흉부 전산화 단층촬영상 임파절종대가 없는 비소세포암 환자에 있어서 술전 병기판정 (Preoperative Staging in Non-Small Cell Lung Cancer without Lymphadenopathy on Computed Tomogram)

  • 연구목적: 수술전 전산화 단층촬영상 임파절종대의 소견이 없는 $T_{1-3}N_0M_0$비소세포폐암 환자들을 대상으로 수술전 후 병기의 차이를 비교하여 이들에 있어 수술전 관혈적인 병기판정의 필요성에 대하여 검토하였다. 방법: 경북대학교병원에서 비소세포폐암으로 개흉절제술을 받았던 환자들 가운데 수술전 병기가 $T_{1-3}N_0M_0$인 41명을 대상으로 수술전과 수술후의 병기의 차이를 비교하였다. 결과: 1) 수술전 병기는 I기의 경우 $T_1N_0M_0$ 3예, $T_2N_0M_0$ 32예로 모두 35예였고 IIIa기 ($T_3N_0M_0$)는 6예였다. 종양의 위치는 중심형 폐암 24예, 말초형 폐암 17예였는데 IIIa기는 모두 중심형 폐암이었다. 2) 수술후 병기는 I기의 경우 $T_1N_0M_0$ 2예, $T_2N_0M_0$ 25예로 모두 27예였고 II기의 경우 $T_1N_1M_0$ 1예, $T_2N_1M_0$ 3예로 모두 4예였으며 IIIa기는 $T_3N_0M_0$ 1예, $T_3N_1M_0$ 2예, $T_3N_2M_0$ 4예, $T_2N_2M_0$ 2예로 모두 9예였고 IIIb기($T_4N_1M_0$)는 1예였다. 3) 수술후 T의 변화가 있은 경우는 $T_2$ 32예 가운데 2예는 $T_3$$T_3$ 6예중 1예는 $T_4$로 판명되었다. 4) 수술후 $N_1$으로 판명된 경우는 7예였고 $N_2$로 판명된 경우는 6예였다. 5) 수술전 T에 따른 임파절전이는 $T_{1-2}$인 경우는 35예 중 8예($N_1$ 5예, $N_2$ 3예)였고 $T_3$인 경우는 6예중 5예($N_1$ 2예, $N_2$ 3예)로 $T_{1-2}$에 비해 $T_3$에서 임파절 전이빈도가 높았고 $N_2/N_1$비도 높았다 그러나 수술전 $T_{1-2}$경우 종양의 위치에 따른 임파절전이의 차이는 없었다. 6) 41예의 대상환자중 $N_2$ 6예와 $T_4$ 1예를 제외한 34예에서 완전 절제가 가능하였다. 결론: 이상의 결과로 전산화 단층촬영상 임파절종대가 없는 비소세포암의 수술전 병기판정시 수술전 $T_3$에서는 종격동경 검사 등의 관혈적인 병기판정방법이 필요하리라 생각된다.

성문암의 방사선치료 (The Result of Radiotherapy in Glottic Cancer)

  • 성문암은 술, 담배가 주 원인적 인자로 밝혀져 있다. 성문암은 쉰목소리 등으로 조기발견이 용이하며 대부분 분화도가 좋은 편평상피암이고 해부학적 위치상 조직생검 및 치료경과 관찰이 용이하며, 성운에는 임파절이 없으므로 전이가 적다. 국소 치료인 방사선치료와 수술이 치료의 주종을 이루며 방사선치료는 음성보전 측면에서 효과적이므로 초기병변에는 방사선치료를 진행된 병변의 경우 수술 혹은 수출 및 방사선 복합요법을 사용하고 있다. 저자들은 1979년 3월부터 1982년 9월까지 서울대학교병원 치료방사선과에서 성문암으로 치료받은 환자들을 추적 조사하여 다음과 같은 결과를 얻었다. 1. 방사선치료 후 3년 무병 생존율이 $T_1N_0 :\;78\%T_1N_0;\;60\%T_3N_0:\;50\%$, 2 구제적 수술후 3년 무병생존율이 $T_1N_0 :\;89\%T_2N_0;\;80\%T_3N_0:\;50\%$ 3. 재발은 대개 3년 이내에 하였다.

제2기 위암에서 3 Subgroup간의 비교 분석 (Comparative Analysis of Three Subgroups in Stage II Stomach Cancer)

  • Purpose: Three subgroups of stage II stomach cancer (T1N2M0, T2N1M0, T3N0M0) by UICC-TNM staging system show obvious survival difference to each other, which becomes the pitfall of the current staging system. We analyzed the survival and relapse pattern of stage II stomach cancer patients in three subgroups retrospectively to prove the need for change in staging system. Materials and Methods: From July 1989 to December 1995, curative gastric resection was performed in 1,037 patients with gastric adenocarcinoma, and among them 268 patients ($26\%$) were in stage II. The number in each of subgroups (T1N2M0, T2N1M0, and T3N0M0) were 17, 139 and 112 respectively. Survival and relapse pattern were analyzed and median follow up period was 46 months. Results: The 3-year cumulative survival rates of T1N2M0, T2N1M0, and T3N0M0 were $50\%,\;80\%,\;and\;76\%$ respectively (p=0.001). And the 3-year cumulative survival rates of T1N2M0 was comparable to those of 2 subgroups of stage IIIa (T2N2M0, T3N1M0), $47\%\;and\;45\%$ (p>0.05). Peritoneal recurrence was the most frequent in T3N0M0. And hematogenous spread was more frequent in T2N1M0 while nodal spread was more frequent in T1N2M0. Ten out of 17 cases of T1N2M0 died of recurrence. Most of them showed submucosal tumor with depressed lesion and mean tumor size was 3.3 cm. Conclusions: Up-staging of T1N2M0 should be considered because it has the lowest survival rate and the worst prognosis among the three subgroups of Stage II stomach cancer patients. In early gastric cancer patients with high-risk factors (large tumor size, invasion into the submucosal layer, and lymphatic vessel involvement), lymph node dissection and postoperative adjuvant therapy is recommended in an attempt to prevent recurrence in the form of lymph node metastasis.

육계에 급여되는 사료중의 단백질 수준과 깔짚 내의 Aluminum Sulfate 첨가가 깔짚 내의 질소 함량에 미치는 영향 (Effect of Aluminum Sulfate Addition to Litter and Dietary Protein Levels of Broilers on Litter Nitrogen Content)

  • The two experiments in this study compared litter nitrogen (N) contents after broiler chicks were raised for 42 days. Experiment I compared litter treated with aluminum sulfate (alum) [Al$_2$(SO$_4$)$_3$ㆍ14$H_2O$] (T$_1$) with nontreated litter (T$_2$) when the broiler chicks were fed the same levels of dietary protein (23% for 0∼3 weeks, 21% for 4∼6 weeks) Experiment 2 compared the alum treated litters of broiler chicks fed high protein diets (T$_3$) having 20.4% protein for 0∼3 weeks and 19.3% for 4∼6 weeks, with lower protein diets (T$_4$) having protein levels of 18.0% for 0∼3 weeks and 17.0% for 4 6 weeks. Each treatment had four replicate cages. As shown in Table 1, T$_1$ had a significantly (P<0.0l) lower pH and significantly (P<0.05) higher total N (TN), NH$_4$-N and inorganic N (IN) than T$_2$. T$_1$ and T$_2$ had similar moisture, organic carbon (OC), NO$_3$-N and organic N (ON). Alum treatment increased available N (AN) significantly (P<0.05) from 13.75$\pm$0.0l mg/g to 14.90$\pm$0.01 mg/g and predicted available N (PAN) significantly (P<0.05) from 15.00$\pm$0.0l to 16.50$\pm$0.02. The C : N ratios were 18.84$\pm$0.40 (T$_1$) and 19.46$\pm$0.10 (T$_2$) while the C : ON ratios were 28.49$\pm$1.15 (T$_1$) and 28.34$\pm$0.20 (T$_2$) although C : N ratios or C : ON ratios did not show any difference between T$_1$ and T$_2$. In Table 2, T$_3$ had significantly (P<0.05) higher moisture, TN, NH$_4$-N, ON and IN than T$_4$, while the pH, OC and NO$_3$-N were similar in both groups. The AN of T$_3$ increased significantly (P<0.05) from 10.99$\pm$0.0l mg/g to 12.98$\pm$0.03 mg/g, while the PAN increased significantly (P<0.05) from 12.39$\pm$0.10 mg/g (T$_4$) to 14.68$\pm$ 0.30 mg/g (T$_3$). The C : N ratios increased significantly (P<0.0l) from 20.07$\pm$0.20 (T$_3$) to 24.40$\pm$0.10 (T$_3$). The C : ON ratios also increased significantly (P<0.0l) from 28.99$\pm$1.15 (T$_3$) to 35.51$\pm$0.20 (T$_4$). These current research results show increased AN contents and PAN contents in alum treated litter or with increased CP levels regardless of alum treatment. However, none of the litters in this study could initially increase mineralization.

Suppose that D(f) is square-free. then we prove that by Theorem 2.1 .mu.(p$^{n}$ -q$^{n-1}$)=.mu.(f)-(n-1)+n(n-2)I(p,q)+1 where .mu.(f), .mu. p$^{n}$ -q$^{n-1}$are the corresponding Milnor numbers of f, p$^{n}$ -q$^{n-1}$, respectively and I(p,q) is the intersection number of p and q at the origin. By one of applications suppose that W$_{t}$ ={(x,y,z):g$_{t}$ =z$^{n}$ -np$_{t}$ $^{n-1}$z+(n-1)q$_{t}$ $^{n-1}$=0} is a smooth family of complex analytic varieties near t=0 each of which has an isolated singularity at the origin, satisfying that the z-discriminant of g$_{t}$ , that is, D(g$_{t}$ ) is square-free. If .mu.(g$_{t}$ ) are constant near t=0, then we prove that the family of plane curves, D(g$_{t}$ ) are equisingular and also D(f$_{t}$ ) are equisingular near t=0 where f$_{t}$ =z$^{n}$ -np$_{t}$ z+(n-1)q$_{t}$ =0.}$ =0.

Effect of Protein Deprivation on Subsequent Efficiency of Dietary Protein Utilization in Finishing Pigs

  • A nitrogen (N) balance trial was conducted to examine the effect of N deprivation on subsequent N retention, blood urea nitrogen (BUN) and IGF-I levels and the ratio of IGF binding protein (IGFBP)-3 to IGFBP-l and -2. Pigs in treatment (T) 1 were given diet A (2.39% N) and those in T2 and T3 were given diet B (1.31% N) and excreta were collected (period 1 (P1)). Pigs in T1 continued to receive diet A while diets for T2 and T3 were changed to diets A and C (2.74% N), respectively. The excreta were collected for two more periods (P2 and P3). During P1, pigs in T2 and T3 retained 50% less N (p<0.001) than those in T1. However, pigs provided T2 (p<0.01) and T3 (p<0.05) retained more N than those assigned to T1 during P2. Pigs in T3 tended to retain more (p=0.10) N than those receiving T2 for the same period. The BUN values were lower (p<0.05) for pigs assigned to T2 and T3 than T1 during P1 and P2. Both IGF-I and IGFBP ratios of pigs assigned to T1 were higher (p<0.05) than those given T2 and T3 during P1 but no differences were found during P2 and P3.

BNR에 의한 하수의 고도처리에 미치는 NH3 스트리핑 전처리의 영향 (Effect of Pre-NH3 Stripping on the Advanced Sewerage Treatment by BNR)

  • The biological nutrient removal from domestic wastewater with low C/N ratio is difficult. Therefore, this study was performed to increase influent C/N ratio by ammonia stripping without required carbon source and for improving treatment efficiencies of sewerage by the combination process of ammonia stripping and BNR (StripBNR). The results of this study were summarized as follows. BOD removal efficiencies of BNR and StripBNR were 95.3% and 93.2%, respectively. T-N and T-P removal efficiencies of BNR were 53.3% and 40.8%, respectively. T-N and T-P removal efficiencies of StripBNR were 72.8% and 62.9%, respectively. Concentrations of $NH_3-N$, $NO_2-N$ and $NO_3-N$ at BNR effluent were 0.03 mg/L, 0.08 mg/L and 9.12 mg/L, respectively. On the other hands, concentrations of $NH_3-N$, $NO_2-N$ and $NO_3-N$ at StripBNR effluent were 5.79 mg/L, 0.01 mg/L and 0.14 mg/L, respectively. Consequently, influent C/N ratio of BNR process was increased by ammonia stripping. Removal efficiency of T-N and T-P was improved about 20% by the process of StripBNR.

침지식 평판형 연속회분식 박반응기에서 유입 유기물 부하의 변화에 따른 영양염류의 제거 특성 (Nutrient Removal Characteristics on Organic Material Loadings in Submerged Flat Sheet Type Sequencing Batch Membrane Reactor)

  • $0.4\;{\mu}m$의 세공크기를 갖고 있는 평막이 침지된 연속회분식 반응기에서 유입 유기물 농도가 영양염류 제거에 미치는 영향을 조사하였다. 분리막의 여과성능과 영양염류 제거효과를 규명하기 위하여 유입 유기물의 농도를 200 mg/L (Run-1), 400 mg/L (Run-2) 및 800 mg/L (Run-3)로 연속적으로 변화시키면서 실험하였다. COD/N 및 COD/P의 비가 증가할수록 T-N 및 T-P의 제거율은 모두 증가하였다. Run-1, Run-2 및 Run-3에서 T-N의 평균 제거율은 각각 28.1, 32.6 및 90.4%이었으며, 투과수의 T-N 평균 농도는 각각 32.0, 30.0 및 4.3 mg/L 이었다. 또한 Run-1, Run-2 및 Run-3에서 T-P의 평균 제거율은 각각 13.6, 35.3 및 93.1%이었으며, 투과수의 T-P 평균 농도는 각각 3.11, 2.33 및 0.25 mg/L이었다.

호소내 퇴적물의 근접도에 따른 수질오염물질 분석(I) - COD, T-N, T-P, pH - (Analysis of Water Quality Pollutants Proximated to sediment in Lake)

  • K 수계 6개 지점의 호소 퇴적물과 근접된 수질에 대해 $COD_{Mn}$, T-N, T-P, pH를 측정분석 하였다. 퇴적물로부터 분리된 수질이 퇴적물과 근접한 수질보다 COD, T-N, T-P 모두 높은 값을 나타내었으며 그 다음으로 퇴적층 바로 위의 수질이 높은 값을 나타내었다. 직상수와 최상수의 $COD_{Mn}$, T-N, T-P에 대한 수질분석 결과는 1.2~19.0mg/L, T-N, 1.3~6.2mg/L, T-P, 0.05~0.26mg/L의 농도범위를 나타내었다. 이상의 결과로부터 퇴적물이 호소내 수질오염에 영향을 미치는 요인임을 알 수 있었다.
