수면무호흡증 예측을 위한 선별검사 개발 (Development of Screening Test for Prediction of Sleep Apnea Syndrome)

  • 연구목적 : 수면무호흡증 환자들은 수면다원검사를 통해 진단이 가능한데, 수면무호흡증이 의심되는 모든 환자를 검사의뢰하기에는 현실적으로 어려움이 많다. 따라서 저자들은 수면다원검사와 치료가 필요한 수면무호흡증 환자를 선별할 수 있는 간단한 검사 방법을 찾고자 본 연구를 시행하였다. 연구방법 : 코골기와 수면무호흡증으로 인해 수면 다원검사가 의뢰된 140명의 환자를 대상으로 하루 밤 동안 1회 수면다원검사를 실시하였다. 수면무호흡증의 증상은 18개의 문항으로 된 설문지로 평가하였고, 우울증은 Zung's 척도, 졸리움증은 Stanford Sleepiness Scale(SSS), 불면증은 이성훈 등(14)이 개발한 불면증척도 등을 이용하였다. 그외 혈압, 키, 체중, 목둘레 등을 측정하였고, 코골기의 가족력, 얼마나 오랫동안 코골기가 있었는지, 술, 담배를 어느 정도 하는지를 조사하였으며, digit symbol test, trail making B test 등의 인지기능검사를 시행하였다. 그리고 이 변수들과 호흡장애지수 간의 상관관계를 SPSS-PC 의 Pearson's correlation 으로 조사하였고, 통계적으로 유의하게 나온 변수들을 가지고 판별분석을 이용하여 호흡장애지수에 대한 이 변수들의 판별계 수를 구하고, 그것으로 판별식을 만들었다. 또한 판별분석상 호흡장애지수에 영향을 미치는 것으로 밝혀진 변수들에 대해 각각 기준점을 정하고, 호흡장애지수 20을 기준으로 하여 선별검사의 민감도를 조사하였다. 연구결과 : 판별분석결과 호흡장애지수에 영향을 미치는 것으로 나타난 6가지 변수 즉, 목둘레, 수면전 수축기혈압, 음주정도, 자다가 숨을 멈추는 정도, 잠잘 때 입안이 마르는 정도, 수면무호흡 설문지 점수를 가지고 판별식을 만들어 보았을 때, 호홉장애지수 20이상인 환자를 92.8%의 민감도로 찾아낼 수 있었다. 또한, 목둘레 40cm이상, 수축기 혈압 125mmHg 이상, 잦은 음주, 수면중 잦은 숨멈춤, 수면중 잦은 구갈증, 코골기 및 수면무호흡증 설문지 총점수 35점 이상 중 3개 이상 해당시 호홉장애지수 20이상인 환자를 87.6%의 민감도로 찾을 수 있었다. 그리고, 목둘레 40cm이상, 수축기 혈압 125mmHg이상, 잦은 음주, 체중 80kg이상 중 2가지 이상 해당 시 호흡장애지수 20 이상인 환자를 83.5%의 민감도로 찾을 수 있었다. 결론 : 외래에서 간단히 시행할 수 있는 설문지조사와 이학적검사를 이용한 위의 방법으로 호흡장애 지수 20이상인 환자를 높은 민감도로 구별해 낼 수 있었다.

발마사지가 항암화학요법을 받는 암환자의 활력징후, 피로 및 기분상태에 미치는 효과 (Effect of Foot Reflexology on Vital Signs, Fatigue and Mood in Cancer Patients receiving Chemotherapy)

  • Purpose: This study was done to investigate the effect of foot reflexology on vital signs, general fatigue, mood and foot fatigue in cancer patients receiving chemotherapy. Method: Data were collected from a convenience sample of 11 patients who were admitted at S and E University Hospital. Changes in systolic blood pressure (SBP), diastolic blood pressure(DBP), pulse rate(PR), general fatigue, mood status, and foot fatigue between pre and post foot reflexology were analyzed using Wilcoxon signed rank test and Friedman test at 0.05 level of significance. Results: There were significant differences in the SBP(p= .009), DBP(p=.014), PR(p=.015), general fatigue (p=.015) mood status(p=.007), and foot fatigue(p= .007) between pre and post foot reflexology. Conclusion: Foot reflexology can improve vital signs, general/foot fatigue and mood status, therefore we recommend to use foot reflexology as an effective nursing intervention in cancer patients receiving chemotherapy.

직장인에서 비만과 혈액지표와의 상관성에 관한 연구 (A Study for the Correlations between Obesity and Blood Parameters at the Worksite)

  • Obesity, a state of having excessive body fat enough to attack one's health, is most blamed for causing chronical degenerative illness such as hypertension, diabetes mellitus as well as arteriosclerosis. According to many studies, the program for the weight control was the most demanded at the worksites. This study was designed to investigate the correlation between the degree of obesity and blood parameters of the employees at the worksites. The results of periodic medical examination for 571 employees(male: 418, female: 153) were analyzed; Age, BMI, systolic blood pressure(SBP), diastolic blood pressure(DBP), hemoglobin(Hb), fasting blood sugar(FBS), total-cholesterol(TC), serum glutamic oxaloacetic transaminase (sGOT), serum glutamic pyruvic transaminase(sGPT) and $gamma$-glutamyl transpeptidase($gamma$-GTP). Pearson correlation coefficient was used to find out the correlation between BMI and each blood parameters. In the case of male, 27.3% of the subjects were over-weight. The subject's age and their blood levels of uric acid, FBS, sGOT and sGPT did not show any significant difference regardless of BMI's, while SBP, DBP, Hb, TC and $gamma$-GTP levels of the overweight group were significantly higher than those of normal group. In the case of female, 9.2% of the subjects were over-weight. This study shows that the implementation of weight control program is required at the worksites

Calcium Channel-blocking Activity of Chinese Balloon Flower (Platycodon grandiflorum) for Producing Blood Pressure-lowering Functional Foods

  • This study was conducted to evaluate the hypotensive properties of the extract of Chinese balloon flower (Platycodon grandiflorum)'s root. In the studies for calcium channel-blocking using Xenopus oocytes, the ethanol-extract ($26.2{\pm}5.2%$) showed higher activity than water-extract. Twenty female rats were fed 25, 35, and 45 mg/kg BW/day of the ethanol-extract for 14 days to observe the changes in blood pressures and heart pulses. Ethanol-extract decreased the systolic, diastolic, and mean blood pressures of the rats. Especially, the rats fed with 45 mg/kg BW/day of the ethanol-extract showed significant decreases in the blood pressures. These results suggested that a decrease in blood pressures was due to the extension of a blood vessel with calcium channel-blocking by ethanol-extract of Chinese balloon flower. Forty %-ethanol showed the highest efficiency for ethanol-extraction of Chinese balloon flower.

복부가온패드 적용이 대장내시경 검사 대상자의 통증과 불안에 미치는 영향 (Effects of Electric Heating Pad on Abdominal Pain and Anxiety during the Colonoscopy)

  • Purpose: The purpose of this study was to examine the effects of an electric heating pad on abdominal pain and anxiety during the colonoscopy procedure. Method: Data were collected from 70 adult patients (experimental group (35), control group (35)) who underwent colonoscopy in a general hospital in Seoul, South Korea between January 6 and May 4 2006. For the experimental group the electric heating pad was applied from 20 minutes before the test through the whole procedure. Objective pain was measured during the test, and subjective pain, state anxiety, blood pressure and pulse rate were measured after the procedure. Results: The experimental group with the electric heating pad reported significantly lower subjective pain and anxiety than did the control group. There were no significant differences in objective pain between experimental and control groups. Patients with an electric heating pad showed significantly lower systolic and diastolic pressure than did those in the control group. There was no significant difference in pulse rate between the groups. Conclusion: Use of an electric heating pad was efficient in reducing subjective pain and anxiety among patients undergoing colonoscopy. This is a convenient and useful nursing intervention to reduce anxiety and pain among patients having a colonoscopy.

영동지역 과체중 및 비만 성인에서 항산화 영양소섭취상태와 혈중 산화 및 염증관련 지표와의 관련성 (Relationship between Antioxidant Nutrient Intakes, Blood Oxidative Stress and Inflammatory Markers of Overweight and Obese Adults in Yeongdong Area)

  • This study was designed to investigate the relationship between the nutrient intakes, antioxidants, and inflammatory markers of overweight and obese adults (46 females and 16 males) in Yeongdong area. The study was conducted through questionnaires, anthropometric checkups, 2-day 24 hr recalls and blood biomarker analyses. Body fat was significantly higher in women compared to men. Diastolic blood pressure (DBP) and systolic blood pressure (SBP) were significantly higher in men compared to women. There were no significant differences in height, weight, body mass index, and waist size among the two groups. The intake of nutrients was significantly higher in men compared to women. Ferric reducing ability plasma (FRAP) was significantly higher in men compared to women. Body fat was positively associated with blood IL-6 and IL-8 levels. DBP was positively associated with FRAP. The intake of protein was negatively associated with IL-6 levels. The intake of carbohydrates was negatively associated with total antioxidant capacity (TAC). Prostaglandin E2 (PGE2) levels were negatively associated with TAC. These results suggest that antioxidant and inflammatory markers may be related to the body fat percentage and dietary intake in overweight and obese adults.

자가 발반사마사지가 중년여성의 우울, 스트레스 반응 및 면역기능에 미치는 효과 (Effect of Self-Foot Reflexology massage on Depression, Stress Responses and Immune Functions of Middle Aged Women)

  • Purpose: This study was aimed to identify the effects of a self-foot reflexology massage on depression, stress responses and functions of the immune system of middle-aged women, Method: This study was a one group pretest-postlest experimental design and the data was collected from August $1^{st}$ 2004 to May $31^{st}$ 2005. The subjects consisted of 46 middle-aged women (40 - 64 years) who were recruited from the Community Health Center in Busan city. Subjects were not treated for 4 weeks, subsequently they were trained in self foot reflexology massage for 2 weeks, and then they did their own daily for 6weeks (2 days at the research center, 5 days at home).The outcome variables were measured 4 times, at baseline, pre training, after training, and after the intervention. The collected data was analyzed using repeated measure ANOVA by the SPSS/WIN program. Result: There was a statistically significant difference in depression, perceived stress, systolic blood pressure, natural-killer cells and Ig G. However, there was not a statistically significant difference in dyastolic blood pressure, pulse or serum cortisol. Conclusion: These results suggest that a self-foot reflexology massage could be utilized as an effective nursing intervention to reduce depression and stress responses, and to strengthen immune systems in middle-aged women.

Effects of Music Therapy on Pain, Discomfort, and Depression for Patients with Leg Fractures

  • Purpose. To determine the effects of music therapy on pain, discomfort, and depression for patients with leg fractures. Methods. Data were collected from 40 patients admitted in an orthopedic surgery care unit. The subjects included 20 intervention group members and 20 control group members. Music therapy was offered to intervention group members once a day for 3 days for 30-60 minutes per day. Pain was measured with a numeric rating scale and by measuring vital signs. Discomfort and depression were measured with self-administered questionnaires. Results. Patients who received music therapy had a lower degree of pain than patients who did not receive music therapy as measured by the numeric pain score (p < 0.001), systolic blood pressure (p < 0.01), diastolic blood pressure (p < 0.001), pulse rate (p < 0.001) and respiration (p < 0.001). Patients who were provided with music therapy also had a lower degree of discomfort than patients who were not provided with this therapy (p < 0.01). Effects of Music Therapy on Pain, Discomfort, and Depression for Patients with Leg Fractures Conclusions. These results demonstrate that music therapy is an effective method for decreasing pain and discomfort for patients with leg fractures.

음악요법이 위내시경 대상자의 검사전 불안에 미치는 효과 (The Effect of Music Therapy on Reduction of Anxiety before Gastroscopy)

  • Purpose: This study examined the effect of music therapy on the pre-exam anxiety of gastroscopy clients. Method: The research period is from June 17 2002 to August 30 2002, and the subjects were 60 individuals, 30 for the experimental and 30 for the control group among adult clients. The data were analyzed by using the SPSS program. Result: The results of this study were as follows: The score of the experimental group was lower than the score of the control group for the situational anxiety (t=-12.56, p=0.000) and systolic blood pressure (t=-3.97, p=0.000) before the examination was supported. The vital sign was partially supported. There was no significant difference(t=-1.05, p=0.297) in the diastolic blood pressure (t=-1.05, p=0.297) and pulse rate(t=-1.30, p=0.199) before the examination of the experimental group and lower than the score of the control group. Conclusion: Music therapy could be useful in the nursing practice, and be utilized as a way of nursing intervention to reduce anxiety of the patients in specific clinical situations. Finally, it is recommended that further studies be conducted on this therapy by utilizing different musical selections.

음악요법이 응급실 환자의 불안에 미치는 영향 (The Effect of Music Therapy on Anxiety of Patients in Emergency Room)

  • Purpose: The purpose of this study was to examine the effects of music therapy on anxiety of patients in emergency room. Methods: The study was designed using a noneqivalent control group nonsynchronized design. For 20 minutes, the experimental group(22 patients) had listened to music and the control group(23 patients) had bed rest. A six-item state anxiety scale developed by Marteau and Bekker, which was based on Spielberger's State-Trait Anxiety Inventory, blood pressure and pulse rate were measured to all study participants before and after the intervention. The data was analyzed by the SPSS 12.0 program. Results: 1) The experimental group's state anxiety level were significantly lower than the control group's(t=2.220, p=.032). 2) There were no significant differences in both group's diastolic(t=-.495, p=.623) and systolic blood pressure(t=831, p=.411). 3) The experimental group's pulse rate was significantly lower than the control group's(t=2.363, p=.023). Conclusion: Music therapy may be applied as a nursing intervention to decrease anxiety in emergency room.
