A Computed Tomography-Based Anatomic Comparison of Three Different Types of C7 Posterior Fixation Techniques : Pedicle, Intralaminar, and Lateral Mass Screws

  • Objective : The intralaminar screw (ILS) fixation technique offers an alternative to pedicle screw (PS) and lateral mass screw (LMS) fixation in the C7 spine. Although cadaveric studies have described the anatomy of the pedicles, laminae, and lateral masses at C7, 3-dimensional computed tomography (CT) imaging is the modality of choice for pre-surgical planning. In this study, the goal was to determine the anatomical parameter and optimal screw trajectory for ILS placement at C7, and to compare this information to PS and LMS placement in the C7 spine as determined by CT evaluation. Methods : A total of 120 patients (60 men and 60 women) with an average age of $51.7{\pm}13.6$ years were selected by retrospective review of a trauma registry database over a 2-year period. Patients were included in the study if they were older than 15 years of age, had standardized axial bone-window CT imaging at C7, and had no evidence of spinal trauma. For each lamina and pedicle, width (outer cortical and inner cancellous), maximal screw length, and optimal screw trajectory were measured, and the maximal screw length of the lateral mass were measured using m-view 5.4 software. Statistical analysis was performed using Student's t-test. Results : At C7, the maximal PS length was significantly greater than the ILS and LMS length (PS, $33.9{\pm}3.1$ mm; ILS, $30.8{\pm}3.1$ mm; LMS, $10.6{\pm}1.3$; p<0.01). When the outer cortical and inner cancellous width was compared between the pedicle and lamina, the mean pedicle outer cortical width at C7 was wider than the lamina by an average of 0.6 mm (pedicle, $6.8{\pm}1.2$ mm; lamina, $6.2{\pm}1.2$ mm; p<0.01). At C7, 95.8% of the laminae measured accepted a 4.0-mm screw with a 1.0 mm of clearance, compared with 99.2% of pedicle. Of the laminae measured, 99.2% accepted a 3.5-mm screw with a 1.0 mm clearance, compared with 100% of the pedicle. When the outer cortical and inner cancellous height was compared between pedicle and lamina, the mean lamina outer cortical height at C7 was wider than the pedicle by an average of 9.9 mm (lamina, $18.6{\pm}2.0$ mm; pedicle, $8.7{\pm}1.3$ mm; p<0.01). The ideal screw trajectory at C7 was also measured ($47.8{\pm}4.8^{\circ}$ for ILS and $35.1{\pm}8.1^{\circ}$ for PS). Conclusion : Although pedicle screw fixation is the most ideal instrumentation method for C7 fixation with respect to length and cortical diameter, anatomical aspect of C7 lamina is affordable to place screw. Therefore, the C7 intralaminar screw could be an alternative fixation technique with few anatomic limitations in the cases when C7 pedicle screw fixation is not favorable. However, anatomical variations in the length and width must be considered when placing an intralaminar or pedicle screw at C7.

A Survey of the Image Conveyed by Different Types of Nurses따 Uniforms (간호사의 복장 형태에 따른 간호사 이미지에 대한 조사 연구)

  • This study is a descriptive study to provide basic data related to types of uniforms worn by nurses. Nurses, other medical center personnel, patients and their famillies were asked for their opinion on changing nurses’ uniforms and how the image of nurses is 1 elated to the type of uniform worn by the nurses. The data for this study were collected during the period from Feb. 25, 1993 to Mar. 26, 1993 at Y-University Medical Center from 132 nurses, 137 other medical center personnel, 117 patients and their families. The instruments used for this study were the Nurses’ Image Scale(NIS) developed by Bown(1986), and an instrument to measure opinions of uniform and cap using photographs of nurses’ uniforms that was developed by the researcher through a literture review. Data were analyzed using the SPPS / PC package, Statistics used for analysis were frequencies, percentages, paired t -test, and oneway ANOVA The results of this study are summarized as follows ; 1. Of the nurses, 84.3%, along with, 51.5% of the medical center personnel, 39.1% of the patients and their families agreed to a change in the color of nurses uniforms. 2. Similarly, 87.4% of the nurses,27.7% of the medical center personnel, 19.3% of the patients and their families were in favor of nurses not wearing caps. 3. Only 11.8% of nurses answered that a white uniform was important to the professional image of nursing. Only 2.4% of the nurses answered that a cap was very important to the professional image of nursing. 4. Only 1.6% of the nurses answered that a white uniform and cap were important to show the role of nurses in the twenty first century. 5. About 1/3, 36.5%, of the medical center personnel, the patient and their families answered that changing the color of the nurses' uniform will make a change in the image of nurses. 6. A White uniform and cap were seen as being im-portant in distinguishing nurses from other medical personnel in the hospital by 76.5% of the medical personnel, the patients and their families. 7. Nurses gave high marks to the idea of no cap regardless of the color of the uniform which would still portray the symbol of the nurses’ role, identity and would differentiate the role from other jobs. The patients and their families gave high marks to a white uniform with cap. 8. Generally, nurses, medical center personnel and patients and their families thought that a colored uniform with cap presented a good image of nurses. 9. There was a significant difference in the response among the three groups(F=24.65, P〈.001) to the figure in a white uniform with a cap, and the patients and their families thought it was the best portrayal of the image of nurses. There was a significant difference among the three groups(F=9.03, P〈.001) to the figure in a white uniform with no cap and the nurses indicated that it was the beat portrayed of the image of nurses. There was no significant difference among the three groups to the figure in a colored uniform with cap. There was a significant difference(F=17.50, P〈. 001) to the figure in a colored uniform with no cap, and the nurses indicated that it was the best portrayal of the image of nurses. In summary, the nurses wanted to change the color of uniform and not to wear a cap. But many of the medical center personnel, patients and their families indicated they did not agree with this idea. Therefore, changing the type of uniform worn by nurses should be done only after consideration has been given to the relation of the change to the professional role of nurses and the image of nurses held people in general. Suggestions arising from this study are as follows : 1. The relation between the role of nurses and their uniform was surveyed, and these results can be used as data when considering a change in uniforms. 2. A change in uniforms should be tried based on the norm established about uniforms. 3. The nurses did not want to wear a cap, but before a decision is definitely made it is necessary to study the philosophy related to the symbol of the cap because the figure of a nurse with a cap portrayed a positive image of the nurses. If the cap is kept, the symbol of the cap should be redefined. 4. In this study, only the image related to the uniform was examined, but in future studies it will be necessary to examine the practicality of various types of uniforms.

Cost Analysis of Home Nursing Care Patients in Rural Hospital (농촌 지역 중소병원의 가정간호사업소 등록환자의 방문비용분석)

  • The home nursing care system is an integral part of the health care delivery system in order to meet the various needs of consummer, in particular, early discharge patient from the hospital, patient with long term care needed and the elderly. To find out the cost of home nursing care services, the home nursing care records of patients registered by home nursing care units established in public hospital with 150beds during the period of 1996 - 1997 were analyzed. The subjects were 102patients, 45 of male patients and 57 of female patients, those who live in a rural area in Kymiggi - Do The results obtained are as follows : 1. The male patients accounted for 44.1% of the total, with 45cases : group aged 60 years and more was the largest group, accounting for 79.5%. 2. The most frequent disease revealed was the osteoporosis which constitute 35.3% of the total registered patients, followed, in order, by malignant tumor, cerebrovascular disease. 3. It revealed that the cost per visit for the male was 47,764won ; the female, 46,078 won per visit. Noteworthy the cost per visit was high in the older patient. It was clearly that the gender, years of age and the cost per visit were statistically significant at 0.01 level and 0.05 level. 4. The cost per visit for the non complicated disease was slightly higher than the complicated disease, but it is not statistically significant, the cost per visit by type of disease varied, the cost per visit for COPD was the highest, followed, in order, by in malignant tumor, cancer, diabetes, osteoporosis etc. 5. It revealed that home nursing care cost for a eligible disease for home nursing care was less than the cost for hospitalization of the same disease, therefore, we expect that the home nursing care is cost efficiency. In conclusion, the home nursing care costs are needed to analyze further in comparison with the hospitalization costs for a certain disease.

A Study on the Care Needs of Family-Caregivers to the Patients with Stroke (뇌졸중환자 가족의 간호요구)

  • The purpose of this study was to identify the care needs of family-caregivers to the patients with stroke. Subjects were 115 family-caregivers caring for the patients while they were in-patients or out-patients with stroke in two general hospitals and one oriental medicine hospital located in Seoul and Kwang-Ju. The instrument used for this study was made by the researcher on the basis of results of literature review and interviews with family-caregivers, composed of 35 items. Internal validity by calculation of cronbach's alpha with data of respondents was 0.91, which was regarded as high. The Data were analyzed by SAS program, with percentage, mean, t-test, and ANOVA. Factor structures of care needs of family-caregivers were elicited by factor analysis(PCA, Varimax rotation). Datum collection had been from July 1 to August 14, 1997. The results of this study were as follows : 1. The mean score of the sum of the care needs of family-caregivers was 3.96 and the highest-mean item was 'need for immediate care(M=4.77)', and the lowest-mean item was 'need for chaplian's visit (M=2.82)'. 2. Care needs of the family-caregivers were : Need to be informed of the disease, treatment and care ; need of education and assistance related to physical functional level ; need of social support and consultation ; need of management of nursing problem related to immobility ; need of appreciation ; need of the way to communicate with patients ; need of immediate care and help. The highest mean factor was the 'need for immediate care and help(M=4.74)', and the lowest mean factor was the 'need of appreciation(M=3.58)'. 3. The variables influencing the degree of care needs perceived by family-caregivers to the patients with stroke were as follows : There were significant differences between need to be informed of the disease, treatment and care and general characteristic factors, which were family caregiver's sex (p=.0178), caring period(p=.0223) and patient's suffering period(p=.0244). There were significant differences between need of education and assistance related to physical functional level and general characteristic factors, which were patient's paralysis(p=.0177), patient's ADL dependency(p=.0032). There were significant differences between need of social support and consultation and general characteristic factors, which were family caregiver's sex(p=.0055), occupation(p=.0159), religion(p=.0093) and patient's sex(p=.0134). There was significant difference in the degree of need of management of nursing problem related to immobility, according to the patient's ADL dependency(p=.0493). There were significant differences between need of appreciation and general characteristic factors, which were family caregiver's age(p=.0107), sex(p=.0133), and patient's age(p=.0338). There were significant differences between need of the way to communicate with patient and general characteristic factors, which were patient's paralysis(p=.0002) and aphasia(p=.0001). There were significant differences between need of immediate care and help and general characteristic factors, which were family caregiver's caring period(p=.0162) and patient's suffering period(p=.0116). 4. The mean score of patient's ADL dependency was 3. 38 and the highest-mean item was 'ascending and descending stairs(M=4.12)', and the lowest-mean item was 'drinking(M=2.60)'. There was no significant difference in the degrees of care needs related to the patient's ADL dependency. 5. The highest information source of family-caregivers was from the doctors about the disease, treatment and care(26.1%). The second highest one was from mass media(20.8%), and the third one was from the nurses. The above findings may be used as the basic data to seek more efficient way of elevating nursing practice and quality for family-caregivers to the patients with stroke.

  • Recently, interest in radiation protection is increasing because of the occurrence of accidents related to exposure dose. So, the nuclear safety act provides to install the shields to avoid exceeding the dose limit. In particular, when the worker conducts the non-destructive testing (NDT) without the fixed shielding structure, we should monitor the access to the workplace based on a constant dose rate. However, when we apply for permits for NDT work in these work environments, the consideration factors to the estimation of the distance and exposure dose are not legally specified. Therefore, we developed the excel model that automatically calculates the distance, exposure dose, and cost if we input the factors. We applied the assumption data to this model. As a result of the application, the distance change rate was low when the thickness of the lead blanket and collimator is above 25 mm, 21.5 mm, respectively. However, we didn't consider the scattering and build-up factor. And, we assumed the shape of the lead blanket and collimator. Therefore, if we make up for these limitations and use the actual data, we expect that we can build a database on the distance and exposure dose.

An Empirical Study on the Oriental Herbal Cosmetics Purchase Behaviors in Women in the Metropolitan Area (한방 화장품 구매행동에 관한 실증적 연구 - 수도권 거주 여성 소비자를 중심으로 -)

  • Recently, the golden age of herbal cosmetics has come. Along with active introduction of oriental herbal lines, diversification of distribution channels is designated as a major feature. In this background, the present study attempts to consider the domestic market for oriental herbal cosmetics, which is growing rapidly with the introduction of various new brands, and examine the perceptions of this new type of cosmetics by women consumers based on their purchase behaviors, and search for the ways for its promotion and development. A survey was conducted to adult women consumers aged 19∼60 residing in Seoul or Gyeonggi-do. Out of a total of 430 surveys distributed, 350 answer sheets were used for the analysis Among the results, the first-hand information on the herbal cosmetics market, their usage, and the consumer needs obtained in the present study will serve as a fundamental data for planning the marketing strategies for the oriental herbal cosmetics.

The Effects of Ascorbic Acid on Full-thickness Skin Wound Healing in Dogs (개에서 Ascorbic Acid가 피부 결손창 치유에 미치는 영향)

  • This study was designed to assess the healing effects of ascorbic acid on full-thickness skin wound in dogs. Four $2.5\pm2.5cm$ full-thickness skin wounds were created bilaterally on the dorsolateral aspect in each of 5 dogs. The ten wounds were treated with ascorbic acid solution in the experimental group, and the ten wounds were treated with $0.9\%$ saline in the control group. The wound healing area was measured every other day for 3 weeks after the wounding. Statistical analysis was conducted with the paired t-test. On the $1^{st},\;3^{rd},\;5^{th},\; 7^{th},\;9^{th},\;11^{th},\;13^{th},\;15^{th},\;17^{th},\;19^{th},\;21^{st}days$, the wound healing rates were $12.6\pm9.6\%,\;9.5\pm9.8\%,\;18.9\pm9.0\%,\;39.2\pm5.4\%,\;62.2\pm4.6\%,\;86.9\pm1.9\%,\;92.9\pm1.8\%,\;96.6\pm1.6\%,\;99.1\pm0.5\%,\;99.8\pm0.2\%,\;99.9\pm0.1\%$ in the experimental group, and $9.1\pm6.0\%,\;9.2\pm6.0\%,\;12.2\pm5.0\%,\;41.3 \pm6.3\%,\;54.2\pm9.2\%,\;81.0\pm2.6\%,\;85.7\pm2.8\%,\;92.3\pm1.5\%,\;96.1\pm0.7\%,\;98.0\pm0.8\%,\;99.5\pm0.2\%$ in the control group On the $15^{th}$ day, the wound closed above $99\%$ in 2 dogs of the experimental group. On the $17^{th}$ day, the wound closed above $99\%$ in 3 dogs of the experimental group. On the $19^{th}$ day, the wound closed above $99\%$ in 5 dogs of the experimental group, and 2 dogs of the control group. On the $21^{st}$ day, the wound closed above $99\%$ in 5 dogs of the experimental group, and 4 dogs of the control group. The wound healing rate of the experimental group was $2\~4$ days faster than that of the control group. The tensile strength was $4,169\pm1,107g/cm^2$ in the experimental group, and $2,438\pm637g/cm^2$ in the countrol group on the$21^{st}$ day after wounding. Tensile strength was significantly higher in the experimental group than in the control group (p<0.05). The histopathological findings of the experimental group were similar to those of the control group on the $1^{st},\;3^{rd}\;and\;5^{th}$days. Inflammation and revascularization of the experimental group were similar to those of the control group on the $9^{th},\;13^{th}\;and\;21^{st}$days. The formation of collagen and reepithelialization were more developed the experimental group than in the control group on the $9^{th},\;13^{th}\;and\;21^{st}$days. Significantly higher wound healing rate, tensile strength and better histopathological findings were observed in the experimental group than in the control group. These results suggest that the topical application of ascorbic acid on full-thickness skin wounds can promote wound healing process.

The Effect of $OPB-K^{(R)}$ on the Osseointegration and Stability of Implant ($OPB-K^{(R)}$가 임플랜트의 골유착 및 안정성에 미치는 영향에 관한 연구)

  • Statement of problem: It has been proved that Pleurotus eryngii Quel and Eleutherococcus senticosus have antiinflammatory action and not only stimulates the proliferation and activity of osteoblast but inhibits the generation and activity of osteoclast in vitro. Pleurotus eryngii Quel and Eleutherococcus senticosus are the main component of $OPB-K^{(R)}$. Purpose: The purpose of this study was to evaluate $OPB-K^{(R)}$ which enhances the healing rate of peri-implant bone and the bone mineral density. Materials and methods: Thirty six specially designed implants were installed in the tibia of rats. The group medicated with $OPB-K^{(R)}$ was the experimental group, and that without was the control group. hen the implant stability was measured by $Periotest^{(R)}$. Bone mineral density and histological measurement were conducted at the 2nd, 4th and 6th week $Periotest^{(R)}$ and bone mineral density values were analyzed statistically with independent t-test at 95% confidence level(p<0.05). Results: The results of this study were as follows : 1. There was no statistically significant difference in $Periotest^{(R)}$. values between the experimental group and control group at the 2nd week, however, on the 4th and 6th week there was significant difference(p<0.05). 2. There was no statistically significant difference in bone mineral density between the experimental group and control group at the 2nd and 4th week, however on the 6th week there was significant difference(p<0.05). 3. Histological analysis showed difference in osseointegration on the 4th and 6th week between the groups. Conclusion: From the results, it is concluded that the $OPB-K^{(R)}$ medicated group showed statistically better results in bone density and stability than the control group. Clinically it would be better to medicate $OPB-K^{(R)}$ to patients for a long period of time after implantation to get superior results.

The marginal fidelity of $Procera^{(R)}$ AllCeram alumina copings and crowns of patients ($Procera^{(R)}$ AllCeram 을 이용한 도재관의 임상적 변연적합도에 관한 연구)

  • Statement of problem: Use of all-ceramic prostheses fabricated with CAD/CAM systems is increasing in the dentistry. Marginal fidelity in production of all-ceramic restoration has important clinical implications and is a key consideration issue in CAD/CAM production as well. Purpose: The objective of this study was to analyse marginal fidelities of $Procera^{(R)}$ Allceram Crown. Material and methods: On 56 patients treated with $Procera^{(R)}$ system Allceram Crown at Dankook Dental Hospital, marginal discrepancies of 101 abutments were measured by stereomicroscope at coping and final restoration stages. Paired t-test and one-way analysis of variance on marginal discrepancy data were conducted to determine the presence of significant differences between measurement and measuring point stages. Results: Marginal discrepancies of final restoration ($45.82{\pm}30.84\;{\mu}m$) were lower than alumina coping ($53.84{\pm}38.83\;{\mu}m$). Furthermore, the differences were found to be statistically significant at 95% confidence level. Anterior marginal discrepancies were lower than posterior marginal discrepancies, but they were not statistically significant. Lingual marginal discrepancies were higher than other measurement sites, and the differences were found to be statistically significant at 95% confidence level. Conclusion: Within the conditions of this study, marginal fidelities of $Procera^{(R)}$ Allceram Crown were acceptable, and after porcelain build-up, marginal fitness improved over alumina coping. More careful scanning is needed for better results.

The Correlation Among Health Status, Burden and Quality of Life of the Adult Stroke Patient's Family and the Elderly Stroke Patient's Family (노인층과 청·장년층 뇌졸중 환자가족의 건강상태·부담감 및 삶의 질과의 관계)

  • The purpose of this study is to investigate the correlation among the stroke patient family's health, burden and quality of life which is based upon the comparative appreciation of the adult stroke patient's family and elderly stroke patient's family. For this purpose, data were collected from the family care-givers for two groups of stroke patients under sixty years old and over sixty years of age, admitted at K Hospital and H Hospital in Seoul. The instruments for this research are based on the tool for measuring physical health and psychological health developed by Yang, Young-hee(1992), the tool for measuring the sense of burden by Seo, Mee-hae and Oh, Ga-sil(1993), and the tool for the quality of life by Noh, Yoo-ja(1988). The sampling for this study was done from December, 2000 until February, 2001. Questionnaire data were drawn up by personal interviews aided by the staff nurses. The analysis of collected data are based on general characteristics calculated at the rate of 100 percent of the average, t-test, ANOVA(some difference on a level with p<.05 being subsquently confirmed by DMR) for Health Status, Burden, Quality of Life and Pearson Correlation to verify the hypothetical correlation among the subjects. The results of this study are as follows: 1. In the adult stroke patient family, the factors influencing the physical health proved to be age, present occupation and family-formation. Here, the factors influencing psychological health turned out to be age, matrimonial status, present occupation and family-formation. In the elderly stroke patient family, the factors influencing physical health proved to be age, gender, final academic status, matrimonial status, present occupation, and relation with the patient. Here, the factors influencing the psychological health were age, final academic status, matrimonial status, present occupation, relation with the patient and family-formation. In the former case, the influencing factors upon the burden were shown to be age, final academic status, matrimonial status, relation with the patient and family-formation. In the latter case, the influences upon the burden were age, gender, final academic status, matrimonial status, present occupation and relation with the patient. In the former case, the influences on the quality of life were gender, and economic situation. In the later case, the influencing factors on the quality of life were age, final academic status, matrimonial status, present occupation, and relation with the patient. 2. The rate of the physical condition in the former case turned out to be 2.83, and the psychological condition 2.37. The physical condition of the latter case was 2.76, and the psychological condition 2.46. The rate of the burden in the former case was 3.14, and that of the latter case was 3.04. The rate of quality of life in the former case proved to be 2.46, and that of the latter case 2.55. 3. The rate of correlation between the burden and the quality of life appeared to be the high counter-correlation (r= -.573). The rate of correlation between the psychological health and the burden of a simialr (r= -.565). The rate of correlation between the physical health and the psychological health proved to be a moderate correlation (r= .372), The rate of correlation between physical health and the burden turned out to be a low counter-correlation (r= -.276). According to this study, there proved to be a very close correlation among the stroke patient family's health, the burden and quality of life.
