high flow velocity.

Usefulness of Silent MRA for Evaluation of Aneurysm after Stent-Assisted Coil Embolization

  • Objective: To determine the usefulness of Silent MR angiography (MRA) for evaluating intracranial aneurysms treated with stent-assisted coil embolization. Materials and Methods: Ninety-nine patients (101 aneurysms) treated with stent-assisted coil embolization (Neuroform atlas, 71 cases; Enterprise, 17; LVIS Jr, 9; and Solitaire AB, 4 cases) underwent time-of-flight (TOF) MRA and Silent MRA in the same session using a 3T MRI system within 24 hours of embolization. Two radiologists independently interpreted both MRA images retrospectively and rated the image quality using a 5-point Likert scale. The image quality and diagnostic accuracy of the two modalities in the detection of aneurysm occlusion were further compared based on the stent design and the site of aneurysm. Results: The average image quality scores of the Silent MRA and TOF MRA were 4.38 ± 0.83 and 2.78 ± 1.04, respectively (p < 0.001), with an almost perfect interobserver agreement. Silent MRA had a significantly higher image quality score than TOF MRA at the distal internal carotid artery (n = 57, 4.25 ± 0.91 vs. 3.05 ± 1.16, p < 0.001), middle cerebral artery (n = 21, 4.57 ± 0.75 vs. 2.19 ± 0.68, p < 0.001), anterior cerebral artery (n = 13, 4.54 ± 0.66 vs. 2.46 ± 0.66, p < 0.001), and posterior circulation artery (n = 10, 4.50 ± 0.71 vs. 2.90 ± 0.74, p = 0.013). Silent MRA had superior image quality score to TOF MRA in the stented arteries when using Neuroform atlas (4.66 ± 0.53 vs. 3.21 ± 0.84, p < 0.001), Enterprise (3.29 ± 1.59 vs. 1.59 ± 0.51, p = 0.003), LVIS Jr (4.33 ± 1.89 vs. 1.89 ± 0.78, p = 0.033), and Solitaire AB stents (4.00 ± 2.25 vs. 2.25 ± 0.96, p = 0.356). The interpretation of the status of aneurysm occlusion exhibited significantly higher sensitivity with Silent MRA than with TOF MRA when using the Neuroform Atlas stent (96.4% vs. 14.3%, respectively, p < 0.001) and LVIS Jr stent (100% vs. 20%, respectively, p = 0.046). Conclusion: Silent MRA can be useful to evaluate aneurysms treated with stent-assisted coil embolization, regardless of the aneurysm location and type of stent used.

The Usefulness of 3T-TOF MR angiography in Patients with Cerebral Infarction (뇌졸중 환자에서 3 Tesla 자기공명혈관조영술의 유용성)

  • Purpose : This study was designed to evaluate the usefulness of 3T-TOF MR angiography (3T-TOF MRA) compared with transcranial Doppler sonography (TCD) and conventional angiography (CA) in patients with suspected cerebral infarction. Materials and Methods : Fifty four patients with clinical symptoms of cerebral infarction were involved in this study, and had undergone 3T-TOF MRA and TCD, with CA in 11 patients. On the basis of divisions of the carotid artery, four groups were designated: group I, both vertebral arteries and basilar artery; group II, segment between 2 cm below bifurcation of common carotid artery and genu portion of internal carotid artery; group III, segment between petrous portion of internal carotid artery and bifurcation of anterior and middle cerebral artery; group IV, from bifurcation of anterior and middle cerebral artery to thier distal branches. Two radiologists retrospectively reviewed the vascular imaging and stenosis in 3T-TOF MRA, TCD, and CA. Results : A total of 432 arteries, 108 in each group, were available. The assessment of vascular imaging quality in 3T-TOF MRA is scored 2.98, 2.96, 2.91, 2.88 in 4 groups, respectively. Agreement among 3T-TOF MR angiography, TCD, and CA was high. Conclusion : 3T-TOF MR angiography may be useful method for the assessment of stenotic lesions of cranial vasculature in patients with cerebral infarction.
